Improvement of Post-inflammatory Hyperpigmentation, Subsequent to Cold Atmospheric Plasma Treatment, in a Patient with Atopic Dermatitis

  • Cold atmospheric plasma (CAP) is a material that generates free radicals through the ionization of air. Despite the application in various medical fields, the efficacy and safety of CAP on post-inflammatory hyperpigmentation due to atopic dermatitis have not been reported. Herein, we report the improvement in post-inflammatory hyperpigmentation of atopic dermatitis, after CAP treatment of three sessions weekly, 10 minutes per session. We examined the clinical severity indexes before and after treatment, including the Investigator's Global Assessment, modified Atopic Dermatitis Antecubital Severity, Eczema Area and Severity Index, and pruritus visual analogue scores. Our results indicate that CAP alleviates the post-inflammatory hyperpigmentation and clinical severity of atopic dermatitis, and effectively improves the atopic skin lesion without severe safety issues. We believe that the previously reported anti-bacterial effects and production of interleukin-6 after CAP treatment are probably responsible for the underlying mechanisms leading to clinical improvement.

A Case of improvement in a patient with dermatitis by using Ortho-Cellular Nutrition Therapy (OCNT) (세포교정영양요법(OCNT)을 이용한 피부염증 환자 개선 사례)

  • Objective: A Case report on the improvement of dermatitis through implementation of OCNT. Methods: OCNT was implemented for 3 months for a Korean man in his 70's with dermatitis with unknown cause since 2015. Results: Following the implementation of OCNT, fever that accompanied dermatitis subsided, occurrence of itchiness and abscess decreased, and the stratum corneum of the palm although displayed improvement of being thinned. As the result of visual follow-up observation upon discontinuation of OCNT, damaged skin due to inflammation was restored to the level of normal skin. Conclusion: OCNT can provide assistance to those experiencing inconvenience due to dermatitis.

Therapeutic Effects of Cheonggi-san Extract on NC/Nga Mice with Atopic Dermatitis-like Skin Lesions (청기산(淸肌散)이 아토피피부염 동물 모델에 미치는 영향)

  • Background and Objectives : Atopic dermatitis is a recurrent or chronic eczematous skin disease with severe pruritus,and has increased in Korea. Although the pathogenic mechanisms of atopic dermatitis are yet unknown, recently skin barrier dysfunction and hyperresponsive Th2 cells in the acute phase have been reported as important mechanisms. Cheonggi-san(CGS) is used in oriental clinics for treatingacute skin lesions of eczema or urticaria. There have been no studies on the therapeutic mechanism of CGS for curing atopic dermatitis. We aimed to find out the therapeutic effects of its internaluse on atopic dermatitis-like skin lesions, induced in NC/Nga mice by the mite antigen D. pteronyssinus and disrupting skin barrier. Materials and Methods : The NC/Nga mice were classified into three groups: control group, atopic dermatitis elicitated group(AD), and CGS treated group (CT). Atopic dermatitis-like skin lesions were induced on the back of female NC/Nga mice, 12 weeks of age, by tape stripping, 5% SDS applied to disrupt skin barrier and painting 3 times a week with D. pteronyssinus crude extract solution for 3 weeks. CT was treated with CGS orally after atopic dermatitis was elicitated. We observed changes of skin damage, mast cells, substance P, angiogenesis, skin barrier, Th2 cell differentiation, nuclear factor-${\kappa}B(NF-{\kappa}B)$ p65 activation and COX-2 in NC/Nga mice with atopic dermatitis-like skin lesions. Results : The skin damages as eczema were seenin AD, but mitigated in CT. The degranulated mast cells in dermal papillae increased in AD, but decreased in CT. The substance P positive reacted cells in CT remarkably decreased. The angiogenesis increased in AD, but decreased in CT. The decrease of lipid deposition and ceramide in AD was seen, but anincrease of lipid deposition and ceramide in CT was seen. The distribution of IL-4 positive reacted cells in dermal papillae increased in AD, but decreased in CT. The distribution of NF-${\kappa}B$ p65 positive reacted cells & COX-2 positive reacted cells in CT decreased. Conclusion : The results may suggest that the CGS per os decreases the dysfunction of the skin barrier, inhibits Th2 cell differentiation and inhibits NF-${\kappa}B$ p65 activation in NC/Nga mice with atopic dermatitis-like skin lesions.

