후각자극이 노인의 균형에 미치는 영향 (The Effects of Olfactory Stimulation on the Balance Ability of the Elderly)

  • Purpose: The purpose of this study was to investigate the effect of olfactory stimulation on the balance ability of the elderly. Design: Randomized controlled trial. Methods: A total of 20 subjects were randomly assigned to the olfactory stimulation group (2 males and 8 females) and deception olfactory stimulation group (3 males and 7 females). Subjects were subjected to 30 second 3 times olfactory stimulation and deception olfactory stimulation, and the assessment measured pre and post postural distance and postural speed. Results: In the elderly, the olfactory stimulation group was significantly improved the postural distance and postural speed in the eyes closed state than the deception stimulation group (p<.05). Conclusion: As the information is provided through the olfactory stimulation, the postural distance and postural speed are significantly improved. therefore, it is expected that the olfactory stimulation in the elderly will be an effective intervention method to prevent fall.

  • Studies on the health problems of the elderly have been increased with increasing life expectancy of Koreans. To assess the health status of people over 50 years old including the elderly (678 free-living people: men (168), women (510)), anthropometric measurements and biochemical nutritional status were analyzed. General characteristics and the prevalences of specific diseases were also examined. The mean anthropometric values for males and females were heights of 162 cm and 149 cm, respectively; and weights of 59 kg and 52 kg, respectively, which were much lower than the Korean standards established in the Korean Recommended Dietary Allowances. The mean BMI, WHR (waist hip ratio), and body fat rate (%), which are obesity indices, were significantly higher in females than in males. As the age went up, height, weight, lean body mass, and mid-arm circumference decreased. Whereas, blood pressure (SBP) for women increased. Mean blood HDL-cholesterol levels of men and women were 46.0 mg/dl and 46.3 mg/dl, respectively; their triglyceride levels were 129.1 mg/d1 and 135.6 mg/dl, respectively, which were not significantly different. Total cholesterol and LDL-cholesterol levels were significantly higher in females than in males. For man there were significantly different in ${\gamma}$-GTP, Na levels by age. As the age went up for women, RBC, hemoglobin, hematocrit, HDL-cholesterol, ${\gamma}$-GTP and $Ca^{2+}$ levels significantly decreased. Levels of total protein and albumin were above the lower normal limit for the majority of the participants. Prevalence of anemia, assessed by hemoglobin using World Health Organization (WHO) definition, was 43.9% for men and 42.1% for women. Also, many of the subjects were out of the normal range in total cholesterol (women), HDL-cholesterol, TAS (total antioxidant status), APase, RBC, hemoglobin, hematocrit levels. Therefore, measures to improve the biochemical nutritional status for the people in rural area are required.d.

농촌지역 노인의 영양상태와 활동량 (Nutritional Status and Energy Expenditure in the Elderly in a Rural Community)

  • In this study, nutrient intake status and energy expenditure were examined to investigate the nutritional status of the elderly in a rural community. The results obtained by questionaries, the 24 hour recall method, and time-diary were as follows: The elderly men surveyed were 73.8 years old, on the average. The elderly women surveyed were 73.5 years old, on the average. The proportion of the elderly with diseases was 51.9%. Most of the subjects (86.1%) had a regular meal pattern of consuming three meals a day. The average daily energy intake of the rural elderly was much lower than the Korean RDA. The dietary assessment data showed that each energy intake of the males and the females was 79.5% and 84.3% of the RDA, respectively. The dietary intake of Ca, Fe, niacin, thiamin, and riboflavin was lower than the Korean RDA, and that of P and Vitamin C was adequate. The Fe intake was significantly different with respect to age and sex (p < 0.05). Although, in both elderly men and elderly women it decreased with age, the elderly men's intake was lower than the elderly women's. The heights of the elderly men and the elderly women was 159.7 cm and 147.5 cm, respectively, and the weights were 60.0 kg and 52.2 kg, respectively, and the BMI was in the moderate range. Heights significantly decreased with age (p < 0.05). According to daily living schedules, leisure time (11.0 hour) was the longest, physiological time (9.6 hours) was next, and work time (3.4 hours) was the shortest. Energy expenditure significantly decreased with age (p < 0.01). Energy intake also decreased with aging. Energy balance (energy expenditure/energy intake) was 93.4% in elderly men and 104.0% in elderly women. Especially, in elderly men in the 65 to 74 age range, the energy balance was the lowest, and the nutrient intake was also much lower than that of elderly women.

