Effects of Early Passive ROM Exercise on ROM of Lower Extremities and Foot Edema in Hemiplegia Patients with Stroke (조기 수동적 하지 관절운동이 뇌졸중 후 편마비 환자의 하지 관절가동범위와 발부종에 미치는 효과)

  • Purpose: The purpose of this study was to examine the effects of early passive range of motion (ROM) exercise on ROM of lower extremities and foot edema in hemiplegic patients after stroke. Methods: The data were collected between August 2009 and April 2010 from 11 patients in the experimental group and 13 in the control group. The passive ROM exercise was performed twice a day for 2 weeks. Results: In the experimental group, ROM of lower extremities (flexion of hip, flexion of knee and ankle) increased significantly compared to that of the control group. There was no significant difference in foot edema between the two groups. Conclusion: The results indicated that the early passive ROM exercise can improve the ROM of lower extremities, but not the foot edema in patients after stroke.

A Case Report of Sequela of Operation of Hallux Valgus (족부 무지외반증 수술 후 후유증 치험 1례)

  • Objectives : This study aims to report the effect of Korean medicine treatments on a sequela of operation of hallux valgus. Methods : The patient was treated with Dangkisoosan-gami, Acupuncture treatment including Herbal-Acupuncture and physical therapy. We evaluated treatment effects by circumference of foot and ankle, Edema index score by Body Composition Analyzer, Ankle-Brachial Index (ABI) and Visual Analogue Scale (VAS). Results : After 25 days of treatment, the symptoms such as lower extremity edema, foot pain and ankle pain were improved. Conclusions : In this case, Korean medicine treatments for a sequela of operation of hallux valgus was effective. But further studies are required to confirm the effect of these methods.

Lower Extremity Edema in Terminal Cancer Patients (말기 암 환자에서의 하지 부종)

  • Purpose: The lower extremity edema (LEE) is a common distressful symptom in advanced cancer patients and is hard to manage. We analyze the characteristics of LEE in patients with advanced cancer to provide the basic information of causes and adequate management. Methods: Physical examination, assessment of the location and severity of edema, blood chemistry (albumin, creatinine), Doppler Sono for patients with suspecting deep vein thrombosis (DVT), and abdomen CT scan for patient with suspecting lymph edema were performed. Severity of edema was classified according to NCI lymph edema scaling and improvement was defined as lowering at least 1 grade of edema after management. Results: Among 154 patient who had been admitted to Hospice Ward from Mar 2003 to Jan 2004, 33 had LEE, and 6 had both upper extremity edema and LEE except generalized edema. Their underlying cancers were stomach (7), lung (6), biliary tract (5), liver (5), colorectal (5), pancreas (2), and others (9). There were 12 patient with grade I, 20 patients with grade II, and 7 patients with grade III edema. The causes were hypoalbuminemia (11), lymph edema (10), DVT (7), obstruction of inferior vena cava (IVC) or portal vein (6), and dependent edema (5). The common managements were including leg elevation and diuretics. Elastic stocking was applied for patients with DVT and leg massage and pneumatic compression was used for lymph edema. The 2/3 patients were improved after management. Conclusion: The incidence of LEE in terminal cancer pts was high (25.3%) and their causes were variable including lymph edema, DVT, hypoalbuminemia and dependent edema. Active noninvasive management according to causes could result in good palliation.

The Effect of Aroma Therapy on Lower Extremity Edema of Terminal Cancer Patients: A Controlled Trial (아로마 요법이 말기 암 환자의 하지부종에 미치는 영향 - 대조군 연구)

  • This study was designed to examine the effect of aroma massage therapy on lower extremity edema of terminal cancer patients. Methods: A total of thirty-six terminal cancer patients with lower extremity edema were divided into two groups: the aroma massage group received massage with blending oil which was applied from toes to 10 cm above the knee of the subject for 15 to 20 minutes in each turn, while the control group received sham aroma massage (applied with carrier oil only). The circumferences of the fore-foot, ankle and calf were measured before massage and 30 minutes, 2 hours, and 12 hours after massage. The blood pressure, pulse and body temperature were also measured to find the change of subject's physiologic conditions. Results: There were no significant differences in blood pressure, heart rate, body temperature and lower extremity circumferences between two groups. However, edema at each site was slightly improved in the treatment group after the aroma massage therapy, compared to baseline data (P<0.05). In addition, the reduction of lower extremity circumference was maximal at 2 hours in foot, 30 min in right ankle and 12 hours in right calf after aroma massage therapy (P<0.05). Conclusion: Our results suggest that aroma massage therapy is not effective on the lower extremity edema of terminal cancer patients.

  • Background: There have been no reports of studies on the effect of a combination of myofascial relaxation technique and passive stretching on the lower extremity body shape of working women. Purpose: This study aimed to examine the effects of myofascial relaxation technique and passive stretching on body composition and body composition analysis (intracellular fluid, skeletal muscle mass, body cell mass), etc. Methods: The subjects of this study were 30 women at a body shape management center who had many problems with their subjective lower extremity body shape. Fifteen subjects were in the experimental group and the control group. The experimental group applied the fascial relaxation technique twice a week for nine weeks. The control group conducted stretches by themselves at least twice a week according to the active stretching instructions. The ANOVA program analyzed the data. Results: In the experimental group, intracellular fluid (p < .05), skeletal muscle mass (p < .048), and body cell mass (p < .047) were significantly increased.   • Purpose: This study was to identify the effects of the application of ankle pumping exercise versus elastic compression stockings for the women lower leg edema and pain due to continuous standing work. Methods: The participants were 20 women with leg edema and pain, divided into two group, 10 ankle exercise group and elastic compression stockings group. Low leg circumference were measured before and after work. Ankle pumping exercise group, elastic compression stockings were divided into groups, individual working one week during the arbitration to arbitration and work around the legs before and after retesting were compared before and after arbitration. Arbitration to stop and keep the information a week after the re-measurement of intervention before and after the arbitration and the degree of intervention were maintained. Statistical methods before and after working around the average value of each the legs was repeated measures ANOVA. Results: The participants had edema and leg problems because of their work. The ankle pumping exercise gorup and elastic compression stockings group showed statistically decrease in leg circumference value. there was no statistically value about the lasting effect of intervention after 1 week. Conclusion: The results of this study revealed that the application of elastic compression stockings and ankle pumping exercises during the hours of work is effective the relief of edema of lower extremity.

