8체질 분류 설문지의 문항 분석-III 발한 상태, 목욕 후 또는 일광욕 후 인체 반응, 생리.병리적인 체질병 특징, 음식 반응을 중심으로 (A Study on the Analasys of the Eight-Constitution Questionnaire)

  • Background : Eight-Constitution Medicine (ECM) devides the human body into eight constitutions. Up until now, checking the pulse types is only way to classify the eight constitution. Recently we try to make a questionnaire that is based on the Eight-Constitution characters and analyze the answer from patients who were classified into one of the eight constitutions according to their pulse types and their responses to constitution-acupuncture therapy Objective : This study intends to analyze each answer of the Eight-Constitution Questionnaire with independent sample t-test. Methods: Participants of this study were outpatients in ECM clinics located in Korea. The resources were collected from 925 patients who were classified into one of the eight constitutions according to their pulse types and their responses to constitution-acupuncture therapy. SPSS 13.0 for Windows was used for independent sample t-test. Results and Conclusions: 1. The proportion of participants' constitutions is in the order of Pancreotonia (28.0%), Colonotonia (17.3%), Pulmotonia (18.1%), Hepatonia (18.1%), Vesicotonia (8.5%), Cholecystotonia (7.0%), Renotonia (2.6%) and Gastrotonia (0.4%). 2. Eight-Constitution Questionnaire consists of 5 parts; Appearance, Character, Body reaction, Physiology & Pathology and Food reaction. 3. 44 items were chosen among 128 question about Body reaction, Physiology & Pathology and Food reaction. 4. Due to the deficiency of Gastrotonia examples, useful items have to be given up. If we can gather Gastrotonia examples, we can use meaningful items more than 44.

한국 참구의학의 체질관점과 실용적 통합치료 의학으로서의 특징 (Constitutionally individualized and practically integrated characteristics of Korean Acupuncture)

  • Objectives: Although Korean Acupuncture (KA) in Korean Medicine (KM) shares its origins with Traditional Chinese Medicine (TCM), it has made its own contribution to the field of acupuncture and general medicine. This study reviews characteristics of KA. Methods: Representative theories of KA were reviewed in light of a constitutionally individualized and practically integrated approach. Individualized approach in KA is based on constitutional energy traits recognized in Saam, Taegeuk, and Eight Constitution acupuncture theories. New practically integrated therapeutic modalities, such as herbal, and bee venom acupunctures, have also been developed in KA. Results: As a form of natural medicine, KA along with KM put emphasis on the vitality and self-correcting mechanism of an individual, which led to the refinement of constitutional approach. KA along with KM theories features a summarized understanding and practical application of integrated therapeutics, which produced creative development of new modalities. Conclusions: KA is expected to make significant contributions to the individualized and practical medicine, based on basic research and clinical trials.

팔체질침 전문 한의사의 체질침 효과에 대한 인식조사 (A Research on the Recognition of the Effect of Constitution-Acupuncture Based on the Expert Constitution-Acupuncture Oriental Medical Doctors)

  • Objectives: The purpose of this research is to obtain basic data about the effects of Constitution-Acupuncture (CA) based on Eight-Constitution Medicine (ECM) by the expert CA Oriental Medical Doctors. Methods: The resources were collected from the internet survey method. 35 samples were collected. The SPSS 13.0 for windows and Answer Tree 3.0 was used for statistical analysis. Results: The results from this research are as follow. 1. The motivation that apply ECM to clinic are 'Because ECM is effective for diagnosis, treatment and prevention against disease' 48.6% and 'Because CA therapy is effective' 25.7%. 2. The advantage that CA therapy compare to the other acupuncture therapy are 'Management and prevention against disease are available by ECM theory to a person who exact constitution is confirmed' 82.9%, 'CA therapy is superior in treatment of hard symptoms to treat by the other acupuncture therapy' 80%, 'Treatment effect is good' 77.1%, 'Needle manipulation of CA is simple comparatively' 57.1%, 'It is definite whether treatment effect is good or not' 54.3% and CA therapy is superior all in diseases of twelve category. 3. When the effects of CA is good, patients expressed much 'Eyesight lightened', 'Took deep sleep at night', 'Fatigue decreased', 'Body is light', 'Feelings became refreshing' as well as 'Symptoms were improved' and also physicians confirmed as those reactions whether treatment is effective. Conclusions: The research suggest that CA is effective therapy in clinic, and so further study is needed for CA therapy.

