요통을 호소하는 ALS환자에 대한 한의학적 치료 효과 증례 보고 (The Clinical Case of Oriental Medical Treatment at Tender Point for Patient with Lower Back Pain Suggesting of Amyotrophic Lateral Sclerosis)

  • ALS를 진단받았으며 2010.5.11부터 2010.6.10까지 ${\bigcirc}{\bigcirc}$한방병원에 입원한 요통을 호소하는 남환에 대하여 화침치료, 전침치료, 침치료, 구치료, 약물치료, 이학요법 등을 시행한 결과 K-ODI, VAS, K-ALSFRS-R 등의 평가 항목에 대하여 호전 경향을 보였다. ALS는 기본적으로 진행성 경과를 밟는 질환이기 때문에, 병의 진행을 늦추는 것과 대증치료를 통해 삶의 질을 높이는 것이 중심이 된다고 볼 수 있다. 본 환자의 경우 상기 한의학적 처치에 의해 일정한 효과가 나타났다. ALS에서 수반되는 요통의 호전에 의해 기능수행능력이 향상된 결과라고 볼 수 있다. 이는 한의학적 치료 방법이 ALS 환자의 치료에 있어서 통증을 감소시키고 일상생활에서의 기능수행능력을 향상시킴으로써 삶의 질을 높이는 대중치료의 역할을 수행할 수 있다는 단서라고 할 수 있다. 치료효과의 검증을 위해서는 추후 지속적인 증례 연구가 필요할 것으로 사료된다.

EAV 측정치(測定値)의 표준화(標準化)에 관(關)한 연구(硏究) (성별(性別), 연령별(年齡別), 체질별(體質別)) (A Study on the standardization of EAV(Electroacupuncture acc.Voll)'s measurement values)

Result As the study for standardization of EAV's measurement values according to constitution and sex and age, the questionnaire on QSCC and QSCC(I) and EAV were carried out to 603 general patients in east : west promotion health center affiliated to Kyung Hee Medical center. The following conclusions were reached as the result of statistical handling of the data of questionnaire and EAV. 1) Each standard scope of BAZ and CMP was suggested. 2) BAZ and CMP examined about constitution did not show any statistical difference. 3) The result of examination by sex on BAZ and CMP is such as followings. A meaningful difference was showed at the level of p<0.001 in HH, HEL, HFR, and FF in case of male on BAZ. meaningful difference were showed at the level of p<0.001 in R3, R4, R18, L3, L16 and L18 and at the level of P<0.01 In R5, R8, R12, L7, L9, L10, L11, and L13 and at the level of P<0.05 in R1, R2, R6, R11, R13, R14, R16, R17, R19, R20, L1, L2, L4, L5, L14, L17, and L19 in case of male on CMP. There was no statistical difference in R7, R9, R10, R15, L12, and L15. 4) In every measure point of both BAZ and CMP a meaningful difference was notified at the level of P<0.01 by age. 2. Suggestion 1)Environmental, examinee and examiner's factors which may cause error in EAV measurement should be excluded. 2)The factor of change by time and space should be clarified. 3)The correlation between disease and meridian should be learned. And scopes of BAZ and CMP according to disease should be studied.

EAV의 측정치(測定値)와 병증유형(病症類型)의 상관성(相關性)에 관(關)한 연구(硏究) (A study on the correlativity of EAV (Electroacupuncure acc.Voll)'s measurement and symptoms of a disease)

  • By making use of the EVA(Electroacupuncture acc. Voll) combined meridian theory of oriental medicine with electronics which was contrived to recognize the physiological and pathological changes of human body, the following conclusions were made in comparison with EAV measurements and types of symptoms(anxiety & headache, fatigue, palpitation, dizzness, abdominal distension, nausea, gastric distubance, constipation & diarrhea, fatty liver, CVA), QSCC, and blood type test. 367 patients including 124 with nervous gastrointestinal problems were selected for this research. 1. From the point of variance of the tested patients 124 nervous gastrointestinal patients, Liver meridian and Spleen meridian showed Hyperenergia and Large intestine meridian, Circulation meridian, Tripe warmer meridian showed hypoergia. 2. In each symptom as the nervous gastrointestinal symptom Liver Meridian showed hyperenergia, Large intestine meridian, Circulation meridian and Triple warmer meridian showed hypoergia. 3. In an objective Comparison with other symptoms, firstly among the headache & anxiety group left Gall Bladder, Triple warmer and Stomach meridian showed remarkable hypoergia, secondly among fatigue group showed hypoergia in Triple warmer meridian and hyperenergia of Stomach meridian, and thirdly among palpitation group showed hypoergia of Kidney meridian, and lastly among dizzness group showed hypoergia of Gall bladder, Stomach, Circulation and Small intestine meridian. 4. All of gastric distubance, nausea, abdominal distention, constipation and diarrhea group showed hyperenergia in Stomach meridian and spleen meridian, gastric disturbance group showed remarkably hypoergia in Circulation, Small intestine, Lung and Large intestine meridian, Nausea group showed hypoergia of large intestine, Constipation and diarrhea group showed hypoergia of Kidney and left Circulation meridian. 5. Fatty liver group showed hyperenergia of Liver meridian of 83.3%, Gall Bladder, stomach and Spleen meridian, Urinary bladder and Kidney meridian showed hypoergia. 6. CVA group showed hyperenergia in Liver and Corculation meridian. 7. Blood type in typical classification had on signigicant bearings on each other. 8. QSCC for the attempt of objective materials of constitutional diagnosis had no correlaticity in comparison with EAV measurements. In conclusion EAV is thought be used as a diagnostic method in oriental medicine and further research is needed regarding it can be used as useful method for verifying the characteristics and early finding of symptoms.

