스크램블러 치료와 경피신경전기자극 치료가 만성 허리 통증 환자의 통증과 기능 장애 및 우울에 미치는 효과 비교 (Comparison of the Effects of Scrambler and Transcutaneous Electrical Nerve Stimulation Therapy on Pain, Functional Disability, and Depression in Patients with Chronic Low Back Pain)

  • Purpose: The purpose of this study was to compare the effects of scrambler and transcutaneous electrical nerve stimulation therapy on pain, functional disability, and depression in patients with chronic low back pain. Methods: Twenty patients with chronic stroke were assigned randomly to an experimental (n=10) or control (n=10) group. The experimental group performed scrambler therapy. The control group performed electrical nerve stimulation therapy. Training was conducted once a day for 30 minutes, five days per week, for three weeks. The pain was measured using the numeric rating scale. Functional disability was measured using the Roland-Morris disability questionnaire. Depression was measured using the Beck depression inventory. Results: As a result of comparison between the groups, the experimental and control groups showed significant difference for pain, functional disability and depression after the experiment (p<0.05). In a comparison between the two groups, the experimental group, in which scrambler therapy was applied, showed a more significant reduction in pain, functional disability and depression than the control group (p<0.05). Conclusion: Based on these results, scrambler therapy shows positive effects on pain, functional disability, and depression in patients with chronic low back pain.

TENS와 SSP가 전류지각역치 및 통증역치에 미치는 효과 (The Effect of Current Perception Threshold and Pain Threshold through Transcutaneous Electrical Nerve Stimulation and Silver Spike Point Therapy)

  • Purpose: This study was designed to compare the effects of transcutaneous electrical nerve stimulation (TENS) and silver spike point (SSP) therapy on current perception threshold (CPT) and mechanical pain threshold (MPT). Methods: Forty-five healthy adult male and female subjects were studied. Fourteen of them were males and twenty-one were females. Subject were randomly assigned to receive; (1) TENS (80/120 Hz alternating frequency), (2) SSP (3 Hz), or (3) no treatment (control group). Electric stimulation was applied over LI4 and LI11 on acupuncture points of the left forearm for 30 minutes. CPT and MPT were recorded before and after electrical stimulation. The data were analyzed using linear mixed models, with group treated as a between subject factor and time a within-subject factor. Results: At 30 minutes after cessation of electrical stimulation the CPT of C fibers and A${\delta}$fibers was reduced in the TENS group that of C fibers was reduced in the SSP group (p<0.05). After cessation of electrical stimulation, the MPT of C fibers and A${\delta}$fibers increased in the TENS group, and that of A${\delta}$fibers increased in the SSP group (p<0.05). Conclusion: After TENS and SSP stimulation, MPT of C fibers and A${\delta}$fibers were selectively increased. In particular, the TENS group showed increases in both C and A${\delta}$fibers, while the SSP group showed increases only in A${\delta}$fibers.

복부 기능적전기자극이 목척수손상환자의 최대기침유량과 노력성폐활량에 미치는 효과 (Effects of Abdominal Functional Electrical Stimulation on Peak Cough Flow and Forced Vital Capacity in Patients with Cervical Spinal Cord Injury)

  • Purpose : The purpose of this study was to investigate the effects of abdominal functional electrical stimulation on peak cough flow and forced vital capacity in patients with cervical spinal cord injury. Methods : The study examined 20 patients with cervical spinal cord injury. The subjects were randomly divided into two groups. All subjects performed conservative physical therapy for 30 minutes. The experimental group also underwent abdominal functional electrical stimulation for at least 20 minutes per day. Abdominal functional electrical stimulation was applied to the rectus abdominis muscle twice each day, three times a week, for four weeks. In all subjects, the peak cough flow was measured using a peak flow meter and forced vital capacity was assessed using a spirometer. Results : The experimental group showed a significant increase in peak cough flow and forced vital capacity in pre-post measurements (p<.05), while the control group showed a significant increase only in peak cough flow. Conclusion : These findings suggest that conservative physical therapy in combination with the abdominal functional electrical stimulation can improve peak cough flow and forced vital capacity in patients with cervical spinal cord injury.

