Effect of strengthening and elongation exercises of upper extremity muscle to forward head posture correction

  • This study was designed to provide basic data for developing exercise program that helps correcting posture by knowing the effect of strengthening and elongation exercises of upper extremity muscle to forward head posture correction. In this study determined subjects whether they had forward head posture or not. On the basis of the New York state posture rating, if a subject's posture is match up with the normal standard posture, gives 5 points and if the posture is slightly get out of the normal standard posture, gives 3 points and if the posture is apparently get out of the standard, gives 1 points. When determining the forward head posture, if talus, humerus and outer ear center are on the same line, it is determined as normal and if outer ear center is off the line less than 1.0cm, it is a slight deformation and if outer ear center is off the line more than 1.0cm, it is a high deformation. In the study selected people who have more than 1 cm gap between two vertical lines start from outer ear center and acromion separately as subjects. Length between the ideal alignment line measured by using goniometer and temporal region showed statistically significant decrease as $2.36{\pm}1.07cm$ before the intervention and $1.06{\pm}0.88cm$ after the intervention. After 4 weeks of neck and chest extensor muscle exercise, the group who exercised both showed increase in range of neck joint motion and neck flexion of the forward head posture. Meanwhile the group who only exercised neck extensor muscle only and the group who only exercised chest extensor muscle didn't showed statistically significant result. That only the group who exercised both muscles showed significant result is the different with studies before. Because this study didn't target patient who had a lesion, couldn't compare effect of the conservative manner and exercise. However, this study provides the fact that the group who exercised both neck and chest muscle had more effect than the control group.

  • The maintenance of skeletal muscle mass is very important for the prevention of life style-related diseases and the improvement of quality of life. It is well-known that resistance exercise and nutrition (especially amino acids) are the most effective interventions for maintaining skeletal muscle mass. It has been reported that many molecules are involved in the regulation of protein synthesis in response to resistance exercise and nutrition. Understanding the molecular mechanisms regulating muscle protein synthesis is crucial for the development of appropriate interventions. The role of intracellular signaling pathways through the mammalian target of rapamycin (mTOR), a serine/threonine protein kinase in the regulation of muscle protein synthesis, has been extensively investigated for these years. Control of protein synthesis by mTOR is mediated through phosphorylation of downstream targets that modulate translation initiation and elongation step. In contrast, upstream mediators regulating mTOR and protein synthesis in response to resistance exercise and amino acid still needed to be determined. In this brief review, we discuss the current progress of intracellular mechanisms for exercise- and amino acid-induced activation of mTOR pathways and protein synthesis in skeletal muscle.

The Comparison Study of Ankle Joint Mobilization and Elongation on the Difference of Weight-bearing Load, Low Back Pain and Flexibility in Flat-foots Subjects (편평족에 대한 관절가동술과 자가신장이 요부의 신장성, 요통과 하지 체중부하 차이에 대한 비교 연구)

  • Background: The purpose of this study was to investigate the relationship between the spine and the flat-foot, the stability and the alignment of the posture of the neck to prevent the alignment of the ankle joint operation and the lower back flexibility of the lumbar region according to the type of treatment using active stretching of the triceps, back pain, and to see how they affect weight bearing differences. Methods: The subjects of this study were 24 chronic low back pain patients. They were randomly divided into experimental group and control group. In the experimental group, ankle joint mobilization and active scraping of triceps were performed three times a week for a total of 6 weeks. The control group was performed in the same way without articulation. The range of flexion and extension motion of the lumbar spine and pain degree and difference of weight-bearing were measured before and after the experiment. Results: The model of ankle joint mobilization and calf muscle elongation of flat foot significantly improved the range of flexion and extension motion of the vertebrae (p<.05) and the VAS and distribution of weight-bearing were decreased in both of two groups (p<.05). In other words, the exercise and mobilization help to recover of the balance of the whole musculoskeletal, the vertebrae. Conclusions: The active exercise of the triceps muscle of the lower leg in this study It affects the flexibility of the lumbar spine, the pain and the difference in the weight support of the lower extremities, when we performed ankle joint mobilization for exercise and cramping, pain and the difference in weight support between the two lower limbs.

