Physiotherapy Approach to Patients with Chronic Plantar Fasciitis : Comparison of the Effects of Specific Stretching Exercise and High-Load Strengthening Exercise (만성 발바닥 근막염 환자에 대한 물리치료적 접근법: 특이적 뻗침운동과 고부하 강화 운동의 효과 비교)

  • Purpose : This study was to investigate the effectiveness of ESWT and plantar fascia-specific stretching exercise vs ESWT and high-load strengthening exercise in patients with chronic plantar fasciitis. Methods : The subjects were randomized to extracorporeal shock wave therapy ( ESWT, for 3 weeks) and daily plantar-specific stretching (Group I: Stretch group) or ESWT and high-load progressive strength (Group II: Strength group) performed every second day. The main outcome measures were ultrasound, visual analogue scale (VAS), and Korean Foot Function Index (KFFI). The ultrasound (plantar fascia thickness), pain intensity I, II (the most painful of the day?, the pain when you first step in the morning?) and KFFI (functional performance) were compared between the groups. Results : No significant difference was observed between the groups in the plantar fascia thickness but pain intensity I, II was significantly lower in Group 2 than in Group 1 at only 12weeks and functional performance was also significantly increased in Group 2 compared to Group 1 at only 12 weeks. Conclusion : The high-load strengthening exercise consisting of the progressive exercise protocol, resulted in superior after 12 weeks compared with plantar-specific stretching. High-load strength exercise may aid in a quicker reduction in pain and improvements in functional performance.

Characteristics of Median Frequency According to the Load During Fatiguing Isometric Exercise (등척성 운동시 운동강도에 따른 중앙주파수의 특성)

  • Median frequency can be regarded as a valid indicator of local muscle fatigue. As local muscle fatigue develops, the muscle fiber conduction velocity decreases, the fast twitch fibers are recruited less, and consequently the median frequency shifts toward the lower frequency area. The aim of this study was to test the characteristics of the median frequency according to exercise load (30% and 60% of MVC on the biceps brachii, 40% and 80% of MVC on the vastus lateralis) during the fatiguing isometric exercise. Thirteen healthy male volunteer students of Yonsei University were recruited. After the testing maximal voluntary isometric contraction, three variables (initial median frequency, regression slope, fatigue index) from the regression line of MDF data were measured in each exercise load. The results showed that the regression slope and fatigue index were significantly different for the biceps brachii, but not for the vastus lateralis initial MDF was not significant difference according to the exercise load on both muscles. The regression slope and fatigue index could monitor physiologic muscle change during fatiguing isometric exercise. The results showed that two MDF variables reflect the local muscle fatigue according to the exercise load.

Effect on Improvement of Muscle Strength Imbalance according to Load Deviation Pattern of Left and Right Arms in Upper Limbs (상지 좌우 운동부하 편차방식이 근력 불균형 개선에 미치는 영향)

  • The purpose of this study was to verify the validation of effect on improvement of muscle strength unbalance according to exercise load deviation during rowing exercise. We performed evaluation of muscular activity and joint torque before the test. We recruited twenty subjects who one side's muscle strength is bigger in more 20% than other side. Subjects divided two groups. One is dominant left side and the other was dominant right side. Subjects performed rowing exercise in electric load deviation rowing equipment (Robo.gym, Humonic Co., Ltd., Daegu, Korea). Exercise performed four sets a day including 25 times a set, and three days a week. Measurements consist of evaluation of muscular activity and joint torque. Exercise load deviation adapted that different value of muscle strength in both arms multiplied 1RM% and added 1RM 50%. The results in adapted load deviation showed that the differences of maximal peak torque in 22.75% were getting increase significantly during exercise in 5.72%. This interpreted that rowing exercise with loading deviation types could provide muscle strength and muscular endurance exercise in same time for balance. Our study found out that loading deviation could provide muscle strength and muscular endurance exercise for improving muscle unbalance.

Evaluation of the rehabilitation system with active load control by using EMG biofeedback (재활운동을 위한 능동형 근전도 바이오피드백 시스템 평가)

  • In this study, the active exercise load control system was developed that monitors patient's muscular condition based on EMG signals and controls the load by biofeedback in real-time. In order to select appropriate muscular parameter for the system, the pre-exercise test was performed to obtain EMG signals from healthy 12 males. Subsequently, the main exercise test was performed to evaluate the active exercise load control system based on IEMG: a selected muscular parameter, on healthy 10 males without musculoskeletal disorder. The accuracy and availability of developed system were confirmed through observing changes between exercise load and IEMG. A correlation was analyzed between the fatigue of muscles and RPE indicating the individual subjective fatigue. As a result, the active exercise load control system that was suggested in this study could be useful to control the initial load-balancing and the amount of exercise appropriate for individual in rehabilitation exercise therapy.

