Objectives : The growing elderly population and social changes have fueled a rapid increase in demand for elderly care facilities, but health care services for the elderly, especially oral health services, have long been overlooked. The purpose of this study was to examine the oral health care of elderly residents in elderly care facilities in an effort to provide some information on the elderly's oral health care and the development of dental care programs geared toward institutionalized elderly people. Methods : The subjects in this study were 230 elderly people who were in elderly care facilities in Jeolla Province. An individual interview was held, and they got a dental checkup. As for data analysis, t-test and ANOVA were utilized, and Scheffe post-hoc test was employed. Results : 1. In relation to the subfactors of the quality of life related to oral health, the elderly people investigated got a mean of 4.58, 4.47, 4.38, 4.09, 3.94, 3.91 and 3.76 respectively in activity disorder, mental disorder, social disorder, mental inconvenience, functional disorder, physical pain and physical disorder. 2. Concerning the overall quality of life related to oral health, there were statistically significant gaps in this aspect according to gender, age, presence or absence of systemic disorder, presence or absence of eating difficulties, subjective health status, subjective oral health state and oral health concern. 3. The presence or absence of root caries had a significant relationship to physical pain and social disorder among the subfactors of the OHIP-14, but that had nothing to do with functional disorder, mental inconvenience, physical disorder, mental disorder and activity disorder. Conclusions : The oral health indexes of the institutionalized elderly people in Jeolla Province were measured, and what factors affected the subfactors of oral health was checked. As a result, there appeared a close relation ship between oral health and the quality of life. Therefore effective oral health plans that cater to the elderly should be carried out to improve elderly people's quality of life related to oral health who stay in long-term elderly care facilities. In the future, prolonged research should be implemented from diverse angles for the sake of institutionalized elderly people.
Background : This study is to identify the inappropriate hospital services for elderly inpatients over 65 years in general hospital with acute care functioning. Consequently elderly inpatient care and the management of long-term care facilities are key issues for current government health policy. Method : The survey was conducted for two months for all inpatients over 65 in 7 general hospitals, 6 work sampling days randomly selected. In each survey day, the subjective judgement by medical staff on the degree of acute care needs and by nursing unit manager on hospital services of each inpatients was also conducted. Result : The total number of cases collected are 2,541 elderly inpatients, according to subjective judgements by medical staff on inpatient condition. However 46.8% of cases are turned out to be non-acute care group. The frequency of medical services provided to non-acute group are 2~3 vital sign checks per day 78.2%, IV injection 40.1%, antibiotics medication 20.2%. Conclusion : Lots of elderly patients' who are staving in acute hospitals, at present need to be transferred to long-term care facilities. However, there was been shortage of long-term care facilities. It is expected to identify the need of elderly inpatients and therefore, to provide cost-effective, appropriate and good quality health services to elderly inpatients depending on their needs.
Objectives : This study aimed to investigate the types of unmet health care needs of the elderly and factors affecting them. Methods : This study used data from the 2012 Community Health Survey. Multinomial logistic regression analysis was conducted to identify the relevance between each type of unmet health care needs and predisposing, enabling, and needs factors, based on the cases without unmet health care needs. Results : Persons with unmet health care needs were 4,460 (9.5%) of the total sample. By types of unmet health care needs, 1,171 (2.5%), 1,026 (2.2%), and 2,263 (4.8%) persons reported inaccessibility, non-accommodativeness, and unaffordability respectively. It was concluded that the there were differences in the associated factors according to the types of unmet medical needs. Conclusions : It is suggested that unmet health care needs in the elderly should be examined from diverse angles rather than from a single aspect of partial limits. In particular, diverse types of unmet health care needs for health care in the elderly according to limited accommodation shoulder be examined. Finally, strategies to decrease unmet health care needs that reflect the associated factors should be developed.
Background: This study was conducted to analyze the impact of having a usual source of care on health behaviors of the middle aged and the elderly, in order to investigate the potential effect of enhancing primary health care in a Korean context. Methods: This study constructed a balanced panel of middle-aged and elderly samples using the Korea Health Panel 2016-2018, and fixed-effect models were used to analyze the data. Results: Among three sets of dependent variables (physical activity, smoking, drinking), statistically significant results were found only in physical activity. Subgroup analysis showed that this effect was not observed in the late elderly (aged 75 and older) and those without chronic diseases. Conclusion: Results of the study implied that enhancing primary health care among middle age and the elderly may have an effect on improving health behaviors. Moving forward to person-centered primary health care from disease-focused primary health care should be considered in high-risk groups such as the middle aged and the elderly with chronic diseases.
The subject of this study is to review the practical approaches of Home Care Services. Included is a brief overview of its nature, providers of Home Care Services, recent history of Home Care Services, and the impact of the national movement toward cost containment in health care. The data used in this study are obtained from the Elderly Program of the Medical Services and other data on the Home Care Services in Japan. With the growing elderly population in Japan, it is to be expected that the medical care expenditure for this sector will continue to increase. With the aim of keeping expenditure for medical care within reasonable bounds, it is essential that this increasing expenditure on the elderly be used effectively. With the Health and Medical Services Law for the Aged was enforced, therefore, remuneration for medical treatment of the elderly and what is known as the staff placement standard at hospital for the elderly were rationalized. In addition to rationalization from the point of view of medical care supply, it is necessary to guarantee the appropriate treatment within the community and at home for those elderly who are bedridden but not in need of hospital care. For this it is required that Home Care Services, such as health services like visiting guidance by public health nurse in hospital of Health Center. So that the elderly can feel secure in receiving treatment within the community and at home, allowances for guidance on leaving hospital and for intermittent nursing and guidance thereafter are to be newly introduced. Home care Services in one aspect of comprehensive health care, it is comprised of health services provided to individuals and families in their homes. Its purposes include promoting, maintaining and restoring health, specifically maximazing independent functioning and minimizing the disabling effects of illness, including terminal illness. Services appropriate to the needs of clients and their families are planned, coordinated, and delivered by providers organized for the delivery of home health care through the use of contractual arrangement, employed staff, or a combination of the two.
