중심외주시 훈련 후 망막 외망상층에서의 신경 재조직화 (Neural Reorganization in Retinal Outer Plexiform Layer Induced by Eccentric Viewing Training)

  • 목적: 단기간의 중심외주시 훈련 후 발생하는 신경의 재조직화의 특성과 호발 위치를 알아보고자 했다. 방법: 정상시력을 가진 성인 14명을 대상으로 21일 간 중심외주시 훈련을 하고 훈련 전후 광지각도와 다국소망막전위도를 측정하여 사후 분석했다. 결과: 중심외주시 훈련 전후 값을 비교한 광지각도 검사(p<0.047)에서 뿐만 아니라 다국소망막전위도 검사에서도 유의한 개선을 보였다(p<0.028). 결론: 시각 말초신경계는 재생이 불가능하지만 단기간의 중심외주시 훈련은 말초신경계에서 신경 재조직화를 발생시킨다.

Effects of Isokinetic Eccentric Training on Lower Extremity Muscle Activation and Walking Velocity in Stroke Patients

  • Purpose: The aim of this study was to determine the effects of isokinetic eccentric training (IET) on lower extremity muscle activation and walking velocity according to slow velocity and fast velocity of isokinetic eccentric training in stroke patients. Methods: Thirty subjects were randomly divided into three groups: experimental group I (n=10), group II (n=10), and control group III (n=10). Each group was provided intervention under three conditions, as follows: isokinetic eccentric training + slow velocity (group I), isokinetic eccentric training + fast velocity (group II), and sit to stand training (group III). The training program was conducted for eight weeks (five times per week; 30 minutes per day). Subjects were measured on lower extremity muscle (vastus lateralis, vastus medialis, gastrocnemius) activation and walking velocity. Analysis of covariance (ANCOVA) were performed for comparison of lower extremity muscle activation and walking velocity between different intervention methods. Results: Significant difference in lower extremity muscle activation and walking velocity was observed in experimental group I and group II compared with the control group III (p<0.01). Results of post-hoc analysis showed a significant in lower extremity muscle activation and walking velocity in group I compared with group II and group III. Conclusion: Findings of this study suggest that slow velocity and fast velocity using isokinetic eccentric training may have a beneficial effect on improvement of lower extremity muscle activation and walking velocity in stroke patients.

  • Purpose : The purpose of this article is to summarize the characteristics of isotonic combination. Method : Some studies of the motor unit activation patterns during isometric, concentric, and eccentric actions, neural strategies in the control of muscle force, and concentric versus combined concentric-eccentric training were reviewed. Results & Conclusions : Eccentric torque may be relatively higher than concentric torque for two potential reasons: 1) stretch responses in the antagonist are not elicited to restrain the motion as can occur concentrically and 2) stretch responses in the agonist may augment eccentric torque production. Concentric-eccentric training has a greater influence on functional capacity than that of concentric training. Both maximal force and average force throughout the motion were significantly higher when the dynamic action was started with preactivation as compared to the mode without preactivation. The peak torques observed during the concentric phase of the eccentric-concentric muscle actions were higher than those noted in the pure concentric contraction.

중등도 이상의 어깨통증을 동반한 근육둘레띠(가시위근) 부분파열 환자를 위한 물리치료 프로그램 : 편심성 훈련과 동심성 훈련의 효과 비교 (Physical Therapy Program for Patients with Partial Rotator Cuff (Supraspinatus) Tears with Moderate to Severe Shoulder Pain : Comparison of the effects of eccentric training and concentric training)

  • Purpose : This study was to verify the effectiveness of eccentric training and to make clinical recommendations on detailed application methods by comparing the effects of changes in acromiohumeral distance (AHD), pain intensity, muscle strength, and functional performance after applying a 12-week eccentric training program (ET group) or concentric training program (CT group) for patients with partial rotator cuff (supraspinatus) tears with moderate to severe shoulder pain. Methods : A total of 29 subjects were assigned to either the "ET group (n=15)" or the "CT group (n=14)" through simple randomization and were measured in the same way at baseline before intervention, 4 weeks and 12 weeks after intervention. All subjects received a physical therapy program 12 sessions 3 times a week for the first 4 weeks, and physical therapy program, 12 sessions a week, from 4 weeks to 12 weeks, for a total 20 sessions. Ultrasound machine was used for AHD, visual analog scale (VAS) was used for pain intensity, electronic hand held dynamometer was used for muscle strength, Korean Constant shoulder score (K-CSS) used for functional performance. Results : AHD, pain intensity, and muscle strength did not show significant differences at 4 weeks, but improved numerically, and showed significant differences at 12 weeks (p<.05), showing superior results in the eccentric training group compared to the concentric training group. Functional performance showed significant changes at both 4 and 12 weeks (p<.05), and excellent results were also found in the eccentric training group. Conclusion : In patients with partial rotator cuff tears with moderate to severe shoulder pain, an eccentric training program increased acromiohumeral distance (AHD), reduced pain, and increased muscle strength. Therefore, it can be clinically presented as an intervention method that can quickly and effectively improve functional performance, which is the ultimate goal of physical therapy.

