Drinking Culture Among Korean College Students (일개 지역 남녀 대학생들의 음주문화 실태 -일개 대학의 사례를 중심으로-)

  • The purpose of this study was to conduct a survey of general status of drinking culture among college students in Korea. Three hundred college students answered the self-administered questionnaire from May 6 to May 10 in 2009. The questionnaire consisted of drinking-related behavior by gender, recognition and need for moderation in drinking program, and the reason for stopping drinking by gender. The statistical method was done by SPSS WIN version 14.0. The first drinking time was high school comprising 41.0% of the respondents. The reason for drinking was to get the personal relationship. The experience of blackout was 50.3% in twice a year and 26.0% in twice per six months. The reasons for stopping drinking were health problem(36.9%), personal relationship(34.5%), and financial difficulty(20.8%). These results will make it possible to establish the right drinking habit, prevention of sexual violence and vehicle accident and health promotion of the students.

Multilevel Analysis of Factors Related to Cost and Length of Stay in Acute Myocardial Infarction Patients with Coronary Stenting: Based on Korean National Health Insurance Service's Customized Database in 2010 and 2015 (관상동맥 스텐트를 삽입한 급성 심근경색 환자의 진료비 및 재원일수 관련 요인에 대한 다수준분석: 2010년과 2015년 국민건강보험공단 맞춤형 데이터베이스 자료를 바탕으로)

  • Background: This study aims to analyze the cost and the length of stay (LOS) of acute myocardial infarction (AMI) patients with coronary artery stenting according to the characteristics of individuals and institutions. Methods: The data was collected from Korean National Health Insurance Service's customized database in 2010 and 2015. Chi-square test, t-test, analysis of variance, and multilevel analysis were performed. Results: The intraclass correlation coefficients for cost were 7.02% in 2010, 5.61% in 2015 and for LOS were 3.17%, 1.40%, respectively. The average costs were 9,067,000 won in 2010 and 9,889,000 won in 2015 (p<0.0001). However, the cost in 2015 was lower than the cost applying increased fee. The costs increased in aged 50-59 years, 60-69 years, and aged ≥70 years versus in aged under 49 years. The cost was higher in Charlson comorbidity index (CCI) 3 to 4 and ≥5 than in CCI 0. The costs were lower in male, medical aid recipients, metropolises, and local hospitals in other regions in 2010. LOS decreased from 8.1 days in 2010 to 7.4 days in 2015. It decreased in male, high income group, and the group of admission via emergency room. However, it increased in higher ages and medical aid recipients, and it also increased when CCI rose. The Internal Herfindahl Index was related to LOS in 2010. Conclusion: The variation of hospital level was small compared to the patient level. Therefore, it is important to implement applicable policies at the patient level in order to reduce cost and LOS of AMI patients.

Risk Factors Associated with Cataract and Macular degeneration by in Korean aged 60 years and over (한국인 60세 이상 노년층의 황반변성과 백내장의 위험요인 -국민건강보험공단 노인코호트 자료를 활용하여-)

  • Purpose: This study was conducted to identify factors associated with cataract and age-related macular degeneration in elders. Methods: In total, 341,588 men(44.18%,) and women(61.887%), aged over 60, were recruited from Korean National Health Insurance Service-Senior(2002-2013). We also analysed the factors which determine the prevalence of contract (ICD-10: H25) and age-related macular degeneration(ICD-10: H353) using Cox proportional hazard regression model Results: The subjects who were women, in older age, the group of higher income level, with hypertension, with heart disease, and with diabetes, the prevalence of both contract and age-related macular were increased(p<0.0001). Conclusion: The prevalence of contract and age-related macular degeneration were higher in old age of Korean who has chronic diseases such as hypertension, heart disease, and diabetes. The management of chronic diseases are essentially required in elderly for more healthy eye in aged society.

  • Based on the rapid development of the computer network technology, the ubiquitous sensor network (USN) was developed to enable us to have access to the communication environment anywhere and anytime without the need for recognizing computers or networks. Moreover, with the increasingly high interest on individual health, the USN technology is being applied to diverse sectors for healthcare and disease prevention. In this paper, a system was designed and implemented using the USN-based RF communication for doctors and nurses who care patients in the hospital to easily measure and control the physiological data on blood pressure and blood sugar. In addition, a database was designed using MS SQL database to store and manage the blood pressure and blood sugar data, which were passively or actively measured from patients. Using the results of this study, the physiological data of patients can be managed in real time and emergency situation can be instantly addressed. It is expected that the healthcare service can be improved and the paradigm of healthcare service environment can be changed.

