전자의무기록의 충실성 검토를 통한 미비기록 개선 활동 (Improvement Activity for Promotion of Incomplete Medical Record through the Review of Electronic Medical Record Completeness)

  • 문제: 전자의무기록(EMR) 시행 후 의무기록 정리율의 저하와 질적인 측면에서의 충실성과 정확성에 대한 문제점이 제기되었다. 목적: 전자의무기록의 정리율과 충실성 검토를 통하여 문제점을 파악하고 개선점 찾아 의무기록 정리율을 향상시키고 충실성을 높이고자 하였다. 의료기관: 서울시에 소재한 대학병원 의무기록과 질 향상 활동: 전자의무기록의 문제점을 개선하기 위하여 사용자 편의를 위한 EMR 프로그램 수정 및 보완, 진단 수술 관련 작업, 업무개선, 교육, 홍보 등의 활동을 실시하였다. 개선효과: 의무기록 정리율, 전자인증미비, 경과기록 기재일수, 퇴원요약 주진단 적합률, 기록지별 필수항목 기재율, 충실성에서 향상이 이루어졌다.

  • The development of the internet and information management has enabled new applications which include: Electronic medical record (EMR), intelligent transportation, environmental monitoring, etc. In this paper, we design and implement the Continuity Care Record(CCR) Data Stream management server that compiled with DSMS and DBMS in EMR system for processing, monitoring the incoming CCR data stream and storing the processed result with high-efficiency. The proposed system enables users not only to query stored CCR information from DBMS, but also enables to execute continue query for the real-time CCR Data Stream. By using of CCR Viewer Application users can view or update their personal health records even compare self health care records with standard health care records in order to monitor the healthy status, and the on line updating information would be minimized and medical error.

  • ICT 힐링 플랫폼은 생체신호 및 생활습관 등의 정보를 기반으로 한 질환조기 경보를 목표로 하는 개념으로서 만성질환 예방을 목적으로 한다. ICT(Information & Communication Technology) 힐링플랫폼의 목표는 개인이 주도하는 건강관리를 위해 여러 건강 관련 서비스 기관들(병원, 피트니스센터, 건강검진센터, 개인건강장치 등)에 산재되어 있는 개인 건강 정보를 개방화하여 개인 단말장치로 모으고, 분석 플랫폼 및 Open API를 제공하여 다양한 부가 서비스들을 활성화하는데 있다. 본 논문에서 제안하는 개인화된 힐링 데이터 접근을 위한 개방형 게이트웨이 프레임워크인 HPAdapter (Healing Platform Adaptor)는 ICT 힐링플랫폼의 데이터 중계를 위해 EMR(Electronic health record), 한방의학, 라이프로그, 웰니스, 만성질활, 피트니스 등 다양한 개인 건강관련 데이터 공급자와 서비스 공급자 사이를 연결해 개인화된 힐링 데이터를 중계하는 소프트웨어 엔진을 뜻한다. 설계된 HPAdaptor는 데이터 및 서비스 공급자 연동을 통해 힐링 레코드 저장소, 모바일 플랫폼 및 분석 플랫폼 등 데이터를 필요로 하는 서비스 혹은 플랫폼의 중계참조 모델로 활용할 수 있다.

  • 인터넷의 발달로 개인정보의 수집 및 이용이 일상화됨에 따라 개인정보의 침해가 급속도로 확대 되고 있다. 의료분야에 대한 개인정보보호에 대해서는 '정보통신망이용촉진 및 정보보호 등에 관한 법률'등에 체계적으로 규정되어 있으나, 법적용 대상이 정보통신 서비스 제공자 위주로 규정되어 의료분야에 적용하는 데 한계가 있다. 때문에 본 논문에서는 국내 의료기관이 전자의무기록 시스템에 보안을 적용하기 위해 우선적으로 선행되어야 할 개인의료정보 보호방안에 대해 정의하고, 적용근거를 위한 법 제도의 검토사항을 제시하였다. 또한, 전자의무기록에 대한 전자서명의 구체적인 적용방안을 예시하여 의료분야에 있어서 보안적용을 위한 기준을 제시하였다.

The Effects of the Electronic Health Record System on Work Overload and Stress Moderation of Hospital Employees

  • Purpose - In endless competition, companies pursue cost reduction and work efficiency. So, entrepreneurs try to increase job intensity, which may lead to job stress and high turnovers because of job burnout. But, Information systems are acknowledged as a work support tool that secures work convenience and the productivity of employees. In this study, we aimed to confirm the effects of information systems in reduing the work overload of employees in a human resource intensive industry. Research design, data and methodology - This is based on the job demands-resources model, conducting an empirical analysis of surveys given to hospital employees working in a human resource intensive industry. Results - The research revealed that information systems reduced the work overload of employees in a human resource intensive industry. Conclusion - This study confirmed the effects of information systems as a job resource based on JD-R theory, and presentation of empirical results indicated that information systems alleviate employee job overload and increases job satisfaction in the medical services industry. In the medical services industry, using electronic health record system decreases in work overload, which results in employees gaining time for self-development and time management, reducing job stress, and leading to job satisfaction.

