A Systematic Review and Meta-Analysis of Pharmacopuncture Treatment for Insomnia Disorder (불면 장애에 대한 약침술 치료법 제안을 위한 체계적 문헌 고찰과 메타 분석 연구)

  • Objectives: The objective of this study was to perform a systematic review and meta-analysis of pharmacopuncture treatment for insomnia disorder to derive clinical evidence and recommendation grades. Methods: Studies that verified effects of pharmacopuncture on primary insomnia were included. Characteristics and quality of included studies were assessed using Risk of Bias (RoB). Results: A total of 25 studies were selected. Pittsburgh Sleep Quality Index (PSQI) and effective rate were primarily used for outcome measurement. Vitamin B12 was the most used pharmacopuncture material. The most frequently used acupuncture point was Anmian (Ex-HN). The volume of the acupuncture solution ranged from 0.25 mL to 2 mL. Acupuncture treatment depth was 0.5 cm to 2 cm. In three studies, the procedure was performed at 16:00. Meta-analysis of studies revealed that the effective rate of the pharmacopuncture group was significantly higher than the group using sleeping pills group (RR: 1.21, 95% CI: 1.01 to 1.45, p=0.04, I2=69%). PSQI was decreased in the intervention group (MD=-2.19, 95% CI: -2.90 to -1.48, p<0.00001, I2=0%). Effective rates of pharmacopuncture and acupuncture groups were higher than that of the acupuncture group (RR: 1.11, 95% CI: 1.05 to 1.17, p=0.0002, I2=0%). PSQI was decreased in the intervention group (MD=-1.87, 95% CI: -2.36 to -1.38, p<0.00001, I2=0%). Although the effectiveness rate of the pharmacopuncture group was not significantly higher than that of the acupuncture group (RR: 1.12, 95% CI: 0.98 to 1.27, p=0.1, I2=9%), the PSQI was decreased in the pharmacopuncture group (MD=-2.10, 05% CI: -3.29 to -0.91, p=0.0005, I2=34%). The quality of clinical studies was poor. Conclusions: Based on results of this study, it is proper to use 0.5 to 2 mL of pharmacopuncture solution such as Danshen and Ciwujia with a depth of 0.5 to 2 cm at around 4 p.m. to treat insomnia disorder, focusing on Anmian (Ex-HN) and Sameumgyo (SP6).

  • Real estate market participants need to have a sense of predicting real estate prices in decision-making. Commonly used methodologies, such as regression analysis, ARIMA, and VAR, have limitations in predicting the value of an asset, which fluctuates due to unknown variables. Therefore, to mitigate the limitations, an artificial neural was is used to predict the price trend of apartments in Seoul, the hottest real estate market in South Korea. For artificial neural network learning, the learning model is designed with 12 variables, which are divided into macro and micro factors. The study was conducted in three ways: (Ed note: What is the difference between case 1 and 2? Is case 1 micro factors?)CASE1 with macro factors, CASE2 with macro factors, and CASE3 with the combination of both factors. As a result, CASE1 and CASE2 show 87.5% predictive accuracy during the two-year experiment, and CASE3 shows 95.8%. This study defines various factors affecting apartment prices in macro and microscopic terms. The study also proposes an artificial network technique in predicting the price trend of apartments and analyzes its effectiveness. Therefore, it is expected that the recently developed learning technique can be applied to the real estate industry, enabling more efficient decision-making by market participants.

A Study on the Anti-wrinkle Properties of Cosmetics Containing Essential Oil from Chrysanthemum boreale MAKINO (산국 에센셜오일을 함유한 화장품의 주름개선 임상효능에 관한 연구)

  • We recently reported the characteristics of Chrysanthemum boreale MAKINO essential oil (CBMEO) at different harvesting stages and its in vitro antioxidant, whitening, skin regeneration, and antiwrinkle properties. In the present study, we investigated the effectiveness of cosmetic creams containing 0.1% of CBMEO in terms of wrinkle reduction and safety. The participants were 22 women aged 42 to 60 y with natural wrinkles. All the participants applied the CBMEO-containing cream to the eye area for 8 weeks. Visual observations of the skin by a dermatologist were used to assess the safety of the product, and improvements in skin wrinkles (roughness) were analyzed using a skin visiometer. Neither the CBMEO-containing cream nor the placebo caused abnormal reactions, such as erythema or allergies, during the test period. The CBMEO-containing cream-exposed test regions showed a greater reduction in the roughness index as compared with those of the placebo-exposed control regions. The R1, R2, and R3 indices showed a statistically significant decrease in the test regions as compared with the placebo control regions after using the cream for 2 weeks (p<0.05). Therefore, this study demonstrated that CBMEO-containing cosmetics are safe for human use and that they have skin wrinkle improvement properties. In the future, CBMEO may be a promising product for use in the development of functional anti-wrinkle cosmetics.

