The Surgical Impact of E-Cigarettes: A Case Report and Review of the Current Literature

  • We report a case of a 51 years old female with a 25 pack year smoking history who underwent bilateral mastectomy and immediate tissue expander reconstruction for newly diagnosed right breast cancer. The patient reported herself as a non-smoker despite significant e-cigarette use, with resulting significant mastectomy skin flap necrosis and breast reconstruction failure. Little is known about the physiologic effect of e-cigarettes on wound healing and tissue perfusion. To this end, we provide an updated review of the impact of e-cigarettes on surgical outcomes. PubMed, Ovid MEDLINE, and PRS GO were searched for the terms "e-cigarette", "electronic cigarette", "e-cig", "electronic nicotine delivery system", "vaping", "surgery", "surgical", "peri-operative", "operate", "operative", and "wound healing". Abstract review of all articles was performed. 123 articles returned that contained both variants of e-cigarettes and surgery as keywords. Of those, manual assessment returned three articles which were found to be relevant to e-cigarette use in the surgical patient. No articles were found that compared perioperative complications in e-cigarette versus traditional cigarette users in humans. In conclusion, our case report depicts the potential dangers associated with e-cigarette use in the surgical patient. There is a public misconception that e-cigarettes are healthier than traditional cigarettes and as such their use may go unreported by patients. Early evidence suggests e-cigarettes may induce some of the same physiologic changes as traditional cigarettes, and may have a significant deleterious effect on wound healing.

Inhallation of e-Cigarette Cartridge Solution Aggravates Allergen-induced Airway Inflammation and Hyper-responsiveness in Mice

  • Electronic cigarettes (e-cigarettes) are becoming increasingly popular worldwide and their cellular effects warrant further evaluation. In this study, we investigated the effects of an e-cigarette cartridge solution on allergen related asthmatic airway inflammation (AI) and airway hyperresponsiveness (AHR), when it is delivered by intratracheal route in mice. Asthmatic AI and AHR were induced by systemic sensitization to ovalbumin (OVA) followed by intratracheal, intraperitoneal, and aerosol allergen challenges in BALB/c mice. The cartridge solution of e-cigarette (containing 16 mg/ml nicotine) was diluted 50 times and $100{\mu}l$ of the diluted solution was intratracheally instilled to OVA-sensitized (OVA-S) mice two times a week for 10 weeks. Long-term e-cigarette inhalation elicited no remarkable changes in the activities of alanine aminotransferase (ALT), aspartate aminotransferase (AST), lactate dehydrogenase enzymes in serum, however, increased infiltration of inflammatory cells including eosinophils, into airways from blood, aggravated the asthmatic AI and AHR, and stimulated the production of cytokines such as interleukin (IL)-4, IL-5 and IL-13, and OVA-specific IgE production. Our data suggest that the inhalation of e-cigarette solutions can function as an important factor to exacerbate the allergy-induced asthma symptoms. Further studies are needed to address the effects of e-cigarette solutions on human health.

한국 성인의 전자담배 사용 관련요인 (Factors associated with Electronic Cigarettes Use in Korean Adults)

  • 이 연구는 한국 성인의 전자담배 사용과 관련된 요소들을 조사하기 위해 수행되었다. 카이제곱 검정, 로지스틱회귀분석을 통해2015년 한국질병통제센터에서 실시한 지역건강조사228,477건을 분석하였다. 전자담배평생 유병율이 남성 11.1%, 여성 0.8%로 나타났다. 현재 전자담배 흡연율은 남성이 24.2%, 여성이 26.1%로 였다. 평생전자담배 흡연경험율은 연령 교육수준, 직업, 소득상태 및 현재흡연상태와 각각 유의미하게 관련되어 있었다. 현재의 보편적인 전자담배 흡연이 현 흡연상태와 유의한 관련이 있는 것은 과거에 흡연했던 그룹보다 현재흡연자 그룹이 현저히 높은 경험을 보이고 있는 것이다. 전자담배의 선호는 높았다. 전자담배는 금연을 목적으로 하는 이들이 전자담배가 일반담배에 비해 금연 효과가 있는 것처럼 인식하고 있었고, 흡연에서 오는 쾌감이나 중독성을 만족시켜 주었다. 따라서 전자담배는 흡연에 대한 딜레마에서 벗어날 수 있는 해결책으로 반영되어 선호도가 높았다. 2014년 전자담배 수입은 전년도 대비 약 3배 증가했고 세계적으로도 전자담배에 대한 관심이 증가하고 있다. 우리는 전자담배의 예방과 중단에 관련된 특징을 고려해야 한다. 흡연자들이 잘못된 흡연관련 건강인식을 개선하기 위한 금연교육 강화와 금연 위해성에 대한 적극적인 홍보가 필요하다. WHO에서도 전자담배의 안전성을 담보하거나 효과가 있다고 인정한 근거가 부족하다고 권고하고 있다. 전자 담배의 사용 동기에 있어서 중요한 환경적인 요인들을 확인하고 전자담배인식과 사용에 대한 연관성을 분석하는 연구들을 모색해야 한다.