This study found a relationship between the characteristics of the elderly with disabilities and extra health care monthly costs caused by disability and suggests methods for effectively managing health care spending of the elderly with disabilities. This study used data from the 2011 Survey on the Disabled conducted by the Ministry of Health and Welfare and the Korea Institute for Health and Social Affairs. The results of the analysis indicate that the relations between the socio-demographic characteristics of the elderly with disabilities and extra health care monthly costs caused by disability of the elderly are influenced by factors such as gender, age, members of the household, average monthly income and type of health insurance. Elderly people with disabilities spend too much for their medical care. To solve this problem, the government needs to find a variety of policy approaches to improve administrative procedure, establish a medical delivery system and enhance health care services.
Objectives : The purpose of this study was to provide basic data for the development of general and oral health improvement programs for the elderly by analyzing the effect of health risk factors on the remaining teeth of the elderly in day care centers. Methods : The subjects were 294 elderly people who used day care centers in Seoul. Results : In terms of health risk factors, the elderly who did not have diabetes or smoke had more remaining teeth. In terms of oral health risk factors, the elderly with no dental caries, periodontal disease and dental pain had more remaining teeth. Conclusions : It is necessary to activate systematic welfare services for the elderly that fit their lifestyle in the light of the health and oral health risk factors of the elderly who use day care centers.
Objectives: Oral health management is important to improve the quality of life among the elderly. This study investigated the performance of elderly oral health management among some care workers in long-term-care hospitals. Methods: The study subjects were 174 care workers in 10 long-term-care hospitals. Data on general characteristics of care workers, attitude, recognition and knowledge of elderly health, performance of elderly oral health management were collected by a self-administered questionnaire. Data were analyzed through descriptive analysis, t-test, ANOVA, correlation and multiple regression analysis by using a SPSS version 23.0 statistical program. Results: The performance score of oral health management was $4.34{\pm}0.64$ on the 5-point Likert scale. The subjects who exercised more than 2 times a month were significantly higher in their performance of elderly oral health management compared to subjects who did not exercise (${\beta}=0.232$, p=0.035). And, the subjects who cared 10-19 persons were significantly higher in performance of elderly oral health management compared to subjects who cared more than 20 elderly (${\beta}=0.246$, p=0.020). The oral health behavior of care worker (${\beta}=0.271$, p<0.001) and the knowledge of oral health care (${\beta}=0.055$, p=0.008) were positively related to the performance of elderly oral health management. Conclusions: The educational program designed to improve knowledge of care workers in accordance with the standard textbook for training care workers should be developed, and the long term education program should be reinforced to improve the performance for elderly oral health care. If care workers can care a proper number of old persons, they will give oral health care to them.

Purpose: Needs of health-welfare-medical service for the elderly is rapidly increasing in Korea. The purpose of this study was to evaluate the needs of health-welfare-medical service for the long-term care elderly in the community and to compare differences by their characteristics. Method: Needs assessment was completed in the homes of 598 persons over 65 years by using the tool of needs assessment, between November and December, 2003. We examined all the health-welfare-medical service of elderly in the community. Data were analyzed using SAS program. Result: The needs of the long-term care elderly in community was largest 'home visiting service of visiting nurse(87.5%)', and then 'religious, psychological and emotional support(73.9%)', 'home visiting therapy of physician(58.5%)', 'social support service(55.7%)', 'health improvement program of public health center and social welfare center(51.8%)', 'health examination(48.8%)' followed. The difference of health-welfare-medical service needs among characteristics(age, medical security, caregiver existence, and regions) was statistically significant by service contents(p<0.05 or p<0.01). Conclusion: We can apply it in the distribution of community resource and the development of service providing programs by figure out the needs assessment for the long-term care elderly in the community, and consequently, through this, realizing the health maintenance and promotion of the long-term care elderly.
Purpose: The purpose of this study was to examine the effect of health literacy and self-care performance on health care utilization of medicaid elderly. Methods: A total of 203 medicaid elderly over 65 and living in B-metropolitan city were interviewed. Data were analyzed with descriptive statistics, t-test, ANOVA, Pearson's correlation coefficients and Hierarchical Multiple regression. Results: The average score of the health literacy was 7.88±2.84 out of 12. The average score of self-care was 3.26±0.77 points on the 5 point scale. The frequency of health care utilization by the subjects was 6.65±5.25 a month. The health literacy and self-care performance showed a statistically significant positive correlation. The health literacy and self-care performance showed a statistically significant negative correlation with health care utilization. The factors affecting health care utilization of the subjects were self-care performance, health literacy, and religion, and the explanatory power was 25%. Conclusion: The results indicate that the self-care performance, health literacy, religion are significant factors of health care utilization in medicaid elderly. Therefore, it is necessary to develop strategies to improve their self-care performance and health literacy for reasonable health care utilization. Also, it is necessary to provide the elderly with correct information about medical use from accessible religious institutions, senior community center, and welfare centers.
