Ultrasound Utility for Predicting Biological Behavior of Invasive Ductal Breast Cancers

  • Purpose: The aim of the study was to evaluate the correlation of ultrasound features with breast cancer molecular status. Materials and Methods: A retrospective review was performed of ultrasound findings in 263 patients diagnosed with breast invasive ductal carcinoma for comparison with immunohistochemistric results were obtained from each lesion. Relationships between ultrasound findings and molecular status were investigated by using multiple regression analysis by means of stepwise logistic regression. Differences in ultrasound criteria were assessed among women with different molecular status. Results: ER positivity was associated with small size, lobulate, angular or spiculated margin contours, absence of calcification, posterior tumor shadowing and low elasticity score; PR positivity was associated with small size, lobulate or angular or spiculated margin contours and absence of calcification; HER2 positivity was associated with presence of calcification and absence of any echogenic halo. The calculated models of predicted molecular status were accurate and discriminating with AUCs of 0.78, 0.74, and 0.74, respectively. Conclusions: Breast cnacer ultrasound features show some correlation with the molecular status. These models may help to expand the scope of ultrasound in predicting tumor biology.

Triple Negative Breast Cancer in People of North East India: Critical Insights Gained at a Regional Cancer Centre

  • Background: Breast cancer is a heterogeneous disease comprising of distinct biological subtypes with many targeted prognostic biomarkers having therapeutic implications. However, no specific targeted therapy for triple negative breast cancer has been discovered to date and hence further research is needed. Aim: The aim and objectives of the present study were to examine the prevalence of triple negative breast cancer (TNBC) in North-East India and to compare the clinicopathological parameters in two study groups defined by immunohistochemistry (IHC) - "TNBC" and "Others". Materials and Methods: We carried out a retrospective study in a cohort of 972 patients diagnosed with invasive breast carcinoma in the Department of Pathology, Dr. B. Borooah Cancer Institute, a Regional Cancer Centre for treatment and research, Guwahati, for a period of 3 years and 10 months from January 2010 to October 2013. Based on IHC findings, patients were divided into two groups - "TNBC" and "Others". All relevant clinicopathological parameters were compared in both. TNBC were defined as those that were estrogen receptor (ER), progesterone receptor (PR), and HER2/neu negative while those positive for any of these markers were defined as "Others". Results: In this study, out of total 972 cases 31.9% (310 cases) were defined as TNBC and 662 cases (68.1%) as "Others" based on IHC markers. Compared to the "Others" category, TNBC presented at an early age (mean 40 years), were associated with high grade large tumours and high rate of node positivity, IDC NOS being the most common histological subtype in TNBC. Conclusions: TNBC accounts for a significant portion of breast cancers in this part of India and commonly present at younger age and tend to be large high grade tumours.