대학생 중복흡연이 우울에 미치는 영향 (The Effects of College Students' Dual Use of Electronic Cigarette and Conventional Cigarette on Depression)

  • Purpose: This study was conducted to find out the association between dual-use of the electronic cigarette (EC) and conventional cigarette (CC) and depression among undergraduate students. Methods: The participants of this study were 225 undergraduate students who were attending two universities located in Ulsan metropolitan city. Descriptive statistics and multiple regression were conducted to analyze the data. Results: Nicotine dependence and depression were significantly higher in the dual-use group than in the single-use group. Dual-use of the EC and CC was significantly associated with depression among undergraduate students. In addition, quality of sleep was significantly associated with depression among undergraduate students. Conclusion: In order to prevent depression among undergraduate students, it is necessary to improve the awareness of the danger of dual use of the EC and CC.

청소년의 금지 약물 사용에 따른 관련 요인 분석 (Analysis of Related Factors According to Using Illicit Substances Among Adolescents)

  • Objectives: The purpose of this study was to analyze related factors in relation to usage of illicit substances for Korean adolescents. Methods: This was a cross-sectional study, which used data from the Twelfth Korean Youth Risk Behavior Web-based Survey (KYRBWS). A total of 65,528 students in 2016 were included in this study. Chi-square test and logistic regression analysis were used for analyzing related factors using illicit substances. Results: Overall, 553 (0.8%) students responded as having ever used illicit substances during their lifetime and 133 (0.2%) students responded as current illicit substance users. It was found that electronic cigarette (EC) use, conventional cigarette smoking, second hand smoking, sad/despair mood, and economic status were significantly associated factors with usage of illicit substances. Comparing 'current EC users' with 'never EC users', the adjusted odds ratios (OR) was 7.76 (95% confidence interval (CI): 5.56-10.82) for life-time use of illicit substances, and 18.74 (95% CI: 8.01-43.87) for current use of illicit substances among adolescents. Additionally, comparing 'students who ever were exposed to second hand smoking during the past week', with 'students who never were exposed to second hand smoking', the adjusted OR was 2.60 (95% CI: 2.13-3.17) for life-time use of illicit substances, and 2.85 (95% CI: 1.68-4.83) for current use of illicit substances among adolescents. Conclusions: Based on the results, electronic cigarette use, second hand smoking, conventional cigarette smoking, sad/despair mood, and economic status were significantly associated with increased odds of substance experience and current substance use. Therefore, the results of this study can provide a useful evidence about adolescent behaviors in predicting substance use.

한국 청소년들의 성별에 따른 전자담배사용과 정신건강 (Electronic cigarette use and mental health among Korean adolescents by gender)

  • 목적: 본 연구의 목적은 청소년들의 전자담배 사용경험과 스트레스, 자살생각, 범불안장애와 같은 정신건강과의 관계를 알아보기 위함이다. 방법: 제19차(2023년) 청소년 건강행태조사를 바탕으로 교차분석과 복합설계 로지스틱 회귀분석을 통해 전자담배사용과 정신 건강과의 관계를 규명하였다. 결과: 남자청소년의 경우 전자담배 평생 사용경험(OR:1.218)과 현재 전자담배 사용경험(OR:1.266), 여자청소년의 경우 전자담배 평생 사용경험(OR:1.587)이 스트레스에 영향을 미치는 요인이었다. 남자청소년의 전자담배 평생 경험(OR:1.866)과 현재 사용(OR:1.651)이 자살생각에 영향을 미쳤으며, 여자청소년의 경우 전자담배 평생 경험(OR:2.375과 전자담배 현재 사용(OR:1.841)이 자살생각에 영향을 미치는 요인으로 나타났다. 또한 남자청소년의 전자담배 평생경험(OR:1.307)과 여자청소년의 전자담배 평생경험(OR:1.616)은 범불안장애에 영향을 미치는 요인으로 나타났다. 결론: 청소년의 전자담배 사용과 정신건강 사이에는 유의한 관계가 있기에 청소년기 초기에 전자담배사용 예방 교육이 필요하다.

