2013년 3월 9일부터 6월 11일까지 대전대학교 부속 대전한방병원 한방재활의학과에 부동(immobility)으로 노쇠가 진행되어 보행장애 및 인지 기능 저하가 발생한 노인 입원환자 1명을 대상으로, 운동요법을 병행한 한방치료가 증상 호전에 미치는 영향에 대해 다음과 같은 결론을 얻었다. 1. 치료 후 양하지 도수근력검사는 4등급으로 근력이 향상되었다. 2. 슬관절 신전 수동운동범위는 -30/-30에서 0/-5도로 호전되었다. 3. 한국형 간이 정신상태 검사는 7점에서 25점으로 호전되었다.

Objective: Elderly people with dementia experience not only cognitive dysfunction but also motor function deficits, such as balance and gait impairments. Recently, transcutaneous electrical nerve stimulation (TENS) has been used to reduce pain as well as to control muscle spasm, spasticity and motor performance in various types of subjects. The purpose of this study determined the effect of a single trial TENS on balance and gait function in elderly people with dementia. Design: A randomized controlled trial. Methods: Twenty-two subjects with dementia were assigned to the TENS group (n=12) and ten subjects were assigned to the control group (n=10) randomly. Subjects were classified into two groups: the TENS group (n=12) and the placebo-TENS group (n=10). The TENS group had electrical stimulation applied on the calf muscle for 15 minutes, while the placebo-TENS group had not received real electrical stimulation. The timed up and go test (TUG) and functional reach test (FRT) were used to measure the balance function, and the 10 meters walk test (MWT) and 6MWT were used to assess gait ability. All tests were performed before and after intervention under a single-blinded condition. Results: After intervention, there were significant improvements in TUG, FRT, 10MWT, and 6MWT results in the TENS group (p<0.05), while the placebo-TENS group did not show significant changes in all outcome measurements. There were also differences in all tests between the two groups at post-measurements (p<0.05). Conclusions: This study demonstrated that a single trial TENS application on the calf may be used to improve balance and gait function in elderly people with dementia.
본 연구는 경도인지장애가 있는 노인을 대상으로 ICF 영역으로 분류하여 지역사회기반 중재의 효과에 대해 체계적 고찰 및 메타분석을 하고자 하였다. 2009년 1월부터 2019년 1월까지 국내 외에 게재된 연구로 PubMed MEDLINE, Cochrane CENTRAL, Google Scholar, 한국학술지인용색인, RISS, 국회도서관 데이터베이스에서 검색하였다. 선정논문의 실험군, 대조군의 대상자 수, 평균, 표준 편차를 사용하여 통계적 이질성, 효과 크기, 출판 편의를 산출하였다. 그 결과 총 5편 연구가 선정되었고, 대상자는 실험군 144명, 대조군 152명 총 296명이었다. 지역 사회 재활 프로그램의 전체 효과 크기는 8.83이였고, ICF 영역별 효과 크기는 신체기능 및 구조에서는 4.77, 활동과 참여에서는 6.17로 큰 효과 크기를 보였다. 본 연구의 결과를 통해 경도 인지장애가 있는 노인의 지역사회기반 재활의 신체기능 및 구조, 활동과 참여에 효과적임을 알 수 있었다.
Purpose : The objective of the current study was to evaluate the impact of the Computer-Based Cognitive Rehabilitation Program (CoTras) on the cognitive function and daily living activities of elderly and adult stroke patients. Methods : Twenty stroke patients were divided into two groups comprising 10 elderly stroke patients and 10 adult stroke patients. The CoTras was applied as the intervention for 30 minutes at a time, three times a week, for nine weeks (i.e., a total of 27 times), to both groups. The Neurobehavioral Cognitive Status Examination was used to assess cognitive function, and the Functional Independence Measure was utilized to evaluate daily living activities, prior to, during, and after the intervention. Results : The CoTras was demonstrated to have a statistically significant and positive effect on the cognitive function and daily living activities of stroke patients. However, the effect of the program on the restoration of weight-shifting capacity, as a component of the daily living activities of stroke patients, was without statistical significance. The program had a greater influence on improving the cognitive function and daily living activities of elderly stroke patients than adult stroke patients. Conclusion : This study makes a meaningful contribution to the literature on the topic as the intervention was demonstrated to lead to a more significant recovery of cognitive function and daily living activities in elderly stroke patients, compared to adult patients. Therefore, it is proposed that the CoTras should be used as a clinical intervention for elderly stroke patients. Future studies that evaluate the application of the CoTras, along with other occupation-based intervention programs, are warranted.
