A Review of Korean Medicine for Clinical Practice Guidelines for Cervical Cancer (자궁경부암의 한의학적 진료에 대한 고찰)

  • The incidence rate of cervical cancer in Korea is still higher than in other developed countries, notwithstanding the national mass screening program. Cervical cancer prognosis is good due to an effective treatment such as surgery or radiation therapy, concurrent chemotherapy in the early stage. However, it is still a poor prognosis when advanced stage or recurrent. Some studies said that combined oriental and western medicine can improve the survival, quality of life, immune function, and decrease side effects in cervical cancer. In the western medical part, clinical practice guideline was published in 2006, 2007 and 2010 in Korea. In China, oriental medicine clinical practice guideline have been published in 2014. But there is no Korean medicine clinical practice guideline. This study will introduce the methods of diagnosis and the medical therapeutics which is commonly utilized for cervical cancer in Korea, and existing Korean medicine clinical practice guideline for Disease Analysis and Treatment (辨證論治). This study will be a meaningful study to establish clinical practice guidelines of Korean Medicine for cervical cancer.

Increased whole grain, fruits and vegetable intake reduced oxidative stress in high school students (전곡류 및 채소, 과일 섭취의 증가가 청소년의 체내 항산화능 개선에 미치는 영향)

  • There is increasing evidence that dietary factors in plant-based diets are important for the prevention of chronic disease. Especially, phytonutrients in fruits and vegetables have been recognized as major contributors for the decreased level of oxidative stress. In this study, the effect of switching the dietary habit to high consumption of fruits and vegetables were evaluated on the parameters of serum antioxidant status in healthy high school students. Forty one students participated in a randomized controlled trial and were assigned to the control group (n = 18) or the intervention group (n = 23). The intervention group was provided for 8 weeks with the main food source being whole grain and vegetables. Anthropometric measurements, blood parameters and dietary intakes were measured, and compared before and after study. After 8 weeks, weight and BMI were significantly decreased in the intervention group (p = 0.000). The serum diacron reactive oxygen metabolites (d-ROMs) test resulted in a significantly decreased level only in the intervention group (p < 0.05) after 8 weeks, but serum biological antioxidant potential (BAP) was increased significantly in both groups (p < 0.001). Intake of energy, total fat, cholesterol and sodium in the intervention group were significantly decreased after 8 weeks (p < 0.05). Also, the intervention group had significantly increased vitamin and phytonutrient intakes of all-trans-${\beta}$-carotene, ${\alpha}$-carotene, ${\alpha}$-tocopherol, ascorbic acid, and total phenols (p < 0.05). Overall, the results of this study suggest that whole grain, fruits, and vegetables supplementation showed improvement of the adolescent health.

One Case Report of Non-small Cell Lung Cancer Patients Treated with Allergen Removed Rhus Verniciflua Stokes(aRVS) (알러젠 제거 옻나무 추출물 투여로 삶의 질 개선과 생존기간 연장을 보인 비소세포성 폐암 환자 1례)

  • Non-small cell lung cancer (NSCLC) is the leading cause of cancer-related mortality worldwide. Standard treatment such as chemotherapy, radiotherapy, and surgery have many limitations in the treatment of this disease. Therefore, new molecular-targeted therapies needed and being developed. Patients with advanced NSCLC have a short life expentancy; therefore, in addition to increasing their survival, improving their quality of life (QoL) is also an important treatment goal. In this case report, we introduce NSCLC patient treated with Allergen Removed Rhus Verniciflua Stokes(aRVS). In this case, survival time incresed as traditional korean medicine using aRVS. And the general condition of the patient became better. Further case study will be needed in order to determine the effects of aRVS on NSCLC.

Universal index for Sasang typology using Cloninger's biopsychological theory (Cloninger의 이론을 활용한 사상체질 생리심리 지표 연구)

  • Introduction Cloninger's Novelty-Seeking (NS) and Harm-Avoidance (HA) were found to be the biopsychological characteristic of Sasang typology, and the So-Yang type has high NS and low HA however the So-Eum type as contrary. The purpose of this study was to show the clinical usefulness of these measures as objective clinical index of Sasang typology in foreign countries lacking validated measures. Methods The Sasang type of 103 university students was diagnosed using Questionnaire for Sasang Constitution Classification (QSCC) II and clinical specialist, and biopsychological and physical features with NS, HA, Sasang Personality Questionnaire (SPQ) and BMI. These measures were normalized to make a noble index of 'NS(100-HA)', and the correlation among these were examined with Pearson's correlation. The differences of biopsychological and physical features between Sasang type groups were attested with ANCOVA, and the comparison of clinical usefulness of SPQ and NS(100-HA) were examined with discriminant analysis. Results SPQ and BMI were clinically useful for Sasang typology as shown in previous studies, and the substitution of SPQ with normalized NS(100-HA) was found acceptable. The NS(100-HA) is significantly correlated with SPQ (r=0.466, p<0.01), however not with BMI (r=0.079, ns). The SPQ and NS(100-HA) scores were found to be distinctive between Sasang type groups, and these were found to predict Sasang type of participants with similar correctness. Discussion and Conclusion The current study reviewed the theoretical backgrounds and confirmed the clinical usefulness of Cloninger's biopsychological theory in Sasang typology. These might provide foundations for integrative medicine and cross-cultural biopsychology of the East and West.