흰쥐의 족삼리 및 태충 전침자극에 따른 뇌대사활성의 변화 (Alterations of Cerebral Metabolic Activation Following Electro-Acupuncture Stimulation on ST36 and LR3 Acu-Points in Rats)

  • Objective : The meridian theory in oriental medicine explains that each acu-point has a characteristic functional effect. It will be supposed that an acupuncture stimulation on different acu-point evokes different activation on different areas in the central nervous system(CNS) according to the meridian theory. On this supposition, our group tried the semi-quantitative [14C]2-deoxyglucose([14C]2-DG) autoradiography on the acupuncture stimulation to the hindlimb acu-points of Sprague-Dawley rats. Methods : A venous catheter for the intravenous administration of isotope was equipped in the right external jugular vein on 3 days prior to the [14C]2-DG study. On the day of the study, two acupuncture needles were inserted into the ST36(Zusanli) or LR3(Taichong) on the left hindlimb. Electro-acupuncture stimulation (2 Hz, 5 ms, 1~3 mA, 15 minutes) started just before the i.v. injection of [14C]2-DG ($25{\mu}Ci/rat$). The brain and the spinal cord were removed and processed for the [14C] 2-DG autoradiography. Results : The EA stimulation on ST36 reveals over 120% metaboilc activation in Arcuate nucleus, Anterior pretectal nucleus, Dorsal cochlear nucleus, Interposed cerebellar nucleus, and Nucleus of Darkschewitsch. The EA stimulation on LR3 reveals over 120% metaboilc activation in Lateral habenula nucleus, Medial vestibular nucleus, Ventromedial thalamic nucleus, Anteroventral thalamic nucleus, Anterior cingulate cortex, Dentate gyrus, Antero cortical amygdaloid nucleus, Anterior pretectal nucleus, and Dorsal tegmental nucleus compared with the non EA stimulation control group. Conclusion : These results demonstrate that the different acu-points evoke the different activations in brain areas. And with this functional brain mapping study, a new scientific elucidation for the basis of the acupuncture-meridian theory in oriental medicine through differences of activated area in CNS according to the each acupuncture point.

기능성 소화불량증의 침치료 연구 동향 : Pubmed를 중심으로 (Trend of Acupuncture Treatment Study for Functional Dyspepsia)

  • Objectives: This study was conducted to help clinical studies for treating Functional Dyspepsia with acupuncture. We inspected tendency of studies of acupuncture treatment for functional dyspepsia and analyzed the assessing instrument for it by scrutinizing Pubmed recent publications. Materials and Methods: We inspected 8 theses and scrutinized their objectives, periods, participants, materials and methods, the instrument of assessment for the studies, and criteria and symptom score for questionaire, results and JADAD score. Results: 1. Most studies were published by oriental doctors except one which was published by an American doctor. More and more studies are being published in America and written in English recently. So it is considered that western doctors think that the acupuncture treatment is useful as unorthodox medicine for functional dyspepsia. 2. Among the 4 clinical studies, 2 studies were RCT and 1 study was not RCT and 1 study with vagueness. All 4 clinical studies were not double-blinded because they were about comparison between acupuncture treatment and medicine. 3. ST36, PC6, CV12, BL21 were chiefly used, and auricular point, ST40, SP6 were also used. Manual acupuncture were used in 2 studies, and low frequency pulse and electroacupuncture were used in other studies respectively. 4. 4 clinical studies used 0~3 points symptom score as the instrument of assessment. Additional assessments were on gastric emptying time, EGG and plasma level of motilin. All thesis were not verified of validity and reliability. 5. Among the 4 reviewed thesis, 2 theses were about acupuncture as one of the various treatment of functional dyspepsia, and the other 2 theses were about efficacy of acupuncture treatment. And the result generally showed effectivity of acupuncture treatment. Conclusion: Acupuncture treatment for functional dyspepsia is considered useful as in unorthodox medicine by western doctors, and it is necessary to provide objective instrument for assessing acupuncture treatment for functional dyspepsia.

