Purpose: The purpose of this study was to identify the factors that affect quality of life in elderly people with dementia. Methods: The participants for this descriptive correlational study were 97 elderly patients with dementia who were attending a daycare center or were being cared for at home. Data were collected from the participants using the Mini-Mental Status Examination-Dementia Scale, the Geriatric Depression Scale (Short Form), Activity of Daily Living (ADL) and Instrumental Activities of Daily Living (IADL) Scale. Results: There were statistically significant differences in quality of life of elders with dementia according to mobility, using dementia day-care center, and cohabitation. In multiple regression analysis, depression, activity of daily of living, using dementia day-care centers and cohabitation were significant predictors of quality of life and the four variables explained 60% of the variance in quality of life. Conclusion: Findings indicate that to enhance the quality of life for elders with dementia, depression needs to be decreased and regular use of daycare service recommended. Further, various programs enhancing physical ability need to be developed and used with these elders.
본 연구는 한국과 일본의 노인입소시설 케어워커를 대상으로 노인에 대한 태도 및 치매지식이 치매에 대한 태도에 어떠한 영향을 미치는지 실증적으로 검증하였으며 나아가 치매에 대한 태도를 결정하는 요인에 있어 한일 간에 차이가 있는지 비교 분석 하였다. 조사는 한국의 케어워커 320명, 일본의 케어워커 350명을 대상으로 하였으며 설문지를 활용한 우편조사방법으로 이루어졌다. 자료 분석에는 t-test, 다중회귀분석을 활용하였으며 이에 따른 연구결과는 다음과 같다. 먼저, 한일 간에 노인에 대한 태도 및 치매지식, 치매에 대한 태도를 비교한 결과 치매지식에서는 유의미한 차이가 없었으나 치매에 대한 태도와 노인에 대한 태도는 한국에 비해 일본의 케어워커가 긍정적인 것으로 나타났다. 다음으로 치매에 대한 태도에 상대적으로 영향력이 큰 변인을 살펴보면 한국은 치매지식(β=.305 p<0.001), 일본은 노인에 대한 태도(β=.348 p<0.001)라는 것을 알 수 있었다. 둘째, 노인에 대한 태도와 치매지식은 한일 양국에서 공통적으로 치매에 대한 태도에 영향을 미치는 변인으로 나타났다.
In modern society, dementia patients are increasing due to busy daily life. Due to the nature of dementia, family members are having a hard time with their caring activities. Dementia diseases reduce the quality of life not only of the patient, but also of the family, and nursing activities lead to social problems such as unemployment or frequent turnover. This study aims to reduce the difficulty and stress of the nursing process by supporting daily life of dementia patients and nursing activities through smart care platform. Ultimately, we will reduce the cost of treatment for people with dementia and delay the worsening of symptoms through a systematic care program for people with dementia. The smart dementia care platform features smart technology to help the dementia patients' daily lives and support their caregivers.
Purpose: AIn recent years, the rapid progress of aging and the increase in the elderly people with dementia is becoming a big social issue. Therefore, this paper studies the development of therapeutic environmental rating scales for the elderly people with dementia in korean nursing homes. Methods: We identify the development process and structure of TESS-NH and understand the details of its nature and features. Then, based on the detailed items, we provide major scale factors which do influence on therapeutic environmental rating scales using PCA. Results: In the results, TESS, in the case of the initial stage of TESS-NH, has an advantage to evaluate on physical environments for a short period of time. TESS-2 + sets a large rage of activities of the elderly people with dementia, and tries to evaluate ongoing cares. TESS-NH has a complete set of well-thought-out assessment features for improving quality of care (Quality of Care) On the other hand, the main factors affecting the therapeutic environmental rating scales in nursing homes are "a consideration for facility management (Care for facilities' maintenance)", "a consideration for spatial cognition (Care for spatial cognition) ", and "a consideration for the safety (Care for safety)". Implications: In the future, to develop the therapeutic environmental rating scales on Korea's dementia nursing home, we should actively consider cares of spaces and facility utilization which the residents (the elderly people with dementia) are more secure and comfortable.
본 연구는 치매의 고위험군인 후기노인을 대상으로 치매인식도, 치매태도 및 신체활동을 조사하고, 치매인식도와 치매태도가 신체활동에 미치는 영향을 파악하여, 후기노인을 위한 맞춤형 복합 신체활동증진프로그램의 개발을 위한 기초자료를 제공하고자 시도되었다. 본 연구는 서술적 조사연구로 K군에 소재하는 9개 노인복지시설을 방문하는 75세 이상 노인을 대상으로 2016년 06월 01일부터 06월 30일까지 일대일 면담을 통한 설문지 작성으로 총 218명의 자료수집을 하였다. 수집된 자료는 SPSS/WIN 18.0 프로그램을 t-test, ANOVA, Pearson's correlation coefficient, hierarchical regression을 사용하여 분석하였다. 연구결과, 연구대상자의 치매인식도, 치매태도 및 신체활동은 모두 낮은 수준이었고, 치매인식도와 치매태도(r=.380, p<.001) 및 신체활동(r=.211, p=.002), 치매태도와 신체활동(r=.405, p<.001) 간은 모두 유의한 정적 상관관계가 있었다. 후기노인의 신체활동에 가장 큰 영향을 미치는 변수는 치매태도(${\beta}=.271$, p<.001)였고, 이외 성별(${\beta}=.207$, p=.003)과 교육수준(${\beta}=.184$, p=.023)이 유의한 영향을 미쳤으며, 이들 변수의 설명력은 28.8%이었다. 결론적으로 노인을 위한 맞춤형 복합 신체활동프로그램 개발시 특히 저학력 여성 후기노인에 초점을 둔 치매인식도 및 치매태도의 제고방안이 포함되어야 한다.