Nutritional Status and Hair Mineral Content of Elementary School Children with Behavioral Problems (문제 행동을 가진 초등학생의 영양 상태와 모발 미네랄 함유량)

  • This study was performed to assess the nutritional status and hair mineral content of children with behavioral problems and compare the values with a gender, age-matched control group. The subjects were recruited from S elementary school children in Seoul, Korea. Students scored higher than 60 points were diagnosed with behavioral problems according to the Korean-Child Behavior Checklist. Nutritional assessment of the two groups (behavior problem group: male n=15, female n=24; $9.6{\pm}1.6$ years, control group: male n=16, female n=18; $9.5{\pm}1.9$ years) was performed using a nutritional survey and by measuring hair mineral contents. In the results, food frequency questionnaire analysis showed that the intakes of anchovy (P<0.05), soybean curb (P<0.01), radish (P<0.05), bean sprouts (P<0.05), spinach (P<0.05), carrot (P<0.05), pumpkin (P<0.05), lettuce (P<0.05), cabbage (P<0.01), apple (P<0.05), and milk (P<0.01) were higher in the control group than the behavior problem group, whereas intakes of ramyeon (P<0.05), cookies (P<0.05), and coke (P<0.01) were higher in the behavior problem group than the control group. Intakes of most nutrients such as plant-derived protein (P<0.05), fiber (P<0.05), plant-derived calcium (P<0.05), phosphorus (P<0.05), plant-derived iron (P<0.05), vitamin B2 (P<0.05), vitamin B6 (P<0.05), vitamin C (P<0.01), vitamin E (P<0.05), and folate (P<0.05) were significantly higher in the control group than the behavior problem group. Hair analysis showed that the levels of arsenic (P<0.05), mercury (P<0.001), uranium (P<0.05), iron (P<0.001), boron (P<0.01), and germanium (P<0.001) were lower, but the levels of phosphate (P<0.05), chromium (P<0.001), sodium (P<0.05), and sulfur (P<0.001) were higher in the behavior problem group than the control group. Conclusively, behavioral problems constitute a complicated condition in which nutritional factors may play major roles. However, it is still under investigation as to whether or not modification of dietary habits or nutritional supplementation can improve children's behavior, since symptoms require a broad understanding of the environmental and genetic interactions.

Preliminary Study for Development of Korean Medicine Clinical Practice Guideline for Pancreatic Cancer (췌장암 한의 임상진료지침 개발 예비 연구)

  • Objectives: The purpose of this study is to investigate preliminarily for development of the Korean medicine clinical practice guideline (CPG) for pancreatic cancer through the analysis of existing CPGs. Methods: Through searching the medical database such as Pubmed, SCOPUS, CNKI, Google Scholar, etc. The global CPGs within recent three years were collected and analyzed. In particular, recommendations related to the Korean medicine or Chinese medicine were made primarily in the Guidelines of Diagnosis and Therapy in Oncology with Traditional Chinese Medicine. Results: The six CPGs were mentioned in this study. The academic societies and organizations developing the CPGs were located in China, Japan, Europe and America. The contents of the CPGs were the clinical questions and statements, surgical therapy, adjuvant therapy, radiation therapy, chemotherapy, palliative medicine, risk assessment, palliation and supportive care, follow-up and recurrence, Tumor-Node-Metastasis (TNM) staging. In the Guidelines of Diagnosis and Therapy in Oncology with Traditional Chinese Medicine, the etiology, mechanisms, herbal drugs, Chinese medicine assessment, complication, syndrome differentiation (SD), Chinese medicine treatment were described. Conclusions: In order to develop the proper Korean medicine CPG for pancreatic cancer and to adapt the correct integrative treatment program on the pancreatic cancer, institutional arrangements for cooperation with Korean medical communities and standardization of SD should be performed.

A Case of Refractory Ascites Due To Hepatocellular Carcinoma Treated with Sasang Constitutional Medicine (간세포암에 동반된 난치성 복수를 호전시킨 소양인 환자 치험1례)

  • 1. Objectives This is a case of refractory ascites due to hepatocellular carcinoma treated with Sasang Constitutional Medication. Refractory ascites, which did not respond to diuretics, was improved with Jeoryeongchajeonja-tang. 2. Methods We recorded the patient's abdominal circumference, body weight and urine output daily. We also investigated the TTKG(transtubular potassium gradient) values 3 times during the treatment course. 3. Results the TTKG value of the patient was siginificantly decreased. TTKG values before administration of Jeotyeongchajeonja-tang were 8.41, 10.82, respectively and 4.25 after administration of Jeotyeongchajeonja-tang. 4. Conclusions We guess that Jeoryeongchajeonja-tang functions as an aldosterone antagonist or increases spironolactone activity through interaction between Jeotyeongchajeonja-tang and spironolactone. But the mechanism of Jeotyeongchajeonja-tang is not clear, so we need more clinical study of refractory ascites improved with Jeotyeongchajeonja-tang and biological study of interaction between Jeotyeongchajeonja-tang and spironolactone.

