Influencing factors of prevention practices against infection exposure among emergency medical technicians in emergency rooms (응급실 응급구조사의 감염노출 예방행위 수행정도에 영향을 미치는 요인)

  • Purpose: The purpose of this study was to investigate the influencing factors of prevention practices against infection exposure among emergency medical technicians (EMTs) in emergency rooms. Methods: A self-reported questionnaire was filled out by 100 EMTs in emergency rooms from June 1 to August 31, 2017. The questionnaire consisted of items concerning the defensive environment for the prevention of infection exposure, perception of preventive behavior, and degree of performance of preventive actions against infection based on a five-point Likert scale. Data were analyzed by descriptive statistics, ${\chi}^2$ test, ANOVA, Pearson's correlation coefficients, and linear regression. Results: The defensive environment for the prevention of infection exposure was 4.12. The perception of preventive action was 4.71, and the degree of performance of preventive actions against infection was 4.54. There was a significant relationship between the degree of performance of preventive actions against infectious exposures and the degree of perception of preventive behavior(r=.506, p=.01) and prevention of infectious exposure(r=.506, p=.01). The protective environment(B=.360, t=3.236, p=.002) and perceived level (B=.904, t=4.662, p=.000) were influenced by the degree of prevention of infection exposure. Conclusion: It is important to manage the protection environment for infection exposure prevention and to enhance the awareness of infection prevention actions against infection exposure among the EMTs in emergency rooms.

  • Due to the improvement in life, we, human beings, are living in the more convenient world. However, as society gets complex, it comes to be faced a dangerous situation that needs to request a help due to an unexpected accident. The promptly emergency relief and emergency treatment from this accident, can be said to be extremely important in increasing a rate of resuscitation. This study analyzed on a case study and problems about the Emergency Medical System out of hospital in our country. The contents in the results of a study can be summarized as follows. 1. given the occurrence of medical disputes in a situation of emergency treatment, the regulation in the legal protection 'Will need to be reinforced. 2. in relation to the mobilization of ambulance, the preferentially passage right in ambulance needs to be guaranteed. In general, the preferentially passage right in ambulance is secured, but it will need to be supplemented by improving the education in civic consciousness and the social system. 3. in case of emergency duties in the emergency medical technician, the countermeasure will need to be strengthened in terms of abusive language and violence from a citizen. 4. in terms of emergency duties in the emergency medical technician, aiming to keep safety from a sudden situation, the possession of the protective equipments will need to be completely provided. 5. the limitation of activity will need to be eased by expanding the range of duties in the emergency medical technician within the ambulance, and the opening of duties will need not to be allowed to the similarly job category. 6. it needs to be reinforced the safety of the emergency medical technician from infectious diseases. Aiming at these emergency medical technicians whose service spirit is thoroughgoing, the nation and the local government will need to form all the conditions in which emergency medical technicians can be active passionately.

Analysis of patients retransferred to a tertiary hospital through paramedics (119구급대를 통해 3차 병원으로 재이송된 환자분석)

  • Purpose: The study aimed to collect the first aid activity log data of patients who were retransferred through paramedics and to analyze the data in the hospital to improve the plan. Methods: We analyzed 434 retransferred patients, out of the 18,197 patients who visited the emergency medical center in the C area in G metropolitan city, from January 2017 through December 2018. The collected data were analyzed using the SPSS software. Results: The patients were retransferred for various reasons: 17.7% (77 cases) due to the absence of specialists, 15.0% (65 cases) required first aid, and 5.3% (23 cases) due to absence of medical department. In addition, the major medical department with the largest number of retransfer was the department of emergency medicine with 38.2% (166 cases). In the prehospital stage, 38.5% (167 cases) were classified as severe, but in the hospital stage, they were classified as mild. In addition, as a result of hospitalization and discharge, 60.4% (262 cases) were discharged. Conclusion: Most of the retransferred patients were non-emergency patients, and were discharged for mild conditions. The overcrowding in the emergency room of a tertiary hospital can be prevented by reducing the retransmission.

