  • Purpose: This study examined the impacts of action observational physical training related to stair walking on the stair walking ability and self-efficacy of chronic stroke patients. Methods: This study was conducted on 24 chronic stroke patients, who were assigned randomly to an action observational physical training group (12 persons) and a landscape observation physical training group (12 persons). To the action-observational physical training group, five videos related to stair walking were presented, and after observing them, physical training was carried out. The landscape observation physical training group observed the videos consisting of landscape, where there were no humans and animals, and then underwent physical training. This study measured the rectus femoris, biceps femoris, tibialis anterior, and gastrocnemius to examine the muscle activity of the lower limb. A timed up and go and step test was performed to examine the balance ability, and a timed stair test was conducted to examine their stair walking ability. A self-efficacy scale was measured to examine the degree of their confidence of performing stair walking. The assessment was conducted three times in total: pre-, post-, and follow-up surveys. Results: As a result of this study, the action observational physical training group significant improvement after the intervention than in the landscape observation physical training group. Moreover, the follow-up study four weeks after the intervention showed significant improvements in the action observational physical training group (p<0.05). Conclusion: These results show that the action observational physical training had a positive impact on the stair walking ability and self-efficacy of chronic stroke patients.

  • Personal creativity is critically important for organizations seeking to survive and thrive in today's highly turbulent business environments. Organizations must effectively identify and mobilize the creative resources of their members. When organizational members perceive a work environment that restricts or fails to encourage individual creative expression, a gap may exist between the level of individual creative potential and the actual amount of individual creativity practiced within the organization. In this situation, this paper will examine the impact of role conflict, role ambiguity, creative self-efficacy, and personal initiative on personal creativity. Creative self-efficacy is the subjective belief in one's personal ability to be creative, that is, a personal assessment of one's own creative potential. A strong internal belief in one's ability to successfully engage in creative behaviors is generally considered an important part of the creative process. Personal Initiative refers to behaviors, mainly directed toward work and organizational issues, that are characterized by the following aspects: self-starting, proactive, and persistent in overcoming barriers. Creativity-related creative self-efficacy and personal initiative are critical components to understand motivation that coordinates the relationship between perception and behavior of individual employees. Based on role theory, researchers have focused on role conflict and role ambiguity as the two key ingredients of role stress. Role ambiguity is defined as an evaluation about the lack of salient information needed to perform a role effectively. Role conflict results from two or more sets of incompatible demands involving work-related issues. Employees are usually pursued work-roles more than one in work-focused organization. Too many work-roles and perceived uncertainties at employee's work can be obstacles to personal creativity. In an analysis of results, while role conflict is not negatively related significantly to creative self-efficacy, role ambiguity is negatively related significantly to creative self-efficacy. While role conflict is significantly related to personal creativity, role ambiguity is negatively related significantly to personal creativity. Creative self-efficacy mediates the relationship among role conflict, role ambiguity, and employee creativity. Personal initiative mediates the relationship between creative self-efficacy and employee creativity. This paper shows that creative self-efficacy and personal initiative are the driving force behind personal creativity. Organizations can get some implications of creative-related role conflict and role ambiguity that employees have experienced. As a result, organizations must not only encourage creativity of employees by greater involvement but also encourage their input towards their-focused own works and tasks. And employees should be developed to pace with the organizational needs and development. Management must enable employees to think of new ideas and practices that promotes personal creativity.

  • A stochastic simulation model incorporated with Reed-Frost approach was derived for evaluating diagnostic performance of a test used for a screening program of an infectious disease. The Reed-Frost model was used to characterize the within-herd spread of the disease using a hypothetical example. Specifically, simulation model was aimed to estimate the number infected animals in an infected herd, in which imperfect serologic tests are performed on samples taken from herds and to illustrate better interpreting survey results at herd-level when uncertainty inevitably exists. From a risk analysis point of view, model output could be appropriate in developing economic impact assessment models requiring probabilistic estimates of herd-level performance in susceptible populations. The authors emphasize the importance of knowing the herd-level diagnostic performance, especially in performing emergency surveys in which immediate control measures should be taken following the survey. In this context this model could be used in evaluating efficacy of a survey program and monitoring infection status in the area concerned.

  • The prime objective of many facial wrinkle-reduction treatments is to achieve visible improvement. Thus the visibility of before/after treatment differences is often part of an efficacy assessment. This paper investigates whether the background knowledge of the people acting as observers in such assessments is of impact on the results: e.g. the subjects themselves are familiar with their faces, skin professionals have much experience in judging skin quality, and thus both might be more sensitive to small changes. In a clinical study 44 Female subjects were regularly treated during a period of 12 weeks with one of three wrinkle-reduction methods: K, Land M (placebo). Photographs were taken before treatment and at 6 and 12 weeks. The photographs were judged by 3 types of observers:ㆍ24 Lay observers were given the 0&6-week and the 0&12-week photo pairs of all subjects to indicate the one with the least wrinkles in a two-alternative forced-choice (TAFC) procedure.ㆍThe subjects themselves were given the 0&6-weel and the 0&12-week pair of their own photos (8 replications) to indicate the photo with the least wrinkles (TAFC).ㆍA trained panel of skin professionals (N=3) each gave 9-point Fitzpatrick wrinkle-severity scores for all individual 0-week and 12-week photos. We found that the lay observers perceived the same differences as the subjects themselves: significant improvements after 12 weeks for treatment K (p<0.0005 and p=0.005, respectively), no visible effects for treatments Land M, and, most importantly, a significant difference between treatments K and M/placebo (p=0.02 and p=0.04, respectively). Also the trained panel found this difference between K and M (p=0.013), but here it was due to a significant deterioration over time of the 'placebo-treated' wrinkles (M, p=0.03). Thus in conclusion we have found no indications that extra knowledge - in the form of familiarity with the own face or in the form of professional training - results in the identification of more treatments that show significantly visible wrinkle-reduction.