본 연구는 주간보호센터의 중풍노인이 이야기치료 집단상담을 통해 치매노인과의 갈등을 해결하는 과정을 탐색하는 연구이다. 중풍노인에 대한 이야기치료 집단상담을 7회 동안 실시한 과정을 사례연구방법을 통해 분석하였다. 분석결과 첫째, 중풍 노인들이 집단상담을 통해 치매노인을 '문제'가 아니라 '치매로 고통받는 사람'으로 보고, 치매노인을 비난하는 대신 도와줄 방법을 적극적으로 찾게 되었으며, 둘째, 자신들을 '치매노인의 피해자'가 아니라 '유능한 사람 으로 규정하게 되었고, 셋째, 중풍노인들끼리 협력적이고 친밀한 관계가 되었으며, 넷째, 중풍노인들이 주간보호센터 운영에 적극적으로 참여하게 된 것으로 나타났다. 이러한 변화에 이야기치료의 문제와 사람을 분리시키기, 클라이언트를 자기 문제의 전문가로 존중하기, 독특한 결과 찾기 등이 도움이 된 것으로 분석되었다. 이러한 결과를 바탕으로 장기요양보호대상자에게 보호를 제공할 뿐만 아니라 참여 기회를 확대하고, 이야기치료를 노인에게 적용할 것을 제안하였다.
본 연구는 레크리에이션 치료 프로그램이 치매 노인들의 인지기능과 균형감각에 미치는 영향을 조사 분석하고자 실시되었다. 연구대상은 알츠하이머형 치매를 진단받은 노인 16명을 대상으로 6 개월 동안 레크리에이션 치료 프로그램을 제공하였으며, 실험군과 대조군 두 그룹의 프로그램 제공전과 후의 인지기능과 균형감각의 변화를 분석하였다. 연구결과 레크리에이션 치료 프로그램은 알츠하이머형 치매 노인의 인지기능과 균형감각을 유지시키거나 증진시키는 것으로 보고되었다. 연구 대상자 모집과 손실 및 레크리에이션 치료 프로그램 기간과 같은 변수 통제의 어려움을 제한점으로 볼 수 있으나 레크리에이션 치료와 같은 비약리적 방법의 개발과 효과성 검증의 측면에서 치매 노인의 재활치료 연구에 기여하였다고 판단된다.
The purpose of this study was to explore the degree of caregiving stress in housewives according to functional impaiment and personal characteristics of demented elderly, and related variables of housewives. The subjects of this study were 141 housewives who had cared for dementia elderly. A questionnaire was used as survey method. The data was analyzed by means of t-test, ANOVA, and Scheffe test. The main findings were as follows: First, there was significant difference in the degree of stress in caring for dementia elderly according to functional impairment of demented elderly. Third, the degree of stress in caring for dementia elderly differed significantly according to education, income, and housework time per day of housewives, and relatinship demented elderly with housewife.
As the population ages, health problems of the elderly, including functional disorders and cognitive impairment, have become some of the most important social issues in Korea. The purpose of this study is to discover the rate of prevalence and factors related to dementia among the elderly in Jeju Province. 2405 elderly in Jeju Province were interviewed, using a questionnaire consisting of sociodemographic characteristics: health behavior, quality of life, and MMSE-K. Prevalence of dementia was 9.4%(11.7% of females, 4.1% of males). Factors related to dementia, analyzed by multiple logistic regression, were age, working status, subjective health status, social support, and subjective economic status. In community health care for the elderly, factors relating to dementia have to be considered. When planning community health care, priority should be given to the elderly who: need care but live alone; lack social support; have a low standard of living; believe they are not in a good state of health; or whose life satisfaction is low.
Concerns about paid-facilities for the elderly with dementia from middle and upper class families have been discussed lately in Korea. Small-scale, home-like group homes for the elderly with dementia were developed in Sweden in the 1980, but they have not been effectively introduced in Korea where they remain a comparatively new concept. The group home, however, would provide a good alternative to Koreans who feel guilty when they have to leave their frail parents in large-scale facilities or hospitals instead of caring for them at home. The aim of this paper is, first, to define the care of the demented elderly who come from upper and middle class families in Korea and, secondly, to discuss the need for group homes to help care for them. A survey was done by questionnaire and was answered by 577 respondents who belonged to middle and upper class families living in Korea in December 2000. Methods of analysis were frequency, mean, and Chi-square. The results of this study were as follows: 1) the traditional notion that the eldest son should be burdened with the primary responsibility for a demented parent has weakened remarkably; 2) a small-scale long-term care facility was considered the most desirable living environment for the demented elderly; 3) the facilities families needed for professional hospitals for victims of dementia, senior citizen centers, large and small-scale long-term care facilities, day-care centers, and short-stay and service housing. The study also revealed that there was low awareness of the group home. The most preferred type of structure for group homes was a single-family detached house, and the most preferred management system was small-scale that could create home-like atmosphere. Additionally the respondents wanted group homes to consist of 6-8 residents with 2-4 persons per room. The results of this study strongly suggest that policy makers should encourage the development of smallscale group homes as an alternative form of housing for the elderly with dementia.
