  • Background and Purpose: The SoUth Korea study to PrEvent cognitive impaiRment and protect BRAIN health through lifestyle intervention (SUPERBRAIN) proved the feasibility of multidomain intervention for elderly people. One-quarter of the Korean population over 65 years of age has mild cognitive impairment (MCI). Digital health interventions may be cost-effective and have fewer spatial constraints. We aim to examine the efficacy of a multidomain intervention through both face-to-face interactions and video communication platforms using a tablet personal computer (PC) application in MCI. Methods: Three hundred participants aged 60-85 years, with MCI and at least one modifiable dementia risk factor, will be recruited from 17 centers and randomly assigned in a 1:1 ratio to the multidomain intervention and the waiting-list control groups. Participants will receive the 24-week intervention through the tablet PC SUPERBRAIN application, which encompasses the following five elements: managing metabolic and vascular risk factors, cognitive training, physical exercise, nutritional guidance, and boosting motivation. Participants will attend the interventions at a facility every 1-2 weeks. They will also engage in one or two self-administered cognitive training sessions utilizing the tablet PC application at home each week. They will participate in twice or thrice weekly online exercise sessions at home via the ZOOM platform. The primary outcome will be the change in the total scale index score of the Repeatable Battery for the Assessment of Neuropsychological Status from baseline to study end. Conclusions: This study will inform the effectiveness of a comprehensive multidomain intervention utilizing digital technologies in MCI.

  • 인권에 기반을 둔 사회복지 실천은 사회복지사의 존재 이유라고 해도 과언이 아닐 것이다. 사회복지사는 사회적 약자의 인권을 옹호하고, 사회의 문제를 해결하며, 사회정의를 실현하는 전문가이기에 인권과는 불가분의 관계에 있다. 최근 한국 사회에서 인권에 대해 많은 논의가 이루어지고 있다. 특히 고령화 사회 속에서 2008년 9월 치매와의 전쟁을 선포한 이후치매관리종합대책을 수립하여 정부가 주도적으로 추진해 오고 있는 시점에서 인권 관점에 근거한 치매노인 돌봄을 이해하기 위해, 인권 관점이 치매노인 돌봄을 위한 사회복지 실천 현장에 주는 장점과 과정은 무엇인지 검토해 볼 필요가 있다. 이에 본 연구에서는 노인의 보호와 치매의 정의, 원인과 치매노인 돌봄을 위한 기술과 장점을 살펴보고, 인권 관점에 근거한 치매노인 돌봄을 위한 방안을 제언하고자 한다.

Correlation Analysis between Cognitive function and Praxis tasks in the Elderly

  • The purpose of this study was to identify differences in cognitive function according to the presence or absence of apraxia and the tasks most relevant to the cognitive function among the various types of tasks in the apraxia test. The subjects were 42 community residents who participated in a cognitive rehabilitation program related to dementia in a Chungbuk area. MMSE-K and BCoS(Birmingham Cognitive Screen) apraxia test were administered to all subjects. The apraxia test includes three types of tasks, gesture production tasks that make meaningful movements according to verbal instructions, gesture recognition tasks that display behavior after make sense of meanings, and meaningless imitation task. Apraxia group(n=30, MMSE-K mean score: 25) showed lower cognitive function than group without apraxia(n=12, MMSE-K mean score: 28). All tasks in the apraxia test showed a significant correlation with cognitive function, but the meaningless imitation task had a negligible correlation. The apraxia test is a good way to assess cognitive function, and it may be more effective to use meaningful behavior to replace cognitive testing.

  • The purpose of this study is to provide a basic data for architectural planning on the wards and special quality of physical remedy spaces of geriatrics hospital. This research, to analyse dependency degree of inpatients in geriatric hospital, shows relativeness between remedy space and wards, and to show behavior of each patients reaction to there space. Result of interviewed inpatients who were sorted by age, sex, dependency degree, and days in hospital, suitable space programming is to need for physical and mental state. On the bases of survey and analysis, the basic data for planning and design the hospital for the elderly with dementia has been proposed.

  • 본 연구는 치매노인환자의 생활의 질을 향상시키기 위한 대화시스템의 개발에 목표를 둔다. 제안된 시스템은 주로 세 가지 모듈, 즉, 음성인식, 시간테이블에 의해 구분된 대화 데이터베이스의 자동검색, 그리고 간호사의 녹음음성에 의한 응답 등으로 구성되어 있다. 우선 치매환자가 요양시설에서 자주 발화하는 대화의 내용을 조사하고, 그들의 발화 음성을 인식하고 적절히 응답하도록 구성하였다. 시스템의 평가를 위해서 시스템이 도입되었을 때와 도입되지 않았을 때를 비교, 조사하였다. 시스템이 도입되지 않았을 때는 간호사가 자유로이 케어서비스를 행할 수 있도록 하였다. 비디오 촬영을 통해서 대상자의 행동 및 반응을 조사한 결과, 치매환자의 요구를 충족시키는데 있어서 대화 시스템이 간호사들보다 더 응답적이었다는 것을 알 수 있었다. 게다가, 제안된 시스템은 상호 대화에 있어서 환자가 더 많이 말하도록 유도함을 알 수 있었다.