Design Suggestion of Active T-shirt According to the Exercise Types in the Silver Generation (실버세대의 운동유형에 따른 액티브 티셔츠 디자인 제안)

  • The silver generation have clothing style of optimal daily life comparing than young generation because they do not participate a specific sport event but daily- life exercise. As the human body ages, the figure of the silver generation shows different body shape because upper body changes to curved figure including the belly and waist part. Therefore, clothing characteristics for the silver generation should be considered with proper function, design and textiles to optimize body movement. This study investigated various exercise types according to motion analysis of the silver generation in order to develop the design of the active T-shirts reflecting the structural properties and providing the optimum exercise circumstance. The results to consider design needs are as followed; As the T-shirts design for the flexible exercise which required frequent movement of upper body such as bending and waist twisting during body stretching, a stretch fabric applied to the waist part considering T-shirts allowance and length to make extreme elongation and support for well-fitting appearance of the T-shirts. As the T-shirts design for the instantaneous reactionary exercise, high elastic four-way stretch fabric is applied to the part of arm hole to optimize skeletal and muscle movement for entire body and arm work. As the T-shirts design for the endurance exercise such as climbing, cycling, and walking, the shoulder line of the back part has cutting line allowance to make optimum movement of the upper body but no change of the waist part.

Effects of silkworm and its by-products on muscle mass and exercise performance in ICR mice.

  • In this study, we investigated the effect of silkworm, and its by-products on exercise endurance, muscle mass, and fatigue recovery using ICR mice model. Powders of silkworm, pupae, dongchunghacho, and silk powder were suspended in water and feed to mice for 2 weeks. The forced swimming time was increased by 4.3, 4.1 and 2.8 seconds after silkworm, pupae, and dongchughacho administration compared to DW fed group. Increases in exercise ability were achieved by difference mechanism according to feeding materials. Pupae increased muscle mass by 129% compared with the control group which may effect on elongation of swimming time. Dongchunghacho improved the plasma concentrations of fatigue markers such as creatinine, blood urea nitrogen (BUN), and D-lactate. Silkworm administration showed dual effect, the muscle mass induction (114% vs. control) and anti-fatigue (plasma creatinine, BUN, and D-lactate were 63, 75, and 78% vs. con) effect which contributed most elongated swimming time. In conclusion, silkworm and its by-products including pupae and dongchunghacho with the predominant protein and bioactive components improved muscle mass and showed anti-fatigue effect which could promote exercise performance ability.

Changes of Elastic Properties in In Vivo Human Tibialis Anterior Aponeurosis Following Maximum Eccentric Exercise (최대 신장성 수축 운동 후 인체 족배굴곡근 건막의 탄성 변화)

  • The purpose of this study was to investigate changes in elastic properties of tendon structure of human ankle dorsiflexor following eccentric exercise. Six male subjects(age: $27.3{\pm}2.0$ years, height: $180.3{\pm}1.4$ cm, weight: $82.6{\pm}5.3$ kg) and three female subjects(age: $26.7{\pm}2.9$ years, height: $170.0{\pm}4.2$ cm, weight: $66.6{\pm}1.4$ kg) performed a single bout eccentric exercise consisting of 120 repetitions of maximum eccentric contractions. Prior to and following the eccentric exercise, isometric ankle dorsiflexion strength along with longitudinal ultrasound image of the tibialis anterior(TA) were collected. Muscle strength decreased about 30% after eccentric exercise. From the muscle strength vs. aponeurosis elongation curve, we obtained an index of stiffness. Stiffness of deep aponeurosis of the TA was assessed and found to be decreased from $87.4{\pm}33.56$ N/mm to $73.1{\pm}23.52$ N/mm. The results of this study suggest that decrease in stiffness of the TA aponeurosis following eccentric exercise might have significant implications to functions of the muscle-tendon complex and the involved joint motion and provide better understanding of eccentric exercise in the fields of training and rehabilitation.