  • The purpose of this study was to assess activation and contribution of muscles of the elderly according to increased loading during the arm flexion extension resistance exercise. Surface electromyographic signals were acquired from biceps brachii, triceps brachii, deltoid posterior, pectoralis major and latissimmus dorsi to determine the difference of the activation of specific muscles between the elderly and young. Five elderly and five young males with no musculoskeletal disease volunteered for the study. Electromyographic activities in the muscles were measured during resistance exercise and normalized to the maximum EMG activity recorded in the maximal voluntary static contraction (MVC). Against the increased loading during arm flexion/extension resistance exercises, the young uses muscles evenly but the elderly uses one specific muscle frequently. Contribution of triceps brachii during extension and deltoid posterior during flexion was principal in the elderly.

한국노인의 식품에 대한 개념 연구 (A study on connotative meaning of foods to elderly Korean)

  • This study was designed to investigate and quantify components of the connotative meaning of foods, and to analyze the correlations between food concepts and food frequencies on elderly Korean. It involves adapting a communications research tools, the semantic differential and demonstrating its use with two population groups, urban and rural aged. The data were collected by interviewing 217 males and females of 70 years of age and older living in urban and rural areas. To assess how they feel and what they know about foods, the instrument which contains concepts of price, taste, goodness of health, interest, usuality, likes and dislikes, appetite, fattening, quality, and nutritive value about foods were developed. The result shows that there are significant correlations between food concepts and food frequencies. The more affirmative concepts the elderly have, the more food frequencies tend to be. positive concepts are appeared on the meat, fishes, vegetables & fruits, Kimchi and the rice, but negative connotations are appeared on the milk and sugar. There are significant differences between the urban and rural elderly on food concepts.

고령 농업인의 위험요인 노출, 건강 문제, 직업 만족도 특성 분석 (Analysis of Hazard Exposures, Health Problems, and Job Satisfaction of Elderly Workers in Agriculture)

  • This study identified hazard exposures, health problems, and job satisfaction among elderly agricultural workers. A total of 1,340 agricultural workers (1,079 males and 261 females) were extracted from the 6th Korean Working Conditions Survey data. Three age groups (A: 40s or below, B: 50s, and C: 60s or above) were statistically compared in hazard exposures, health problems, and job satisfaction. Elderly agricultural workers (60s or above) had significantly more exposure to ergonomic hazards, severe health problems, and lower job satisfaction than young agricultural workers. The study findings may help understand the characteristics of hazard exposures, health problems, and job satisfaction of elderly agricultural workers and will be useful for improving these conditions.

일부 농촌지역 시설노인의 건강상태 (Health Status of Institutional Elderly in a Rural Area)

  • 일부 농촌지역의 시설노인의 인지기능과 일상생활 수행능력을 중심으로 건강상태를 측정하고자 하였다. 전라남도 10개의 노인복지시설에 수용된 485명 중 466명(96.1%)을 대상으로 조사를 하였다. 조사내용은 사회인구학적 특성, MMSE, ADL, IADL, 및 COOP chart이었다. 남자가 148명(31.8%)이었고 여자는 318명(68.2%)이었다. 평균연령은 남자 $73{\pm}8.2$, 여자 $79{\pm}8.5$세였다. 인지기능은 남자 45명(30.4%), 여자 115명(36.2%)에서 중증 인지기능장애를 나타내었다. ADL은 6항목 전체를 독립적으로 수행하는 경우가 남자 74명(50.0%), 여자 136명(42.8%)으로 나타났고, 6항목 전체를 수행하지 못하는 경우는 남자 46명(31.1%) 여자 83명(26.1%)으로 나타났다. IADL은 남자에서는 교통수단 이용 불가가 78명(52.7%)으로 다른 항목에 비해서 높았고, 여자에서는 빨래하기 항목이 가장 낮은 수행률을 보였다. COOP 차트로 살펴본 건강상태는 남녀 모두에서 신체기능에 비해서 감정상태와 사회활동에서 좋은 점수를 보였다.

경기일부 농촌노인의 식생활 향상을 위한 영양교육 효과 (A Study on Nutrition Education for Rural elderly of Kyungki province in Korea)