전기 자극이 여성 노인의 만성 무릎통증, 운동자기효능 및 삶의 질에 미치는 효과 (Effects of Electrical Stimulation Therapy on Chronic Knee Pain, Exercise Self-Efficacy, and Quality of Life in Korean Elderly Women)

  • Purpose: This study was to examine the effects of electric stimulation therapy on chronic knee pain, exercise self-efficacy, and quality of life in Korean elderly women. Methods: The design was an unequivalent control pretest-posttest study. Samples were total of 60 (experimental: 30, control: 30) elderly women with healthy cognitive and communication abilities aged 65 years old and above. The experimental treatment involved was the electric stimulation of both thigh quadriceps muscles for 15 minutes per treatment, 3times per a week, for a total of 12 weeks. Measurements taken were S-F MPQ and AIMS for chronic knee pain, exercise self-efficacy measurement for exercise self-efficacy, and S-F 36 scale for quality of life. Data were analyzed using SPSS PC+ 12. Results: Chronic knee pain according to the Short-Form McGill Pain Questionnaire (t=43.563, p=.000) and Arthritis Impact Measurement Scale (t=31.364, p=.000) were significantly decreased in the experimental group by the application of electrical stimulation therapy. Exercise self-efficacy (t=107.116, p=.000) and quality of life (t=76.429, p=.000) were significantly increased in the experimental group by the application of electrical stimulation therapy. Conclusion: Electrical stimulation therapy could be a more effective primary nursing intervention in decreasing chronic knee pain, and on increasing exercise self-efficacy and quality of life for Korean elderly women.

신경근 전기자극이 뇌졸중 환자의 주관절 굴곡근 경직에 미치는 영향 (The effect of neuromuscular electrical stimulation on the spasticity of elbow flexor in stroke patients)

  • The purpose of this study was to examine the effects of neuromuscular electrical stimulation(NMES) on the spasticity of elbow flexor. Ten spastic hemiplegia who have been at EUL-JI medical college hospital in Taejon with age range of 35 to 70 years$(56.8\pm9.5)$, were participated in this study. The subjects (5male, 5female) took neuromuscular electrical stimulation therapy on the triceps of the affected elbow. To compare the effect of the treatment, the grade of spasticity of affected elbow flexor was measured at pre-treatment and the end of each weeks using modified Ashworth scale. Then, the range of extension of the affected elbow was measured at pre-treatmen and the end of each weeks using full circle goniometer. The data were analyzed with ANOVA to determine significant differences with the passage of time. The results were as follows: 1. ANOVA test showed significant differences in reducing spasticity of NMES group with the passage of time (p<.01). 2. ANOVA test showed significant differences in increasing range of motion with the passage of time of NMES group(p<.01). The neuromuscular electrical stimulation is a effective method to reduce the spasticity of elbow flexor.

경피신경전기자극과 초음파가 전기생리학적 반응에 미치는 영향 (Effects on Electrophysiologic Responses to the Transcutaneous Electrical Nerve Stimulation and Ultra Sound)

  • The purpose of this study was to investigate the influnce of afferent stimuli, transcutaneous electrical nerve stimulation and ultra sound, on the electrdiagnostic study of normal subjects. Electrodiagnostic study was performed before and after the application of afferent stimulation of the right popliteal fossa on 18 healthy female volunteers. After the transcutaneous electrical nerve stimulation, there is no significantly change of latencies and amplitudes of SEP, H-reflex, peroneal nerve F-wave, and sensory nerve conduction. After the ultra sound, there is no significantly change of latencies and amplitudes of SEP, H-reflex, peroneal nerve F-wave, and sensory nerve conduction. Tibial nope F-wave and motor nerve shows prolonged latency after TENS and US (p<0.01). Ultrasound may have a similar mechanism of action compared to transcutaneous electrical nerve stimulation by having localized inhibitory effects of the peripheral nerve. However, further investigation is needed to assess their mechanism of action and the precise relevance of stimulation modality.

${\beta}-Endorphin$의 내피세포의존성-세로토닌 유도-근 수축 억제와 저빈도-주파수 은침점전자극의 혈장 ${\beta}-endorphin$ 증가 (Inhibition of Endothelial Cell-dependent Serotonin-induced Contraction of ${\beta}-endorphin$ and Increment of Plasma ${\beta}-endorphin$ of Silver Spike Point Low Frequency Electrical Stimulation)

  • The purpose of this study was to demonstrate the effects of silver spike point (SSP) low frequency electrical stimulation on plasma ${\beta}-endorphin$ activities measured by radio- immunoassay from normal volunteer and the effects of ${\beta}-endorphin$ on 5-hydroxytryptamine (5-HT, serotonin)-induced contraction investigated by isometric tension methode in rats. The current of 3 Hz continue type, but not 100 Hz continue type, of SSP low frequency electrical stimulation significantly increased in plasma ${\beta}-endorphin$ from normal volunteer. The endothelial cell-dependent 5-HT-induced contractions were inhibited by ${\beta}-endorphin$ $1{\mu}M$. These results suggest that the ${\beta}-endorphin$ regulates nociceptive-like substance, such as 5-HT, in part and that the SSP low frequency electrical stimulation, specifically current of low frequency of 3 Hz continue type, significantly increases plasma ${\beta}-endorphin$ from normal volunteer.