Effects of Air Pollution on Asthma in Seoul: Comparisons across Subject Characteristics (서울지역 대기오염이 천식에 미치는 급성영향: 연구대상의 특성에 따른 비교)

  • Objectives: Korean epidemiological studies have used reduced samples according to the subject's characteristics, such as the health services provided, the historical note with asthma, and age, to examine the acute effect of air pollution on asthma using the Korean National Health Insurance records. However, there have been few studies on whether the effects shown in these reduced samples are different from those of all samples. This study compared the effects of air pollution on asthma attacks in three reduced samples with those of entire samples. Methods: The air pollution data for $PM_{10},\;CO,\;SO_2,\;NO_2,\;and\;O_3$, and weather conditions including temperature, relative humidity, and air pressure in Seoul, 2002, were obtained from outdoor monitoring stations in Seoul. The emergency hospital visits with an asthma attack in Seoul, 2002 were extracted from the Korean National Health Insurance records. From these, the reduced samples were created by health service, historical notes with asthma, and age. A case-crossover design was adopted and the acute effects of air pollution on asthma were estimated after adjusting for weather, time trend, and seasonality. The model was applied to each reduced sample and the entire sample. Results: With respect to the health service, the effects on outpatients were similar to those for the total sample but were different for inpatients. These similar effect sizes were also observed in the reduced samples according to the historical note with asthma and age. The relative risks of $PM_{10},\;CO,\;SO_2,\;NO_2,\;and\;O_3$, among the reduced and entire samples were 1.03, 1.04-1.05, 1.02-1.03, 1.04-1.06, and 1.10-1.17, respectively. Conclusions: There was no clear evidence to show a difference between the reduced samples and the entire samples.

Are Primary Health Care Workers Aware of Cervical Cancer Risk?

  • Background: We here examined the awareness of female health employees (doctors, nurses, midwives) working in primary health care service about cervical cancer and its risk factors. Additionally attitude and behavior for gynecologic examination and pap smear screening wwere researched. Materials and Methods: This cross-sectional, descriptive study con cerned female health employees working at primary health care services in two southern cities of Turkey, over a four month period in 2013. Participants were recently or previously sexually active research was explained and verbal informed consent was obtained face to face. The questionnaire consisted of two parts; socio-demographic characteristics and level of knowledge about cervical cancer and its risk factors. Results: The average age of the participants (midwives 43.7%, n=143; nurses 40.4%, n=132; doctors 6.4%, n=21; emergency medical technicians and others, 9.5%, n=31; total, n=327) was $30.9{\pm}6.41$ years. 64.2% (n=210) were working in Diyarbakir and 35.8% (n=117) in Batman. A large proportion reported low knowledge and inadequate screening practice Conclusions: Health employees should be better informed about the importance of screening for cancers, given their preventive roles for the general population.

Design and Implementation of Pulse Monitoring System for U-Healthcare (U-Healthcare 지원을 위한 맥박 정보 모니터링 시스템의 설계 및 구현)

  • U-Healthcare is one of the major applications in ubiquitous sensor network. U-Healthcare has potential to become a critical service for the people who immediately require emergency ambulatory attention. This paper describes about the real time pulse monitoring and reporting system, consisting of two components: thus, the one is a reliable bio-sensor that continuously monitors the pulse information of the subject, and the other is the automatic transfer system that transmits pulse information to both his/her family and hospital care system through the Base Station. In the hospital, this bio-information can be used to treat the patient accordingly. I designed the pulse information monitored by a bio-sensor module that transfers the pulse information to both the Base Station and the central monitoring system through transmitting protocols such as Zigbee and TCP/IP, as well as designed the architecture of information packets for the corresponding protocols. Furthermore, the central monitoring system automatically parses the pulse information of the subject into the web database server, which can continuously provides the real time information and status of the subject via an internet browser to the clients who are family members of the subject and the authenticated medical care personnel as well.

