Purpose: The purpose of this study was to investigate factors which can delay hospital arrival for patients with acute stroke. Method: The participants for this study were 102 patients who were admitted to the Department of Neurology in a hospital in Chungnam province. Data were collected from Oct, 2002 to June, 2003. The data were analyzed with frequencies, percentages, and $x^2$ test using the SPSS PC program. Result: The results of this study were as follows: 1. Only 33.8 % of participants arrived at the hospital within 3 hours. 2. Educational level, economic status, people living in same residence, and place of residence were factors which significantly affected prehospital delay time. Also there were significant differences in prehospital delay time according to family history of stroke, day of the week when symptoms began, perception of seriousness of symptoms, type of hospital first used, and transportation. Conclusion: Therefore, to reduce prehospital delay time, educational programs which focus on the above factors need to be developed. Also, a public campaign for utilizing emergency service to reduce transportation time for acute stroke patients need to be facilitated.

Purpose: This study was performed to identify the level of Korean elderly's knowledge regarding concepts of end-of-life (EOL), Life-sustaining-treatment (LST), and advance directives (AD) which are critical aspects for establishing AD in Korean society. Methods: A questionnaire survey was done between October 2011 and February 2012. Knowledge of AD was evaluated with 3 aspects including EOL, LST, and AD utilizing a questionnaire that was developed by authors for the study. Data were collected from 268 community dwelling elderly from three cities and analyzed using descriptive statistics, t-tests, one-way analysis of variance, and a Scheffe post hoc test with SAS Ver. 9.1 program. Results: Overall, Korean elderly were poorly acquainted with AD related concepts. Significant differences in awareness of AD including understanding of EOL, the level of comprehension of LST, and knowledge about AD were revealed by gender, education level, economic state, and acquaintance with terms of AD or LST. Conclusion: To acknowledge autonomy and support quality of life for elderly and to meet the purpose of AD, attention should be given to target populations including elderly in terms of knowledge level related to AD, social marketing, and infra structure relevant to practice AD in our society.
Purpose. 1) To identify the time taken from symptom onset to the arrival at the hospital (pre-hospital delay time) and time taken from the arrival at the hospital to the initiation of the major treatment (in-hospital delay time) 2) to examine whether rapid treatment results in lower mortality. 3) to examine whether the pre- and in-hospital delay time can independently predict in-hospital mortality. Methods. A retrospective study with 586 consecutive AMI patients was conducted. Results. Pre-hospital delay time was 5.25 (SD=10.36), and in-hospital delay time was 1.10 (SD=1.00) hours for the thrombolytic therapy and 50.24 (SD=121.18) hours for the percutaneous transluminal coronary angio-plasty (PTCA). In-hospital mortality was the highest when the patients were treated between 4 to 48 hours after symptom onset using PTCA (p=.02), and when treated between 30 minutes and one hour after hospital arrival using thrombolytics (p=.01). Using a hierarchical logistic regression model, the pre- and in-hospital delay times did not predict the in-hospital mortality. Conclusion. Pre- and in-hospital delay times need to be decreased to meet the desirable therapeutic time window. Thrombolytics should be given within 30 minutes after arrival at the hospital, and PTCA should be initiated within 4 hours after symptom onset to minimize in-hospital mortality of AMI patients.
Purpose: This study aimed to determine the influence of a breastfeeding coaching program (BCP) for mothers of late preterm infants (LPIs) on the breastfeeding rate and neonatal morbidity within 1 month after discharge. Methods: This was a non-randomized quasi-experimental study with a time series design. The participants were 40 LPIs and their mothers who were hospitalized in a neonatal intensive care unit at a university hospital. Nineteen LPIs were assigned to the control group, and 21 to the experimental group. The mothers of the LPIs in the experimental group received the BCP once on the discharge day and then once a week for 1 month. Neonatal morbidity was defined as an outpatient department or emergency room visit due to an LPI's health problem. Results: The breastfeeding rate in the experimental group was significantly higher than in the control group at the fourth week after discharge (χ2=7.17, p=.028). Five and two LPIs in the control group and the experimental group, respectively, visited a hospital due to neonatal jaundice. Neonatal morbidity was not significantly different between the two groups (χ2=1.95, p=.164). Conclusion: The BCP was useful for improving the breastfeeding rates of LPIs and may have potential to reduce neonatal morbidity.