소음인(小陰人)의 장부경락론적(臟腑經絡論的) 고찰(考察) (Overview of four subgroups in Soeum(Lessor Yin) Constitution, Which are classified based on 'Chang(=Viscera) and Fu(=Bowels) meridian theory')

  • 이제마선생에 의해 제창된 사상체질론은 지난 백년간 후세 사상연구가들에 의하여 발전되어 왔다. 그 중 하나가 바로 권도원 선생에 의해 제창된 체질침(體質針)의 출현이었던 것이다. 체질침범에서는 종래의 방법과는 다른 새로운 맥법(脈法)과 맥도(脈圖) 그리고 체질에 따른 체질침방(體質針方)이 운용되고 있다. 이 체질침방의 경우 실제로는 장부경락(臟腑經絡)의 오행혈을 운용한 사암(舍岩)선생의 오행침을 체질침화(體質針化)한 것이다. 이제마 선생의 체질이론과 사암선생의 오행침이 결합된 것은 실로 역사적인 일이라 하지 않을 수 없다. 권도원 선생은 여덟 개의 맥도와 함께 8체질론을 내세웠으며 스물네개의 사암선생 침방 중 여덟 개를 체질침화하여 이 여덟 체질의 기본 치료처방으로 사용하였다. 본 논문의 저자인 염태환은 지남 1986년 미국 라스베가스에서 열린 동양의학 학술대회에서 이미 24체질론과 24체질 맥도를 제시한 바 있다. 본 논문은 우선 체질의학의 발달에 대한 역사를 간단히 정리하였다. 4체질론에서 시작하여 24체질론까지의 발전되어 온 단계 단계가 여기에 설명되어 있다. 이 설명 속에서 십이경락이 어떻게 사상의학과 이론적으로 결부되는가를 이해하게 되면, 24체질론의 출현이 쉽게 받아들여질 것이다. 본 논문은 이 24체질 중 특히 소음인에 속하는 네 체질에 대하여 초점이 맞추어져 있다. 이 네 개의 체질에 대한 체질맥진법, 맥도, 침법 그리고 한약처방 등이 이 논문에 다루어지고 있는 것이다. 그리고 이 논문의 뒷부분에는 이 네 체질에 대한 저자들의 임상 실례들이 실려 있다. 이 논문의 목적은 새로운 학설을 임상실례와 더불어 소개함으로써, 더욱 많은 한의사들이 관심을 가지고 체질침을 임상에 응용하게 하며 또한 앞으로 치료효과를 객관적으로 밝히기 위한 이중맹검시험(Double blind controlled study) 등이 시행될 수 있는 토대를 마련하고자 하는 것이다.

8체질(體質) 설문지(說問紙)의 타당도(妥當度), 신뢰도(信賴度)에 관한 연구(硏究) (A Study on the Validity and Reliability of the Eight-Constitution Questionnaire)

  • Background: Eight-Constitution Medicine (ECM) classifies the human body into eight constitutions. Diagnosis of discrimination of the eight constitutions depends on a unique pulse diagnosis. However, pulse diagnosis is subjective and requires vigorous training. Objectives: This study aims to analyze the validity and reliability of the Eight-Constitution Questionnaire as a diagnostic method. Methods: Participants of this study were outpatients in six ECM clinics located in Seoul. The resources were collected from 409 patients who were classified into one of the eightconstitutions according to their pulse types and their responses to constitution-acupuncture therapy. SPSS 13.0 for Windows was used for statistical analysis: factor analysis, reliability analysis, independent sample t-test, and multinomial logistic regression were used to verify the results. Results and Conclusions: 1. The proportion of participants' constitutions is in the order of Pancreotonia (23.7%), Colonotonia (19.8%), Pulmotonia (18.1%), Hepatonia (16.9%), Vesicotonia (8.1%), Cholecystotonia (7.3%), Renotonia (5.3%) and Gastrotonia (0.7%). 2. Sevencomponents and 74 items were selected through factor factor and relaibility analysis performed on about 251 items. 3. The firstcomponent's mean is significantly higher in Pancreotonia than that in other constitutions (p<0.05). The second is in Pulmotonia and Colonotonia, whereas the third is in Hepatonia and Cholecystotonia. Fifth is in Vesicotonia, the sixth is in Colonotonia, and the fourth and seventh are not significant in specific constitutions. 4. The percentage that Pancreotonia is correctly predicted is 96.9%, Pulmotonia is 91.9%, Colonotonia is 91.4%, Hepatonia is 88.4%, Vesicotonia is 81.8%, Gastrotonia is 66.7%, Renotonia is 66.7%, Choleeystotonia is 30.0%, and the total percentage is 85.3%.