High Fat Diet or Exercise Training Alters Hepatic Total and Phospholipid fatty Acid Compositions in Rats (고지방식이 및 운동훈련이 흰쥐 간조직의 총지방산 및 인지질 지방산패턴에 미치는 영향)

  • Effects of high fat diet and/or endurance exercise training on hepatic total and phospholipid(PL) fatty acid compositions were evaluated in rats fed one of the following diets for 31 days. control diet(CD, 5 wt% corn oil) or high fat diet(HFD, 35 wt% corn oil). Half of the rats in each group were exercise-trained regularly on a treadmill for 90 minutes/day during the entire feeding period. Total and PL fatty acid compositions of hepatic lipid extracts were determined by a gas-liquid chromatograph),. Endurance exercise training did not change the daily food intake, but significantly reduced body weight gain and feed efficiency ratio of rats, which were most prominent in animals fed HFD. Exercise training did not significantly change the percentages of ∑saturated fatty acids (SFA) and ∑polyunsaturated fatty acids(PUEA), but decreased the percentage of ∑monounsaturated fatty acids(MUFA) in hepatic total fatty acids, which might be associated with the decrease in (equation omitted) 9-desaturation index of hepatic total fatty acid metabolism. Exercise training significantly lowered the percentages of 16 : 0 and 22 : 5$\omega$3, and increased the percentages of 20 : 1 and 20 : 3$\omega$3 in both total and PL fatty acid compositions in rat liver. Both total fatty acid and PL fatty acid compositions of rat liver responded more sensitively to changes in dietary fat content than to endurance exercise training in this study. Feeding HFD, whoch contains high level of linoleic acid(LA, 18 : 2$\omega$6), significantly decreased the percentages of ∑SFA and $\Sigma$MUFA, and increased the percentages of ∑PUFA and ∑$\omega$6 fatty acids of hepatic total fatty acids. Hepatic total fatty acid composition was affected by dietary fat content and dietary fatty acid composition more sensitively than those found in hepatic PL fatty acid composition. HFD significantly decreased most of desaturation indices, while exercise training significantly decreased elongation index(20 : 5$\omega$3⇒22 : 5$\omega$3) of hepatic total and PL fatty acid metabolism in rats. The Effect of Exercise Program on Chronic Low Back Pain in Female Teachers of Elementary School (만성요통 여교사에 대한 운동프로그램의 효과 - 근력, 근지구력, 유연성, 통증, 기능장애, 우울 및 생활만족도를 중심으로 -)