Background: This study had been carried out with 20 elderly subjects as its object for about one month from November 3, 2008 to December 14, 2008 in order to observe the effect of Underwater Treadmill on the elderly's walking and balance ability. Methods: Subjects were assigned either experimental group (n=10) or the control group (n=10), experimental group received Underwater Treadmill program (30 min per course, 3days a week for 6weeks). Subjects were assessed for muscle power (Nicolas Manual Muscle Test), balance (Functional Reach Test), gait ability (Time Up and Go, 10M walking test) before experiments and after experiments. Results: The results of this study were as follows; 1. After underwater treadmill exercise, the change of isometric contraction indicated a beneficial increase on lower extremity muscle power of experimental group and functional test of balance function; FRT, TUG indicated beneficial difference between groups. 2. beneficial difference between groups in walking speed of hourly index change of walking function. 3. between muscle power and balance, gait ability, we could find out there's high correlation ship between lower extremity muscle power increasing and balance and gait ability of the elderly. Conclusion: Aerobic exercise using underwater treadmill effects on muscle power strengthening of the elderly, and because of this, increase of lower extremity muscle power is very helpful not only to improvement of balance ability, but also to improvement of gait ability, so it will be used as a physical therapy program on clinic and used as an exercise program for protecting the elderly from falling down very well.

Objective: The purpose of this study was to analyze and compare vertical ground reaction forces during sit to stand (STS) and gait between female elderly and young individuals using the Wii Balance Board (WBB). Design: Cross-sectional study. Methods: Fifty-one female elderly people (age: $75.18{\pm}4.60years$), and 13 young people (age: $29.85{\pm}3.69years$) performed the five times STS test and gait respectively on the WBB. We analyzed time (s), vertical peak (%), integral summation (Int_SUM, %), and counter variables (%) in STS and 1st peak (body weight, BW%), 2nd peak (BW%), peak minimum (BW%), time (second), center of pressure (COP) path length (mm), and Int_SUM (BW%) in gait. The independent t-test was used to assess for differences in STS, gait ability, and general characteristics between the female elderly group and young adults group. With the first and last trials excluded, the mean value was obtained from the middle three of the five trials. Results: During STS, Int_SUM and time of young adults were significantly less than of the female elderly subjects. There were no significant differences in peak and counter variables. In gait, all variables (1st peak, 2nd peak, min, time, COP_path, and Int_SUM) showed significant differences between groups (p<0.05). This study demonstrated that the validity of vertical ground reaction forces occurring during STS and gait was significant in female elderly and young adults. Conclusions: Based on the measurement of vertical ground reaction forces in STS and gait using the WBB, it is possible to clinically improve the quality of geriatric physical therapy. Further studies are necessary to examine concurrent validity of elderly patients who have undergone total hip or knee replacement.
Purpose: The purpose of this study was to determine the effects of a fall prevention program on gait ability, one leg standing-right, one leg standing-left, fear of falling, fall efficacy, and depression in the elderly. Methods: A fall prevention program was consisted of fall preventing exercise with laughter therapy. Quasi-experimental design with one group pre- and post-test was used. Twenty-two elderly were participated in this study from a community health center located in a rural area, South Korea. SPSS/WIN 18.0 was used for descriptive analysis and paired t-test. Results: After 12 weeks with a fall prevention program, gait ability (t=3.44, p=.002), one leg standing-right (t=-3.73, p=.001), one leg standing-left (t=-4.17, p<.001) and fear of falling (t=2.12, p=.046) in the elderly were significantly improved. Conclusion: A fall prevention program was effective on physical and psychological functions. This study suggested that control groups and a larger sample should be included in order to validate the effects of a fall prevention program for the elderly. Objectives: Sarcopenia is a common disease in the elderly population that causes disability, poor quality of life, and a high risk of death. In the current study, we conducted a meta-analysis to report basic knowledge about the prevalence of sarcopenia in the elderly in Korea. Methods: We searched for articles in the MEDLINE, Cochrane Library, Embase, and Scopus databases published until December 28, 2020. Studies investigating the prevalence of sarcopenia in elderly Koreans aged ≥65 years were included. The methodological quality of the studies was evaluated using the Newcastle-Ottawa scale. Publication bias was evaluated using the Egger test and funnel plots. Results: In total, 3 studies and 2922 patients were included in the meta-analysis. All 3 studies used the European Working Group on Sarcopenia in Older People criteria for the diagnosis of sarcopenia. The total prevalence of sarcopenia was 13.1-14.9% in elderly men and 11.4% in elderly women. Conclusions: This meta-analysis is the first to estimate the pooled prevalence of sarcopenia in elderly Koreans, and its findings suggest that sarcopenia is common in this population. Therefore, attention should be paid to the prevention and control of sarcopenia.
Purpose : To estimate the prevalence of and identity the associates of cognitive function and depression among people over 70 years of age, and to examine the relationship between cognitive impairment and depression, Methods : The elderly in Pusan area were studied on the Korea version Mini Mental Scale Examination instrument(MMSE-K), Short form of Geriatic Depression Scale(SGDS). Results : The prevalence of cognitive impairment($MMSE{\leq}24$) was 64.4% and that of depression($GDS{\geq}8$) was 40.7% Lower education, no religion and no work experience were associated with cognitive impairment. Depression in demented group was 68.2%, in suspect dementia group was 43.7% and in non-demented group was 9.5%. Conclusion : The prevalence of cognitive impairment and depression in elderly was high and depression was in association with cognitive impairment.