사상체질과 인체 흉복부길이와의 관계 (Relationship between Sasang Constitution and Length of Chest and Abdominal Region)

  • Objectives : This study was performed to investigate the relationship between Sasang Constitution and length of chest and abdomen region. Methods : Forty eight participants were enrolled. Weight, height, and lengths from CV22 to CV16, from CV16 to CV8, and from CV8 to CV2 were measured. Ratios of last three lengths to total CV22 to CV2 were calculated respectively. On the other hand, Sasang Constitution was analyzed with QSCCII(Questionnaire of Sasang Constitutional Classification II). The analysis of relationship between the measurements and Sasang Constitution was carried out with SPSS to compare the mean values according to Sasang Constitution. Results : All the mean values were not significantly different depending on the Sasang Constitution, except the ratio of the length from CV16 to CV8 to the length from CV22 to CV2, only in female participants. The ratio was higher in the female Soeumin participants than any other constitutional groups. Conclusions : This result showed that Soeumin could be assumed that they might have weak digestion abilities.

김홍경의 기경팔맥관(奇經八脈觀)에 관한 연구(硏究) - 음양유교맥(陰陽維蹻脈)과 사상체질(四象體質)의 상관성을 중심으로 - (The reserch of Kim hong-kyung's view of extra meridians -With the connection yin yang wei qiao mai With four trigram constitution theory-)

  • Subject : Kim hong-kyung(金洪卿)'s view of eight extra meridians Object : Kim hong-kyung has not only deeply studied Saam acupuncture but also the function and characteristics of extra meridian at the same time. Especially, he has tried the study to connect yin yang wei qiao mai(陰陽維蹻脈) with Lee je-ma(李濟馬)'s four trigram constitution theory, therefore the author felt the need to make public the theory and started reserch on the theory. Method : I researched the physiological properties of extra meridians through the past eastern medical literature, I reported on the types of people to show well the properties of extra meridians gathering Kum-oh's verbal explanation when I had classes from him. Result : Kum-oh Kim hong-kyung defined meridians as passways of consciousness and feeling, he interpreted 12 meridians and extra meridians with the view of mind, which has very important meanings in the history of acupuncture in the point of first trial to interpret meridians from this view. He explained Ren mai(任脈) and Du mai(督脈) are meridians which show clearly properties of yin-yang to appear trusts of affirmation and distrusts of denial in the mind, as Ren(任) means entrusting and Du(督) means supervising in English. He explained Chong mai(衝脈) is the meridian representing impulsive man that has a meaning of impulse as being suggested the name of Chong mai, because Chong mai is borrowed from Foot shao yin kidney meridian(足少陰腎經). Dai mai(帶脈) is the meridian representing the characteristics of woman that has a meaning of binding as being suggested from the name of Dai mai which acts when people have something on their flank or they are pregnant, because Dai mai borrowed from Foot shao yang gall bladder meridian. He thought that four trigram divided from yin and yang is innate trigram, so Yang wei mai(陽維脈), Yin wei mai(陰維脈), Yang qiao mai(陽蹻脈), Yin qiao mai(陰蹻脈) among extra meridians have connections with Tai-yang ren(太陽人), Tai-yin ren(太陰人), Shao-yin ren(少陰人), Shao-yang ren(少陽人) of Four trigram constitution theory respectively which was created by Lee je-ma. He explained yang means opponents and wei(維) means binding. Yang wei mai is the meridian to be active when opponents bind or intervene in you, also the meridian is well active to the people who easily criticize others who are rude, to the people who well refuse restraints from organization. Because he explained yin means me and wei(維) means binding. Yin wei mai is the meridian to be active when I bind and intervene in opponents. He also state that the meridian is easily active to the people who have a tendency to gather people and make party apparatus by nature. He explained Yang qiao mai is the meridian to be active in the situation that people are waiting for others to bridge gap because yang means opponents and qiao(蹻) which has a meaning of standing on one's tiptoes means bridging gap between people. He also state the meridian is easily active to Shao yin ren who has a tendency to be passive, to easily settle down and not to move by nature. He explained Yin qiao mai is the meridian to be active in the situation that I bridge gap between me and others to make friends, because yin means me and qiao(蹻) which has a meaning of standing on one's tiptoes means bridging gap between me and others. He also state the meridian is easily active to Shao yang ren who is active in every business and who has remarkable ability to make friends. Conclusion : Kim hong-kyung stated the physiological properties of extra meridians as below. Ren mai is the meridian representing positive confidence which is easily active when people have a mind to trust others, Du mai is the meridian representing negative distrust which is easily active when people have doubts. Chong mai is the meridian representing the characteristics of man that has impulsive characteristics. Dai mai is the meridian representing the characteristics of woman to be easily active when she is pregnant. Yang wei mai has close connections with Tai-yang ren who has a tendency to refuse restraints from others and to criticize on opponent's remarks and behaviors that are wrong. Yin wei mai has close connections with Tai-yin ren, as the meridian is active in the situation that people enjoy restraining and intervening in others. Yang qiao mai has close connections with Shao yin ren, as the meridian is active to the people who are negative and passive by nature. Yin qiao mai has close connections with Shao yang yin who has a strong tendency to make friends easily by nature.