The Study of Critical Indicators Development for Establishing Patient Classification System in the ER (응급실의 환자분류체계 확립을 위한 결정지표 개발 연구)

  • Purpose: The purpose of the study was to identify critical indicators for the development of efficient patient classification system in a emergency room. Method: This study involved following five steps. Step 1. Selection of the lists direct nursing services in the ER. Step 2. Measurement of the time of direct nursing services from Aug. 31st to Nov. 30th, 2005. Step 3. Classification of the patients according to the nursing care time. Step 4. The determination the critical indicators for different patient classes. Result: Determinate indicators were as follow: 3 items in the first group (vital sign checking, IV route starting, blood sampling), 3 items in the second group (vital sign checking, fluid infusion, blood sampling), 9 items in the third group (I/O checking, $O_{2}$ inhalation, suction, fluid infusion, IV bolus, Central catheter preparation & management, blood sampling, intubation preparation & management, postmortem management), 7 items in the fourth group (EKG monitoring, BP monitoring, $O_{2}$ inhalation, fluid infusion, using the specific drugs, CPR, postmortem management). Conclusion: This study can help future studies which measure nursing services standard time or assigns value to emergency nursing services.

A study on the cardiopulmonary resuscitation by the emergency medical dispatcher (구급상황관리사에 의한 심폐소생술 안내 실태 연구)

  • Purpose: The purpose of the study is to check up the status of 119 emergency control centers usage. Therefore, the status of use of 119 emergency control centers and the incidence of pre-hospital cardiac arrest patients were investigated. Methods: The emergency activity daily reports and first aid diaries of 119 emergency control centers from January to December 2018 were reviewed. For more accurate status analysis, Among the first aid guidance received in the emergency rescue standard system, the cardiopulmonary resuscitation guide log was reviewed. Results: In 2018, the total usage of the 119 emergency control centers was 1,358,356 calls, hospital guidance werethe most commom (n=629,676, 46.4%), followed by first aid (n=428,027, 31.5%), disease consultation (n=170,238, 12.5%), medical oversight (n=111,188, 8.2%), and interhospital transfer (n=5,052, 0.4%). Regarding the user number per 1,000 persons, Jeju was the greatest at 48.0, whereas Changwon was the lowest at 13.0. A total number of dispatcher-assisted cardiopulmonary resuscitation was 12.181. The time from report to chest compression were 156.2±80.8 seconds for those with previous cardiopulmonary resuscitation training and 168.0±79.3 seconds for those without such training (p<.05). Conclusion: The ratio of first aid instructions, including dispatcher-assisted cardiopulmonary resuscitation, among total usage of the 119 emergency control centers increased. Therefore, additional efforts are required to improve the quality and expertise of information provided through the 119 emergency control centers.

Identifying research trends in the emergency medical technician field using topic modeling (토픽모델링을 활용한 응급구조사 관련 연구동향)

  • Purpose: This study aimed to identify research topics in the emergency medical technician (EMT) field and examine research trends. Methods: In this study, 261 research papers published between January 2000 and May 2022 were collected, and EMT research topics and trends were analyzed using topic modeling techniques. This study used a text mining technique and was conducted using data collection flow, keyword preprocessing, and analysis. Keyword preprocessing and data analysis were done with the RStudio Version 4.0.0 program. Results: Keywords were derived through topic modeling analysis, and eight topics were ultimately identified: patient treatment, various roles, the performance of duties, cardiopulmonary resuscitation, triage systems, job stress, disaster management, and education programs. Conclusion: Based on the research results, it is believed that a study on the development and application of education programs that can successfully increase the emergency care capabilities of EMTs is needed.

A study on standardization & completion of transfer consultation record for patients transferred to emergency medical center (응급의료센터로 전원된 환자의 진료의뢰서 표준화 및 충실도에 관한 연구)