Health Care Utilization of Workers with Skin Disease in Inchon (인천지역 일부 근로자들의 피부질환으로 인한 의료이용에 관한 연구)

  • Objectives: This study was carried out to identify relation of health care utilization due to skin disease(ICD-10, L00-L99) and characteristics(sex, age, exposure status, type of industry, size of enterprise) of workers. Methods: We made new database composed of 30,536 workers' health examination results in one specific health examination institute in Inchon and data of medical insurance utilization due to skin disease in 4 medical insurance associations for enterprise from January, 1995 to December, 1997 And we analyzed determinants of health care utilization due to skin disease of workers. Results: Among 30,536 study subjects, 8,837(28.9%) workers and 4,181 (13.7%) workers utilized medical insurance due to total skin disease(ICD-10, L00-L99) and contact dermatitis(ICD-10, L23-L25), respectively. Female workers(p<.001), workers exposed to organic solvents(p<.05), workers if manufacturing industries(p<.05, p<.01, respectively) and in large scale enterprises(p<.001) utilized more medical insurance due to total skin disease and contact dermatitis than male workers, workers not exposed to organic solvents, workers in non-manufacturing industries and small scale enterprises. With multiple logistic regression analysis, significant explanatory variables affecting workers' medical utilization due to total skin disease and dermatitis and eczema(ICD-10, L20-L30) in total workers were sex, age, specific chemicals and size of enterprises. And age, type of industries, organic solvents, specific chemicals and size of enterprises were significant explanatory variables related to medical insurance utilization due to dermatitis and eczema and contact dermatitis in male workers. Conclusion: From the above results, we found that workers exposed to organic solvents utilized more medical insurance due to skin disease than workers not exposed to. And, comparing to workers in large scale enterprises, workers in small scale enterprises may have unmet medical care utilization due to skin disease. Therefore we have to establish counterplan to manage occupational skin disease of high risk group(organic solvent exposure group) and to satisfy unmet medical care utilization of workers in small scale enterprises.

A Statistical Analysis of Outpatients in Dermatology of Korean Medicine at Kyung Hee University Korean Medicine Hospital in 2011-2015 (2011-2015년 경희대학교 한방병원 한방피부과 외래환자에 대한 통계적 분석)

  • Objectives : This study was performed to find out the characteristics of outpatients and diseases in dermatology of Korean medicine and compare to previous studies.Methods : We reviewed the medical records of total 3,041 outpatients in dermatology of Korean medicine at Kyung Hee Korean medicine hospital from January 1, 2011 to December 31, 2015. We found out demographic characteristics of outpatients and observed the diseases in order of frequency by age and sex. Then, we compared these results to previous studies performed at same hospital in 2000 and 2007.Results and Conclusions : 1. Among the total 3,041 outpatients in 2011-2015, male patients were 1,184(38.87%) and female patients were 1,857(61.07%). 2. There were 203(6.68%) patients in 0-10 years old, 455(14.96%) patients in 11-20, 918(30.19%) patients in 21-30, 582(19.14%) patients in 31-40, 359(11.81%) patients in 41-50, 265(8.71%) patients in 51-60, and 259(8.52%) patients in more than 61 years old. 3. The common diseases in 2011-2015 were seborrheic dermatitis(17.33%), eczema(13.32%), urticaria(12.89%), atopic dermatitis(12.36%), pruritus(9.90%), acne(8.85%), and alopecia(6.08%). 4. The most common diseases by age groups were atopic dermatitis(0-10, 11-20 years old), seborrheic dermatitis(21-30, 31-40, 41-50 years old), urticaria(51-60 years old), and pruritus(more than 61 years old). 5. The common diseases in male patients were seborrheic dermatitis(20.69%), atopic dermatitis(15.46%), and urticaria(12.25%) and female patients were seborrheic dermatitis(15.19%), eczema(15.13%), and urticaria(13.30%). 6. The most increasing disease in the distribution between 2004-2006 and 2011-2015 is rosacea(+462.93%), and the most decreasing disease is vitiligo(-72.05%).

The Statistical Study of Dermatoses Outpatients in Oriental Hospitals Located in Gwangwon Province (강원지역 소재 한방병원 피부과 외래환자의 임상적 분석)

  • Objective : The purpose of this study is to expand our treatment field in oriental dermatology by understanding the patterns of outpatients who had visited the oriental hospitals located in Gangwon province. Methods : We studied 510 outpatients out of 2240 people from Sangji University Oriental Hospital(SUOH) and 333 outpatients out of 2030 people from Chuncheon Oriental Hospital(COH) from January 1, 2007 to December 31, 2009. We categorized the results by gender, age, disease types, residences, months and number of visiting times. Results : The results were as follows 1. Comparing the number of outpatients that had visited the hospital by gender in SUOH shows that female's visiting rate is 57% and male is 43%. In COH shows that female's visiting rate is 54% and male is 46%. 2. Most frequent visiting age group were 21-30 year-old group(30%) in SUOH and 11-20 year-old group(23%) in COH. 3. In regard to the monthly visiting period, January(13%) is the highest in SUOH and July(10%) is the highest in COH. 4. Regional distributions of the outpatients that had visited SUOH and COH show Wonju(73%) and Chuncheon(74%). 5. In terms of treatment methods, SUOH showed the external application(65%), herb medication(54%), and acupuncture(52%) while COH showed acupuncture(74%), the external application(69%), and herb medication(66%). 6. Visiting frequency of outpatients in SUOH showed that 1 time visiting cases were 51%. In COH case, 1 time visiting cases were 19%. 7. The most prevalent diseases were urticaria(15%), eczema(14%) acne(12%), and seborrheic dermatitis(11%) in SUOH and atopic dermatitis(27%), urticaria(20%), acne(16%), and psoriasis(10%) in COH. 8. Most outpatients had visited SUOH for 1 time(51%) and COH for over 11 times(32%) with the major 16 diseases. Conclusions : This study suggests that outpatients who had visited the department of oriental dermatology preferred to get a treatment for urticaria, eczema, atopic dermatitis and acne. Considering this aspect, it needs to improve current research to expand the oriental medical treatments further in dermatological field.