  • The purpose of this study was to assess the nutrition education program for older adults at rural area. Eighty three adults (mean age ;55.6$\pm$11.9) participated in nutrition education program. Data collection includes serum and urine collections for health state. The impact of nutrition KAP (knowledge attitude and practice) and retention was examined among participants who were assessed at program entry 3 times of education and 1 month of follow-up. The results were as follows : The participants were composed 52 persons of under 60-year-old and 31 persons over 60 years. Of the 83 participants, 60 were females and 23 were males. Serum albumin and calcium content of females (3.5$\pm$1.2g/dl, 7.5$\pm$3.9mg/dl) were significantly lower than males(4.7$\pm$0.9g/dl, 11.2$\pm$4.6mg/dl). Serum TG and cholesterol contents of females (143$\pm$117mg/d1, 205$\pm$31mg/d1) were higher than males (101$\pm$86mg/dl, 192$\pm$19mg/dl) but not significantly different. The calcium (13.4$\pm$9.0mg/dl) and urea (743$\pm$440mg/dl) contents of urine excretion were also significantly lower in female than in males (19.7$\pm$11mg/dl, 1041 $\pm$535mg/dl). Via the question of nutrition attitude, the participants improved significantly between pre and post education (the score of pre, post and after 1 month, 6.7$\pm$1.8, 7.2$\pm$1.5, 7.2$\pm$1.8), and were also attributed to better personal feeling health by modified CMI test but food habit was not significantly improved. As the differences of age and gender groups were compared, under 60-year-old group had better nutrition practice score than over 60 years group.

양로원 재원노인의 정신건강에 관한 연구 II. 우울증을 중심으로 (A study on Depression in the Elderly at the Home for the Aged)

  • 1986년 6월부터 1986년 8월까지 대구직할시 및 경상북도내 소재된 9개 양로원 재원노인 296명을 대상으로 실시한 Zung의 자가평가 우울척도상 평가의 결과는 다음과 같다. 1. 우울척도의 총점은 남자노인 $38.80{\pm}11.92$, 여자노인 $43.21{\pm}14.33$이었으며 남녀간 유의한 차이가 있었다(P<0.05). 2. 우울의 항목별 성적은 남녀노인 공통으로 절망감, 무가치감, 공허감, 혼동 등에서 높았고, 항목별 남녀간 비교에서 여자노인군이 남자노인군에 비해서 유의하게 높은 성적을 보인 항목은 울음, 빈맥, 식욕감퇴, 주간 감정변화 등 4개 항목이었다. 3. 우울성적 분포는 50점 이상 고득점자가 남자노인 9명(16%), 여자노인 47명(33%) 등 56명(29%)으로 여자노인군에서 더 많은 수를 보였다. 4. 우울과 관련된 사회정신의학적 요인들중 남녀노인군에서 공통적으로 영향력을 미친 요인은 연령의 증가, 군 이하의 출생, 대도시에서의 성장 등이었으며, 남자 노인군보다 여자 노인군에서 우울성적이 유의하게 높게 나온 요인은 무직, 무학, 면에서의 성장, 종교시 천주교나 기독교의 경우와 사별(P<0.05), 흡연을 하는 경우 등이었다.

노인의 영양섭취상태에 영향을 미치는 인구사회학적 요인 분석 (Sociodemographic Factors Associated with Nutrients Intake of Elderly in Korea)

  • In recent years, the number and proportion of Korean elderly have grown rapidly, and elderly individuals show a disproportionate risk for poor nutritional status. The purpose of this study was to examine the relationship of sociodemographic background to nutrient intake of persons 65 years of age or older, living in 15 cities in Korea. Data on 1973 subjects (603 males, 1370 females), who participated in the Korean Elderly Nutrition Survey (2000), were analyzed. Their mean age was 72.3 years and their mean body mass index (BMI) was 24.2 kg/$m^2$. Basic sociodemographic data were obtained through personal interviews. The 98-item semi-food frequency questionnaire, developed and previously validated for Korean middle-aged and elderly subjects, was administered. “Percentage of subjects who consumed under 75% Korean RDA,” “number of nutrients consumed below 75% Korean RDA,” “mean nutrient adequacy ratio,” and “nutrient density” were used to determine nutritional status. Male elderly had better nutritional quality than female elderly. Nutritional quality decreased with age, especially in older elderly (over 75). Elderly who were underweight (BMI 〈 20 kg/$m^2$) showed poorer nutritional quality than those who were normal weight (BMI 20∼25 kg/$m^2$) and overweight (BMI $\geq$ 25 kg/$m^2$). Elderly who lived alone had significantly poorer nutritional quality than those who lived with a spouse, and/or with children. Lower education level and economic dependence also showed lower nutritional quality. A stepwise multiple regression analysis was performed to examine the effects of specific sociodemographic factors on nutritional quality. For number of nutrients under 75% RDA as a dependent variable, education level explained 4.8% of the variance, followed by living status, age, body mass index, gender, and living expense support (Model $R^2$ = 0.091). For mean nutrient adequacy ratio as a dependent variable, model $R^2$ was 0.098. Therefore, sociodemographic variables such as gender, age, body mass index, living status, educational level, and economic status influenced elderly nutrition status. These results indicate that an elderly nutrition intervention should focus on subjects who are poorly educated, living alone, age 75 or older, and/or underweight.
