개인용 고주파 전기자극기의 주파수 가변 설계 및 출력 특성의 분석 (Analysis of Output Characteristics and Frequency Variation Design for Personal High Frequency Electrical Stimulation Medical Devices)

  • In this paper, personal electrical stimulation medcial devices using bioelectrical stimulating was developed. Therapy effect of RET(resistive electric transfer) was more effective than CET(capacitive electric transfer), but CET was adopted because of safety issue. Then, the optimum parameters that may be effective in thin skin or facial wrinkles was set. For example, the frequency of the pulse voltage for stimulation is 1.8[MHz], burst frequency is 7[kHz] or 400[Hz], the development of devices was to have ON/OFF control and frequency control. When burst frequency was adjusted 7[kHz], heat was generated in the electrode. The case of 400[Hz] the heat was little generated. The microcontroller ATmega128-based experimental results show that the proposed personal high frequency electrical stimulation devices can be applied to medical equipment using therapy effect successfully.

흰쥐 좌골신경손상 후 전기 자극이 대뇌피질에서의 MAP2 발현에 미치는 영향 (The Effect of Electrical Stimulation on MAP2 Expression in the Cerebral Cortex following Sciatic Nerve Crush Injury in Rat)

  • The purpose of this study was to investigate the effect of electrical stimulation(EST) on MAP2(Microtubule Associated Protein 2) expression in cerebral cortex following sciatic nerve crush injury in rats. Twelve Sprague-Dawley adult female rats, six for control and six for experimental, were anesthetized and their sciatic nerves were crushed. The electrical stimulation (EST) was applicated with 3 Hz for 10 minuties in a day for muscles innervated sciatic nerve. The MAP2 expression in cerebral cortex was identified from immunohistochemistry against MAP2. The result of this study were as follow: 1) In control group, MAP2 immunoreactive neurons were observed but there no significant increase for 3 days. 2) MAP2 immunoreactive neurons were increased markably in experimental group than control group. 3) MAP2 immunoreactive neurons were increased markably after applicating with EST in sciatic nerve crush injury induced group from 2nd day. This study showed that the application of EST for muscles after sciatic nerve crushed injury made MAP2 immunoreactive neurons in the cerebral cortex increased. Therefore, the electrical stimulation on the peripheral site, denervated muscle, may facilitate MAP2 expression in the cerebral cortex.

  • The purpose of this case study was to introduce functional electrical stimulation(FES) for paraplegic patients. FES provides the ability to rise from sitting to standing, maintenance of a standing position, and the ability to walk with a reciprocal gait. Six channels of electrical stimulation are sufficient for synthesis of a simple reciprocal gait pattern in these patients. During the double-stance phase, knee extensor muscles of both knees are stimulated, providing sufficient support for the body. Only one knee extensor muscle group is excited during the single-stance phase. The swing phase of the contralateral lower extremity is accomplished by eliciting the synergic flexor muscle response through electrical stimulation of afferent nerves. The transition from the double-stance phase to the swing phase is controlled by two hand switches used by the therapist or built into the handles of the walking frame for using by the patient. A twenty-five years old male was with a T9/T9 spinal cord injury due to a traffic accident and admitted to Yonsei Rehabilitation Hospital for comprehensive treatment. After 30 days of training using the Parastep(R) he was able to stand for 10 minutes. After 43 days, he was able to walk and at discharged he could walk for 100 meters.

뇌졸중 환자의 상지 기능에 기능적 전기 자극이 미치는 영향: 무작위대조군연구에 기초한 체계적 고찰 (Effect of Electrical Stimulation on Upper Extremity Function in Stroke Patients: A Systematic Review Based on Randomized Controlled Trials)

  • Objective: Electrical stimulation is an assistive technology used to aid the recovery of upper limb use after stroke. The purpose of this systematic review was to determine the effects of electrical stimulation on upper extremity function in individuals with hemiparetic stroke and to develop an evidence base that supports the use of electrical stimulation for upper limb recovery after stroke. Design: A systematic review based on randomized controlled trials (RCTs). Methods: Studies published before April 20 2021 were collected for this review by searching PubMed, four other databases, and RCTs that reported the effects of electrical stimulation on upper extremity function in individuals with the characteristic stroke type. Information on the following parameters was extracted from each study: surname of first author, published year, country, participants, intervention, intervention's intensity, comparison, outcomes, additional therapy, and summary of results. This review also evaluated the bias within each study, including any selection bias, performance bias, detection bias, attrition bias, and reporting bias. Results: This review included five RCTs, and 208 stroke patients were included in the analysis. Stroke patients who underwent electrical stimulation showed significantly improved grip and pinch strengths, wrist range of motion, and basic daily living compared to those in the control group; however, there was no improvement in upper extremity function. Of the selected papers, 60% showed a "high risk" of performance bias, and 20% showed a "high risk" of detection bias. Conclusions: The results of this systematic review suggest that electrical stimulation provides some benefits to stroke patients, such as improved hand strength and range of motion. However, future studies are needed to provide clinical evidence of the effects of electrical stimulation on upper extremity function in stroke patients.

