재난트라우마에 대한 이침의 활용 - NADA 프로토콜의 소개 (Ear Acupuncture Treatment for Disaster-Related Trauma: Introduction of the National Acupuncture Detoxification Association (NADA) Protocol)

  • The number of disasters caused by natural or human-made events such as earthquakes, tsunamis, floods, typhoons, industrial disasters, and the recent outbreak of COVID-19 has increased and the damage caused by such disasters is increasing every year. Disasters can cause physical destruction and also have unexpected psychological impacts, such as post-traumatic stress disorder (PTSD). Ear acupuncture is a treatment modality that can be helpful for both physical and mental health problems. Since ear acupuncture is a cost-effective, flexible, and safe treatment tool, it has the potential to provide medical assistance in disasters. The National Acupuncture Detoxification Association (NADA) auricular acupuncture protocol, originally developed for addiction treatment, has been used for mental illness and behavioral symptoms in both community health settings and in a variety of disaster, terror, and refugee settings. In this review, we introduced the NADA protocol, including its history, features, training, clinical evidence, and cases in disaster settings, suggesting that the NADA protocol could be utilized as a disaster medical support model in Korean medicine.

A Fully Digital Auricular Splint Workflow for Post-Keloid Excision

  • Ear keloids are challenging lesions to treat due to high recurrence rates postexcision. Conservative compression techniques as adjunct treatment have been reported to be effective. An innovative technique of using computer-aided design/computed-aided manufacturing to print a customized auricular splint improves efficiency and comfort level for patients compared with conventional methods. The ear is scanned using an intraoral scanning 2 weeks postsurgery. A two-piece auricular splint is designed on the digital model, incorporating perforated projections for three nylon screws for retention of the splint. The splint is printed with clear acrylic material, postprocessed, and finished. The patient is taught to assemble the components of the splint and instructed to wear for at least 8 hours daily. The surgery site reviewed for any ulceration, pain, or recurrence of keloid for 6 months. During the 6-month review, the excision scar remained flat and pink. The patient also reports unrestricted daily activities. The digital workflow increases comfort for the patient and reduces the number of hours required to produce a customized auricular splint compared with conventional methods. A fully digital workflow for a printed auricular splint should be considered for adjunctive treatment to excision of ear keloids.

표준한국인 체적소 모델 HDRK-Man의 외형 보정 및 선량 산출에 미치는 영향 평가 (Deformation of the Reference Korean Voxel Model and Its Effect on Dose Calculation)

  • 최근 한양대학교에서는 고해상도 연속절단면 컬러해부영상자료를 이용하여 표준한국인 성인남성 체적소 모델 HDRK-Man을 개발하였다. HDRK-Man 은 기존의 다른 모델들과 마찬가지로 누워있는 인체의 영상을 사용하여 제작하였기 때문에 등과 엉덩이 부분이 납작하다는 문제가 있다. 이러한 문제를 해결하기 위하여 본 연구에서는 HDRK-Man 체적소 모델을 변형이 용이한 면(surface) 모델로 변환하였고 전신외형을 보정한 후 외형 보정이 선량 계산에 미치는 영향을 평가하였다. 면 모델로의 변환 작업은 상용 소프트웨어인 $3D-DOCTOR^{TM}$, $Rapidform^{(R)}$2006, $Rhinoceros^{(R)}$4.0을 사용하였고, 전신외형의 보정작업은 $MAYA^{(R)}$8.5을 사용하였다. 면의 형태로 변환된 모델은 곧바로 몬테칼로 선량계산에 사용한 수 없으므로, 다시 체적소 모델로 변환하여 선량계산을 수행하였다. MCNPX 몬테칼로 코드를 이용하여 보정 전후의 유효선량을 계산하여 비교해 본 결과 전체적으로 큰 차이는 없었으며, 예상대로 전 (AP), 좌(LLAT), 우(RLAT) 방향에 비하여 후 (PA)방향에서 상대적으로 가장 큰 차이를 보였다. 본 연구를 통하여 변환된 면(surface) 모델은 방사선 방호 분야에서 활용할 수 있는 다양한 자세를 갖는 변형된 인체모델 개발에 사용될 수 있을 것으로 기대된다.

