A Clinical Analysis of Acute Acromioclavicular Dislocation (견봉쇄골 관절탈구의 수술적 치료의 비교)

  • The management of acute acromioclavicular dislocations has usually followed the accepted principles of obtaining an anatomical reduction of the joint and maintaining it until soft tissue healing has occurred. So, the preferred treatment for acute acromioclavicular dislocation is controversial. We analysed operatively treated twenty-eight cases for acute acromioclavicular dislocation between February 1994 and January 1997 and reviewed postoperatively to evaluate the results of three different methods. We collected retrospectively the data via clinical history, associated injury, type of injury, radiographic review, Taft score, and final results. Follow up time averaged 14 months. (range, 12 to 21 ) In according to Rockwood's classification, ]7 cases were type Ⅲ, 1 case was type IV, and 10 cases were type V. Ten cases were treated with the modified Phemister method, ten cases with the modified Bosworth method and eight cases with the modified Weaver-Dunn method. ]n patients treated by modified Phemister method, the Taft score was 9.4 points and 8 cases achieved good or excellent results. In patients treated by modified Bosworth method, the Taft score was 9.8 points and 8 cases achieved good or excellent results. In patients treated by modified Weaver-Dunn method, the Taft score was 10.3 points and 7 cases achieved good or excellent results. The overall Taft score was 9.9 points and 23 cases achieved good or excellent results. There were four complications, such as calcification or metallic loosening or breakage of K-wire, but did not influence late results. In conclusions, there was no significant difference of results regarding the different three methods. However, our results indicated that the coracoclavicular ligament reconstruction by transfer of coracoacromial ligament produced better results.

  • In this paper, we proposed an electromyography(EMG) based control method of a virtual robot arm as an adaptive human supporting system or remote control system, which consists of an shoulder control part, elbow control part, and wrist control part. The system uses four surface electrodes to acquire the EMG signal from operator. It is shown from the experiments that the EMG patterns during arm motions can be classified sufficiently by using SOM and LVQ. The interface system based on PC environment is constructed to 3-D graphic user interface(GUI) program. Experimental results show that proposed method obtains approximately 94 percent of success in classification.

  • Software vulnerabilities are becoming more and more increasing, their role is to harm the computer systems of companies, governmental organizations and agencies. The main objective of this paper is to propose a method that will cluster future software vulnerabilities that may spread. This method is developed by combining the Multiple Correspondence Analysis (MCA), the Elbow procedure and the Kmeans Algorithm. A simulation was done on a dataset of 15713 observations. This simulation allowed us to identify families of future vulnerabilities. This model was evaluated using the silhouette index.

Evaluation of the Surgical Treatment for Chronic Acromioclavicular Joint Injury; Weaver and Dunn Method Versus Acromial Bone Block Transfer (오구견봉인대만을 이용한 술식과 견봉골편을 포함한 오구견봉인대를 이용한 변형 Weaver and Dunn 술식에서의 만성 견봉쇄골관절 손상 치료결과 추시)

  • Purpose: The purpose of our study was to compare treatment results of two different surgical techniques for chronic acromioclavicular joint dislocations. Materials and Methods: Fifty consecutive patients diagnosed as chronic acromioclavicular joint dislocations between January 1997 and June 2009 were included in the study. Patients were randomized into two different groups. Patients in the first group (n=20) were treated using a modified Weaver and Dunn method using a simple coracoacromial ligament transfer method. Patients in the other group (n=30) were treated with acromial bone-block transfer containing coracoacromial ligament. Mean follow-up times for the two groups were 13.1 and 14.9 months, respectively. Results: At 1 year postoperatively, mean coracoclavicular distance, the VAS score and the ASES score for the Weaver-Dunn method group; for the bone block transfer method group were 3.8 mm (-3 to 6 mm), 3.5 (1.0 to 7.0) and 91.1 (81.66 to 95); 3 mm (-2 to 6 mm), 4.2 (1.0 to 7.5) and 79.6 (31.66 to 95). There were no significant differences in radiologic (p=0.377) and functional (p=0.093) results between the two groups. Failures in the former and latter group were, respectively, two and one. Conclusion: The bone block transfer method shows a tendency to maintain coracoclavicular distance and appeared to yield similar results as the modified Weaver Dunn method.

A Virtual Robot Arm Control by EMG Pattern Recognition of Fuzzy-SOFM Method (가상 로봇 팔 제어를 위한 퍼지-SOFM 방식의 근전도 패턴인식)

  • We proposed a method of a virtual robot arm controlled by the EMG pattern recognition using an improved SOFM method. The proposed method is simple in that the EMG signals are used as SOFM's input directly without preprocessing but nevertheless input patterns are reliably classified and then used for fuzzy logic systems to automatically tune the neighborhood and the learning rate. In order to verify the effectiveness of the proposed method, we experimented on EMG pattern recognition of 6 movements from the shoulder, wrist, and elbow. Experimental results show that the proposed SOFM method has 21.7% higher recognition rate than the general SOFM method, the average number of learning iterations has been decreased, and then the virtual robot arm is controlled by EMG pattern recognition.