An Approach of Traditional Korean Medicine to Cancer, the Leading Cause of Disease Burden (질병 부담이 증가하는 암의 한의학적 치료 접근)

  • Objective: The purpose of this study was to investigate the disease burden of cancer, to investigate how many cancer patients use CAM (complementary and alternative medicine) therapies with or without conventional medicine, to know reasons for use of cancer CAM therapies, and finally to discuss viewpoints on treating cancer with traditional Korean medicine (TKM). Method: In order to know the disease burden of cancer, the prevalence and mortality of cancer patients of Korean, American, and WHO reports were investigated, and the usage of cancer CAM therapies was investigated in several reports. The viewpoints about cancer treatment with TKM were suggested with the characteristics of TKM. Results: One hundred thousand Korean people were diagnosed [in time period] as new cancer patients, and cancer patients are gradually increasing every year. Cancer is a leading cause of deaths in Korea and worldwide. From 21% to 63% of Korean cancer patients, 80% of American cancer patients, and 35.9% of European cancer patients used CAM therapies, and the most common therapy was an herbal therapy. Conclusions: TKM has been used to treat cancer since initial use of acupuncture and herbs with basic viewpoints on human health and disease. First, TKM regards occurrence of cancer as an environmental problem of the whole human being. Second, the pathologic concept centered on antipathogenic Qi (Zheong Qi) can reinforce the functioning of the patient's own natural vitality to overcome cancer. Third, TKM deals with cancer patients through mind, body, and spirit based on the monoism of mind and body.

Weight misperception and its association with dieting methods and eating behaviors in South Korean adolescents

  • BACKGROUND/OBJECTIVES: There is little information on the association between weight misperception and eating behavior in Korean adolescents. Therefore, we investigated the association of food intake habits and dieting method and disturbed eating behavior (DEB) in relation to weight misperception. SUBJECTS/METHODS: Data was collected by using a nationwide online panel survey from 6,943 adolescents enrolled in middle/high school. DEB was measured with the Eating Attitudes Test (EAT-26) and those who scored ${\geq}20$ on the EAT-26 were considered to have eating disorder. Logistic regressions were conducted to examine the association between weight misperception based on self-reported weight status and dieting method and eating behaviors. RESULTS: The proportion of weight underestimation was 23.5% and that of overestimation was 24.0%. Weight overestimating girls were more likely to engage in various unhealthy dieting practices (OR = 1.69 for fasting; OR = 1.88 for laxative or diuretic use; OR = 2.05 for self-induced vomiting after meals; P < 0.05). Moreover, there was a strong association between overestimation and undesirable eating behaviors, especially among girls, e.g.: having breakfast (OR = 0.85), high consumption of fast foods (OR = 1.28) and regular sodas (OR = 1.39), but not among boys. In both genders, weight overestimation appears to be a major risk factor for DEB (OR = 1.34 for boys and OR = 1.41 for girls; P < 0.05). CONSLUSIONS: Weight overestimation is associated with unhealthy weight control practices and eating behaviors. We particularly found a significant association between weight overestimation and DEB among nationwide Korean adolescents.

Combination Therapy of Gefitinib and Korean Herbal Medicines Could be a Beneficial Option for Patients with Non-Small-Cell Lung Cancer

  • Lung cancer has a high mortality rate and is often diagnosed at the metastatic stage. Gefitinib is a targeted molecular therapeutic drug used to treat patients with non-small-cell lung cancer (NSCLC). Korean herbal medicines may also have therapeutic efficacy against lung cancer, reduce the side effects associated with chemotherapy, and improve patient quality of life (QOL). This case report describes the effects of a Korean herbal medicine regimen combined with gefitinib in a patient with NSCLC and bone metastasis. The Korean herbal medicine regimen included woohwanggeosa-dan, hwanggibujeong-dan and geonchilgyebok-jeong. The computed tomography (CT) findings showed that following combination treatment, the size of the tumor was markedly decreased without serious adverse events. Moreover, the Eastern Cooperative Oncology Group (ECOG) performance status was improved and cancer-related pain was decreased. These results suggest that a combination of Korean herbal medicines and gefitinib may be an effective therapeutic option for patients with advanced NSCLC and bone metastasis. Further studies are needed to examine the mechanism and the clinical efficacy of Korean herbal medicines against NSCLC.