Development and Practical Application of a Psychological Skill Training Program for National Wheelchair Curling Players -Frontal EEG Asymmetry (휠체어컬링 국가대표선수의 경기력 향상을 위한 심리기술훈련 프로그램의 개발 및 적용 -EEG 뇌파활용 연구)

  • The present study aimed to develop a sports psychological skill training program and to test its applicability to improve the performance of national wheelchair curling players. A total of 4 participants completed the study. Subjects have been to 12 sessions of 6 different psychological skills training programs (routine training, attentional focus training, writing practice training in diary, relation training, self-control training, and positive self-talk training). The effectiveness of psychological skills training has been evaluated with 6 questionnaires and EEG measurement. The result was that psychological skills training had improved coping with adversity, peaking under pressure, and concentration, self-management and reduced anxiety of Korean national wheelchair curling player. The result was that the EEG inter-hemispheric asymmetry index showed negative emotional states before psychological skill training but positive emotional states after. The result implies that psychological skill training plays an important mediating role in bringing about positive effects in the psychological elements and competitiveness in national wheelchair curling players.

Effectiveness after Designation of a Trauma Center: Experience with Operating a Trauma Team at a Private Hospital

  • Purpose: The present study aimed to evaluate the influence of how the trauma care system applied on the management of trauma patient within the region. Methods: We divided the patients in a pre-trauma system group and a post-trauma system group according to the time when we began to apply the trauma care system in the Halla Hospital after designation of a trauma center. We compared annual general characteristics, injury severity score, the average numbers of the major trauma patients, clinical outcomes of the emergency department, and mortality rates between the two groups. Results: No significant differences were found in the annual patients' average age ($54.1{\pm}20.0$ vs. $52.8{\pm}18.2$, p=0.201), transportation pathways (p=0.462), injury mechanism (p=0.486), injury severity score (22.93 vs. 23.96, p=0.877), emergency room (ER) stay in minutes (199.17 vs. 194.29, p=0.935), time to operation or procedure in minutes (154.07 vs. 142.1, p=0.767), time interval to intensive care unit (ICU) in minutes (219.54 vs. 237.13, p=0.662). The W score and Z score indicated better outcomes in post-trauma system group than in pre-trauma system group (W scores, 2.186 vs. 2.027; Z scores, 2.189 vs. 1.928). However, when analyzing survival rates for each department, in the neurosurgery department, in comparison with W score and Z score, both W score were positive and Z core was higher than +1.96. (pre-trauma group: 3.426, 2.335 vs. post-trauma group: 4.17, 1.967). In other than the neurosurgery department, W score was positive after selection, but Z score was less than +1.96, which is not a meaningful outcome of treatment (pre-trauma group: -0.358, -0.271 vs. post-trauma group: 1.071, 0.958). Conclusions: There were significant increases in patient numbers and improvement in survival rate after the introduction of the trauma system. However, there were no remarkable change in ER stay, time to ICU admission, time interval to emergent procedure or operation, and survival rates except neurosurgery. To achieve meaningful survival rates and the result of the rise of the trauma index, we will need to secure sufficient manpower, including specialists in various surgical area as well as rapid establishment of the trauma center.

  • Objective: The aim of this study was to report the effectiveness of traditional Korean medicine treatment with rehabilitation exercise on a chronic intracerebral hemorrhage patient with a severe gait disturbance. Method: The patient was treated with Korean herbal medicine (Gami-sibjeondaebo-tang) along with acupuncture, moxibustion, and rehabilitation exercise. The treatment effect was evaluated through the Manual Muscle Test (MMT) and the Korean version of the Modified Barthel Index (K-MBI). The gait of the patient was evaluated by Functional Ambulatory Category (FAC) and the Timed Up and Go (TUG) test. The spatiotemporal parameters were evaluated by a walkway system (GAITRite(R) system) and gait symmetry was evaluated by Symmetry Criterion (SC). Results: After 111 days of traditional Korean medicine treatment, the spatiotemporal parameters and symmetry of the patient's gait improved. Conclusion: This study suggested that traditional Korean medicine treatment with rehabilitation exercise could be effective for gait disturbance in patients with chronic intracerebral hemorrhage.