  • Im sog. Kemptener-Fall aus dem Jahre 1994 in Deutschland hat der 1. Strafsenat des BGH die Bedeutung des Patientenwillens erstmals $f{\ddot{u}}r$ einen solchen Fall hervorgehoben, in dem die Krankheit noch keinen irreversibel $t{\ddot{o}}dlichen$ Verlauf genommen hat. Der 1. Senat des BGH entwickelt aus den $Pr{\ddot{a}}missen$, die er in drei $Leits{\ddot{a}}tzen$ $festh{\ddot{a}}lt$, $schl{\ddot{u}}ssig$ und im $gel{\ddot{a}}ufigen$ Modus der Dogmatik eine $Begr{\ddot{u}}ndung$, die gleichwohl den weitaus ${\ddot{u}}berwiegenden$ Teil der eigentlichen Probleme des Falles nicht einmal sichtbar genacht, geschweige denn einer $L{\ddot{o}}sung$ $n{\ddot{a}}hbar$ $br{\ddot{a}}chte$. Die $Leits{\ddot{a}}tze$ formulieren rechtliche Prinzipien, beziehen aber unvermeidlich zugleich und $selbstverst{\ddot{a}}ndlich$ Stellung zu einigen der schwierrigsten und $ungel{\ddot{o}}sten$ ethischen Fragen der modernen Medizin. Im sog. Kemptener-Fall brachte die $L{\ddot{o}}sung$ des Senat $f{\ddot{u}}r$ Mediziner, Ethiker und Juristen keine befriegende $L{\ddot{o}}sun$, sondern allein zahlose Zweifel und Unklarheiten. Denn dabei geht es nicht um moralisch $unerw{\ddot{u}}nschten$ Ergebnis der Rechtsanwendung im Einzelfall. Vielmehr hat die moderne Medizin $f{\ddot{u}}r$ den ${\ddot{a}}rztlichen$ Eintscheidungsbereich zwischen Leben und Tod einen Typus professionellen Alltagshandelns gebracht. Deshalb ist es eine Illusion, wollte man von der strafrechtlichen Dogmatik befriegende Resoltate erwarten. Trotzdem kann man nicht die dogmatische Untersuchung $aufh{\ddot{o}}ren$, weil eine Gesetzgebungslehre oder eine Strafrechtspolitik von den dogmatischen Probleme ausgehen muss. In diesem Hintegrund im Beitrag wurde dieser Fall durch kriti sche Auseinandersetzung strafrechtsdogmatisch zu begrunden versucht, Dabei wird die $Interessenabw{\ddot{a}}gung$ zwischen Lebens- und Sterbensinteressen als der $Rechtfertigungsbegr{\ddot{u}}ndung$ beim $t{\ddot{o}}dlichen$ Behandlungsabbruch vertreten.

Branched-chain Amino Acids Reverse the Growth of Intrauterine Growth Retardation Rats in a Malnutrition Model

  • This experiment was conducted to determine the effect of dietary supplementation with BCAA (branched-chain amino acids: leucine, isoleucine and valine) on improving the growth of rats in a malnutritional IUGR (Intrauterine Growth Retardation) model, which was established by feeding restriction. In the experimental treatment, rats were fed purified diets supplemented with BCAA (mixed) during the whole gestation period, while arginine and alanine supplementation were set as the positive and negative control group, respectively. The results showed that, compared to the effect of alanine, BCAA reversed IUGR by increasing the fetus weights by 18.4% and placental weights by 18.0% while fetal numbers were statistically increased. Analysis of gene and protein expression revealed that BCAA treatment increased embryonic liver IGF-I expression; the uterus expressed higher levels of estrogen receptor-$\alpha$ (ER-$\alpha$) and progesterone receptor (PR), and the placenta expressed higher levels of IGF-II. Amino acid analysis of dam plasma revealed that BCAA supplementation effectively enhanced the plasma BCAA levels caused by the feed restriction. BCAA also enhanced the embryonic liver gluconeogenesis by augmenting the expression of two key enzymes, namely fructose-1,6-biphosphatase (FBP) and phosphoenolpyruvate carboxykinase (PEPCK). In conclusion, supplementation of BCAA increased litter size, embryonic weight and litter embryonic weight by improving the dam uterus and placental functions as well as increasing gluconeogenesis in the embryonic liver, which further provided energy to enhance the embryonic growth.

