Effect of flexion degrees in elbow joint on muscle activation of the extensor carpi radialis and biceps brachii muscles in healthy young adults

  • Objective: Chronic lateral epicondylitis is a condition which becomes sore and tender on the lateral side of the elbow joint damaged from overuse and repetitive use of the extensor muscles of the forearm. The purpose of this study was to investigate the effects of flexion degrees in the elbow joint on extensor carpi radialis longus and brevis and biceps brachii muscles in individuals with healthy young adults. The main purpose of this study was to suggest the feasibility of optimal elbow angle during therapeutic eccentric exercise with resistance for strengthening of wrist extensors. Design: Cross-sectional study. Methods: Thirty health young adults (male 15, female 15) participated in this study. This study measured muscle activation in four different conditions of elbow flexion, $0^{\circ}$, $30^{\circ}$, $60^{\circ}$, and $90^{\circ}$ during eccentric exercise with weight loading in wrist extensors, extensor carpi radialis longus and brevis and biceps brachii muscles using surface electromyography. Results: The muscle activation of extensor carpi radialis showed a negative relationship with the degrees of elbow joint flexion. With increasing elbow flexion angles, the ECRL muscle activation amount was significantly lower (p<0.05). In contrast, the muscle activation of the ECRB muscle activation amount was significanlty higher (p<0.05). Conclusions: This study suggests that the eccentric exercise of wrist extension with selected activation of wrist extensor muscles according to elbow flexion positions, and suggests that the extensor carpi radialis longus and brevis will need to be strengthened for preventing and treating chronic lateral epicondylitis regardless of degrees of elbow joint flexion.

Influence of Head-Neck Rotation on Elbow Flexor and Extensor Muscle Activity and Strength in Normal Adults

  • Purpose: This study examined the effects of the directions of neck rotation position on the muscle activity and strength of the elbow flexor and extensor muscle. Methods: Forty-one healthy adults participated in this study. The subjects were asked to their elbow 90° flexion in three different neck rotations (neutral, ipsilateral, and contralateral) in the sitting position. The muscle activities of the biceps and triceps brachii muscle were measured using surface electromyography. And the muscle strength of the elbow flexor was measured using dynamometer. One way repeated measures ANOVA was used to compare the muscle activity and strength of the elbow flexor and extensor depending on the different neck turning directions. Results: There were significant differences between contralateral neck rotation and ipsilateral neck rotation, contralateral neck rotation and neutral position. But, there was no significant difference in the triceps brachii muscle activity in comparison with the neck rotation. There were significant differences between contralateral neck rotation and ipsilateral neck rotation, contralateral neck rotation and neutral position. Conclusion: To summarize this study, the elbow flexor and extensor muscle activity and strength was highest in the contralateral neck rotation position. In other words, it was possible to confirm the effect of Asymmetrical Tonic Neck Reflex in healthy adults whose primitive reflexes were inhibition, and head and neck positions should be considered during clinical evaluation and treatment.

미세전류 자극 강도에 따른 지연성 근육통의 통증과 관절가동범위에 미치는 영향 (The Comparison of Effect of MC Intensity in Pain and ROM in Delayed Onset Muscle Soreness)

  • Purpose : We investigated the effects of microcurrent(MC) electrical stimulation on each intensity($100{\mu}A,\;200{\mu}A,\;500{\mu}A$ - 30pps frequency was same) on delayed onset muscle soreness(DOMS). Methods : Subjects were assigned randomly divided into three groups of eight for three different treatment protocoals($100{\mu}A,\;200{\mu}A,\;500{\mu}A$-experimental groups). Twenty-four healthy males and females subjects were participated in this study. All subjects performed eccentric exercise of elbow flexor(biceps brachii) until exhausted. The measured items of elbow flexor muscle strength were Nicholas Manual Muscle Taster(NMMT). The measured items of elbow joint range of motion ROM) were Goniometer. The measured items of elbow flexor muscle pain were visual analogue scale(VAS). Treatment were applied at 30 minute exercise after and again at 24 hours and at 48 hours and at 72 hours after. Measurements were taken after treatment. Analysis of Results using repeated measures analysis of variance(ANOVA) and post hoc tests were as follows: two-way ANOVA with repeated measurement for muscle strength, flexion ROM, extension ROM and VAS. Results : This results showed eccentric exercise casused DOMS, DOMS response to eccentric exercise were reduces by microcurrent therapy. DOMS was significant decreased at $100{\mu}A,\;200{\mu}A,\;500{\mu}A$. Muscle strength was significant difference at all intensity. Elbow flexion ROM was significant difference at all intensity but elbow extension ROM was insignificant difference at all intensity. VAS score was significant difference at $100{\mu}A$ and $500{\mu}A$ but insignificant difference at $200{\mu}A$. All experimental groups showed insignificant difference with all intensity MENS. Conclusion : These findings indicate that microcurrent therapy is had effect on recovery from exercise induced muscle damage. In our's suggestion, microcurrent therapy is particularly more appropriate therapeutic modality.