손목터널증후군에 사용된 침 치료 보고의 질 평가 (A Systematic Review on the Reporting Quality of Acupuncture Treatment for Carpal Tunnel Syndrome)

  • Objectives : The aim of this study is to analyze the details of acupuncture treatment methods and the reporting quality of acupuncture on Carpal Tunnel Syndrome (CTS). Methods : Search was conducted in Pubmed, EMBASE, and Cochrane Library for acupuncture studies on CTS. The reporting quality of acupuncture treatment was assessed using the following guidelines: Standards for Reporting Interventions in Clinical Trials of Acupuncture (STRICTA) for analyzing the method of acupuncture treatment, Consolidated Standards of Reporting Trials (CONSORT) for analyzing study design and study process, and Risk of Bias (ROB) for analyzing bias. The number of reported items was calculated and evaluated as a proportion. The reported proportion of each study was classified into three grades: Grade A (% score ≥75), Grade B (50≤ % score <75), and Grade C (% score <50). Results : A total of 9 Randomized Controlled Trials (RCTs) were included in this study. All trials reported 12 items (66.67%) on average in STRICTA guidelines. Five studies were conducted with manual acupuncture and 3 studies were conducted with electroacupuncture. PC7 (Daereung) was most frequently used to treat CTS. In STRICTA guideline evaluation, 3 studies were classified as Grade A, 5 studies were classified as Grade B, and 1 study was classified as Grade C. In the CONSORT statement assessment, all trials reported an average of 20.56 items. Of the 9 RCTs, 6 studies were classified as Grade B and 3 studies were classified as Grade C. In ROB assessment, most studies showed a low (63.49%) or unclear (26.98%) risk of bias. The selective reporting bias and the incomplete outcome data bias were found to have the lowest risk of bias, and the allocation concealment of selection bias was found to have the most unclear risk of bias. Conclusions : Recent acupuncture studies on CTS showed moderate reporting quality. However, more detailed reports on acupuncture are still needed to establish more solid evidence of acupuncture treatment.

비만환자의 비만감소를 위한 침술요법에 대한 국내외 논문 분석 (Analyses of stidies on acupuncture therapies for reducing obesity in obeses patients)

  • 본 연구의 목적은 비만환자의 비만을 감소시키기 위한 침술요법의 현황을 파악하고 침술요법의 내용과 효과를 파악하는 것이다. 문헌검색은 '비만', '침', '이침', '전기침' 등을 검색어로 하여 한국학술정보, 한국교육학술정보원, 국회도서관, Pubmed, CINAHL 등을 통해 2003년 1월부터 2017년 12월까지 발표된 연구논문을 검색하였다. 연구자가 원본을 확인한 후, 선정한 19편의 논문을 최종 분석 하였다. Scottish Intercollegiate Guideline Network의 체크리스트로 논문의 질적 평가를 실시한 결과 10점 만점에 9점인 연구가 1편, 10점 만점에 7-8점인 연구가 15편, 10점 만점에 6점인 연구가 2편, 10점 만점에 3점인 연구가 1편이었다. 침술요법의 내용을 파악한 결과 비만을 감소시키기 위한 경혈점은 shemen, hunger, LI-11(Quchi), LI-4(Hegu), St-25(Tianshu), St-44(Neiting) 등이었다. 침술요법 총 시행 횟수는 평균 16.9회이었고, 침술요법 중재 기간이 20일 혹은 72일인 연구에서의 침술요법 중재 빈도는 1회/1-2일이었고, 침술요법 중재 기간이 4-18주인 연구에서의 침술요법 중재빈도는 1-3회/주이었다. 분석한 연구결과에 의하면 침술요법은 신체계측수치의 감소, 혈당 및 혈중 지질 농도의 감소, 혈중호르몬 변화 등을 일으켜 비만환자의 비만을 감소시키는 효과가 있었다. 향후 비만환자의 비만감소를 위한 침술요법의 효과성을 평가하기 위해 잘 설계된 연구가 시급히 필요하다.

