Malperfusion of a major organ with aortic dissection has various clinical features according branch. The morbidity and mortality rate can increase without suspicion especially postoperative period. Surgical outcomes and prognosis are influenced by early expeditious diagnostic and therapeutic measures are mandatory for successful treatment. The authors report four successful cases of acute aortic dissection with mal perfusion of various organs, such as the brain, kidney, and the lower extremities.

Background and Objectives : Artecoll(R) is an injectable soft tissue filler, which is a suspension of polymethylmethacrylate microspheres in $3.5\%$ bovine collagen solution. The authors aimed to determine the clinical of Artecoll of Artecoll(R) as an injection material into the vocal fold to correct the glottal insufficiency caused by unilateral vocal cord paralysis. Materials and Methods : Forty-one consecutive patients with unilateral vocal cord paralysis received percutaneous Artecoll injections under local anesthesia. Acoustic, aerodynamic and stroboscopic analyses were prospectively provided before, 1 week and 3 months after injection. Perceptual GRBAS grading by speech language pathologists and subjective ratings of the hoarseness and aspiration by the patients were also obtained. Results : Aerodynamic parameter(maximal phonation time) were significantly improved after the injection (p<0.05). Acoustic parameters (jitter and shimmer) were improved at the 3rd month follow-up. GRBAS uading and patients own subjective scaling of hoarseness and aspiration also showed significant improvement (p<0.05). Early or delayed significant side effects were not observed. Conclusion : Vocal fold injection with Artecoll is a convenient, safe and useful method of temporarily correcting the glottal insufficiency. Further long-term follow-up studies will answer the usefulness and safety of the Artecoll injection laryngoplasty.

After nearly three decades of reconst-ructing our completely devastated country, Korea has become a modern, cosmo-politan, fast-paced and dynamic country. In order to further increaser our manufac- turing capabilities and double the per-capita income by the beginning of the 21st century, we must improve productivitiy and encourage creatitivity in all sectors of our country. The S & T community's efforts in research and development. education, and interna-tional cooperation will be invaluable in determining our direction and reaching these goals. The general public is eager to peacefully unite our foreign powers since 1945. Toward that end. members of the S & T community are willing to coope-rate with our north Korean counterparts in all areas of S & T with the exception of defense related endeavors. Establi-shing scientific ties with north Korea will result in mutual economic benefit and greater regional stability in East Asia. In particular. scientists and manu-facturers in each province are eager to initiate cordial and professional relation-ships with north Korea in the hopes of securing these benefits. The Korean Federation of Science and Technology Societies(KOFST) has played an important role both in developing S & T policies, and in gostering the frowty of S & T societies. A non-governmental umbrella organization composed of over 251 professional societies, KOFST facili-tates the transfer of research outcomes and technology from research institutions to the various industries. KOFST also seeks to increase cooperation between the countries of the Pacific Rim. As evidence of that, we have pursued joint research and industrial ventures with China. Established projects include those on environmental conditions in the Yellow Sea, and improvement of aircraft manufacturing, telecommunica-tion equipment, and agricultural produ-cts. Increased cooperation between the Pacific Rim countries, particularly in S & T, would foster social and economic development for the region as a whole.
Purpose: The purpose of this study is to describe the characteristic clinical findings and treatment of suprascapular nerve entrapment by ganglion and to evaluate its results. Materials and Methods: Seven paitents with suprascapular nerve entrapment were evaluated on an average 13 months(range, six months to three years two months) after surgical excision and decompression. There were six males and one female. The mean age at operation was 31 years(range, 23 to 40 years), Suprascapular nerve entrapment were caused by compression of ganglion cyst in suprascapular notch or spinoglenoid notch in all cases. All patients complained of pain located over posterolateral area of the shoulder. Two patients had atrophy of both the supraspinatus and infraspinatus muscles, In four patients, only the infraspinatus muscle was involved. Muscle strength on both forward flexion and external rotation was decreased in two patients. In four patients, only external rotation was decreased. All patients underwent open excision of ganglion cyst and decompression. Results: The most dramatic effect of operation was prompt disappearance of pain in all patients. The average visual analog scale had improved from 7.2 to 0.6 point at the latest follow-up evaluation. An atrophy of the supraspinatus or infraspinatus muscle partially disappeared in four of six patients and muscle strength of forward flexion or abduction improved in all of six patients. The overall result was excellent for five patients and good for two. Conclusion: Surpascapular nerve entrapment by ganglionic cyst had clinically unique symptoms and signs on physical examination. Surgical excision is effective for symptomatic and functional outcomes. We believe that early intervention can be one of treatment modality before an irreversible damage occurs if the ganglion is large enough to compress suprascapular nerve, and to develop severe pain and muscular atrophy.

