Qualitative Assessment of Breast Cancer Early Detection Services Provided through Well Woman Clinics in the District of Gampaha in Sri Lanka

  • Background: Breast cancer is the most common cancer diagnosed among women in Sri Lanka. Early detection can lead to reduction in morbidity and mortality. The objective here was to identify perceptions of public health midwives (PHMs) on the importance of early detection of breast cancer and deficiencies of and suggestions on improving existing breast cancer early detection services provided through Well Woman Clinics. Materials and Methods: A qualitative study using four focus group discussions (FGDs) were conducted among 38 PHMs in the Gampaha district in Sri Lanka and the meetings were audio-recorded, transcribed and analyzed using constant comparison and identifying themes and categories. Results: All the PHMs had a firm realization on the need of breast cancer early detection. The four FGDs among PHMs revealed non-availability of guidelines, inadequacy of training, lack of skills and material to provide health education, inability to provide privacy during clinical examination, shortage of stationery, lack of community awareness and motivation. The suggestions for the improvements of the programme identified in FGDs were capacity building of PHMs, making availability of guidelines, rescheduling clinics, improving the supervision, strengthening the monitoring, improving coordination between clinical and preventive sectors, and improving community awareness. Conclusions: Results of the FGDs can provide useful information on components to be improved in breast cancer early detection services. Study recommendations were training programmes at basic and post basic levels on a regular basis and supervision for the sustainance of the breast cancer early detection program.

  • Delirium is a common psychiatric disorder and occurs in many hospitalized older patients and has serious consequences including increased mortality rate. Despite its importance, health care clinicians often fail to recognize delirium or misdiagnosed as other psychiatric illness. Awareness of the etiologies and risk factors of delirium should enable clinicians to focus on patients at risk and to recognize delirium symptoms early. To improve early recognition of delirium, emphasis should be given to terminology, psychopathology and knowledge regarding clinical rating scale for delirium in the specific medical and surgical clinical settings. In this study, authors introduce rating scales for delirium and knowledge of clinical diagnostic process for delirium and give rise to appropriate assessment of delirium in the clinical situation.

  • Recent years, many industries acknowledge that environmental substantiality of products must be an essential role and it is one of the major importances for industries to consider the environmental impacts of products at the early stages of product development. This study investigated eco-design parameters and $CO_2-eq$. emissions in each stage of raw material acquisition, manufacturing, transportation, use and disuse in life cycle of touch monitor stand based on Eco-Design. In this study, to fulfill of Eco-Design, the environmental impact assessment of through LCA(Life cycle assessment) was carried out with benchmarking monitor stand and we suggested the direction of new design of touch monitor stand mechanism based on eco-friendly considerations. New design based on LCT(Life Cycle Thinking) showed that the following eco-friendly considerations at the early stage of design to be helpful to reduce GWP(Global Warming Potential) [kg $CO_2-eq$.] in new monitor stand development and it is necessary for Eco-Design process of the product.

  • The risk assessment of thermal hazard to identify chemical or process hazard during early process developments have been considered. The early identification of thermal hazards associated with a process, such as rapid heats of reaction, exothermic decompositions, and the potential for thermal runaways before any large scale operations are undertaken. This paper presents to evaluate the safe operating parameters/envelope for exist plant operations. The assessment of thermal hazard with operating conditions such as amount of process materials, inhibitor, and catalyst on esterification process in manufacture of concrete mixture agents are described. The experiments were performed by a sort of calorimetry with the Multimax reactor system as a screening tool. The aim of the study was to evaluate the thermal risk of process material and mixture in terms of safety security to be practical applications in esterification process. It suggested that we should provide the thermal hazard of reaction materials to present safe operating conditions with cause of accident through this study.

  • The influence of nutrition during early life on physical growth as well as mental development has been thoroughly discussed in the literature. The physical dimensions of the body are greatly influenced by nutrition, particularly during the period of rapid growth in early childhood. Nutritional status affects every pediatric patient's response toillness. Good nutrition is important for achieving normal growth and development. It is indicated that permanent impairment of the central nervous system may result from dietary restriction of imbalance during certain periods of life. If children under 3 years of age show a good nutritional status, it may be assumed that they are well nourished. Several common diseases of children such as iron deficiency, chronic constipation and atopic dermatitis are known food related diseases. Patients with chronic illness and those at risk of malnutrition should have detailed nutritional assessments done. Components of a complete nutritional assessment include a medical history, nutritional history including dietary intake, physical examination, anthropometrics (weight, length or stature, head circumference, midarm circumference, and triceps skinfold thickness), pubertal staging, skeletal maturity staging, and biochemical tests of nutritional status. The use of age, gender, and disease-specific growth charts is essential in assessing nutritional status and monitoring nutrition interventions. Nutrition assessment and dietary counseling is helpful for the cure of disease, and moreover, the prevention of illness.

Development and Validation of the Strengths Assessment Indicators for Daycare Centers (어린이집 강점평가지표 개발 및 타당화)

  • Objective: The purpose of this study was to develop assessment indicators and to verify the validity and reliability of the developed assessment indicators. Methods: A Delphi survey, focus group interviews, and content verification were conducted in order for experts to develop an evaluation index of the strengths of the day care center. A main survey was conducted on 438 daycare center principals and teachers to test their item quality, validity and reliability. Results: The final assessment indicators consisted of three areas, seven assessment criteria, 19 evaluation factors, 41 assessment items and a five-point rubric rating scale. As for the common strengths indicators, there were three assessment areas, five assessment criteria, 12 assessment elements and 22 assessment items. In regard to the selective strengths indicators, there were 3 assessment areas, 5 assessment criteria, 12 assessment elements and 16 assessment items. Conclusion/Implications: The efforts to confirm the strengths of daycare centers are expected to facilitate the identity building of the daycare center itself and for its organizational members to make a contribution to the qualitative improvement of childcare.

  • Recently, as a performance based design concept is introduced, assurance of expected performances on serviceability and safety in the whole span of life is exactly requested. So, quantitative assessments about durability related properties of concrete in early-age long term are come to necessary, Especially in early age, deterioration which affects long-term durability performance can be occurred by hydration heat and shrinkage, so development of reasonable hydration heat model which can simulate early age behavior is necessary. The micor-pore structure formation property also affects shrinkage behavior in early age and carbonations and chloride ion penetration characteristic in long term, So, for the quantitative assessment on durability performance of concrete, modelings of early age concrete based on hydration process and micor-pore structure formation characteristics are important. In this paper, a micromechanics based hydration heat evolution model is adopted and a quantitative model which can simulate micro-pore structure development is also verified with experimental results. The models can be used effectively to simulate the early-age behavior of concrete composed of different mix proportions.

  • This study investigated the effects of a teaching process using dynamic assessment for a unit on young children's measurement ability. Subjects were 45 5-year-old children in a kindergarten in Busan. The instrument was the Dynamic Assessment Tools for Young Children's Measurement Ability (Hwang & Ko, 2(07). Assessment consisted of four steps: pre-test, learning, transfer, post-test. Results were that at post-test, there were significant differences in scores of measurement ability between the dynamic assessment and control groups. In the dynamic assessment group there was significant improvement in length, width and weight between pre- and post-tests and there were significant shifts of measurement strategies and measurement errors between the pre- and post-tests.

A study on the investigates and compares the surveyed degree of recognitions of workers who works in companies executing the risk assessment By the investigation and comparison, it is expected to bring the better solution for early and efficient application for those companies which are not taking the risk assessment.