A Methodology for Selection of Habitat Management Areas for Amphibians and Reptiles Considering Soil Loss (토양유실을 고려한 양서파충류의 서식지 관리지역 선정방법)

  • As disaster risk and climate change volatility increase, there are more efforts to adapt to disasters such as forest fires, floods, and landslides. Most of the research, however, is about influence of human activities on disaster and there is few research on disaster adaptation for species. Previous studies focusing on biodiversity in selecting conservation areas have not addressed threats of disaster in the habitats for species. The natural disasters sometimes play role of drivers of ecological successions in the long run, but they might cause serious problems for the conservation of vulnerable species which are endangered. The purpose of this study is to determine whether soil loss (SL) is effective in selecting habitat management areas for amphibians and reptiles. RUSLE model was used to calculate soil loss (SL) and the distribution of each species (SD) was computed with MaxEnt model to find out the biodiversity index. In order to select the habitat management area, we estimated the different results depending if value of soil loss was applied or not by using MARXAN, a conservation priority selection tool. With using MARXAN, conservation goals can be achieved according to the scenario objectives, and the study has been made to meet the minimum habitat area. Finally, the results are expressed in two; 1) the result of soil loss and biodiversity with MATRIX method and 2) the result of regional difference calculated with MARXAN conservation prioritization considering soil loss. The first result indicates that the area with high soil loss and low species diversity have lower conservation values and thus can be managed as natural disturbances. In the area where soil loss is high and species diversity is also high, it becomes where a disaster mitigation action should be taken for the species. According to the conservation priorities of the second result, higher effectiveness of conservation was obtained with fewer area when it considered SL in addition to SD, compared to when considered only biodiversity. When the SL was not taken into consideration, forest area with high distribution of species were important, but when SL considered, the agricultural area or downstream of the river were represented to be a major part of habitats. If more species data or disaster parameters other than soil loss are added as variables later, it could contribute as a reference material for decision-making to achieve various purposes.

A Prospective, Multi-Center, Double-Blind, Randomized Study to Evaluate the Efficacy and Safety of the Synthetic Bone Graft Material DBM Gel with rhBMP-2 versus DBM Gel Used during the TLIF Procedure in Patients with Lumbar Disc Disease

  • Objective : This study is to evaluate the efficacy and safety of demineralized bone matrix (DBM) gel versus DBM gel with recombinant human bone morphogenetic protein-2 (rhBMP-2) used in transforaminal lumbar interbody fusion (TLIF). Methods : This study was designed as a prospective, multi-center, double-blind method, randomized study. All randomized subjects underwent TLIF with DBM gel with rhBMP-2 group (40 patients) as an experimental group or DBM gel group (36 patients) as a control group. Post-operative observations were performed at 12, 24, and 48 weeks. The spinal fusion rate on computed tomography scans and X-rays films, Visual analog scale pain scores, Oswestry disability index and SF-36 quality of life (QOL) scores were used for the efficacy evaluation. The incidence rate of adverse device effects (ADEs) and serious adverse device effects (SADEs) were used for safety evaluation. Results : The spinal fusion rate at 12 weeks for the DBM gel with rhBMP-2 group was higher with 73.68% compared to 58.82% for the DBM gel group. The 24 and 48 weeks were 72.22% and 82.86% for the DBM gel with rhBMP-2 group and 78.79% and 78.13%, respectively, for the DBM gel group. However, there were no significant differences between two groups in the spinal fusion rate at 12, 24, and 48 weeks post-treatment (p=0.1817, p=0.5272, p=0.6247). There was no significant difference between the two groups in the incidence rate of ADEs (p=0.3836). For ADEs in the experimental group, 'Pyrexia' (5.00%) was the most common ADE, followed by 'Hypesthesia', 'Paresthesia', 'Transient peripheral paralysis', 'Spondylitis' and 'Insomnia' (2.50%, respectively). ADEs reported in control group included 'Pyrexia', 'Chest discomfort', 'Pain', 'Osteoarthritis', 'Nephropathy toxic', 'Neurogenic bladder', 'Liver function analyses' and 'Urticaria' (2.86%, respectively). There was no significant difference between the two groups in the incidence rate of SADEs (p=0.6594). For SADE in the experimental group, ''Pyrexia' and 'Spondylitis' were 2.50%. SADE reported in the control group included 'Chest discomfort', 'Osteoarthritis' and 'Neurogenic bladder'. All SADEs described above were resolved after medical treatment. Conclusion : This study demonstrated that the spinal fusion rates of DBM gel group and DBM gel with rhBMP-2 group were not significantly different. But, this study provides knowledge regarding the earlier postoperative effect of rhBMP-2 containing DBM gel and also supports the idea that the longer term follow-up results are essential to confirm the safety and effectiveness.