팔체질의학 맥진의 신뢰성 연구 (A Pilot Study on Reliability of Pulse Diagnosis in Eight-Constitution Medicine)

  • 배경 : 체질의학에서 진단의 중요성에도 불구하고 팔체질의학의 맥진의 신뢰성에 대한 과거 연구가 거의 없었다. 목적 : 본 예비연구는 팔체질의학의 맥진에 대한 진단자 간의 신뢰성 및 진단자 내의 신뢰성을 검사하기 위함이다. 재료 및 방법 : 팔체질의학을 이용한 진료 경력이 3년 내지 5년 되는 한의사 두 명이 실험에 참여하였다. 31명의 건강한 대학생들의 팔체질을 맥진을 통해 진단하고 그 결과를 확신도와 함께 각각 기록하도록 하였으며 이 중 5명은 각각 진단자로부터 3회씩 진단을 받도록 하였다. 그 결과를 분석하여 진단자 간의 신뢰성과 진단자 내의 신뢰성을 검사하였다. 아울러 진단에 대한 확신도와 결과와의 상관성이 있는지도 조사하였다. 대상자와 진단자는 서로를 알 수 없도록 가리개를 하고 실험을 진행하였다. 결과 : 진단자 간의 일치도는 35.7%였으며 카파 계수는 0.232로 보통 이하 수준인 것으로 나타났다. 진단자 내 일치도는 진단자 별로 각각 89%, 66.83%로 나타났다. 다만 두 진단자 사이의 결과가 일치하는 경우 불일치하는 경우보다 진단자의 확신도가 높은 것으로 드러났다 결론 : 본 연구에서는 팔체질의학의 맥진의 진단자 간, 진단자 내의 신뢰성이 비교적 낮은 것으로 나타났다. 이는 진단의 변수가 많고 진단자의 숙련도나 대표성의 문제 등 여러 가지 요소들로 인한 결과로 파악된다. 팔체질의학의 진단자 간, 진단자 내의 신뢰성에 대한 향후 연구가 절실히 요구된다.

