한방병원에 입원한 수족냉증을 동반한 추간판탈출증 환자에 대한 한방 치료 1례 (A Case of Cold Hypersensitivity of Hands and Feet Treated with Korean Medicine Including Gyejifabuja-tang-gmibang and Acupuncture in a Patient with Lumbar Spinal Herniated Intervertebral Disc - A Case Report)

  • Objective: The purpose of this study is to report the efficacy of using Korean medicine treatment with cold hypersensitivity of the hands and feet with lumbar spinal herniated intervertebral disc. Case presentation: We used Korean medicine treatment (Gyejigabuja-tang, acupuncture) to treat a hospitalized patient with cold hypersensitivity of the hands and feet with lumbar spinal herniated intervertebral disc. To evaluate the treatment, we used the Numerical Rating Scale (NRS). The degree of cold hypersensitivity of the hands and feet was also evaluated using evaluation paper. The generic health status was measured using the European Quality of Life-5 Dimensions (EQ-5D) scale. The 31-year-old female underwent 13 days of inpatient treatment. Her pattern identification was that both hands and feet are cold (BiYangHeo) types. The treatments were herbal medicine and acupuncture. She took Gyejigabuja-tang for 13 days. She also received acupuncture for 20 minutes twice a day at LI11 (曲池), L14 (合谷), TE5 (外關) on her hands, LR3 (太衝), SP6 (三陰交), SP9 (陰陵泉) ST36 (足三里), GB41 (足臨泣) on Feet, GV4 (命門), GV3 (腰陽關), BL23 (腎兪), BL24 (氣海兪), BL25 (大腸兪), and BL26 (關元兪) on the low back. After 2 weeks of treatment, the NRS scale of her cold hypersensitivity of hands and feet symptoms dropped from NRS 7 to NRS 3, and her low back pain dropped from NRS 6 to NRS 3. Her cold hypersensitivity of hands and feet evaluation paper score decreased from 64 to 32, and her EQ-5D increased from 0.487 to 0.681. After the end of treatment, continued symptom improvements and no significant side effects were confirmed. Results: These findings suggest that Korean medical treatment (Gyejigabuja-tang, acupuncture) may be effective for treating cold hypersensitivity of hands and feet with lumbar spinal herniated intervertebral disc. The NRS, evaluation paper, and EQ-5D scores improved after treatment, with no side effects.

리튬함유 폐액으로부터 D2EHPA에 의한 리튬의 회수 (Recovery of Li from the Lithium Containing Waste Solution by D2EHPA)

  • 리튬함유 폐액에서 $D_2EHPA$를 추출제로 사용하여 용매추출법에 의해 리튬의 회수에 관한 연구를 실시하였다. 수용액상의 pH, 추출제 농도 및 상비 변화 등 리튬의 추출에 영향을 미칠 수 있는 인자들에 대한 실험을 실시하였다. 실험 결과, 평형 pH가 증가할수록 리튬의 추출율이 증가하였고, pH 6.0에서 20% $D_2EHPA$에 의해 최대 50%의 리튬 추출율을 보였다. McCabe-Thiele diagram 분석으로 부터 리튬은 상비(O/A) 3.0에서 4단으로 95%이상 추출이 가능하였다. 한편 탈거액으로 황산을 사용하였고, 리튬 탈거의 최적 황산 농도는 90 ~ 120 g/L 이었다. 연속탈거 공정을 통하여 리튬이 11.85 g/L까지 농축 가능하였으며, 이 용액으로 부터 침전법에 의해 탄산리튬의 제조가 가능하였다.

True Digestibility of Phosphorus in Different Resources of Feed Ingredients in Growing Pigs