Purpose: Laparoscopic appendectomy (LA) has become a gold standard for children even in complicated appendicitis. The purpose of this study was to compare the postoperative surgical site infection rates between laparoscopic and open appendectomy (OA) group in pediatric complicated appendicitis. Methods: A total of 1,158 pediatric patients (age ${\leq}$ 15 years) underwent operation for appendicitis over a period of 8 years. Among these patients, 274 patients (23.7%) were diagnosed with complicated appendicitis by radiologic, operative and pathologic findings, and their clinical outcomes were retrospectively analyzed. Results: Of the 274 patients with complicated appendicitis, 108 patients underwent LA and 166 patients underwent OA. Patients in the LA group returned to oral intake earlier (1.9 days vs. 2.7 days; p<0.01) and had a shorter hospital stay (5.0 days vs. 6.3 days; p<0.01). However, rate of postoperative intra-abdominal infection (organ/space surgical site infection) was higher in the LA group (LA 15/108 [13.9%] vs. OA 12/166 [7.2%]; p<0.01). Readmission rate was also higher in the LA group (LA 9/108 [8.3%] vs. OA 3/166 [1.8%]; p<0.01). Conclusion: The minimally invasive laparoscopic technique has more advantages compared to the open procedure in terms of hospital stay and early recovery. However, intra-abdominal infection and readmission rates were higher in the laparoscopy group. Further studies should be performed to evaluate high rate of organ/space surgical infection rate of laparoscopic procedure in pediatric complicated appendicitis.
This paper examines how South Korean government regulators and environmental scientists endeavored to improve the environmental quality of Seoul in preparation for the Olympic Games in 1988. When Seoul was nominated as a host city of the Summer Games in 1981, it was suffering from severe environmental pollution. The municipal government of Seoul and the Ministry of Environment initiated a series of pollution survey projects, which were followed by relevant regulatory procedures. One of the outcomes of these projects was the beginning of odor pollution regulation that used the Environmental Official Testing Method. Although the Pollution Prevention Act of 1963 made odor pollution such as unpleasant order, stench, and stink subject to environmental regulation, proper administrative procedures to reduce odor pollution were not in place until the 1980s. In order to reduce odor pollution in urban areas, governmental officers and environmental scientists tried to regulate odor-causing chemicals from factories. In June 1988, however, severe odor pollution was detected not from factories but from unregulated areas in Seoul, such as the waterside parks of the Han river, the downtown, and the marketplaces. The local regulators and scientists were concerned that the odor pollution could make a "wrong" impression on foreign athletes and visitors about the degree of "modernity" of Korea. This paper shows that the 1988 Summer Olympics was a transition point in the early history of odor regulation in South Korea, when the scope of odor regulation could be expanded.

Objective: These experiments were conducted to investigate the optimal expose length of propidium iodide (PI) and bisbenzimide on differential staining of mouse blastocysts. Materials and methods: A total 964 blastocysts (early${\sim}$hatched) was exposed to PI (n=831) (group I: $\leq$ 10; II: $11{\sim}15$; III: $16{\sim}20$; IV: $\geq$21 sec) and bisbenzimide (n=133) (group A: $\leq$1; B: $1{\sim}$3; C: $\geq$ 4 hr) in several periods for differential staining. Statistical analysis was performed using t-test with SigmaPlot-2001. P-values < 0.05 were accepted as statistically significant. Results: In case of PI exposure, differential staining rates were significantly higher (p<0.05) in group I (89.8%) than in any others (group II: 77.6%; III: 29.6%; IV: 22.2%) and higher (p<0.05) in group II than in group III and IV. In case of bisbenzimide exposure, differential staining rates were not statistically differences in three groups (group A: 97.4%; B: 87.8%; C: 93.3%). Conclusion: The differential staining rates of mouse blastocysts are not affected by the exposure length of bisbenzimide. However, blastocysts were exposed to PI with period of shorter than 15 sec show best outcomes of differential staining rates.