Behavior Patterns of Health Care Utilization in Terminal Cancer Patients (말기암 환자들의 의료이용행태)

  • Background : In order to improve the quality of life of dying patients, they need to receive not only the physical, psychological, social, and spiritual care, but also systematic and continuous care to die with dignity. However, no adequate medical services are available for these terminal cancer patients. We studied their behavior patterns of health care utilization to understand more of their medical and social needs. Methods : We investigated 108 bereaved families through the telephone interview with structured questionnaires. They were randomly selected through the retrospective chart review of the terminal patients who passed away due to cancer. Results : Most of the terminal cancer patients received their care from proper medical services including admission to hospital (45.4%), outpatient clinic (22.2%), emergency room (16.7%), and oriental medicine (12.0%). But during the terminal phase of their illness, 32.4% of patients never received medical care including oriental medicine, and 28.7% received alterative natural care. 26 bereaved families (24.1%) pointed out the indifference of medical staff as a problem receiving proper hospital care, and 22 (20.4%) emphasized emotional strain of their helplessness with the patients' suffering as a problem of caring at home. Over 90% suggested availability of continuous care, hospice care, home care, and 24 hour telephone service to be improved. Conclusions : Due to various reasons, adequate medical care is not delivered to the terminal cancer patients in our present medical system. These problems can be approached with the establishment of proper education and medical delivery system. The role of comprehensive medical specialty cannot be overly emphasized to accomplish this most effectively.

Case Study on the Building Organization of Medibio Research Laboratory Facilities in Research-driven Hospital (연구중심병원 의생명연구원의 실험실 구성 사례 조사)

  • Healthcare technology has been growing and fostering cooperation between industry, university and hospitals as growth engines in korea. So, the medibio research institutes in hospital have been constructed to promote research and industrialization centering on healthcare technology. The purpose of this study is to investigate the cases of research institutes in hospitals, and search the characteristics of building organization of medibio research laboratory facilities. Case study is investigated by floor plan, homepage and site visits about five research institutes selected in research-driven hospitals. The facility title and size of research laboratory is originated from site area and research building location. The building function include not only the research lab and business office reflecting on the development platform, and but assembly and meeting room in the ground level. Laboratory floor plans have three types, rectangular, rectangular+linear and linear type, one is traditional and efficient, the others are people and friendly. And building core types are correlated with lab space unit modules, single and double side core are shown in rectangular type. All the laboratories are open lab, composed with laboratory bench and research note writing desk facing the lab service and enclosed lab-support area. And they have communication space looking as warm and cozy common area for the innovation, convergence and collaboration. As the high risk of contamination and high standard for safety and security, equipment and facilities are well managed with biological environment including BSC, fume hood, PCR classification, eye washing and emergency shower.

Demonstration Project on Utilization of Telephone Consulting and Telemedicine System for Home Health Care of the Elderly (노인건강상담전화 운용과 가정간호사업 활성화를 위한 원격의료 시범사업)

  • Advanced countries such as the USA and Japan are eagerly seeking ways to improve health and welfare of the elderly. One of the services is home health care service using the telephone. Various types of services using the telephone have been developed, improved and are being utilized ranging from the basic consulting to emergency response systems in the area of health care for the elderly. A demonstration project was launched to study the feasibility of a consulting system and telemedicine for the elderly using the public phone system in Korea. For this project, a gathering site for the elderly was selected and those who visited this place were interviewed to find out what kinds of services they wanted and what kind of system they needed to provide the required services. Based on the users' requests and the surrounding environment, a telephone consulting facility was established at the Research Institute of Nursing Science at Seoul National University and consulting personnel was recruited, trained and posted at the center. An Application program for home health care nurses to use when they visited the patients at their homes was developed. This system operates on a notebook Computer and allows nurses to communicate with a doctor at a local hospital through a modem and telecommunication line. These systems were implemented for three months and problems which developed during operation of the systems were identified and progressively modified. Through system evaluation, it was found that a consulting system using phone service will be an invaluable system for the welfare of the elderly in the future. But in order to meet the elderly's need, more services than mere consultation are needed. That is, communication with physicians and hospitals are needed. Thus, when there is any need for physicians' attention, physicians or hospitals should be contacted directly. Similarly for telemedicine, when the home health care nurse visits elderly patients she can assess the patient's problem and provide nursing care, access a physician or hospital to refer her patient to or consult directly using the telecommunication the system. The above mentioned system is a basic form of futuristic telemedicine for the elderly and those who have chronic disease problems. This kind of system will be of great value when it is used on the national information super-highways in the future. In order to get to that stage, of course, this project needs great improvement in the technical, academic, and legal aspects.