Factors Predicting Microinvasion in Ductal Carcinoma in situ

  • Background: Whether sentinel lymph node biopsy (SLNB) should be performed in patients with pure ductal carcinoma in situ (DCIS) of the breast has been a question of debate over the last decade. The aim of this study was to identify factors associated with microinvasive disease and determine the criteria for performing SLNB in patients with DCIS. Materials and Methods: 125 patients with DCIS who underwent surgery between January 2000 and December 2008 were reviewed to identify factors associated with DCIS and DCIS with microinvasion (DCISM). Results: 88 patients (70.4%) had pure DCIS and 37 (29.6%) had DCISM. Among 33 DCIS patients who underwent SLNB, one patient (3.3%) was found to have isolated tumor cells in her biopsy, whereas 1 of 14 (37.8%) patients with DCISM had micrometastasis (7.1%). Similarly, of 16 patients (18.2%) with pure DCIS and axillary lymph node dissection (ALND) without SLNB, none had lymph node metastasis. Furthermore, of 20 patients with DCISM and ALND, only one (5%) had metastasis. In multivariate analysis, the presence of comedo necrosis [relative risk (RR)=4.1, 95% confidence interval (CI)=1.6-10.6, P=0.004], and hormone receptor (ER or PR) negativity (RR=4.0, 95%CI=1.5-11, P=0.007), were found to be significantly associated with microinvasion. Conclusions: Our findings suggest patients presenting with a preoperative diagnosis of DCIS associated with comedo necrosis or hormone receptor negativity are more likely to have a microinvasive component in definitive pathology following surgery, and should be considered for SLNB procedure along with patients who will undergo mastectomy due to DCIS.

Clinicopathologic Characteristics of Male Breast Cancer: A Report of 21 Cases in Radiotherapy Center of Hamedan, Iran

  • Background: Male breast cancer accounts for less than 1% of all cancer in men and only around 1% of all diagnosed breast cancer. Despite a significant raise in the last 25 years, it still remains a rare disease. Materials and Methods: We conducted a retrospective study from 2004-2011 with 21 male breast cancer patients. We aimed to analyze the epidemiologic data (age, personal and family history), tumor characteristics (size, histological type, location, TNM stage, receptors), surgery, adjuvant chemotherapy and radiation therapy, hormonal therapy and survival (relapse, follow up, death) who reffered to our center with breast cancer. Results: The median age was $49.2{\pm}14.2$ years (range 30-83 years). A family history of breast cancer was noted in four cases. The main clinical complaint was a retroareolar mass in 85.7%of patients (n=18). Histologically, 85.7% (n=18)were invasive ductal carcinoma and 4.7% (n=1) had ductal carcinoma in situ and 9.4% (n=2) had mixed histology including invasive medullary and ductal carcinoma. Hormonal therapy was delivered to 16 cases (76.1%) due to ER or PR positivity. During median follow up of 30 months (3-84 month), distant metastases were evident in 4 cases (19%). During the follow-up period, only one patient died due to metastatic disease. The mean time to recurrence detection was 30 months. Conclusions: The percentage of cases of male breast cancer is very low compared to breast cancer in females, explaining why very few investigations have been conducted in Iran. Limited coverage in the literature make gender-specific findings difficult so future research of this entity involving multi-institutional cooperation and longer follow up is essential to provide new insights about the biological and clinical factors of this rare cancer.

흰점박이꽃무지 유충의 혈구세포에 대한 형태학적 특성 (Ultrastructure Characterization of Hemcytes in Larvae of Protaetia brevitarsis seulensis)

  • 본 연구는 흰점박이꽃무지(Protaetia brevitarsis seulensis (Colbe) (Cetoniidae, Coleoptera) 유충의 혈림프에 존재하는 혈구세포들의 형태학적 특성분석을 위하여 수행하였다. 흰점박이꽃무지 유충 혈강 내에는 과립혈구세포, 세포질혈구세포, 편도혈구세포, 구상적혈구세포, 전혈구세포, 지방혈구세포 총 여섯 종류의 혈구세포들이 관찰 되었다. 그 중 과립혈구세포는 핵, 미토콘드리아, 골지체를 포함하여 잘 발달된 세포소기관들이 관찰 되었고 외래물질 침입시 면역학적 식균작용을 수행하는 것으로 밝혀졌다. 특히, 과립혈구세포의 세포질에는 잘 발달된 리소좀(<$1{\mu}m$ 직경)들이 세포막 주변으로 분포되어 존재하고 있음을 알 수 있었다. 식균된 외래물질은 다양한 크기의 리소좀들과 서로 합쳐지면서 외래물질을 제거하는 것으로 판단된다. 그 외 다섯 종의 혈구세포들은 외래물질 침입시 면역학적 활성화와 관계가 없는 것으로 관찰되었다.