Purpose: The purpose of this study was to explore health care workers awareness and performance of standard precautions. Methods: Participants were 296 health care workers including nurses, physicians, and medical technicians. Awareness and performance of standard precautions were measured with 4-point Likert scales. The data were analyzed with t-tests and one-way ANOVA by using SPSS 18.0. Results: The mean scores of awareness were 3.72 in nurses, 3.62 in physicians, and 3.47 in medical technicians. There was a significant difference of awareness by occupational classification (F=12.39, p<.001). The mean scores of performance of standard precautions were 3.45 in nurses, 3.19 in physicians, and 3.23 in medical technicians. There was a significant difference of performance by occupational classification (F=10.98, p<.001). In addition, the score of performance of standard precautions was significantly lower than that of awareness (t=11.89, p<.001). Conclusion: The results of this study indicated that awareness and performance of standard precautions were different by occupational classification. To improve performance of standard precautions in hospitals, it is necessary to provide a distinct infection control program by occupational classification.
This research has been conducted in order to compare and analyze the role-conflict, job stress and job satisfaction of nurses according to their current work posts. The subjects of this research were carefully selected 426 nurses from 2 university hospitals. The research tool were a role-conflict measurement index, job stress a tool, and job satisfaction Minnesota Satisfaction Questionnaire. The data have been collected using a structured questionnaire from March to May 2003. The collected data have been analyzed using the SPSS PC 10.0V program. The nurses at Intensive Care Unit (ICU) showed highest level of job stress while the nurses at surgical ward showed the lowest level of job stress (F=8.37, p=.000). There was no significant difference in role-conflict and job satisfaction according to the current work posts. According to the results of this research, there was a difference in stress according to the current work posts, and it turned out that the nurses at ICU had the highest level of stress compared to the nurses at other departments, and it causes a fall in job satisfaction.

Purpose: This study was dont: to evaluate the factors that influence secondary traumatic stress, burnout, and physical symptoms experienced by firefighters. In addition, a hypothetical model was developed. Method: The data were collected using self-reported questionnaires from 410 field service firefighters. Data analysis was done with the SPSSWIN 10.0 program for descriptive statistics and Windows LISREL program to determine the causal relationship between the measurement variables. Results: The hypothetical model which was developed fits well with actual data(${\chi}^2$=3.16, p=0.99, GFI=1.00, AGFI=0.99, NFI=0.99, and NNFI=1.06). The variable, personality type, social support, and impact mobilization frequency had a significant effect and accounted for 7% of the secondary traumatic stress in firefighters. The variables, career, social support, and secondary traumatic stress had a significant effect and accounted for 24% of the burnout in firefighters. Social support, impact mobilization frequency, and secondary traumatic stress were important variables and accounted for 31% of the physical symptoms in firefighters. Conclusion: It is necessary for firefighters to manage themselves according to their personality types. Strategies that can increase the firefighters' social support and decrease their perceived secondary traumatic stress also need to be developed to minimize or prevent a negative effect on their health.
Purpose: This study was to investigate the effects of upper meridian massage on cerebral blood flow, emotions, and sleep of the institutionalized elderly. Methods: This study was a nonequivalent control group pre- and post-test design. The participants were 50 elderly (26 in the experimental group; 24 in the control group) living in the institutions. Data were collected between July 9 and September 1, 2011. The data were analyzed using descriptive statistics, t-test, and $x^2$-test, Repeated Measures ANOVA, and Cronbach's ${\alpha}$ coefficient. Each participant in the experimental group received the upper meridian massage for 10 minutes, 4 times per week for 2 weeks. Each participant's cerebral blood flow and self-reported questionnaires were tested before treatment, after 1 week and 2 weeks during treatment sessions. Results: There were significant differences in sleep and emotions after 1 week and 2 weeks during treatment sessions. But cerebral blood flow measured by common carotid artery pulsatility index (CCA PI) and common carotid artery resistance index (CCA RI) demonstrated significant differences in 2 week point in time. Conclusion: These results indicated that upper meridian massage could be an effective intervention for improving cerebral blood flow, emotions, and sleep of the institutionalized elderly.
Purpose: The purpose of this study was to identify the effects of the phytoncides on serum cortisol level, vital signs and life stress of college students. Methods: This study was nonequivalent control group non-synchronized design. The subjects consisted of 60(control group: 28, experimental group: 32). The phytoncides mixed the pine(pinus sylvestris) oil with the cypress(cupressus sempervirens) oil in the same rate. In the experiment, it was put in an aroma-necklace bottle and inhaled 3 times per day(9AM, at noon, before going to bed) for 2 weeks. The data were analyzed by the SPSS version 12.0 program. Results: In the experimental group systolic BP(F=15.603, p=.000), diastolic BP(F=29.489, p=.000) and serum cortisol level(F=4.968, p=.000) were significantly decreased. Conclusion: The phytoncides inhalation is a partly help to reduce college students' life stress. The phytoncides is the incense of the familial tree. We recommended to examine what differences between the green shower and the phytoncides inhalation for the stress reduction.