약물부작용 감시를 위한 공통데이터모델 기반 임상데이터웨어하우스 구축 (Development and Lessons Learned of Clinical Data Warehouse based on Common Data Model for Drug Surveillance)

  • Purposes: It is very important to establish a clinical data warehouse based on a common data model to offset the different data characteristics of each medical institution and for drug surveillance. This study attempted to establish a clinical data warehouse for Dankook university hospital for drug surveillance, and to derive the main items necessary for development. Methodology/Approach: This study extracted the electronic medical record data of Dankook university hospital tracked for 9 years from 2013 (2013.01.01. to 2021.12.31) to build a clinical data warehouse. The extracted data was converted into the Observational Medical Outcomes Partnership Common Data Model (Version 5.4). Data term mapping was performed using the electronic medical record data of Dankook university hospital and the standard term mapping guide. To verify the clinical data warehouse, the use of angiotensin receptor blockers and the incidence of liver toxicity were analyzed, and the results were compared with the analysis of hospital raw data. Findings: This study used a total of 670,933 data from electronic medical records for the Dankook university clinical data warehouse. Excluding the number of overlapping cases among the total number of cases, the target data was mapped into standard terms. Diagnosis (100% of total cases), drug (92.1%), and measurement (94.5%) were standardized. For treatment and surgery, the insurance EDI (electronic data interchange) code was used as it is. Extraction, conversion and loading were completed. R language-based conversion and loading software for the process was developed, and clinical data warehouse construction was completed through data verification. Practical Implications: In this study, a clinical data warehouse for Dankook university hospitals based on a common data model supporting drug surveillance research was established and verified. The results of this study provide guidelines for institutions that want to build a clinical data warehouse in the future by deriving key points necessary for building a clinical data warehouse.

Association between Electronic Medical Record System Adoption and Healthcare Information Technology Infrastructure

  • Objectives: The objective of this study was to investigate the relationship between the level of Electronic Medical Record (EMR) system adoption and healthcare information technology (IT) infrastructure. Methods: Both survey and various healthcare administrative datasets in Korea were used. The survey was conducted during the period from June 13 to September 25, 2017. The chief information officers of hospitals were respondents. Among them, 257 general hospitals and 273 small hospitals were analyzed. A logistic regression analysis was conducted using the SAS program. Results: The odds of having full EMR systems in general hospitals statistically significantly increased as the number of IT department staff members increased (odds ratio [OR] = 1.058, confidence interval [CI], 1.003-1.115; p = 0.038). The odds of having full EMR systems was significantly higher for small hospitals that had an IT department than those of small hospitals with no IT department (OR = 1.325; CI, 1.150-1.525; p < 0.001). Full EMR system adoption had a positive relationship with IT infrastructure in both general hospitals and small hospitals, which was statistically significant in small hospitals. The odds of having full EMR systems for small hospitals increased as IT infrastructure increased after controlling the covariates (OR = 1.527; CI, 1.317-4.135; p = 0.004). Conclusions: This study verified that full EMR adoption was closely associated with IT infrastructure, such as organizational structure, human resources, and various IT subsystems. This finding suggests that political support related to these areas is indeed necessary for the fast dispersion of EMR systems into the healthcare industry.

클라우드기반 u-헬스케어 시스템을 위한 보안 이슈 및 요구사항 분석 (Security issues and requirements for cloud-based u-Healthcare System)

  • 디지털 기기 간 융합과 무선 통신 기술의 발전, 생체신호 측정 센서의 소형화, 종이나 수기로 관리되던 의료관련 정보를 디지털화한 전자의무기록(EMR, Electronic Medical Record) 구축 및 전자건강기록(EHR, Electronic Health Record)의 도입 등으로 인해 '언제 어디서나' 자신의 건강 상태를 모니터링하고 개인 맞춤 건강관리 서비스를 받을 수 있는 Ubiquitous Healthcare (u-헬스케어) 시대가 도래하였다. 또한 클라우드 컴퓨팅(Cloud computing) 기술의 등장은 u-헬스케어 서비스의 발전을 더욱 가속화시키고 있는 요인 중 하나이다. 그러나 이러한 u-헬스케어 서비스 활용 과정에서 개인의 정보가 악의적으로 사용될 경우 정확하고 신뢰성 있는 헬스케어 서비스를 제공받지 못할 뿐만 아니라 단순 건강 검진 및 치료의 수준을 넘어 크게는 개인의 생명과 직결되는 심각한 문제를 초래한다. 이에 본 논문에서는 클라우드 컴퓨팅 환경에서의 u-헬스케어 서비스와 관련된 다양한 보안 이슈를 분석하고 이를 토대로 안전한 보안 의료정보 공유 시스템 구축을 위한 보안 요구사항에 대하여 서술한다. 더불어 향후 국내 u-헬스케어 산업 활성화를 위한 발전방향에 대하여 논하고자 한다.

위 절제술 환자의 진료계획표 개발 및 전자 의무 기록화 (Development of a Flexible Critical Pathway with Electronic Medical Record for Gastrectomy Patients in a University Hospital)

  • Objectives : This study was conducted to evaluate the effect of fixed critical pathway with emr (electronic medical record) on the length of hospital stay, the cost and quality of care provided to gastrectomy patients in a university hospital and to develop flexible critical pathway with emr which can be used excluded or drop-out patients. Methods : Thirty-eight patients with gastrectomy were included as case group and Thirty-four patients included as control group. The comparison between control and case with using fixed critical pathway were done. To develop and to evaluate usefulness of flexible critical pathway with flexible data base, simulation was done for flexible critical pathway with drop-out patients. Result : The major results of this study were as follows: There were no significant differences in patient clinical conditions and no sign of deterioration of quality from critical pathway. The length of hospital stay was 11 days in control group, 8 days in path group(P<0.01). The total costs during the hospital stay were reduced in path group. However the cost per day was significantly increased from reduction of hospital stay(554,352 won in control, 645,669 won in path group). One hundred percentage of drop out patients(60) in the simulation of flexible critical pathway was successful. Conclusion : Computerized critical pathway reduced the length of hospital stay, total hospital costs and resource utilization without harming quality of patient care. The flexible critical pathway program can be used as one of the powerful management tools for reducing the practice variations and increasing the efficiency of care process and decreasing the workload of doctors and nurses in Korean hospital settings.