하지 절단 환자의 환상통에 대한 한방 치험1례 (A Case Report on a Phantom Limb Pain Patient after Below Knee Amputation using Korean Medicine Treatment)

  • Objectives: This study presented the case of a 49-year-old Korean female with phantom limb pain after below right knee amputation and aimed to assess the effectiveness of Korean medicine treatment. Methods: The patient was treated with scalp acupuncture, electroacupuncture and herbal medicine. We executed a numerical rating scale (NRS), conducted a global assessment (G/A), administrated a Short-Form McGill Pain Questionnaire 2 (SF-MPQ-2) and measured total daily sleep time to evaluate symptom improvement. Results: The patient's G/A scores decreased from 10 to 2 and SF-MPQ-2 points decreased from 20 to 6 after treatment. The total daily sleep time did not changed due to anxiety. Conclusions: This study suggests that Korean medicine treatment could be effective in treating phantom limb pain after amputation. Further studies are needed.

고도비만 환자 대상 한의 비만 상담을 포함한 한의치료의 체중 감량 효과: 후향적 차트 리뷰 (Effect of Korean Medicine Treatment Including Korean Medicine Counselling on Weight Loss in Patients with Morbid Obesity: A Retrospective Chart Review)

  • Objectives: We conducted a retrospective chart review to investigate the effects of Korean Medicine (KM) treatment on weight loss in patients with morbid obesity (body mass index [BMI]≥30 kg/m2) and to compare the effects of KM counseling and non-counseling on weight loss. Methods: We performed a retrospective chart review of patients with morbid obesity who received KM treatment for at least 1 month. We evaluated the effects of KM treatment on body weight, BMI, body fat, fat mass, EuroQol-5D, and the Korean version of the obesity-related quality of life scale (KOQOL). We also assessed the liver and kidney functions, and adverse events. We performed a comparative analysis between the counseling and non-counseling groups. Results: A total of 37 patients who underwent 4 weeks of KM treatment involving Wolbi-tang, electroacupuncture, and KM counseling were included in this study. Twenty-one patients were assigned to the counseling group and sixteen patients to the non-counseling group. There was a significant decrease in weight, BMI, fat mass, and KOQOL (P<0.05) with improvements in total cholesterol, low-density lipoprotein, and triglycerides. The counseling group had a greater reduction in body weight, BMI, body fat, and fat mass compared to the non-counseling group, although it was not significant. In the BMI below 35 (n=25), the reduction ratio of body weight, body fat mass, and body fat was higher in the counseling group than in the non-counseling group; additionally, body fat decreased significantly in counseling group (P<0.05). No severe adverse effects were observed. Conclusion: KM treatment could be effective for weight loss, especially in patients with morbid obesity, KM counseling could also be a good tool for weight loss, typically in patients with BMI<35 kg/m2.

한방 치료로 고안드로겐혈증을 개선한 다낭성 난소 증후군 연속증례연구 (A Case Series of Polycystic Ovary Syndrome with Improved Hyperandrogenism Treated with Korean Medicine)

  • Objectives: This study aims to analyze the medical record of eight cases of polycystic ovary syndrome (PCOS) in a local Korean medicine clinic, in order to provide evidence on Korean medicine treatment of PCOS and to suggest the direction of future studies. Methods: The medical records of eight cases diagnosed with PCOS based on 2003 Rotterdam Criteria and whose total testosterone level was 0.53 ng/ml and over were retrospectively analyzed. The primary outcome was total testosterone level, and the secondary outcomes included luteinizing hormone (LH) and follicle stimulating hormone (FSH) levels, LH/FSH ratio, weight, last mentrual period (LMP), numerical rating scale (NRS) score of dysmenorrhea, and other associated symptoms pre- and post-treatment. Korean medicine treatment consists of the complex herbal prescription consisting of Yukmijihwang-tang, Baekho-tang, Daesiho-tang, and Gyejibokryeong-hwan, modified according to symptoms; acupuncture and electroacupuncture on 中脘 (CV12), 下脘 (CV10), 關元 (CV4), 石門 (CV5), 合谷 (LI4), 太衝 (LR3), 三陰交 (SP6), and 懸鍾 (GB39); and indirect moxibustion on 神闕 (CV8). Results: The average total testosterone level lowered statistically significantly after the treatment. The average FSH and LH levels lowered with insignificance, while the average LH/FSH ratio and weight lowered statistically significantly. During the treatment period of three to six months, every patient had the menstrual cycle shortened except for one case; and two cases with infertility problem succeeded in being pregnant. There was no adverse event. Conclusions: The Korean medicine treatment was found effective in treating PCOS, especially regarding hyperandrogenism, amenorrhea or oligomenorrhea, and infertility.