A Murine Model of Toluene Diisocyanate-induced Contact Hypersensitivity

  • Background: Toluene diisocyanate (TDI) can cause contact allergy and occupational asthma, but the mechanism underlying sensitization to this chemical compound remains controversal. Also the correlation of mast cell with contact hypersensitivity (CHS) and the role of mast cell in the TDI-induced CHS is unknown. This issue was investigated by administrating TDI on the skin of genetically mast cell-deficient WBB6F1/$J-Kit^{W}/Kit^{W-v}$ ($W/W^{V}$) and congenic normal WBB6F1/J-Kit+/+ (+/+) mice. Methods: To development of animal model of TDI-induced CHS and to investigate the correlation of mast cell with CHS and the role of mast cell in the TDI-induced CHS, $W/W^V$ and +/+ mice were sensitized with TDI on the back skin at day 1 and day 8, and then challenged with 1% TDI on the ear at day 15. At 1, 2, 4, 8, and 24 hours after 1% TDI challenge, the ear thicknesses were measured. It was investigated the histologic changes of dermis in the ear of $W/W^V$ and +/+ mice at 24 hours after 1% TDI challenge. Results: TDI induced a significant ear swelling response in $W/W^V$ and +/+ mice. TDI induced the significant infiltrations of polymorphonuclear leukocytes and eosinophils in $W/W^V$ and +/+ mice, but not of mast cells in normal mice. And TDI increased a characteristic extent of mast cell degranulation in normal mice. There were no significant differences in the ear swelling and the infiltrations of polymorphonuclear leukocytes and eosinophils of normal versus $W/W^V$ mice, either at baseline or after TDI-induced CHS. Conclusion: From the above results, TDI can be used as a murine CHS model, and the mast cells may not be essential in TDI-induced CHS.

청각 보철을 위한 자극패턴 추출에 관한 연구 (A Study on the Extraction of the Excitation Pattern for Auditory Prothesis)

  • In this study, the excitation pattern, which can be sensated by a man having hearing loss due to the damage of inner ear, is extracted, and the procedure of the auditory speech signal processing is simulated with the computer. Therefore, the excitation pattern is extracted by the neural tuning model satisfying the physiological characteristic of the inner ear and by the infor.ation extracted from speech signal. The firing pattern is also extracted by inputting this excitation pattern to the auditory neural model. With this extracted firing pattern, the possibility that the patient can sensate the speech signal is studied by the computer simulation.

봉독이 여드름 균으로 유도된 염증성 동물모델에 미치는 효과 (Effects of Bee Venom on Propionibacterium acnes-induced inflammatory Skin Disease in Mice)

  • Bee venom (BV) has been used as a treatment for a wide variety of ailments such as inflammatory diseases in korean traditional medicine. Despite its well documented anti-inflammatory property, it has not been fully demonstrated regarding the influence of BV against Propionibactierium acnes (P. acnes), which promotes follicular inflammation (inflammatory acne). This study evaluated the anti-inflammatory property of BV against P. acnes in vivo. To induce inflammation in vivo using P. acnes, $1{\times}10^7$ CFU of living P. acnes were intradermally injected into the ear of mice. BV (1, 10, 100 ${\mu}g$) in vaseline was applied epicutaneously on the ear resulting in P. acnes-induced ear swelling and inflammation. Epicutaneous administration of BV with P. acnes decreased the number of infiltrated inflammatory cells and inflammatory cytokines in the ear, thereby relieving P. acnes-induced ear swelling and granulomatous inflammation, especially at the dose of 1 ${\mu}g$ of BV. In this report, we demonstrated the therapeutic effects of BV on P. acnes-induced inflammation in vivo using the mouse model. These data highlight the potential of using BV as an alternative treatment to the antibiotic therapy of acne vulgaris.