Clinical and microbiological effects of adjunctive local delivery of minocycline (Periocline®) in patients receiving supportive periodontal therapy: a pilot study

  • Purpose: This study aimed to evaluate the clinical and microbiological efficacy of adjunctive local delivery of minocycline (Periocline®) in patients receiving supportive periodontal therapy (SPT) after initial treatment. Methods: The participants were 16 men and 8 women (age, 20-65 years) who had at least 15 natural teeth, underwent SPT for more than 1 year due to chronic periodontitis, had 4 or more periodontal pocket sites deeper than 5 mm, and showed >25% gingival bleeding on probing (BoP). They were randomly assigned to the test and control groups. In the test group, mechanical debridement and local antibiotic delivery were performed for all periodontal sulci/pockets; in the control group, mechanical debridement and saline irrigation were performed. In patients who underwent SPT for more than 1 year, clinical and microbiological examinations were performed at baseline and 1 and 3 months after SPT. The clinical examination included an assessment of the periodontal pocket depth, clinical attachment level, plaque index, and BoP. Microbial tests were performed using real-time polymerase chain reaction; the relative ratios of Porphyromonas gingivalis and Fusobacterium nucleatum were determined. Results: Both groups showed significant improvements in clinical parameters at 1 and 3 months from baseline; there were no significant changes between months 1 and 3. Intergroup differences were insignificant. The microbiological analysis revealed no significant differences in P. gingivalis and F. nucleatum ratios across time points. While intergroup differences were insignificant, there was a tendency for the P. gingivalis and F. nucleatum ratios to decrease in the test group. Conclusions: Mechanical debridement in patients receiving maintenance therapy resulted in clinically significant improvement; the effectiveness of additional local delivery of antibiotics was not significant. The ratios of P. gingivalis and F. nucleatum showed a tendency to decrease in the test group, although it was not significant.

Collision Risk Assessment by using Hierarchical Clustering Method and Real-time Data (계층 클러스터링과 실시간 데이터를 이용한 충돌위험평가)

  • The identification of regional collision risks in water areas is significant for the safety of navigation. This paper introduces a new method of collision risk assessment that incorporates a clustering method based on the distance factor - hierarchical clustering - and uses real-time data in case of several surrounding vessels, group methodology and preliminary assessment to classify vessels and evaluate the basis of collision risk evaluation (called HCAAP processing). The vessels are clustered using the hierarchical program to obtain clusters of encounter vessels and are combined with the preliminary assessment to filter relatively safe vessels. Subsequently, the distance at the closest point of approach (DCPA) and time to the closest point of approach (TCPA) between encounter vessels within each cluster are calculated to obtain the relation and comparison with the collision risk index (CRI). The mathematical relationship of CRI for each cluster of encounter vessels with DCPA and TCPA is constructed using a negative exponential function. Operators can easily evaluate the safety of all vessels navigating in the defined area using the calculated CRI. Therefore, this framework can improve the safety and security of vessel traffic transportation and reduce the loss of life and property. To illustrate the effectiveness of the framework proposed, an experimental case study was conducted within the coastal waters of Mokpo, Korea. The results demonstrated that the framework was effective and efficient in detecting and ranking collision risk indexes between encounter vessels within each cluster, which allowed an automatic risk prioritization of encounter vessels for further investigation by operators.