아급성기 뇌졸중 환자의 보행동안 청각적 피드백과 기능적 전기 자극 적용이 균형, 보행 및 하지 기능에 미치는 영향 (Study on Effects of Auditory Feedback and Application of Functional Electrical Stimulation During Gait on Balance, Gait and Lower Extremity Function in Patients with Subacute Stroke)

  • PURPOSE: Examine the effects of auditory feedback and functional electrical stimulation on balance, walking ability, and lower extremity function of subacute stroke patients. METHODS: Twenty-seven subjects diagnosed with subacute stroke within six months were randomly divided into three groups: test group 1, which performed walking exercises with auditory feedback and functional electrical stimulation; test group 2, which performed walking exercises only with functional electrical stimulation; control group applied only functional electrical stimulation, with nine subjects each. RESULTS: There were significant pre- to post-intervention differences in BBS in the gait training group with auditory feedback and functional electrical stimulation treatment, and significant pre- to post-intervention differences in BBS, sit-to-stand time, and average step speed in the gait training group with functional electrical stimulation, but no statistically significant differences in between-group comparisons. CONCLUSION: Gait training with auditory feedback and functional electrical stimulation can improve the balance and gait performance in stroke patients. Therefore, in the future, gait training with auditory feedback and functional electrical stimulation therapy may be suggested as a gait rehabilitation training tool for stroke patients.

경피신경전기자극이 지연성근육통의 교감신경활동에 미치는 영향 (The Effects of Transcutaneous Electrical Nerve Stimulation on Sympathetic Nerve Activity in Delayed Onset Muscle Soreness)

  • This study conducts eccentric exercise ti non-dominant elbow flexor of 20 persons in order to examine the effects of transcutaneous electric nerve stimulation on sympathetic nerve activity in delayed onset muscle soreness, induces delayed onset muscle soreness, divides them into 10 persons respectively as experimental and control groups. And a stimulation for 2 min. with 100 pps is given to elbow flexor after repeated three times of 10 minutes rest, temperature, blood pressure and pulse are measured and as a result of two-way ANOVA, change of temperature didn't show a significant difference according to the elapse of times(p>0.05) and systolic pressure and pulses in showed a significant difference between experimental and control groups(p<0.05). These results suggest that transcutaneous electrical nerve stimulation has a direct or indirect influence on sympathetic nerve activity in delayed onset muscle soreness under a restricted condition of electrical stimulation.

경피전기자극을 이용한 연접 전 억제가 만성 뇌졸중 환자의 global synkinesis에 미치는 영향 (The Effect of Presynaptic Inhibition Using the Transcutaneous Electrical Stimulation for Global Synkinesis on the Post-Stroke Hemiplegic Patients)

  • Purpose : This study was performed as follows in order to investigate the effect of presynaptic inhibition mechanism using the transcutaneous electrical stimulation (TES) for global synkinesis (GS) on the post-stroke hemiplegic patients. Methods : The subjects consist of 38 post stroke hemiplegic patients; experiments were performed on thirty patients excluding eight. The experiment was performed on sham group, sensory level stimulation group, and motor level stimulation group for 20 minutes a day 5 times a week for 6 weeks total. We compared the differences in GS levels and walking ability. The measurements were carried out pre, immediated, post 10th, and 20th, for a total of four measurements. Results : The GS level using sEMG found significant differences between groups at the post 10th and post 20th in dorsiflexion, and post 20th in plantarflexion (p<0.05, p<0.01). The motor level group indicated more significant differences when the number of electrical stimulations increased. TUG and 10 m walking test indicated a significant difference at immediated, post 10th, and post 20th. The motor level group showed more significant decreasing tendency than the sensory level group. Conclusion: From these results, electrical stimulation using presynaptic inhibition mechanism of transcutaneous electrical stimulation (TES) had positive effects for walking ability on inhibition of muscle tone in lower extremity. The motor level stimulation group experienced a more significant effect than the sensory level stimulation group. Therefore, the transcutaneous electrical stimulation (TES) is considered to be effective on walking ability increasing through inhibition of muscle tone in lower extremity for rehabilitation of post stroke hemiplegic patients.