Study of Load According to the Position of Sling Strap during Sling Bridge Exercises for Improvement of Life Care (라이프케어 증진을 위한 슬링교각운동 시 슬링스트렙 위치에 따른 부하량 연구)

  • The purpose of this study was to accurately measure the load generated on the leg according to the position of the sling strap supporting the leg during the sling bridge exercise for improvement of life care. Ten Healthy university students participated in two trials: knee bridge exercise and ankle bridge exercise in supine position. The loads transmitted to the knees and ankles were measured when a bridge exercise was performed with a sling strap on both knees and ankles. As a result of the study, it was confirmed that the load of sling bridge exercise with both knees supported was statistically greater than the load of sling bridge exercise with both ankles supported(p<.01). On the other hand, there was no statistically significant difference in the load between both knees and both ankles (p>.05). Therefore this study is meaningful in that it objectively measured the load on the leg during sling bridge exercise, and is thought to be helpful in setting exercise intensity during sling exercise.

Effect of the Short Foot Exercise with 2nd~5th Toe Extension on the Abductor Hallucis Activity and Medial Longitudinal Arch under Various Loads (다양한 부하에서 2~5번째 발가락 폄을 동반한 짧은 발 운동이 엄지벌림근 근활성도와 안쪽세로활에 미치는 영향)

  • Background: In this study, the abductor hallucis activity and medial longitudinal arch angle were compared by performing four exercises, namely the short foot exercise, the short foot exercise with 2nd~5th toe extension, the short foot exercise with 2nd~5th toe extension with a load of 1kt on the sole, and the short foot exercise with 2nd~5th toe extension with a load of 2kg on the sole. Methods: Four short foot exercises as described above were performed by 20 healthy adult males and females. The abductor hallucis activity and medial longitudinal arch angle were measured and analyzed by surface electromyography and the Kinovea software program. Results: The short foot exercise with 2nd~5th toe extension, short foot exercise with 2nd~5th toe extension with a load of 1kg on the sole, and the short foot exercise with 2nd~5th toe extension with a load of 2kg on the sole showed significantly higher abductor hallucis activity than the short foot exercise alone. Among these, the short foot exercise with the 2nd~5th toe extension was the most effective. All exercises showed a significantly decreased medial longitudinal arch angle post-exercise than pre-exercise, and the short foot exercise with the 2nd~5th toe extension showed a significantly decreased medial longitudinal arch angle compared to the other three exercises. Conclusion: It is believed that the short foot exercise with the 2nd~5th toe extension can be proposed as an effective exercise that can replace the short foot exercise alone.

The Effect on Improvement of Muscle Strength Imbalance According to Load Deviation Protocol of Whole Body Vibration Exercise (부하 편차 방식의 전신진동운동이 하지 근력 불균형 개선에 미치는 효과)

  • The purpose of this study was to verify the effect on improvement of muscle strength unbalance according to load deviation protocol during whole body vibration exercise. Seventeen female volunteers (age $22{\pm}3$ years, height $160{\pm}4.9$ cm, weight $49{\pm}.8$ kg) participated in this experiment. The subjects performed squat exercise in WBV platform. Exercise were performed five set a day including 15 time a set, three days a week, and during 4 weeks. Vibration stimulus was 25Hz as frequency and intensity was dominant leg 1mm and non-dominant leg 4mm. The results in WBV group showed that the differences of peak torque in 16% were getting decreasing significantly after 4 weeks from 16.2% to 5.2%. This result means that WBV with load deviation protocol could provide muscle strength exercise for muscle strength balance. Our study found out that WBV of load deviation protocol could provide muscle strength exercise for improving muscle imbalance.