Background: Prolonged sitting at work can lead to adverse health outcomes. The health risk of office workers is an increasing concern for the society and industry, with prolonged sitting work becoming more prevalent. Objective: This study aimed to explore the variation in muscle activities during prolonged sitting work and found out when and how to take a break to mitigate the risk of muscle symptoms. Methods: A preliminary survey was conducted to find out the prevalence of muscle discomfort in sedentary work. Firstly, a 2-h sedentary computer work was designed based on the preliminary study to investigate the variation in muscle activities. Twenty-four participants took part in the electromyography (EMG) measurement study. The EMG variations in the trapezius muscle and latissimus dorsi were investigated. Then the intervention time was determined based on the EMG measurement study. Secondly, 48 participants were divided into six groups to compare the effectiveness of every break type (passive break, active break of changing their posture, and stand and stretch their body with 5 or 10 mins). Finally, data consisting of EMG amplitudes and spectra and subjective assessment of discomfort were analyzed. Results: In the EMG experiment, results from the joint analysis of the spectral and amplitude method showed muscle fatigue after about 40 mins of sedentary work. In the intervention experiment, the results showed that standing and stretching for 5 mins was the most effective break type, and this type of break could keep the muscles' state at a recovery level for about 30-45 mins. Conclusions: This study offers the possibility of being applied to office workers and provides preliminary data support and theoretical exploration for a follow-up early muscle fatigue detection system.
Background and Objectives: Improvements in the fields of neonatology and surgical subspecialities make tracheotomy possible to the younger population. But complication rates for infantile tracheotomy are significantly higher than that for the other pediatric tracheotomy. This study was designed to present our 9-year experiences of infantile tracheotomy and to evaluate the effect of several factors of complications. Materials and Methods: From 1996 through 2004, 60 tracheotomies were performed. Charts were reviewed with respect to indications for tracheotomy, underlying diseases, success rate in decannulation and length of support time until decannulation, complication and mortality rate. Results: There were 41 male patients and 19 female patients. Ventilatory support for neurological impairment(38.3%) was the leading indication for tracheotomy, followed by subglottic stenosis(36.7%), laryngomalacia(13.3%). Decannulation was accomplished in 60.0% of infants with an average of 56.5momths with tracheotomy. Complications occurred in 43.3%. There was one tracheotomy-related mortality in case of tracheal atresia. Most common complication was subglottic stenosis. Conclusion: Infantile tracheotomy had significant morbidities and its outcomes are thought to be usually related to the underlying disease and age. To prevent complication, early decannulation is advisable, and for long-term tracheotomy patients, regulation of reflux and infection may be necessary.

Objectives: The purpose of this study was to evaluate and compare the treatment outcomes of partial glossectomy with or without elective neck dissection in patients with tongue squamous cell carcinoma (SCCa). Materials and Methods: A total of 98 patients who were diagnosed with tongue SCCa and underwent partial glossectomy between 2005 and 2014 were evaluated. Only 14 patients received elective neck dissection, and 84 patients received only partial glossectomy. Results: There were 56 men and 42 women with a mean age of 57 years and mean follow-up period of 33.7 months. There were 70 patients graded as T1 and 28 as T2. The total occult metastasis rate was 17.3%. The 5-year overall survival rate was 83.3% with elective neck dissection and 92.4% with observation. The 5-year disease-free survival rate was in 70.7% in the elective neck dissection group and 65.3% in the observation group. Conclusion: We retrospectively reviewed the records of 98 patients with tongue SCCa. These patients were divided into two groups, those who underwent elective neck dissection and those who did not. There was no statistically significant difference between the groups undergoing partial glossectomy with or without elective neck dissection.