  • Eccentric muscle contraction is more effective than concentric and iosmetric muscle contraction in increasing muscle strength. Also, eccentric or concentric-eccentric training has greater effective in neural activation and muscle hypertrophy than concentric training. In some study, eccentric exercises have been shown to reduce pain and improve function on Achilles tendinopathy. The purpose of this study was to evaluate the effect of eccentric isokinetic exercise in a patient with dislocation of the tarsometatarsal joint by traffic accident. After eccentric isokinetic training, peak torque, average work, and average power were increased. Also, the patient was fully weightbearing with a pain free normal gait thus making good recovery.

Thera-Band를 이용한 단축성 & 신장성 수축 저항 훈련 프로그램이 어깨의 최대 토크와 최대 파워에 미치는 영향 (Effects of Combined Resistance Training Program of Concentric and Eccentric Contraction Using Theraband on Shoulder Rotation Torque Max and Peak Power)

  • 본 연구는 Thera-Band를 이용해 단축성 수축과 신장성 수축이 혼합된 저항운동을 실시하는 것이 단축성 수축 저항 운동만을 실시하는 것보다 근력증진에 더 효과적인지를 알아보고자 하는 논문이다. 연구대상자는 부산광역시 소재 S 대학교에 재학 중인 학생 30명 이었으며, 30명을 무작위로 단축성 수축과 신장성 수축 운동군, 단축성 수축 운동군, 무운동군으로 나누어 4주 동안 운동을 실시하였다. 근력 측정은 등속성 운동기기인 CON-TERX를 이용하였으며, 각속도는 $60^{\circ}$/sec, $120^{\circ}$/sec이었다. 4주 운동 후, 단축성 수축과 신장성 수축 운동군의 경우 각속도 $60^{\circ}$/sec에서 단축성과 신장성 최대 토크와 최대 파워, 각속도 $120^{\circ}$/sec에서 단축성 최대 파워가 통계적으로 유의하게 증가하였다. 반면 단축성 수축 운동군은 각속도는 $60^{\circ}$/sec와 $120^{\circ}$/sec의 최대 파워에서만 유의한 증가가 있었다. 반면 무운동군은 변화가 없었다. 따라서 Thera-Band를 이용해 단축성 수축과 신장성 수축이 혼합된 운동을 실시하는 것이 단축성 수축 운동만을 실시하는 것보다 근력증진에 더 효과적임을 알 수 있었다.

중심외주시 훈련 후 주변시야에서의 대비감도 변화 (Change of Contrast Sensitivity in Peripheral Vision Following Eccentric Viewing Training)

  • 목적: 중심외주시 훈련 전후 대비감도를 검사하여 망막 주변부의 기능적 개선 여부를 알아보고자 하였다. 방법: 정상 시각을 가진 14명의 성인 남녀의 우안을 대상으로 중심와에서 $20^{\circ}$ 이측으로 떨어진 망막 주변부의 대비감도를 검사했다. 60분간 영상을 21일 동안 중심외주시 훈련 후 대비감도를 다시 검사하여 사후 검정했다. 결과: 시간적 가중에 따른 임계점 분석에서 중심외주시 훈련 전 0.7 cpd에서 임계점은 2.67(467 ms)이었으며 훈련 후 2.79(616 ms)로 증가했다 (p<0.05). 훈련 전 3.0 cpd에서 임계점 역시 2.53(341 ms)이었으며 훈련 후 3.04(1102 ms)로 증가했다(p<0.05). 결론: 중심외주시 훈련시 선명하고 작은 글씨체의 문자로 훈련하는 것을 권장하며 짧은 시간동안 자주 반복하는 것이 보다 효과적이다. 아울러, 저시력 환자에 대한 각 병변별 시재활훈련을 할 수 있는 기준에 대한 연구가 절실하다.