Effects of Bridging Exercise Using Weight Loads on Trunk and Lower Limb Muscles Activity in Healthy Adult Males (무게부하를 이용한 교각운동이 건강한 성인 남성의 몸통 및 다리 근육의 활성화에 미치는 영향)

  • Purpose: This study observed the activity of trunk and lower limb muscles during a modified bridging exercise with various weight loads. Methods: The participants in this study consisted of 15 male adults. The muscle activity of the elector spinae, rectus abdominis, gluteus maximus, gluteus medius, vastus medialis, vastus lateralis, tibialis anterior, and soleus muscles was measured with an EMG-8 system during a modified bridging exercise performed with various weight loads (indicated as percentage of body weight). Differences in muscle activity during the bridging exercise according to the weight load applied were analyzed using a one-way ANOVA, and post hoc analysis was performed using LSD. Statistical significance was accepted at a p-level of 0.05. Results: When the subjects performed the modified bridging exercise with various weight loads, the muscle activity of the gluteus maximus and vastus medialis peaked at a load of 0.5%. The activity of the gluteus medius showed a remarkable difference when the modified bridging exercise was performed at loads of 0% and 0.5%, 0% and 1%. In addition, the activity of the vastus medialis showed a remarkable difference between modified bridging exercises performed at a load of 0% and that performed at a load of 0.5%. Conclusion: The results suggest that performing modified bridging exercises with a load of 0.5% of body weight results in significant differences in the activity of the gluteus medius and vastus medialis muscles. Thus, it is suggested that performing the modified bridging exercise at 0.5% of body weight may selectively strengthen the gluteus medius and vastus medialis muscles.

Validation of Exercise Effect in Rehabilitation during Knee Extension of the Lower Limbs using Pneumatic Exercise System (공압운동기기의 하지 슬관절 운동 시 재활운동 효과의 유효성에 관한 연구)

  • Our study is to verify validation of exercise effect that developing exercise and rehabilitation instrument using pneumatic actuators available rehabilitation and healthcare, while at the same time safer and more convenient for the user to select the desired exercise. Shares subject to the usual exercise is conducted in less than a week in the knee extension and flexion at the undergraduate level there is no more than 10 of the 20 patients were female. Subject performed knee exercise 3 set a day with 12times a set using fluid type exercise instrument and pneumatic exercise instrument that build in this study. Experiment has progressed in 4-weeks with 3-days a week. Our exercise load consist of two type. One is active exercise load with developed instrument in our study and the other one is passive exercise load with fluid instrument. Exercise program based on real-time muscle activation patterns and muscle activity before and after exercise by comparing the effects of exercise for rehabilitation has been verified. The results showed that muscular activity is higher in active exercise load than passive load, and improvement of muscle strength more increased too. Equipment more user-centered exercise more effectively exercise may enhance muscle strength was found. This study can adapt rehabilitation program and exercise for patient and weaker. Also our system can provide easily and convenient prescription of rehabilitation for expert related rehabilitation.

Relationship Between Modified Physiological Cost Index for Isokinetic Ergometer Exercise Test and Oxygen Consumption (등속성 에르고미터 운동을 이용한 수정된 생리적 부담 지수와 산소소비량 변화량과의 상관성)

  • The purpose of this study was to establish modified physiological cost index (PCI) for predicting energy consumption by heart rate (HR) at isokinetic ergometer exercise testing. The subjects were twenty-eight healthy men in their twenties. All of them performed upper and lower extremity isokinetic ergometer exercise tests which had six loads (400, 500, 600, 700, 800, and 900 kg-m/min) and five loads (400, 500, 600, 700, and 800 kg-m/min) respectively. The exercise sessions were finished when HR was in plateau. HR and oxygen consumption were determined during the final minute. Resting heart rate and oxygen consumption were used for calculating heart rate, oxygen consumption changes and modified PCI. Regression analysis established the relationship between each variable to work load, HR and oxygen consumption. The results were as follows: 1) In the lower extremity ergometer exercise test, oxygen consumption increased continuously as work load increased, but in the upper extremity ergometer test, oxygen consumption only increased until work load was 700 kg-m/min. 2) HR increased as work load increased in both exercise tests, but in the upper extremity ergometer test, HR decreased from the 700 kg-m/min. 3) The modified PCI increased as work load mcreased until the 700 kg-m/min point in the lower extremity ergometer test and until the 500 kg-m/min point in the upper extremity ergometer test when it started to decrease in both tests. 4) In the lower extremity ergometer exercise test, regression analysis established the relation as $dVO_2$ = -.0215HR - .2141 where $dVO_2$ is given in l/min and HR in beat/min ($R^2$ = .2677, p = .000). ln the upper extremity ergometer exercise test. regression analysis established the relation as $dVO_2$ = -.0115HR + .2746 ($R^2$ = .1308, p = .000). The results of this study were similar to previous studies but were different under high work load conditions. So modified PCI should be used with only low intensity work load testing. Subjects for upper extremity ergometer exercise testing should complete a prescribed training course prior to testing, and only low intensity work load should be used for safety considerations.
