신뢰할 수 있는 전자의무기록에 관한 연구 (A Study on Reliable Electronic Medical Record Systems)

  • 기존의 EMR 방식은 병원 내에 서버를 두고 있어 환자의 개인정보들이 병원관계자나 악의적인 목적을 가진 사람들에게 쉽게 노출되었다. 그리고 이외에도 환자의 의료기록들이 병원 내에 저장되어 있어 의료사고가 발생하더라도 병원관계자들이 수정할 여지가 있다. 이러한 정보 노출 문제점을 해결하기 위해 안전한 전자의무기록을 제안한다. 제안한 전자의무기록은 의료과실이 일어났을 때 중요한 정보를 제공함으로서 신뢰할 수 있는 정보로 이용될 수 있다. 그리고 제안한 시스템은 안전하고 효율적으로 환자를 인증하고 환자 개인의 의료정보를 보호할 수 있으므로 보다 높은 보안성을 제공할 수 있다.

  • 인터넷의 발달로 개인정보의 수집 및 이용이 일상화됨에 따라 개인정보의 침해가 급속도로 확대 되고 있다. 의료분야에 대한 개인정보보호에 대해서는 '정보통신망이용촉진 및 정보보호 등에 관한 법률'등에 체계적으로 규정되어 있으나, 법적용 대상이 정보통신 서비스 제공자 위주로 규정되어 의료분야에 적용하는 데 한계가 있다. 때문에 본 논문에서는 국내 의료기관이 전자의무기록 시스템에 보안을 적용하기 위해 우선적으로 선행되어야 할 개인의료정보 보호방안에 대해 정의하고, 적용근거를 위한 법 제도의 검토사항을 제시하였다. 또한, 전자의무기록에 대한 전자서명의 구체적인 적용방안을 예시하여 의료분야에 있어서 보안적용을 위한 기준을 제시하였다.

  • The rapid development and distribution of information communication industry facilitates the changes of hospital administration, introducing EMR(Electronic Medical Record) instead of paper-based medical record in the medical field. The developed countries such as U.S. have established EMR system after in the middle of 1970s because the primary advantages of EMR is to store and handle vast amounts of records efficiently and increase the quality of health care. Most of health organizations in Korea also apply medical record system to their administration. As the result, they have accomplished a scientific administration system through the use of medical record to handle a variety of patient's information including patient's confidentiality and privacy such as family history, social status, income level, and so on. However, access to and the misuse of EMR causes illegal infringement of patient's information and finally it becomes a very serious medical issue. Potential leakage and misuse of records may seriously infringe patient's privacy rights. In this respect, the related agencies in the public and private sector have been making efforts to prevent patient's records leakages. Especially, the revision bill of Medical Law in 2002 establishes the ways on the security and standards of electronic records. However, it does not provide the proper guidelines which is applied to the rapid changes of the medical environment. One of the most priorities in the hospital administration is the production and maintenance of an accurate medical records fulfilled by medical recorders. Therefore, it is very important for health care providers to hire ethical-based medical recorders. But, unfortunately most of hospitals overlook the importance of their roles. All parts including government, physician and patient must have more concerns on the problems related to EMR. Therefore, this study aims to propose the proper ways to resolve the problems coming from EMR.

  • 개인정보를 포함한 다양한 데이터를 활용하기 위해서는 정보주체의 권리를 보장하며 데이터 활용을 할 수 있는 모델이 필요하다. 정보주체를 고려하는 개인정보 활용 패러다임의 변화는 마이데이터를 기반으로 하는 다양한 데이터 활용 모델을 만들어내고 있으며, 다양한 개인정보가 생겨나고 다뤄지는 의료분야에서도 이러한 움직임이 진행되고 있다. 이번 논문에서는 마이데이터 기반 개인 건강/의료 데이터 활용 생태계가 확산되었을 때, 생겨날 수 있는 이해관계자들 간의 데이터 거래모델을 제안하였고 다양한 실험 결과를 기반으로 한 수학적 모델링을 통해 정보주체와 데이터 활용자 모두의 요구사항을 만족할 수 있는 개인 건강/의료 데이터 거래모델이 성립할 수 있음을 보였다.

공유 전자의무기록을 위한 MFER과 HL7 표준 적용 (Adoption of MFER and HL7 Standard for Shared Electronic Medical Record)

  • Medical environments incorporate complex and integrated data networks to transfer vast amounts of patient information, such as images, waveforms, and other digital data. To assure interoperability of images, waveforms and patient data, health level seven(HL7) was developed as an international standard to facilitate the communication and storage of medical data. We also adopted medical waveform description format encoding rule(MFER) standard for encoding waveform biosignal such as ECG, EEG and so on. And, the study converted a broad domain of clinical data on patients, including MFER, into a HL7 message, and saved them in a clinical database in hospital. According to results obtained in the test environment, it was possible to acquire the same HL7 message and biosignal data as ones acquired during transmission. Through this study, we might conclude that the proposed system can be a promising model for electronic medical record system in u-healthcare environment.