팔체질의학론의 원리에 대한 고찰 (Study on Principle of the Theory of Eight Constitutional Medicine)

  • In Sasang constitutional medicine, I have researched the process of visceral structure in the Eight Constitutionals under circulation of the five elements and the formation of primary source of illness. From this research, I could draw following conclusions through combination of the auxiliary psycho-formulas by applying the constitutional acupuncture therapy on the five elements diagnostic calculation. Since the arrangement for the five dimensions of organs in the eight constitutions has been formulated by circulation of the five elements in Sasang constitutions, if the five elements begin circulating count-clockwise from the reference point at the organs in Sasang constitutional medicine, the positive constitutional arrangement of organs is built up such as, the positive constitution of metal, earth, wood and water, while begin circulating clockwise, the negative constitutional arrangement of organs is formulated, such as the negative constitutions of metal, earth, wood and water. The source of illness results from imbalance of the organic force being generated by transfer of the five elements from compatibility to incompatibility when the five elements circulate. Hence, it has been acknowledged that if the source of illness comes from the strongest organ, it is the time when circulation of the five elements progresses from the second organ (the second strongest) in incompatibility, and if the source of illness comes from the weakest organ, it is the time when circulation of the five elements progresses from the third organ (the middle) in incompatibility. It is considered proper to diagnose meridians of the pericardium and the Triple Burners rather than to diagnose meridians of the heart and the small intestine which forms the visceral arrangement of the eight constitutions. For instance, the auxiliary psycho-formula obtains its prescription by attenuating the first organ (the strongest) while augmenting the fourth organ (the second weakest) when the axis of incompatibility in the five elements circulation crosses the second and the third organs, and it gets its prescription by attenuating the second organ(the second strongest) while augmenting the fifth organ(the weakest) when the axis of incompatibility in the five elements circulation crosses the third and the fourth organs. In addition, when medicating, the $4{\sim}5$ times of repeated performance can be assumed to represent the amount of an energy that each organ bears depending on the phase in the arrangement of the eight constitutional organs.