Various Aspects, Patterns and Risk Factors in Breast Cancer Patients of Balochistan

  • Purpose: Breast cancer is the commonest malignancy of females throughout the world with one million new cases each year. In Pakistan, the burden of breast cancer disease is high with late stage presentation being a common feature, more than half being stage III or stage IV. The objective of this study was to study various aspects, patterns and risk factors in breast cancer patients of Balochistan. Method: Present study was performed on 134 patients of breast cancer who were registered in CENAR. The patients were interviewed by providing a questionnaire. Informed consent was taken from all the patients who took part in this study after explanation of the study aims. Body mass index (BMI) was calculated andbiopsy reports were obtained from patients files. All the cases were classified with respect to age, gender, ethnic group (Baloch, Pashtoon, Punjabi, Afghani, Hazara) BMI, cancer type, cancer grade, hormonal status, side of the cancer, fertility and marital status. Results: Out of 134 patients, the most common ethnic group was Pashtoon with a total of 42 and the common age group was 41-50 years with a total of 51. Invasive ductal carcinoma (IDC) was the most common type, accounting for in 128 patients (95.5%) followed by invasive lobular carcinoma (ILC). Conclusion: Pashtoon was the most common ethnic group, IDC was common type and most of the patients had an ER/PR positive hormonal status.

Prognostic Significance of HER-2/neu and Survival of Breast Cancer Patients Attending a Specialized Breast Clinic in Kolkata, Eastern India

  • Introduction: The worldwide incidence of breast cancer has increased rapidly in recent years. The scenario of Eastern India is also showing the same trend. It is necessary to study the utility of HER-2/neu as a prognostic factor in breast cancer survival. However, there have not been detailed studies in this respect with the breast cancer patients of Eastern India. Thus this study was conducted. Materials and Methods: In this hospital-based study 86 breast cancer patients attending a breast clinic of a reputed institute of Eastern India and having invasive ductal carcinomas were observed for a period of 5 years after surgery. Associations between 5 years observed survival and status of ER, PR and HER-2/neu of the patients were critically evaluated. Results: There was statistically significant association between survival pattern for 5 years and the HER-2/neu status (p=0.00001). Better survival was observed for the patients with HER-2/neu negative tumors 67(100%) compared to HER-2/neu positive tumors 7(36.8%). Conclusions: There is strong interaction between survival and HER-2/neu expression of breast cancer patients. Thus the patients with HER-2/neu positive tumors need to be treated aggressively.

Silencing of Lysyl Oxidase Gene Expression by RNA Interference Suppresses Metastasis of Breast Cancer

  • Objective: The aim of this study was to investigate possible mechanisms of LOX gene effects on invasion and metastasis of breast cancer cells by RNA interference. Methods: LOX-RNAi-LV was designed, synthesized, and then transfected into a breast cancer cell line (MDA-MB-231). Expression of LOX, MMP-2 and MMP-9 was determined by real-time PCR, and protein expression of LOX by Western blotting. Cell migration and invasiveness were assessed with Transwell chambers. A total of 111 cases of breast cancer tissues, cancer-adjacent normal breast tissues, and 20 cases of benign lesion tissues were assessed by immunohistochemistry. Results: Expression of LOX mRNA and protein was suppressed, and the expression of MMP-2 and MMP-9 was significantly lower in the RNAi group than the control group (P<0.05), after LOX-RNAi-LV was transfection into MDA-MB-231 cells. Migration and invasion abilities were obviously inhibited. The expression of LOX protein in breast cancer, cancer-adjacent normal breast tissues and benign breast tumor were 48.6% (54/111), 26.1% (29/111), 20.0% (4/20), respectively, associations being noted with clinical stage, lymph node metastasis, tumor size and ER, PR, HER2, but not age. LOX protein was positively correlated with MMP-2 and MMP-9. Conclusion: LOX displayed an important role in invasion and metastasis of breast cancer by regulating MMP-2 and MMP-9 expression which probably exerted synergistic effects on the extracellular matrix (ECM).