  • The Electronic Medical Record (EMR) is a valuable source of medical data intelligence in e-health systems. The watermarking techniques have been used to authenticate the owner and protect the EMR from illegal copying. The existing distributive strategies, successfully operated to secure the EMR, are found to be inadequate. Blockchain technology, mainly, is employed by a sharing database that allows the digital crypto-currency. It rapidly leads to the magnified expectations acme. In this excitement, the use of consortium adopting the technology based on Blockchain, in the EMR structure, is found improving. This type of consortium adds an immutable share with a translucent record of the entire business and it is accomplished with responsibility, along with faith and transparency. The combination of watermarking and Blockchain technology provides a singular chance to promote a secured, trustworthy electronic documents administration to share with the e-records system. The authors, in this article, present their views on consortium Blockchain technology which is incorporated in the EMR system. The ledger, used for the distribution of the block structure, has team healthcare models based on dissimilar multiple image watermarking techniques.

Study on the Categorical Structure Standardization for Representation of 3D Human Body Position System

  • This study presents the categorical structure for ther epresentation of a 3D human body position system in the WD stage after NP approval by the International Organization for Standardization (ISO), analyzes the needs of electronic medical record users and establishes future implementation plans for expanding its use in Korea. Research was conducted on the needs of doctors, nurses, health and medical information managers, and radiology departments, which are the main stakeholders of electronic medical records. The overall requirements for electronic medical records were derived from the results, and the requirements for each stage of use of electronic medical records were analyzed. Based on the results of the study, the study proposes plans to expand the use of the categorical structure for the representation of the 3D human body position system, and also aims to establish a standard system for health and medical terminology in Korea and contribute to the development of health and medical information standards through international standardization.

  • 문제: 전자의무기록(EMR) 시행 후 의무기록 정리율의 저하와 질적인 측면에서의 충실성과 정확성에 대한 문제점이 제기되었다. 목적: 전자의무기록의 정리율과 충실성 검토를 통하여 문제점을 파악하고 개선점 찾아 의무기록 정리율을 향상시키고 충실성을 높이고자 하였다. 의료기관: 서울시에 소재한 대학병원 의무기록과 질 향상 활동: 전자의무기록의 문제점을 개선하기 위하여 사용자 편의를 위한 EMR 프로그램 수정 및 보완, 진단 수술 관련 작업, 업무개선, 교육, 홍보 등의 활동을 실시하였다. 개선효과: 의무기록 정리율, 전자인증미비, 경과기록 기재일수, 퇴원요약 주진단 적합률, 기록지별 필수항목 기재율, 충실성에서 향상이 이루어졌다.

  • EMR(Electronic Medical Record)이란 종이문서 방식을 사용하는 의료차트 대신에 모니터로 출력되는 형식의 의료차트를 말한다. 현재 통용되는 EMR은 전문 의료인이 전문적인 용어 써놓은 양식이기 때문에, 그 의료기록의 실제 주인인 환자는 의료인의 부가적인 설명 없이는 자신의 자세한 증세는 물론 병명조차 정확히 확인하기 힘들다. 이에 환자에게 있어 매우 시간 소모적이고 노동집약적인 과정으로 환자가 의료인을 거쳐서 자신의 의료정보를 얻는 방식보다는 환자가 직접 그 정보를 얻는 방식이 필요하다. 따라서 본 연구는 EMR이 갖는 이러한 문제를 해결하기 위해 환자가 의료인을 거치지 않고 직접 자신의 의료정보를 얻기 위한 환자 접근행 EMR 응용 시스템의 비즈니스 모델을 제시한다.

  • In recent years, two major streams in medical information systems are:1) system integration among OCS(Order Communication System), EMR(Electronic Medical Record), PACS(Picture Archiving and Communication System), and ERP(Enterprise Resource Planning) and 2) system integration through medical collaboration between East and West medical service providers. One of the characteristics which differentiate the Korean medical industry from the western medical industry is the East-West medical collaboration. In many respects there are many differences between East and West medical treatment. Although East and West medical treatment have developed from different medical philosophies and standards, we assume that the better medical care can be provided by integrating their medical procedures effectively. The two possible approaches to the integration of East and West medical information systems are suggested in this paper:One is loosely coupled model and the other is tightly coupled model. EMR improves the quality of medical record which reflects the quality of clinical practice. It provides more efficient and convenient way of input, retrieval, storage, communication and management of medical data. We abstracted the standard medical procedures from the two medical procedures performed in Daejeon Oriental Hospital and Hehwa Clinic at Daejeon University and also abstracted database schema by analyzing the characteristics of information needed in East-West medical collaboration. Our EMR is composed of two types of data:one is structured data and the other is unstructured data, which are formalized by SOAP(Subjective, Objective, Assessment, Plan) format. Currently the integrated system is implemented and operated successfully for six months.