  • The purpose of this study was to compare the enamel fluoride uptake and anticariogenic effect following topical application with different physical form of fluoride and application time. The results can be summarized as follows: 1. While the 4-minute applicaton increased the enamel fluoride uptake significantly than 1-minute application regardless of physical form of applied topical fluoride agents, no siginificant difference could be found between foam and gel type when applied for the identical period of time. 2. The application time or the physical form of topical agents had no singificant influence on the reduction of caries lesion. 3. Based upon the above-mentioned results of this study, it cannot be concluded that the amount of enamel fluoride uptake has an absolute impact on the change of lesion depth. Therefore, the clinical application of 1-minute topical fluoride application which appears to have a similar anticariogenic efficacy to 4-minute fluoride application may be encouraged.

  • Cervical cancer resulting from prior infection with human papillomavirus (HPV) is a significant public health threat against young Japanese women. A national immunization plan to vaccinate 13~16 year old female students against HPV infection has been started in Japan since 2010, and may reach almost full coverage by the end of 2012. Older age females who may already be sexually active are not targeted by this plan but should follow safer sex practices as well as periodic screening of the cervix cytology to reduce their risk of developing cervical cancer. HPV vaccination alone does not offer full protection either, because only some HPV types are covered by the vaccines and the long-term efficacy of the vaccines has not been determined yet. Therefore, we did a survey at an international university in Japan to study the knowledge and attitude of female college students towards prevention of cervical cancer, to examine the age when they start sexual activity and other related attributes that may influence the risk of cervical cancer. We discuss the results of our survey and what they imply for the possible impact of an HPV immunization plan on the risk of cervical cancer in Japan, and conclude by an emphasis on the need to increase awareness among Japanese female adolescents and to enhance the cervical screening rates among older females who are already sexually active.

  • The effective management of damage in tunnels is crucial for ensuring their safety, longevity, and operational efficiency. In this paper, we propose an educational management model tailored specifically for addressing damage in tunnels, utilizing numerical solution techniques. By leveraging advanced computational methods, we aim to develop a comprehensive understanding of the factors contributing to tunnel damage and to establish proactive measures for mitigation and repair. The proposed model integrates principles of tunnel engineering, structural mechanics, and numerical analysis to facilitate a systematic approach to damage assessment, diagnosis, and management. Through the application of numerical solution techniques, such as finite element analysis, we demonstrate the efficacy of the proposed model in simulating various damage scenarios and predicting their impact on tunnel performance. Additionally, the educational component of the model provides valuable insights and training opportunities for tunnel management personnel, empowering them to make informed decisions and implement effective strategies for ensuring the structural integrity and safety of tunnel infrastructure. Overall, the proposed educational management model represents a significant advancement in tunnel management practices, offering a proactive and knowledge-driven approach to addressing damage and enhancing the resilience of tunnel systems.

  • Background: Pharmacists communicate with a variety of healthcare experts to prevent medication errors. Situation-Background-Assessment-Recommendation (SBAR) is a tool used for concise and accurate communication. In 2018, we developed the pharmacy-SBAR (P-SBAR) to deliver pharmacists intervention more quickly and effectively through quality improvement activities. Objectives: This study evaluates the efficacy of P-SBAR on pharmacists' intervention activities before and after the implementation of P-SBAR applications. We assessed the impact of P-SBAR on reducing the burden of intervention work, promoting pharmacists' participation, and enhancing the acceptance rate. Methods: This is a retrospective study of the two groups before and after P-SBAR implementation. All pharmacists' intervention records during two periods (2016-2017 and 2019-2020) were extracted from the data warehouse system at Kyunghee University Hospital at Gangdong, Seoul. The outcome was the number of inpatients and pharmacists who participated in the prescription monitoring activity, the number of interventions, and the physicians' acceptance rate. Results: Although the total number of inpatients decreased (364,753 vs. 348,229), the number of pharmacists who participated in intervention activity increased (monthly mean: 15.8 vs. 18.0, p=0.001). The total number of interventions (2,767 vs. 4,389), the frequency of full acceptance (2,018 vs. 3,710), and the monthly acceptance rate increased significantly (73.8% vs. 83.8%, p<0.001). Conclusion: P-SBAR improved accessibility and convenience by digitalizing the intervention activities performed in an offline environment. Improvement in work burden and acceptance rate using P-SBAR is expected to contribute toward reducing medication errors.