Effect of Bridge Exercise Combined with Functional Electrical Stimulation on Trunk Muscle Activity and Balance in Stroke Patients

  • Background: Stroke patients have weak trunk muscle strength due to brain injury, so a single type of exercise is advised for restoring functionality. However, even after intervention, the problem still lies and it is suggested that another intervention method should be applied with exercise in order to deal with such problem. Objectives: To Investigate the effect of bridge exercise combined with functional electrical stimulation (FES) on trunk muscle activity and balance in stroke patients. Design: Randomized controlled trial. Methods: From July to August 2020, twenty stroke patients was sampled, ten patients who mediated bridge exercises combined with functional electrical stimulation were assigned to experiment group I, and ten patients who mediated general bridge exercises were assigned to experiment groupII. For the pre-test, using surface EMG were measured paralyzed rectus abdominis, erector spinae, transverse abdominis/internal oblique muscle activity, and using trunk impairment scale were measured balance. In order to find out immediate effect after intervention, post-test was measured immediately same way pre-test. Results: Change in balance didn't show significant difference within and between groups, but muscle activity of trunk was significant difference rectus abdominis and erector spinae within groups I (P<.01), also between groups was significant difference (P<.05). Conclusion: Bridge exercise combined with FES could improve trunk function more effectively than general bridge exercise due to physiological effect of functional electrical stimulation.

Effects of Gastrocnemius Neuromuscular Electrical Stimulation Training on Ankle mobility and Gait in Patients with Stroke

  • Objective: The purpose of this study was to investigate the effects of gastrocnemius neuromuscular electrical stimulation training on ankle mobility and gait in patients with stroke. Design: A randomized controlled trial. Methods: 31 patients with stroke were selected and classified into an experimental group (n=16) and a control group (n=15). Both groups were assessed for ankle mobility using the Knee to Wall Test and gait parameters using G-walk before and after the intervention. The intervention was applied five times a week for four weeks. The experimental group performed gastrocnemius neuromuscular electrical stimulation followed by ankle control exercises, while the control group only applied NMES to the tibialis anterior muscle of the paretic side for 30 min per session five times a week for 4 weeks. Results: Experimental group showed significant improvements in Knee to wall test. and lumbar flexibility after the intervention. both group showed significant improvements in gait parameters after the intervention. However, when comparing between the two groups, the experimental group showed a more significant effect than the control group. Conclusions: Gastrocnemius neuromuscular electrical stimulation training can be considered an effective approach to improve ankle mobility and gait ability in patients with stroke.

인체에 적용한 전기자극이 Serum myoglobin과 Aldolase에 미치는 영향에 대한 생리학적 효과 (A experimental study on the physiological effects of electrical stimulation treatment of serum myoglobin and aldolase in human body)

  • Prior studies have revealed that several stimulation to the muscle have released serum myoglobin into the blood vessel and increased aldolase activity. The present authors carried out a study which effect of electrical stimulation treatment (induced a isotonic wrist exerceise) on serum myoglobin(Mb) levels and aldolase(Al) activity were investigated in 6 healthy female. There were four groups of female: 1. no electrical stimulation control: 2. electrical stimulation 10min (EST10'); 3. electrical stimulation 20 min (EST20'); 4. electrical stimulation 30min (EST30'). Each groups is all the same one. Radioimmunoassay and Ultraviolet Spectrophotometry were performed to increased or decreased of serum myoglobin and aldolase. Serum myoglobin significantly increased in electrical stimulated groups[EST10' $(30.20{\pm}5.27ng/ml)$, EST20'$(31.65{\pm}3.96ng/ml)$, EST30'$(31.95{\pm}2.0ng/ml)$] to be compared with control group$(24.43{\pm}2.20ng/ml)$. Aldolase significantly increased in electrical stimulated groups [EST10' ($6.85{\pm}1.17$ Sigma U/mL), EST20'($6.70{\pm}1.46$ Sigma U/mL), EST30'($6.56{\pm}1.01$ Sigma U/mL)) to be compared with control group($5.03{\pm}1.86$ Sigma U/mL). The results of this study show that isotonic exercise result in electrical stimulation treatment increased serum myoglobin content and aldolase activity. In conclusion, our results support that stimulation release serum myoglobin and increase aldolase activity.