  • 세계적으로 전자담배 사용의 급속한 증가와 함께 미국을 중심으로 전자담배와 관련된 사망보고들이 발생함에 따라 전자담배 관련 급성 폐 손상은 중요한 사회적 문제로 대두되었다. 이 증례 보고는 급성 호흡기 증상과 전자담배 흡연력이 있는 24세 남자 환자에서 전자담배 관련 급성 폐 손상을 진단한 국내 첫 사례이다. 흉부 CT에서 흉막하부 보존을 보이는 미만성간유리음영과 폐경화 및 중심소엽성 미세결절의 소견을 보여 전자담배 관련 폐 손상의 특징적 형태인 기질화폐렴과 미만성폐포손상에 해당하는 증례로 생각되었고, 검사실 소견에서 감염이 배제되고, 환자의 질병을 설명할 다른 원인이 없었다. 따라서, 임상 소견 및 부합하는 영상 소견으로 진단 기준을 충족하여, 다학제 진료를 통해 전자담배 관련 급성 폐 손상을 진단하였다. 이를 통해 영상의학과 의사들이 전자담배 관련 급성 폐 손상의 진단에 중요한 역할을 할 수 있을 것으로 생각된다.

최근 한 달 이내 전자담배 흡연율 및 관련요인 (Smoking rate of Electronic Cigarettes and its related factors within the last one month)

  • 이 연구는 19세 이상 성인의 최근 한 달 이내 전자담배 흡연율과 인구사회학적 요인과 건강수준 및 건강행태 요인 측면의 관련요인을 파악하여, 전자담배 흡연율 감소를 위한 금연교육과 보건정책 수립의 근거를 생성하기 위해 시도되었다. 2018년 서울시 지역사회 건강조사에 참여한 19세 이상 22,908명의 설문자료를 이용한 이차자료 분석 연구이며, 분석은 SAS 프로그램을 이용하였다. 최근 한 달 이내 전자담배 흡연율은 4.5%이었으며, 다변량 로지스틱 회귀분석에서 전자담배 흡연 관련요인은 성, 연령, 교육수준, 직업, 일반담배 흡연상태이었다. 전자담배 흡연을 예방하고 흡연율을 낮추기 위하여 우선적으로 대졸이상의 학력을 가진 전문직을 대상으로 전자담배의 위험성을 알리는 교육을 실시하여야 한다. 또한 일반 국민을 대상으로 전자담배 흡연도 일반담배 흡연과 차이가 없음으로 알리는 홍보와 캠페인을 시행하고 나아가 전자담배를 일반담배와 동일하게 취급하는 동일한 금연정책의 전개가 요구된다.

액상형 전자담배 용액에 의한 급성 니코틴 중독: 증례보고의 체계적 고찰 (Acute Nicotine Poisoning due to Electronic Cigarette Liquid: Systematic Review of Case Reports)

  • Purpose: Acute nicotine poisoning by liquid nicotine in electronic cigarettes is becoming an increasing problem worldwide. The current systematic review aimed to determine the harm of acute nicotine poisoning by reviewing published case reports. Methods: An online literature search with PubMed, Embase, Cochrane Library, and KoreaMed database was performed to identify relevant studies addressing acute nicotine poisoning with electronic cigarettes. Two investigators searched the case reports written in English or Korean. Results: Twenty-six cases were included in this study. The routes of intoxication included ingestion in 18 cases, intravenous injection in three cases, subcutaneous injection in two cases, and ocular exposure in two cases. Ten cases had a cardiac arrest, and seven of them died. Seven out of 12 cases with intentional poisoning had a cardiac arrest. Nine children under 18 years were reported, and three of them had a cardiac arrest. Sixteen cases without a cardiac arrest recovered well, except for one case with sudden sensorineural hearing loss. Conclusion: The authors reviewed the risks of electronic cigarette liquid in terms of acute poisoning through a systematic review. The nicotine solution of an e-cigarette can be life-threatening in cases of acute poisoning. Therefore, active emergency treatment with early recognition is necessary. In addition, various management methods and regulations for preventing acute nicotine poisoning, such as restriction of distribution and nicotine concentration, should be considered.

흡연 대학생의 전자담배에 대한 인식과 전자담배 사용 영향 요인 (Electronic cigarettes recognition and influence factors of electronic cigarettes of among smoking university)

  • Objectives: This study is purpose to recognition the of smoking behavior and the electronic cigarette of university students have a current smoking. Methods: The subjects were university living in Busan, the survey was conducted from March 23 to April 12, 2015, 314 except for 24 copies of non-response and error response among a total of 340 questionnaires were analyzed. Statistical analysis methods used in this study are $x^2$-test, Linear Regression Analysis and other basic statistics such frequency, percentage using SPSS version 22.0. Results: Electronic cigarette has been analyzed to be recognized non smoking, smoking reduces, good health than conventional cigarettes, convenient to use, there is no smell, can smoke in any place and as safe. Gender, grade, non smoking experience, non smoking and accept factors were analyzed to influence the use of electronic cigarettes. Conclusions: We suggest an established institutional arrangements and regulations, take advantage of various health programs development, and ongoing health education.