Radiologic Comparison of Humeral Position according to the Implant Designs Following Reverse Shoulder Arthroplasty: Analysis between Medial Glenoid/Medial Humerus, Lateral Glenoid/Medial Humerus, and Medial Glenoid/Lateral Humerus Designs

  • Background: The currently available reverse shoulder arthroplasty (RSA) designs can be classified into medial glenoid/medial humerus (MGMH), lateral glenoid/medial humerus (LGMH), and medial glenoid/lateral humerus (MGLH) prosthesis designs. The purpose of this study was to radiologically analyze the effect of different RSA designs on humeral position following RSA. Methods: A total of 50 patients who underwent primary RSA were retrospectively analyzed. Among 50 patients, 33 patients (group A: MGMH) underwent RSA with Aequalis system (Wright, Inc, Bloomington, MN, USA), 6 (group B: LGMH) with Aequalis system using bony increased offset, and 11 (group C: MGLH) with Aequalis Ascend Flex system. The acromiohumeral distance, acromioepiphyseal distance (AED), lateral humeral offset (LHO), LHO from the center of rotation ($LHO^{COR}$), and deltoid length were radiologically measured to quantify the distalization and lateralization of the humerus. Results: The increment in postoperative AED was $19.92{\pm}3.93mm$ in group A, $24.52{\pm}5.25mm$ in group B, and $25.97{\pm}5.29mm$ in group C, respectively (p=0.001). The increment in postoperative LHO was $0.13{\pm}6.30mm$, $8.00{\pm}12.14mm$, and $7.42{\pm}6.88mm$, respectively (p=0.005). The increment in postoperative $LHO^{COR}$ was $20.76{\pm}6.06mm$, $22.04{\pm}5.15mm$, and $28.11{\pm}4.14mm$, respectively (p=0.002). Conclusions: The radiologic analysis of the effect of different RSA designs on humeral position following RSA showed significant differences in the increment in postoperative AED, LHO, and $LHO^{COR}$ between the 3 groups. Therefore, MGLH design seems to be more effective for humeral distalization and lateralization compared to original Grammont design.

요골 두 절제 유무에 따른 비구속형 전 주관절 성형술 (Nonconstrained Total Elbow Arthroplasty with or without Radial Head Resection)

  • Purpose : To evaluate the effect of radial head resection on longevity in nonconstrained total elbow arthroplasty, Materials and Methods : The results of 20 cases of radial head replaced Pritchard ERS and 18 cases of radial head excised Kudo elbow was followed for 6 to 16 years, 10.6 years in average. The rate of loosening, osteolysis, radial head subluxation, joint dislocation and valgus deformity were compared between the two groups. Results'There was no exaggerated cubitus valgus in Pritchard ERS group. However there developed one case of delayed subluxation of radial head occurred in 6 years postoperatively, one case of loosening, and one case of sponta- neous fracture of humeral shaft due to osteolysis. In Kudo elbow group,5 cases (27.8%) showed exaggerated valgus deformity with instability. There were one case of loosening and one case of delayed dislocation occurred in a year postoperatively. Conclusion : The most clear feature of Kudo elbow was exaggerated cubitus valgus. However, the ostolysis was developed in both groups and there was no difference in rate of the other complications between the two groups.

곡관의 하류에 설치된 전자기유량계의 유량신호 특성에 관한 실험적 연구(난류 유동) (An Experimental Study on Installation Effects of Pipe Elbow on the Electromagnetic Flowmeter Characteristics (Turbulent Flow))

  • An electromagnetic flowmeter(EMF) essentially averages the velocity distribution over the pipe cross- sectional area, and the measured value is dependent on the velocity profiles. In this study, installation effects of 90$^{\circ}$long elbow(KS B 1522, ISO 3419) on the EMF characteristics was investigated. A commercial EMF was adopted and the distribution of magnetic field in the electrodes cross section was measured. In the experiment, the national flow standard system, of which measurement uncertainty was evaluated in accordance with ISO 17025 recommendation, was used fur characterization of EMF. The leading line has 150D long straight pipe to established a fully developed flow before entering into the elbow and the elbow was installed downstream of it. then the flowmeter was tested within 50 D from the elbow. The installation effects of the flowmeter were investigated by varying the mean velocity(Reynolds No.)in pipe section, the locations and the direction of electrodes plane.($\phi$) From the experimental results, we find the optimal conditions to get most accurate measurements. Generally, the deviations from the calibration value were less than 0.5 % in farther than 10D distance from the elbow and the direction of electrode plane. $\phi$ = 90$^{\circ}$yielded the smallest measurement deviation. These characteristics were shown consistently in turbulent region regardless of the mean Reynolds number.
