  • The objective of this study is to explore the situations and extension roles for rural elderly welfare program in North Dakota, USA. With a growing older population, aging has become an important issue for extension. Services for rural elderly available in North Dakota were adult day care, home health care, senior insurance counseling, nutrition and medication assistance programs, support groups, legal assistance, meals on wheels, nursing homes and more. With a growing older population, aging has become an important issue for extension. Extension provides programs and services for rural elderly. This study was conducted by literature review. First, many rural elderly Americans are actively engaged in volunteer work and have made substantial contributions to their communities. Second, extension educators from interdisciplinary areas should work together to develop programs. Extension programs can include intergenerational programs to help younger generations learn about the issue. Third, extension can collaborate with other agencies and groups to offer support groups. Offering educational programs is a key to empowering older people. Fourth, elderly residents may be the only increasing natural resource for volunteering in general, and for participation in community improvement in particular. Fifth, extension educators should be proactive in working with agencies to provide social access and in helping older people be actively engaged in their lives, especially in rural areas.

The data were collected using a questionnaire from March 16 to April 15, 2007. The questionnaire return rate was 43.8%. Result: The hospital-based home care agencies were able to visit 66.5% of the national administrative districts. Of the home care service recipients, over 50% were 65 years old and older. About 43% of the agencies reported that over 50% of their patients would be subject to the long-term care insurance. They expressed concern that home care services would be withdrawn once the insurance system is initiated. Conclusion: This study suggests that hospital-based home care agencies need to manage home care services with long-term care insurance. • This study investigates the effects of serving medicinal food items on the dietary intake of elderly inpatients. A total of 77 elderly inpatients at a long-term senior care hospital in Yeongju participated in this study. Control and medicinal food items were provided for 20 days (March 24 through April 12, 2014, and April 14 through May 3, 2014, respectively). A satisfaction survey was conducted for both control and medicinal food items on the last day of the respective serving period. Data were analyzed by using SPSS (Windows ver. 21.0). The satisfaction level was higher for medicinal food items than for control food items. The leftover rate was higher for control food items than for medicinal food items. The intake of energy, carbohydrates, proteins, fats, vitamin A, thiamin, niacin, potassium, and zinc was higher for medicinal food items than for control food items. These results indicate that the dietary intake of elderly inpatients was higher for medicinal food items than for control food items. This suggests that medicinal food items can help increase the nutrient intake of elderly inpatients and implies that such food items may be more desirable for a long-term senior care hospital diet. A prospective analysis of the long-term control is needed to establish the effects of medicinal food items on elderly patients.

  • Purpose: The purpose of this study was to examine self care behaviors according to the risk levels of vascular complications in elderly women with hypertension. Methods: The subjects of this study were 162 women living in the community who had been diagnosed with hypertension in clinics. The data were analysed by the SPSS 10.0 program using descriptive statistics, t-test, and ANOVA with Scheff$\dot{e}$ post-hoc test. Results: The average self care behavior score of the elderly women was 2.79. Total self care behavior was significantly different depending on the risk levels of vascular complications such as systolic blood pressure, diastolic blood pressure, ankle-brachial index, and the framingham point score. Among sub-scores of self care behaviors, exercise management was the poorest performance compared with other self care behaviors. Conclusion: This study proved the differences in self care behaviors according to the risk levels of vascular complications. To decrease the prevalence of vascular complication, it is necessary to develop programs specifically to enhance self care behaviors of elderly women with hypertension.

  • Purpose: The purpose of this study was to examine the mediating and moderating effects of fall-related efficacy and social support on the relationship between frailty and health-related quality of life among rural community-dwelling elderly. Methods: A cross-sectional survey was conducted with a convenient sampling method, and data of 438 elderly residents living in a rural community was used. The structured questionnaire included items from the Euro Quality of life-5 Dimensions (mobility, self-care, usual activities, pain/discomfort, anxiety/ depression), fall-related efficacy and social support. Results: Analysis of the mediating effect of fall-related efficacy and social support showed that there was significant mediating influence of fall-related efficacy on the relationship between frailty and health-related quality of life. There were no moderating effects of fall-related efficacy and social support. Conclusion: The findings suggest that fall-related efficacy may play a role in reducing the effect of frailty on health-related quality of life and underscore the need to consider ways of enhancing fall-related efficacy in interventions for rural community-dwelling frail elderly.

  • Purpose: This study investigated evidence for the effects of community-based cognitive rehabilitation programs for demented elderly. Methods: The literature review covered studies between 2004 and 2014, and searched international databases including Ovid-MEDLINE, EMBASE, and the Cochrane Library. The Risk of Bias Assessment Tool for Non-Randomized Studies was used by two reviewers to evaluate independently the quality of the selected articles. Results: Eighteen studies were selected through a systematic search of the literature. About three quarters of the studies showed that the community-based cognitive rehabilitation programs provided by nurses were significantly effective in enhancing cognition among elderly individuals with dementia. Conclusion: Based on these results, we suggest that nurse is an appropriate profession to manage cognitive rehabilitation or cognitive enhancement programs for demented elderly individuals. Therefore, the current system in which caregivers provide cognitive rehabilitation programs for demented elderly individuals must be reformed.

  • The purpose of this study was to understand experiences of successful aging of community-dwelling elderly living in the Korean community using mixed methods. This study is a mixed methods research using convergent parallel design. 483 elderly people living in the 2 cities were selected as subjects to collect quantitative data. For qualitative data collection, 6 elderly people participated. The level of successful aging for elderly people living at home scored an average of 4.30 ± .59 out of 5 points (4.46 ± .64 for self-efficacy, 4.31 ± .65 for good self-control, 4.29 ± .70 for satisfaction with children's success, and 4.26 ± .61 for partnership with spouses). The main themes of successful aging derived by interview were as follows: 1) fulfill responsibilities of raising children as parents, 2) establish a stable life to overcome difficulties, 3) accept the body being different from the past, and 4) live in harmony with people around them. Based on these results, it is necessary to provide useful health care services that can be used in the local community and to provide service utilization information through various routes in order to achieve a successful aging.

  • Purpose: This study aimed to investigate the effects of a nurse-led community comprehensive exercise program on the body composition, physical function, and health-related quality of life in elderly patients with osteoporosis. Method: The study was conducted with one group pretest-posttest design. A total of 57 elderly patients participated in 8 weeks of intervention. Data was analyzed with the SPSS ver. 23.0 using descriptive statistics and paired t-test. Results: At the end of the intervention, body mass index was significantly increased (t=2.93, p=.005), but right leg balance (t=2.40, p=.02) was significantly improved. In addition, the total Korean-Western Ontario and McMaster Universities Osteoarthritis Index (K-WOMAC) (t=3.48, p=.001), knee pain (t=2.61, p=.012), stiffness (t=2.53, p=.014), and physical function (t=3.51, p=.001) were significantly decreased. EuroQoL Visual Analogue Scale (EQ-VAS) scores (t=4.25, p<.001) were significantly improved. Conclusion: The nurse-led eight-week community comprehensive exercise program did not show desirable change in the body composition but was effective on the physical function and health-related quality of life for older people with osteoartritis.