Applicability of the mα-tangent Method to Estimate Plastic Limit Loads of Elbows and Branch Junctions (선형탄성해석과 mα-tangent방법을 이용한 배관 한계하중 평가 적용성)

  • In this study, the limit loads calculated by the $m_{\alpha}-tangent$ method based on the linear finite element analysis are compared with the closed form solutions that are proposed by various authors. The objects of the analysis is to select the elbow and the branch pipe which are representative structure of piping system. The applicability of the $m_{\alpha}-tangent$ method are investigated by applying it to cases with various geometries. The internal pressure and the in-plane bending moment are considered and the $m_{\alpha}-tangent$ method is in good agreement with the existing solutions in case of elbows. However, the limit loads calculated by the $m_{\alpha}-tangent$ method for branch junctions do not agree well with the existing solutions and do not show any tendency. The reason is a biased result due to the stress concentration of the discontinuous parts.

  • In the present study, a new method of generating boundary-fitted coordinates systems controlled by control function is introduced. Application of the method to a three-dimensional simply-connected region is the demonstrated. The numerical grid generation has following feat ures, (a) The generated boundary fitted coordinates is well concentrated in near wall region and satisfied orthogonality, (b) The grid control function is fully automatic and well controlled in sharp convex boundary.

  • CF8M cast austenitic stainless steel is used for several components such as primary coolant piping, elbow, pump casing, and valve bodies in light water reactors. These components are subject to thermal aging at the reactor operating temperature. Thermal aging results in spinodal decomposition of the delta-ferrite leading to increased strength and decreased toughness. In this study, three kinds of the aged CF8M specimen were prepared using an artificially simulated aging method. The objective of this study is to summarize the method of estimating ferrite contents, Charpy impact energy and J-R curve, and to evaluate the thermal embrittlement of the CF8M cast austenitic stainless steel piping used in the domestic nuclear power plants.

An Analysis of Acoustic Field for Turbo Chiller Discharge Duct by Using Boundary Element Method (경계요소법을 이용한 터보냉동기 덕트의 내부 음향장 해석)

  • The turbo chiller uses centrifugal compressor, which operates at about 14,500 rpm. Due to the high rpm of the impeller, the noise of chiller males one of the serious problems. The possibility of the sound reduction by using absorbing material is studied in this paper. The generated sound propagates through the duct and then radiates to the outer field. So, the use of sound absorption material inside the duct is one of the effective methods. To study the effect of location of the material, we use Boundary Element Method to analyze the sound field inside the duct system. Numerical study shows the highest sound pressure region is near the elbow of curved duct. From the numerical study, it is also shown that appropriate use of sound absorbing material at this region makes 8dB reduction of the highest noise level.

Arthroscopic Rotator Cuff Repair: Serial comparison of outcomes between full-thickness rotator cuff tear and partial-thickness rotator cuff tear

  • Purpose: To compare the results of arthroscopic rotator cuff repair and subacromial decompression in partial thickness rotator cuff tear (PTRCT) with those in full thickness rotator cuff tear (FTRCT). Subjects and method: Of the 46 patients who were rested of the rotator cuff tear based on the operational findings, 42 patients who were able to receive a serial follow-up for 2 years were selected as the study subjects. The average age of the patients at the time of the operation was 55 years, and the mean duration of the follow-up was 34 months. The subjects included 22 cases of PTRCT and 20 cases of FTRCT. In terms of rotator cuff repair, the average number of tendon to tendon repair (TTR) was 1 in both PTRCT and FTRCT, and that of tendon to bone repair (TBR) was 1 and 3 in PTRCT and FTRCT, respectively. The average number of use of suture anchor was 1 and 2 in PTRCT and FTRCT, respectively. The level of shoulder pain and function of the subjects were measured using shoulder functional evaluation score of American shoulder and elbow society (ASES score) at before and 2 years following the operation. Results: At the final follow-up following the operation, PTRCT group showed changes in scores from 7.2 to 0.9 on average pain score and 34 to 91 on ASES score, whereas FTRCT group showed changes in scores from 7.6 to 1.2 on pain score and 29 to 88 on ASES score. There were no significant differences between the two groups (P > 0.05). The average range of motion of shoulder significantly increased in both groups at the final follow-up in comparison with the pre-operative time point. The evaluation at the final follow-up showed that 93% of the total subjects showed good or excellent results, and 95% showed satisfactory results from the procedure with regard to pain reduction and functional outcomes. Two cases of the 3 fair results were caused by acromioclavicular arthritis. Conclusion: It may be anticipated that arthroscopic rotator cuff repair and subacromial decompression may bring satisfactory post-operative outcomes in both PTRCT and FTRCT on pain relief and functional recovery. However, careful preoperative examination of the acromioclavicular joint is critical to avoid failures of these procedures.