  • The purpose of this research which was conducted by surveying the transfer consultation records from 360 medical institutions such as general hospitals, hospitals, clinics to the Emergency Medical Center at E University Hospital for six months(Jan. 1, 2000 - Jun. 30, 2000) are to standardize & complete transfer consultation record of hospitals at the 1st & 2nd referral level and to give patients transferred emergency medical center medical information services on a better quality. The conclusions and suggestions from this study were summarized as follows; (1) Examing the distribution of the referral medical consultation(transfer) sheet type, surgery part local clinic sheet types were 34.4%, medical part local clinic sheet types were 26.7%, undifferentiated local clinic sheet types were 23.9% and hospital level sheet types were 15.0%. (2) The items of the transfer consultation records had been standardized more than 75% in the order of patient's name, date, doctor's name, diagnosis, patient's status, impressions. (3) That the degree of recording completion on these items is in the order of patient's name, date, diagnosis, impressions was revealed. (4) Because the standardization and the degree of recording completion are very low in the patient's gender, age, address, electronic recording system was needed for more perfect input of initial patient informations. (5) This standardizing & complete recording on examination and medication will prevent re-examination and abuse of medication for patients transferred emergency medical center. (6) EMT Transfer System should be fixed in all medical institute for the standardizing & complete recording on care period and departure time will give many emergency patients the proper treatments at the proper time. (7) It was revealed that developing new standardized transfer consultation record & using electronic recording system are needed. (8) The complete recording & Fast Track System were needed for higher rate of bed operation at emergency medical center and more hospital profit.

The appropriate scope of work of EMT-paramedics in emergency medical institutions according to expert opinion (전문가들이 생각하는 응급의료기관 내 종사하는 1급 응급구조사에 대한 인식과 적정 업무 범위는)

  • Purpose: This study aimed to assess and improve the work scope of emergency medical technicians (EMT)-paramedics in emergency medical centers. Methods: Data were collected from February 1 to 14, 2018. The questionnaire comprised a total of 56 questions: eight general questions, six questions about the recognition of EMT-paramedics in emergency medical centers, and 42 questions about their work. Data were analyzed via frequency and correlation analysis using SPSS version 18.0. Results: Regarding the work of EMTs in emergency medical centers, positive opinions were found in 23 items, negative opinions in 4 items, and both positive and negative opinions in 9 items. Regarding the questions about the necessity, work importance, professionalism, and the need for an increase4 in EMR-paramedics, depending on respondents' career, there was no statistically significant difference between the group of beginners and the group comprising beginners and that of experienced workers (p>.200). Conclusion: The expert survey revealed that the work performance of EMT-paramedics in regional emergency medical centers was trusted and showed positive results of improvement and expansion.

An Analysis of Demand on EMD Education in Emergency Medical Care Information Centers (응급의료정보센터의 응급통신관리교육 요구도 분석)

  • The purpose of this study which was done by questionnaire survey on doctors, paramedics, radio operators, computer technicians, administrators in Emergency Medical Care Information Centers was to analyze demand on EMD education. The significant 101 data were collected in 12 Emergency Medical Care Information Centers from Dec. 17, 2003 to Jan. 31, 2004 and analyzed by using SPSS. The conclusions from this study were summarized as follows. Composition of respondents who work in Emergency Medical Care Information Centers were 40.7% 26-30 years old in age, 56.4% male in sex, 55.6% medical direction in duty, 76.2% paramedics in certificate. 54.5% out of the paramedics had two years present career, 62.3% had one year past career, 31.0% didn't receive EMD education, 39.0% wanted 5-8 hours continuing education. The paramedics received more EMD education on Introduction to Emergency Medical Concepts, Obtaining Information from Callers, Providing Emergency Care Instructions and wanted more continuing education on Providing Emergency Care Instructions, Key Questions & Pre-Arrival Instructions, Obtaining Information from Callers. This study will be helpful to build up an education system for EMDs such as continuing education, curriculum, certification.

The Basic Design on Dispatch Life Support Pre-arrival Instruction Protocol (응급통신소생술 도착전지시 프로토콜 기본설계 방안)

  • As technology advances rapidly, emergency medical dispatching systems must also advance. Optimal emergency medical dispatching requires the courage to practice medicine at dispatch. Modern emergency medical dispatch provides appropriate resource responses with the use of an (Advanced Medical priority Dispatch System(AMPDS) in U.S.A. The AMPDS is a systematic protocol for all aspects of the dispatch process. Based on the AMPDS chief complaint list, we initiated a basic design on pre-arrival instruction protocol and completed the scripted pre-arrival instruction protocol, using Cardiopulmonary resuscitation(CPR) protocol. Implementation of this study creates computerized emergency medical dispatching system for future enhancements, such as the AMPDS.