신경근 전기자극이 뇌졸중 환자의 주관절 굴곡근 경직에 미치는 영향 (The effect of neuromuscular electrical stimulation on the spasticity of elbow flexor in stroke patients)

  • The purpose of this study was to examine the effects of neuromuscular electrical stimulation(NMES) on the spasticity of elbow flexor. Ten spastic hemiplegia who have been at EUL-JI medical college hospital in Taejon with age range of 35 to 70 years$(56.8\pm9.5)$, were participated in this study. The subjects (5male, 5female) took neuromuscular electrical stimulation therapy on the triceps of the affected elbow. To compare the effect of the treatment, the grade of spasticity of affected elbow flexor was measured at pre-treatment and the end of each weeks using modified Ashworth scale. Then, the range of extension of the affected elbow was measured at pre-treatmen and the end of each weeks using full circle goniometer. The data were analyzed with ANOVA to determine significant differences with the passage of time. The results were as follows: 1. ANOVA test showed significant differences in reducing spasticity of NMES group with the passage of time (p<.01). 2. ANOVA test showed significant differences in increasing range of motion with the passage of time of NMES group(p<.01). The neuromuscular electrical stimulation is a effective method to reduce the spasticity of elbow flexor.

미세전류치료가 류마티스 관절염 유발 흰쥐의 Interleukin-1 발현에 미치는 영향 (Influence of Microcurrent Therapy in Interleukin-1 Expression in Rhueumatoid Arthritis Rats)

  • Purpose: Electrical stimulation is one of several treatments recommended for RA patients. Electrical stimulation of RA patients, reduces pain, or facilitates joint motion prior to exercises. However, there is still limited evidence on the efficacy of electrical stimulation and thus any conclusions drawn about this method remain controversial. Recently, Microcurrent Electrical Neuromuscular Stimulation (MENS) has received significant attention as a potential method of electrical stimulation. In this study, we investigated the effect of microcurrent treatment in rheumatoid arthritis rat. Methods: Subjects were allocated either to the control group or experimental group, which was subject to microcurrent stimulation. Interleukin-1 expression in the metatarsophlangeal joint and the oedema index in the ankle were used for classification and subsequent evaluation of pathology. Subjects were assessed at 1, 7 and 14 days after inducing rheumatoid arthritis through adjuvant injection. Thirty-six subjects, 18 in each group, were used in this study. Statistical analysis was performed by calculating the differences between the two groups and between each interval assessment. Categorical variables were compared between the two groups with the paired-T test. The one-way ANOVA test was performed to assess changes in ordinal variables. Results: Baseline characteristics were similar in both groups. Statistically significant differences were found between the two groups. The biological marker of pro-inflammatory cytokine and oedema index were decreased in response to this treatment. Conclusion: These data show that treatment of rheumatoid arthritis with a microcurrent stimulation device reduced the oedema index and pro-inflammatory cytokine IL-1.

전기자극이 흰쥐 탈신경 근육의 위축에 미치는 영향 (Effect of Electrical Stimulation on the Denervated Muscle of the Albino Rat. Sprague-Dawley)

  • The purpose of this study was to determined the effect of low-frequency electrical stimulation on the denervated gastrocnemius muscles of the albino rats, Sprague-Dawley. Fifteen Sprague-Dawley adult male albino rats were divided into non-treated (normal) group, denervated (control) group, denervated and electrical stimulated (experiments). The gastrocnemius muscles of the right leg were submaximally stimulated with 30 Hz electrical stimulation. After 4-week period, the animals were sacrificed, and muscle were removed, fixed by immersion, and processed for light and electron microscopy. The numbers of Ag-NOR increased significantly (p<0.001), but significant reductions of girth(p<0.01), wet muscle weight (p<0.001), high glycogen content fiber (p<0.01), and mitochondrial number (p<0.05) were found in denervated control group. In comparison with control group, significant increase of right leg girth (p<0.05), wet muscle weight (p<0.001), high glycogen content fiber (p<0.05), numbers of Ag-NOR(p<0.001), number of mitochondria (p<0.01), mitochondrial volume found in electrical stimulated experimental group. The results suggest that the electrical stimulation of the muscle partially prevented the denervated atrophy in the rat gastrocnemius muscles.