Kaposi's Varicelliform Eruption in Atopic Dermatitis treated with Korean medicine

  • Objectives: This case report is to present a complete recovery from Kaposi's varicelliform eruption (KVE) that occurred in a patient with atopic dermatitis by applying Korean Medicine therapies. Methods: Hwangyeonhaedoktang pharmacopuncture (HP), 0.3 mL, and 25% bee venom pharmacopuncture (BVP), 0.1 mL, were injected, 0.2 mL each, at both BL13 acupoints once a day in the morning. Acupuncture was applied at Sama Upper, Middle and Lower of the Master Tung acupuncture points and at ST44 on the left lateral for 30 minutes twice a day. The affected face was gauze dressed with mixture of 2.0 mL HP and 1.0 mL 25% BVP with 20 mL of normal saline twice a day. Herbal Medicine, Seungmagalgeuntang, was administered three times a day after each meal. Results: Rashes and papules on the face were completely cleared after 10 days of treatments. Conclusion: KVE, an acute and urgent dermatitis, can be effectively treated with Korean medicine.

Simultaneous Determination of the Seven Phenylpropanoids in Xanthii Fructus Using a HPLC-PDA and LC-MS

  • Xanthii Fructus has been traditionally used for the treatment of rhinitis, rheumatoid arthritis, and eczema. In this study, a high-performance liquid chromatography-photodiode array (HPLC-PDA) method was developed and then used for the simultaneous analysis of eight phenylpropanoids in Xanthii Fructus. The analytical column used for this separation was a $SunFire^{TM}$ $C_{18}$ column, maintained at $40^{\circ}C$. The mobile phase used was 1.0% acetic acid in distilled water and 1.0% acetic acid in acetonitrile with gradient elution. For identify of each component, the mass spectrometer (MS) was used a Waters triple quadrupole mass spectrometer requipped with electrospray ionization (ESI) source. The HPLC-PDA method showed good linearity: correlation coefficients were ${\geq}0.9996$. The limits of detection and quantification of the eight compounds were 0.02 - 0.04 and $0.06-0.14{\mu}g/mL$, respectively. The extraction recoveries ranged from 97.51 to 108.67%. The relative standard deviation values of intra- and inter-day precision were 0.06 - 1.55 and 0.09 - 1.68%, respectively. The validated HPLC-PDA method was applied to simultaneously analyse the amounts of eight phenlypropanoids in Xanthii Fructus.

A Observational Study on Children with Atopic Dermatitis in Atopic-Free Village (아토피 안심마을로 이주한 아토피 환아의 관찰연구)

  • Objectives This study aimed to evaluate the effects of atopic care program through observing atopic patients who have lived in atopic-free village located Geumsan of Chungnam province. Methods Seventeen children (boy 7, and girl 10) with atopic dermatitis had moved into atopic-free village of Geumsan, and they have been cared with atopic care program. Atopic dermatitis symptoms were measured every month using SCORAD (Scoring of Atopic Dermatitis) and EASI (Eczema Area and Severity Index) from March, 2012 to September, 2013. The changes of scores were analyzed using paired t-test. Results The median month of residence period was 19.9 months (range from 6 to 42 months). The atopic symptoms were significantly improved on SCORAD (p<0.01) and EASI (p<0.1) respectively. Conclusion Despite the limitation of the small number of study subjects, this study may provide the possibility of natural environment-based therapy for children with atopic dermatitis.

The Role of the Immune System in the use of Probiotic Lactic Acid Bacteria in Preventing and Treating Allergic Diseases

  • The immune system is generally divided into the innate and the adopted immune systems, both protecting the body from pathogens. Recently, allergies, a disease associated with an imbalanced immune system, have increased rapidly in developed countries. Prevailing symptoms of allergic diseases are eczema, allergic rhinitis, asthma, inflammatory bowel disease, and food allergy. Probiotic bacteria, mainly consisting of lactic acid bacteria, are used in the prevention and treatment of allergic diseases. The function of them is to stimulate the intestinal immune cells and form a complex signal network to activate other immune cells. Beneficial health effects of probiotics are based on the hygiene hypothesis, which suggests that sanitary environment is important for health, but limited exposure to environmental factors increases allergic diseases. An immunoregulatory effect of probiotic bacteria is demonstrated by controlled trial, animal model, in vitro, in vivo and ex vivo designs. However, the immunoregulatory effect of probiotic bacteria is controversial because it depends on probiotic strains, a dose and a type of diseases. In this review, we discussed clinical evidences on immunoregulatory effects of probiotic bacteria.