  • To determine the true digestible phosphorus (TDP) requirement of growing pigs, two experiments were designed with the experimental diets containing five true digestible P levels (0.16%, 0.20%, 0.23%, 0.26% and 0.39%) and the ratio of total calcium to true digestible P (TDP) kept at 2:1. In Experiment 1, five barrows (Duroc${\times}$Landrace${\times}$Yorkshire) with an average initial body weight of 27.9 kg were used in a $5{\times}5$ Latin-square design to evaluate the effect of different dietary P levels on the digestibility and output of P and nitrogen. In Experiment 2, sixty healthy growing pigs (Duroc${\times}$Landrace${\times}$Yorkshire) with an average body weight (BW) of 21.4 kg were assigned randomly to one of the five dietary treatments (12 pigs/diet), and were used to determine the true digestible phosphorus (TDP) requirement of growing pigs on the basis of growth performance and serum biochemical indices. The results indicated that the true digestibility of P increased (p<0.05) linearly with increasing dietary TDP level below 0.26%. The true P digestibility was highest (56.6%) when dietary TDP was 0.34%. Expressed as g/kg dry matter intake (DMI), fecal P output increased (p<0.05) linearly with increasing P input. On the basis of g/kg fecal dry matter (DM), fecal P output was lowest for Diet 4 and highest (p<0.05) for Diet 5. The apparent digestibility of crude protein (CP) did not differ (p>0.05) among the five diets, with the average nitrogen output of 12.14 g/d and nitrogen retention of 66% to 74% (p>0.05), which suggested that there was no interaction between dietary P and CP protein levels. During the 28-d experimental period of Experiment 2, the average daily gain (ADG) of pigs was affected by dietary TDP levels as described by Eq. (1): $y=-809,532x^4+788,079x^3-276,250x^2+42,114x-1,759$; ($R^2=0.99$; p<0.01; y = ADG, g/d; x = dietary TDP, %), F/G for pigs by Eq. (2): $y=3,651.1x^4-3,480.4x^3+1,183.8x^2-172.5x+10.9$ ($R^2=0.99$; p<0.01; y = F/G; x = dietary TDP, %), and Total P concentrations in serum by Eq. (3): $y=-3,311.7x^4+3,342.7x^3-1,224.6x^2+195.6x-8.7$ (R2 = 0.99; p<0.01; y = total serum P concentration and x = dietary TDP, %). The highest ADG (782 g/d), the lowest F/G (1.07) and the highest total serum P concentration (3.1 mmol/L) were obtained when dietary TDP level was 0.34%. Collectively, these results indicate that the optimal TDP requirement of growing pigs is 0.34% of the diet at a total Ca to TDP ratio of 2:1.

  • The purpose of this study is to report the effects of Korean medicine treatment in baastrup's disease. We treated the patient for 34 days using Korean medicine treatment such as acupuncture, pharmacopuncture, herbal medicine, Chuna manual therapy and help motion style acupuncture treatment. Numerical rating scale (NRS), EuroQol five dimension (EQ-5D) index, Oswestry disability index (ODI), and lumbar range of motion (ROM) were used as evaluation tools for the state. NRS decrease from 5 to 3, and EQ-5D index, ODI, L-spine ROM were improved by the treatment. In conclusion, this case study shows that Korean medicine treatment can be effective for baastrup's disease.

한국 중년남성의 건강관련 삶의 질에 대한 잠재계층분석 (Latent Class Analysis for Health-Related Quality of Life in the Middle-Aged Male in South Korea)

  • Purpose: This study aimed to identify types of quality of life (QoL) based on the 5 dimensions of EQ-5D and predict factors affecting types of QoL. Methods: This study was a secondary analysis using data from the Korean Health Panel Survey-II(2012). Participants were 2,071 middle-aged men who had completed the additional survey in 2012 and the data were analyzed using SPSS 20.0 and Mplus 5.21 for latent analysis. Results: Three latent classes of QoL were identified: serious (2.4% of the sample), threatened (15.5%), and stable types (82.0%). The types and characteristics of QoL among the latent classes differed. On comparing latent type 1 with latent type 2, the socioeconomic status (p<.05), employment status (p<.05), and subjective health status (p<.001) were found to be significant. On comparing latent type 1 with latent type 3, the socioeconomic status (p<.05), current smoking status (p<.001), and subjective health status (p<.001) were found to be significant. On comparing latent type 2 with latent type 3, the socioeconomic status (p<.05), subjective health status (p<.001), stress (p<.001) were found to be significant. Conclusion: The results showed significant heterogeneity in types of QoL and the predictors of QoL by types were different. These findings provide basic information for developing nursing interventions to improve QoL. Specific characteristics depending on the subtypes should be considered during the development of interventions.

쌀의 생산과정에서 발생하는 탄소배출량 산정을 위한 전과정평가 적용 (Estimation of Carbon Emission and Application of LCA (Life Cycle Assessment) from Rice (Oryza sativa L.) Production System)

  • 농산물 탄소성적표지제도 도입을 위한 농업분야 LCI database에 대한 연구를 위하여 쌀의 생산체계를 대상으로 전과정평가를 수행하였다. LCI 구축을 위한 영농 투입물과 산출물에 대한 데이터결과 유기질비료와 복합비료가 각각 4.29E-01 kg $kg^{-1}$ rice, 2.30E-01 kg $kg^{-1}$ rice으로 영농단계에서 가장 높은 투입량을 보였고, 영농단계에서 발생하는 직접대기배출물이 3.23E-02 kg $kg^{-1}$ rice의 배출값을 나타내었다. 쌀생산체계에서 발생하는 온실가스를 중심으로 전과정 목록분석을 수행한 결과 탄소성적값은 8.70E-01 kg $CO_2$-eq. $kg^{-1}$ rice이었다. 이 중 $CO_2$가 7.02E-01 kg $CO_2$-eq. $kg^{-1}$ rice로 온실가스 발생의 약 80%가 $CO_2$였고, $CH_4$$N_2O$의 배출량은 각각 13%, 5%의 비중을 차지하였다. 전과정 영향평가 결과 지구온난화에 기여하는 주요인자는 유기질비료 생산 공정이 42%, 무기질비료 생산공정이 40%로 비료생산공정에 의한 비중이 가장 높았다.