Effects of Foot Bath and Spray Application of Peppermint and Grapefruit Essential Oils on Lower Extremity Edema, Pain, and Fatigue (페퍼민트, 그레이프후룻 에센셜 오일을 이용한 족욕 또는 스프레이 적용이 하지부종, 통증 및 피로에 미치는 효과)

  • Work involving standing or walking for a long time can cause swelling, pain, and fatigue in the lower extremities due to the effect of gravity on the circulation of the body. This study aimed to confirm the effects of aromatherapy essential oils, as a complementary and alternative therapy, on lower extremity edema, pain, and fatigue when applied as a foot bath or spray. The study was conducted after daytime work over two consecutive days on nurses who volunteered at the E University Hospital in D Metropolitan city between 15 August 2018 and 11 October 2018. This randomized controlled study. The foot bath group received a foot bath in 9 L of water containing 0.5 ml of essential oil blended with grapefruit and peppermint in a 1:1 ratio. The spray group received the same solution in 3% dilution in the form of a spray. Edema was assessed by measuring calf and ankle circumference, while pain and fatigue were evaluated using a numerical rating scale. Data were analyzed by ANOVA and repeated-measures ANOVA. The calf circumference in the foot bath and spray groups significantly reduced compared to that in the control group (F = 14.053, p <.001) after the interventions. Lower extremity pain (F = 42.497, p < .001) and fatigue (F = 20.641, p < .001) also significantly reduced in the foot bath and spray groups compared to those in the control group after the interventions. Foot bath is recommended to relieve swelling and pain in the lower extremities, and aroma spray is highly recommended considering the ease of application.

Effect of Sequential Intermittent Pneumatic Compression for Lymphedema (임파부종 환자의 간헐적 공기압박 치료의 효과)

  • The purpose of this study was to investigate the effect of sequential intermittent pneumatic compression in patients with lymphedema who were hospitalized for three days at Samsung Medical Center. Thirty-three subjects with lymphedema was selected by physician referral. A selection of the patients was made according to the following criteria : 1) no known metastases, 2) no infection after the a difference of at least 10% in the volumes of edematous limb. All patients were treated with the Lympha-Press(Model 103-A). Circumferential limb mesurements was done before and after a 3-day treatment period. As a result of sequential intermittent pneumatic compression therapy. All extremity showed a decrease in circumferential measurements with the maximal reduction occurring at the wrist(55.75 %) for the upper extremities and at the lower 1/3 of calf (40.61%) for the lower extremities. Upper extremity arm was reduced by 42.1% and lower extremity leg by 33.61%. In contrast with this, the proximal levels of arm patients and leg showed comparatively poor reduction than distal levels. Almost 44.44% of arm patients and 5.26% of leg patients experienced significant reduction(> 50%) after compression therapy. These data clearly indicates that Sequential Intermittent Pneumatic Compression Therapy was effective treatment for reducing of extremity volume in patients with lymphedema. but this was variable degree and dependent on the amount of pre-existing lymphedema. Other factors such as duration of edema, etiology, previous history of radiology, age did not appear to influence the extend of improvement. Further sutdies will be necessary to determine long term benefit of this therapy.

  • Rhabdomyolysis is a syndrome caused by injury to skeletal muscles and involves leakage of large quantities of potentially toxic intracellular contents into the plasma. It is known that rhabdomyolysis results in peripheral nerve injury, however, reports of bilateral sciatic neuropathy following rhabdomyolysis are rare. We report a case involving a 42-year-old female patient with no past medical history, who presented with sudden bilateral calf pain, redness, and burning sensation with weakness of both lower extremities after sleeping on an electric heating pad following alcohol drinking. Lower extremity magnetic resonance angiography (MRA) revealed multifocal edema with enhancement of bilateral lower extremity muscles. Clinical and electrodiagnostic tests were consistent with the diagnosis of bilateral sciatic neuropathy following rhabdomyolysis. This is a rare case of bilateral sciatic neuropathy following rhabdomyolysis.

Massive Edema of the Lower Extremity after Surgery for Abdominal Aortic Aneurysm, A case report (복부 대동맥류 수술후 발생한 하지의 심한 부종)

  • 71 years old man was operated on due to abdominal aortic aneurysm associated with complete occlusion of left common iliac artery. The coexisting chronic deep vein thrombosis of the left femoral and iliac vein was not diagnosed preoperatively. Resection of aneurysm and Y-graft interposition was performed. Recurrent edema and pain occured to the left lower extremity immediately postoperatively, which aggrevated with the lapse of time, resulting in fatal extensive venous thrombosis. This report regards the surgical treatment and complication of the aortoiliac occlusive disease associated with chronic deep vein thrombosis.