소풍산물추출물이 DNCB로 유발된 접촉성피부염에 미치는 영향 (Effect of Sopung-san on Contact Hypersensitivity induced by Repeat Elicitation of DNCB)

  • Sopung-san(SPS; prescription No. DO27), a herbal medicine prescribed by oriental hospital of Daegu fanny University, has been used as cure for pruritus and psoriasis. To evaluate anti-inflammatory effect of Sopung-san, we treated Sopung-san extract in animal model system induced contact hypersensitivity. Contact hypersensitivity, a local inflammatory response of the skin, was induced by 1% DNCB on the right ear of BALB/C mouse. Sopung-san was prepared as water extract and administrated everyday for 2 weeks per oral. A right ear of mouse was potently swelled by 1% of DNCB treatment, but a mouse ear thickness was significantly reduced by Sopung-san after 2 weeks. treatment. Sopung-san reduced IgG in serum obtained from blood of 1 % DNCB-treated mouse. IgE in serum was not changed by DNCB and/or Sopung-san treatment. $IL-1{\beta}$ and IL-6 from ear tissue were significantly increased by DNCB, Dut were not reduced by Sopung-san. From these results, anti-inflammatory effect of Sopung-san, especially reduction of ear swelling, might be partly due to reduction of IgG in serum.

PZT 적층 압전체 기반의 인공중이용 트랜스듀서 설계 및 구현 (Design and Implementation of a Vibration Transducer based on PZT Multi-layered Actuator for Implantable Middle Ear Hearing Devices)

  • 최근 기존 공기 전도형 보청기의 문제점과 불편함을 해결하기 위해 이식형 보청기인 인공중이 (Implantable middle ear hearing devices, IMEHDs)가 미국, 일본, 한국 등에서 활발히 연구 개발되고 있다. 본 논문은 인공중이용 진동체로서 PZT 적층 압전체 기반의 플로팅 매스 트랜스듀서의 설계 및 구현에 관한 것이다. 인공중이용 압전 플로팅 매스 트랜스듀서 (piezoelectric floating mass transducer, PFMT)의 기계적 진동 특성을 구하기 위한 근사적 모델링과 이의 해석을 위한 전기적 변환을 통한 진동 특성 분석을 수행하여 트랜스듀서 설계에 반영하여 구현하였다. 구현된 PFMT를 본 연구팀이 개발한 완전 이식형 인공중이 시스템 시작품에 적용하였으며, PFMT의 성능을 확인하기 위해 PFMT를 사체 이소골에 장착한 후 이소골 진동변위를 측정하는 실험을 행하였다. 이를 통해 본 논문에서 설계 및 구현된 PFMT의 인공중이용 진동 트랜스듀서로서 유효성을 확인하였다.

Balb/c 생쥐에 대한 어싱 매트리스에 의한 항염 효과 (Anti-inflammatory Effects of Earthing Mattress in Mouse)

  • Earthing, caused by direct skin contact with the Earth's surface, is used to reduce the symptoms of inflammation (fever, fever, swelling and pain). However, there is little evidence to support the anti-inflammatory effects of earthing mattresses. Therefore, this study was conducted to investigate whether anti-inflammatory effect of earthing mattress using an in vivo animal model. The anti - inflammatory effect was evaluated by measuring ear thickness and foot volume in 12-O-tetradecanoylphorbol-13 acetate (TPA) - induced ear edema and carrageenan - induced paw edema model, respectively. Balb/c mouse in carrageenan paw edema model showed significant anti - inflammatory effect in the group treated with earthing mattress for 4 hours or 24 hours for 3 days. For females, the anti-inflammatory effect was greater when the earthing mattress was added to the mattress than the mattress alone treatment. From the above results, it was found that the female responds more to the effect of the earthing as well as the mattress effect. In addition, when the male and female Balb/c mice were exposed to mattresses and earthing mattresses for 24 h for 3 days, respectively, the mattress and earthing mattresses showed significant inhibition of IL (Interleukin)-1β levels compared to the control. In the TPA ear edema model, Balb/c mouse showed significant anti - inflammatory effect in the group treated with the earthing mattress for 4 hours or 24 hours for 3 days. Both males and females showed more anti-inflammatory effects when they were exposed to earthing mattresses with mattresses added to the mattresses. From the above results, it was found that both male and female respond to the effect of earthing as well as the mattress effect in the TPA ear edema model. In conclusion, in this study, we have verified that earthing mattress shows inhibitory effects on TPA and carrageenan-induced inflammation. From these results, it is suggested that the anti-inflammatory effect can be expected by applying the earthing mattress to patients suffering from inflammatory diseases. However, there is a need to pinpoint exactly how the earthing mattress relieves inflammation, and further research is needed to investigate the mechanism.