Comparison of the Duration of Maintained Calf Muscle Flexibility After Static Stretching, Eccentric Training on Stable Surface, and Eccentric Training on Unstable Surfaces in Young Adults With Calf Muscle Tightness

  • The objective of this study was to determine the duration of maintained calf muscle flexibility gained in young adults with calf muscle tightness, as measured by increases in ankle active and passive dorsiflexion range of motion (DFROM) after three stretching interventions. Twenty subjects (5 men and 15 women) with calf muscle tightness received the following three stretching interventions in one leg (assigned at random): static stretching (SS), eccentric training on stable surface (ETS), and eccentric training on unstable surfaces (ETU). The subjects received all three interventions to the same leg, applied in a random order. Each intervention had a break of at least 24 h in-between, in order to minimize any carryover effect. Each intervention used two types of stretching: with the calf muscle stretched and both knees straight, and with the knee slightly bent in order to maximize the activation of the soleus muscle. All three interventions were performed for 200 seconds. We measured the duration of maintained calf muscle flexibility through active and passive ankle DFROM before intervention, immediately after intervention (time 0), and then 3, 6, 9, 15, and 30 min after intervention. We found a difference in the duration of maintained calf muscle flexibility between the three interventions. In the ETS and ETU interventions, a significant improvement in calf muscle flexibility, both ankle active and passive dorsiflexion ranges of motion (ADFROM and PDFROM), was maintained for 30 min. In the SS intervention, however, ADFROM before 9 min and PDFROM before 6 min were statistically different from the baseline. Our results suggest that ETS and ETU may be more effective than SS for maintaining calf muscle flexibility in young adults.

만성 뇌졸중 환자의 종아리 근육에 적용한 편심성 훈련이 근긴장도와 근력 및 보행에 미치는 영향 (The Effects of Eccentric Training Applied to Calf Muscles on Muscle Tone, Muscle Strength and Gait of Patients with Chronic Stroke)

  • Purpose: This study aimed to investigate the effects of eccentric training applied to the calf muscles on muscle tone, muscle strength, and gait variables in patients with chronic stroke. Methods: Twenty-two participants were divided into experimental (n=12; eccentric training) and control (n=10; static stretching and stretching board) groups. The participants completed 30-minute physical therapy sessions five times a week for three weeks. Calf muscle tone, muscle strength, and gait variables were measured using MyotonPRO, a hand-held dynamometer, and Optogait, respectively, before and after each intervention. Results: Two-way analysis of variance (ANOVA) indicated a significant interaction effect between measurement points and groups in frequency, stiffness, and decrement of the lateral gastrocnemius, medial gastrocnemius, and soleus muscles (p<.05). Paired t-tests showed that the experimental group exhibited significantly decreased frequency and stiffness scores for the lateral gastrocnemius, medial gastrocnemius, and soleus muscles (p<.05), as well as significantly increased decrement and muscle strength scores, gait speed, step length, and stride length (p<.05). Conclusion: The application of eccentric training to the calf effectively reduced muscle tone, increased muscle strength, and improved the gait speed, step length, and stride length of patients with chronic stroke.

한글기반 중심외보기 훈련 프로그램이 읽기속도에 미치는 효과: 사례연구 (Effects of the Eccentric Viewing Training Software Based on Korean for Reading Speed: A Case Study)

  • 본 연구는 중심부 암점 환자에게 적용하기 위한 한글기반 중심외보기 훈련 소프트웨어를 구성하고, 정안인을 대상으로 프로그램의 효과성을 파악하고자 하였다. 한글기반 중심외보기 훈련 소프트웨어는 신속순차시각제시를 이용한 한글자극으로 구성하였다. ABA 설계(ABA design) 개별실험연구설계를 위해 정안인 남자 대학생 2명에게 훈련 전후 기초선 각 3회기, 훈련은 3주간 8회기를 시행하고, 매회기 단문읽기속도를 측정하였다. 중심외보기 훈련 프로그램은 단어읽기 훈련 3단계와 문장읽기 훈련 2단계, 총 5단계로 구성하였다. 구성된 프로그램 훈련 결과, 단문읽기속도는 훈련전보다 훈련 후 2표준편차 이상 증가하였음을 나타냈다. 본 연구를 통하여 한글기반의 체계적인 중심외보기 훈련 소프트웨어를 구성하여 국내 임상 현장에서 적용할 근거를 마련하였고, 사전연구를 통하여 중심외보기 훈련의 가능성을 확인할 수 있었다.