  • 요통(腰痛)은 다른 어느 질병(疾病)보다도 많은 사람이 경험(經驗)을 하였던 질환(疾患)중의 하나로써 요통(腰痛)의 治療法(치료법)에는 전통적으로 침요법(鍼療法), 구요법(灸療法), 한약요법(韓藥療法)이 주(主)가 되어 왔으며, 최근에는 물리치료(物理治療), 약침요법(藥鍼療法), 추나요법(推拏療法) 등이 쓰이고 있다. 여기서는 1994년 9월 14일부터 1996년 5월 25일까지 "우리한의원"에서 고통(顧痛)으로 진료 받은 73명의 환자들을 중심으로 이름, 성별, 나이, 발병원인, 주증(主證), X-ray, CT, MRI 등의 소견, IRCT(InfraRed Computer Thermography) 소견(所見), 체질(體質), 사상처방(四象處方), 약침요법(樂鍼療法), 치료기간(治療期間), 경과(經過) 등을 분석(分析)하고 이것을 바탕으로 기존의 증치의학(證治醫學)과는 다른 사상의학적(四象醫學的)인 방법으로 요통(腰痛)을 접근하는 방법을 알아보고자 하였으며 아울러 X-ray, CT, MRI 등의 소견(所見)과 한의학적(韓醫學的)으로 이용할 수 있는 적외선체열진단(赤外線體熱診斷)과 비교하여 보며, 또 악침(樂鍼)에서의 사상의학적(四象醫學的)인 접근 방법도 알아보았다. 조사방법(調査方法)은 체질별(體質別) 분포, 사상처방(四象處方)의 사용, 치료경과(治療經過), 치료기간(治療期間)을 중심으로 하였으며, 체질(體質) 감별(鑑別)의 방법은 체형기상(體形氣像), 용모사기(容貌詞氣), 성질재간(性質材幹), 체질증(體質證), 체질병증(體質病證), 음식물의 반응 등을 중심으로 하여 임상적인 감별을 하였고, X-ray는 추나요법(推拿療法)을 위하여 Open mouth, C-spine AP&Lat, TH-spine AP&Lat, L-spine AP&Lat을 의뢰하였으며, 적외선체열진단(赤外線體熱診斷)은 피판(皮板)(Dermatomes)을 중심으로하여 각각의 디스크가 피판(皮板)에 미치는 부분을 관찰하여 양쪽의 체열(體熱)의 차이가 섭씨 1도 이상 차이가 나는 것으로 진단을 하여 다음과 같은 결론을 얻었다. 1. 전체 환자 73명중 태음인(太陰人)이 47명(66.4%)으로 가장 많았고 다음은 소음인(少陰人)이 16명(21.9%)이었고, 소양인(少陽人)은 10명(13.7%)으로 가장 적었으며 이것은 동의수세보원(東醫壽世保元)에서의 태음인(太陰人) 오천인(五千人), 소양인(少陽人) 사천인(三千人), 소음인(少陰人) 이천인(二千人), 태양인(太陽人)은 드물다는 구성 비율과는 달리 소양인(少陽人)과 소음인(少陰人)의 구성 비율이 바뀌었으며, 전체적으로는 소양인(少陽人)이 적게 나오고 태음인(太陰人)이 많이 나왔음을 알 수 있다. 2. 각 체질별(體質別) 치료의 경과에 대한 만족도는 전체적으로 60.3%였으며 태음인(太陰人)에서 만족도(66.0%)가 높았고 소음인(少陰人)에서 만족도(56.3%)가 가장 낮았으며 소양인(少陽人)의 만족도 (60%)는 평균과 비슷하였다. 3. 각 체질별(體質別) 치료에 만족하는 환자(患者)에게 보인 사상처방(四象處方)을 분석하여 보면 소양인(少陽人)에 있어서는 표병약형방지황탕(表病藥荊防地黃湯)과 리병약(裡病藥)인 육미지황탕(六味地黃湯)이 고르게 썼으며, 소음인(少陰人)은 리병약(裡病藥)인 십이미관중탕(十二味寬中湯)이 가장 많이 쓰였으며, 태음인(太陰人)은 리병약(裡病藥)인 청심연자탕(淸心蓮子湯), 청폐사간탕(淸肺瀉肝湯), 열다한소탕(熱多寒少湯)이 많이 쓰였음을 알 수 있었다. 따라서 소음인(少陰人)과 태음인(太陰人)은 리병약(裡病藥)이 치료 경과가 좋았으며 소양인(少陽人)은 표병약(表病藥)과 리병약(裡病藥)에서 고르게 쓰였음을 알 수 있다. 4. 치료에 만족하는 환자의 평균 치료기간은 약 6주였으며, 각 체질별(體質別)로는 소양인(少陽人)에서 4.2주로 가장 낮았고, 다음은 태음인(太陰人)이 5.7주 였으며, 소음인(少陰人)에서 6.8주로 가장 오래 걸림을 알 수 있다. 5. 사상의학(四象醫學)적인 요통(腰痛)의 치료방법은 외상성에 의한 요통(腰痛) 따른 치료와 그 외의 체질적(體質的) 소인에 따라 소음인(少陰人)의 표병(表病)온 승양익기(升陽益氣)를 하고, 리병(裡病)은 이음강기(裡陰降氣)를 하며, 소양인(少陽人)의 표병(表病)은 표음강기(表陰降氣)를 하고, 리병(裡病)은 청양상승(淸陽上升)을 시키며, 태음인(太陰人)의 표병(表病)은 폐양승기(肺陽升氣)시키고 리병(裡病)은 청간조열(淸肝操熱)을 하며, 태양인(太陽人)은 보간생음(補肝生陰)의 치법(治法)을을 쓸 수 있다. 6. 사상처방(四象處方)외의 방법으로 약침요법(藥鍼療法)은 체질적(體質的)으로 적용하는 방법은 약침(藥鍼)의 선별 방법과 약침(藥鍼)을 치료하는 부위의 선별방법으로 나눌 수 있다. 약침(藥鍼)의 선별에서 소음인(少陰人)은 蜂藥鍼(B.V.)을, 소양인(少陽人)은 홍화(紅花)(H.O)를 태음인(太陰人)은 호도(胡挑)(I), 웅담(熊膽), 우황(牛黃), 사향(麝香)(V,O.K.) 등을 써 볼 수 있고, 팔망약침법(八網樂鍼法)도 기존에 쓰는 체질별(體質別) 사상처방(四象處方)을 그대로 약침(藥鍼)으로 만들어 쓸 수 있다. 약침(藥鍼)을 치료하는 부위의 선별방법은 사상의학(四象意學)의 체질증(體質證), 체질병증(體質病證)을 충분히 응용할 수 있는 태극침법(太極鍼法)의 혈위(穴位)와 사초부위(四焦部位)에 따라 생리(生理), 병리(病理)를 적용시켜 쓰는 방법올 활용하여 볼 수 있다.