  • This study was performed to probe the effect of exercise program on muscle strength, endurance, flexibility, pain, disability level and life satisfaction in female teachers of elementary school who complain of low back pain. For this study, 44 female teachers aged 30-50 years with mechanical low back pain of 6 months' duration, who had the structural normalities in the lumbar spine, were recruited from April 1 to July 10 1999. Twenty three out of them were assigned to the experimental group and twenty one to the control group. The exercise program consisted of education on right postures, the etiology and diagnosis of low back pain, and exercise intervention such as muscle relaxation, elongation and strengthening. With 8 weeks program, the subjects received two sessions of education and six sessions of group exercise in the 1st week, while three sessions of group exercise and four sessions of individual exercise weekly and two sessions of education during the later 7 weeks. The muscle strength and endurance were measured by Cybex 770, the flexibility by flexibility measurement machine, the intensity of pain by Visual Analogue Scale (VAS), the level of disability by Oswestry low back pain disability scale, depression by Beck depression inventory (BDI), and life satisfaction by Life satisfaction index-Z. Study measurements were taken before and after 8 weeks exercise program. Data were analyzed using paired t-test, t-test, and ANCOVA. The results were as follows ; 1. The flexors and extensors peak torque and flexors peak torque per body weight of experimental group were significantly increased at test velocities $30^{\circ}$/sec, $60^{\circ}$/sec compared with those of control group. There was no significant difference in extensors peak torque per flexors peak torque at $30^{\circ}/sec$, $60^{\circ}/sec$ between experimental and control group. 2. The flexors and extensors total work and flexors total work per body weight of experimental group were significantly increased at $120^{\circ}/sec$, compared with those of control group. 3.The flexibility of lumbar spine in experimental group was significantly increased compared with that of control group. The pains in anterior, posterior, left lateral and right lateral bending and in rotation of experimental group were significantly increased compared with those of control group. 4. The Oswestry disability scores of experimental and control group were significantly decreased, and there was no difference in the Oswestry disability score change between experimental and control group. 5. The scores of BDI of experimental group were significantly decreased compared with those of control group. Life satisfaction index-Z scores of experimental group were not changed, but those of control group were significantly decreased. There was no difference in the score change of Life satisfaction index-Z between experimental and control group. 6. ANCOVA analysis for the data variables of inhomogeneous baseline represented that there was no significant difference in extensors peak torque and extensors total work at $120^{\circ}/sec$ and extensor total work per body weight at $120^{\circ}/sec$ change between experimental group and control group. These findings indicate that the exercise program could be effective in increasing the muscle strength, endurance, flexibility and decreasing pain, improving depression in female teachers of elementary school with chronic low back pain. It is suggested that the exercise program could be an essential factor for the effective nursing intervention to the patients suffered from chronic low back pain.

Development of Hospice Model Networked with the Existing Welfare Systems in Gimhae City (김해지역 호스피스 보건복지연계 모형개발에 관한 연구)

  • Purpose : With the elongation of the average life span, the main causes of death are chronicle illness including cancers resulting in a large amount of medical resources. And there are still many patients to whom a sufficient medical care is not given. All these bring on the uneven distribution of medical resources together with the increase of medical cost. Hence, an efficient system should be set up to solve these problems. Methods : The hospice draws a great attention as a resolution of high medical cost and uneven medical resource distribution, and has been proved to be an alternative to the existing medical system. So, the characteristics of the hospice has been reviewed, particularly with respect to its scopes and related resources. And by tracking the actual cases, the necessary services and supports are investigated. Results : The intrinsic characteristics of hospice is that it executes not only the medical exercise but also all the subjects related with patients and their families. And the hospice is performed not only by the medical experts but also by all the disciplines including social and spiritual affairs. This indicates that the hospice requires the integrated system comprised of medical, social and other welfare entities. Conclusion : To establish the actual hospice, an efficient and systematic integration of all the existing medical and other welfare resources in the local society is necessary. The most practical way is the networking of resources, which practices the hospice more efficiently without additional investment.

Dual Mobility Cup for Revision of Dislocation of a Hip Prosthesis in a Dog with Chronic Hip Dislocation

  • A 6-year-old, 36.5 kg castrated male Golden Retriever presented for revision surgery for left total hip replacement. The patient underwent removal of the cup and head implants due to unmanageable prosthetic hip dislocation, despite revision surgery. On physical examination, the dog showed persistent weight-bearing lameness after exercise of the left hindlimb with mild muscle atrophy. Radiographic examination revealed dorsolateral displacement of the femur with a remnant stem and bony proliferation around the cranial and caudal acetabulum rims. The surgical plan was to apply the dual mobility cup to increase the range of motion and jump distance to correct soft tissue elongation and laxity caused by a prolonged period of craniodorsal dislocation of the femur. The preparation of the acetabulum for cup fixation was performed with a 29-mm reamer, and the 29.5-mm outer shell was fixed with five 2.4-mm cortical screws. The head and medium neck of the dual-mobility system were placed on the cup, and the hip joint was reduced between the neck and stem. The dog exhibited slight weight bearing on a controlled leash walk the day after surgery. The patient was discharged 2 weeks postoperatively without any complications. Six months postoperatively, osseointegration and a well-positioned cup implant were observed, and the dog showed excellent limb function without hip dislocation until 18 months of phone call follow-up.