Convergence factors among their physical state, function and activities influencing on the cognition of elderly residents in a community (지역사회 재가 노인의 인지수준에 영향을 미치는 신체상태와 기능 및 활동의 융합요인)

  • A descriptive research is studied to identify the physical factors affecting to cognitive function among elderly residents over 65 years old in a community. The data were collected from 352 participants as part of a health-related survey by home visits in Seoul in 2010-2012. Their cognitive function was measured using the Korean form of Hasegawa Dementia Scale(HDS-K) and the collected data were analyzed by using t-test, ANOVA, and multiple regression analysis. From the analysis, the prevalence rate of cognitive impairment was 13.6%. Elderly residents in a community showed different cognition levels by the status of age, change of weight, change of BMI, walking and flexibility exercise. Especially, the lowest cognition level was found in the normal BMI group with decreased BMI change by over 3. The factors influencing on cognition level of the elderly with normal BMI were age, change of weight, change of BMI, and walking exercise. The variance indicated 12.2% as their cognition level. Therefore, for preventing the cognitive impairment of the elderly that were rapidly decreased of BMI, we need the program to manage their nutrition and walking exercise.

Relationship Between Amyloid Positivity and Sleep Characteristics in the Elderly With Subjective Cognitive Decline

  • Background and Purpose: Alzheimer's disease (AD) is a neurodegenerative disease characterized by a progressive decline in cognition and performance of daily activities. Recent studies have attempted to establish the relationship between AD and sleep. It is believed that patients with AD pathology show altered sleep characteristics years before clinical symptoms appear. This study evaluated the differences in sleep characteristics between cognitively asymptomatic patients with and without some amyloid burden. Methods: Sleep characteristics of 76 subjects aged 60 years or older who were diagnosed with subjective cognitive decline (SCD) but not mild cognitive impairment (MCI) or AD were measured using Fitbit® Alta HR, a wristwatch-shaped wearable device. Amyloid deposition was evaluated using brain amyloid plaque load (BAPL) and global standardized uptake value ratio (SUVR) from fluorine-18 florbetaben positron emission tomography. Each component of measured sleep characteristics was analyzed for statistically significant differences between the amyloid-positive group and the amyloid-negative group. Results: Of the 76 subjects included in this study, 49 (64.5%) were female. The average age of the subjects was 70.72±6.09 years when the study started. 15 subjects were classified as amyloid-positive based on BAPL. The average global SUVR was 1.598±0.263 in the amyloid-positive group and 1.187±0.100 in the amyloid-negative group. Time spent in slow-wave sleep (SWS) was significantly lower in the amyloid-positive group (39.4±13.1 minutes) than in the amyloid-negative group (49.5±13.1 minutes) (p=0.009). Conclusions: This study showed that SWS is different between the elderly SCD population with and without amyloid positivity. How SWS affects AD pathology requires further research.

Association between Cognitive Function, Behavioral and Psychological Symptoms of Dementia and White Matter Hyperintensities in Patients with Alzheimer's Disease and Mild Cognitive Impairment (알츠하이머병 및 경도인지장애 환자에서 인지기능 및 행동심리증상과 백질고강도신호와의 연관성)

  • Objectives : The aim of this study is to investigate correlation between degree of white matter hyperintensities (WMH) and neurocognitive function along with behavioral and psychological symptoms of dementia (BPSD) in Korean patients with Alzheimer's disease (AD) and mild cognitive impairment (MCI). Methods : Participants were 115 elderly subjects diagnosed with Alzheimer's disease or mild cognitive impairment in this retrospective study. WMH in brain MRI were rated with standardized visual rating scales (Fazekas scales) and the subjects were divided into two groups according to Fazekas scale. Cognitive function was evaluated with Korean version of the consortium to establish a registry for Alzheimer's Disease (CERAD-K), and BPSD was evaluated with Korean neuropsychiatric inventory (K-NPI). Independent t-test was performed to analyze the relationship between the degree of WMH and neurocognitive functions & BPSD. Results : Especially, the group with high severity of WMH showed significantly lower language fluency (p<0.05). In addition, the group with high severity of WMH showed significantly higher score in K-NPI. Conclusions : There was a significant association between WMH and neurocognitive test related with executive function. Moreover, WMH seems to affect BPSD severity. Evaluation of WMH would provide useful information in clinical settings.

  • This study has a purpose to devise an education program in relation to Gerontology and geriatrics applicable to Korean nurses through literature review and analysis of education programs utilized by the Geriatric Education Centers (GECs) of USA. Educational contents on gerontology and geriatrics are very diverse in precedent literatures. The education programs of GECs for health care personnel are equally very diverse. Among educational contents, subjects considered important are age-related changes, health problems of the aged, pharmacology, death, fall and osteoporosis, delirium, dementia, depression, urinary incontinence, communication with elderly. Methods used in education program are mainly seminar, workshop and lecture through internet. In addition, case study, small-group discussion and conference are also adopted. The program proposed in this study for nurses in relation to gerontology and geriatrics consists of 32 hours' education; 6 hours for age-related changes, 10 hours for health problems of the elderly, 14 hours for health problems in old age and 2 hours of communication technique. Educational method proposed is to utilize lecture through internet, direct education, workshop, practical exercise, case study and role play in parallel. This study proposed an education program on the basis of precedent literature and the program of GECs. Therefore, it is desirable to develop in the future more practical education program applicable to and required in practicing fields. Of course, this development needs to be based on nurses' educational needs by field in relation to gerontology and geriatrics.

  • A bedridden patients, elderly people, and dementia who are subject to special care at a medical institution can not handle the feces themselves and need the help of a guardian or care-giver. In particular, toxic substances are contained in the stools, which can cause eczema, dermatitis and urticaria, so it is important to replace diapers. In this paper, we propose a wearable device design for the detection of excretions in consideration of the various excretion requirements of the elderly. The device is a form in which a module are attached to an adult diaper used in a nursing hospital to detect excreta, and the presence or absence of a wearer can be confirmed by an LED. The measured data is transmitted to the smartphone app in real time via Bluetooth in the module and can be checked for popup notification. The validity of this study was verified by comparing the actual excretion with the data collected through the designed module.