  • High-speed flight vehicle have various cavities. The supersonic cavity flow is complicated due to vortices, flow separation and reattachment, shock and expansion waves. The general cavity flow phenomena include the formation and dissipation of vortices, which induce oscillation and noise. The oscillation and noise greatly affect flow control, chemical reaction, and heat transfer processes. The supersonic cavity' flow with high Reynolds number is characterized by the pressure oscillation due to turbulent shear layer, cavity geometry, and resonance phenomenon based on external flow conditions, The resonance phenomena can damage the structures around the cavity and negatively affect aerodynamic performance and stability. In the present study, we performed numerical analysis of cavities by applying the unsteady, compressible three dimensional Reynolds-Averaged Navier-Stokes(RANS) equations with the ${\kappa}-{\omega}$ turbulence model. The cavity model used for numerical calculation had a depth(D) of 15mm cavity aspect ratio(L/D) of 3, width to spanwise ratio(W/D) of 1.0 to 5.0. Based on the PSD(Power Spectral Density) and CSD(Cross Spectral Density) analysis of the pressure variation, the dominant frequency was analyized and compared with the results of Rossiter's Eq.

영남권역에서 가정용 인공호흡기를 사용하는 환자 가족간병인의 간병 부담과 삶의 질 (Survey on the Care Burden and Quality of Life in Family Caregivers of Patients Using Home Mechanical Ventilator in Yeongnam Region, Korea)

  • 목적 : 본 연구는 영남권역 내 가정에서 인공호흡기 사용 환자를 돌보는 가족간병인이 느끼는 간병 부담과 삶의 질을 알아보고자 하였다. 연구방법 : 영남권역 내에 거주하며 가정에서 인공호흡기를 사용하는 환자를 돌보는 주간병인을 대상으로 설문조사를 시행하였고 가족간병인의 응답 내용을 분석하였다. 설문지는 환자의 정보, 간병 실태 및 추가 지원 요구 사항 등으로 구성하였다. 가족간병인의 간병 부담과 삶의 질을 평가하기 위해 각각 Korean version of Short Form Zarit Burden Interview(K-ZBI-12)과 한국형 가중치 모형에 따라 교정한 3-Level version of EuroQol-5 Dimension(KEQ-5D-3L)을 사용하였다. 통계적 유의수준은 p<0.05로 하였다. 결과 : 총 150명 중 주간병인이 가족간병인인 98명을 분석하였다. 환자의 연령은 54.10±17.94세, 성별은 남자 74명(75.5%), 여자 24명(24.5%)이었고 주 진단은 신경근육계 질환이 83명(84.7%)으로 가장 많았다. 가족간병인의 K-ZBI-12 점수는 33.08±10.34로 간병에 대한 고부담을 보였고 EQ-5D-3L 점수는 0.71±0.25로 두 항목은 음의 상관관계를 보였다(p=.038). 환자의 연령, 인공호흡기 사용기간, 의료비에 대한 가족간병인의 경제적 부담, 간병비에 대한 가족간병인의 경제적 부담 및 전문간병인의 참여 시간은 K-ZBI-12 점수와 각각 통계적으로 유의한 상관관계를 보였다. KEQ-5D-3L 점수는 가정용 인공호흡기의 사용기간과 음의 상관관계를 나타냈다(p=.017). 환자가 침습적 인공호흡기를 사용하는 경우 비침습적 인공호흡기를 사용하는 경우보다 가족간병인의 KEQ-5D-3L 점수가 낮았다(p=.008). 간병에 참여하는 인원이 두 명 이상인 경우가 한 명인 경우보다 K-ZBI-12 점수가 낮았다(p=.001). 가족간병인이 추가지원을 바라는 요구사항 중 경제적 지원에 대한 요구도가 가장 높게 조사되었다. 결론 : 영남권역에서 가정용 인공호흡기를 사용하는 환자들의 보호자가 느끼는 간병 부담이 크고 삶의 질이 낮다는 것을 알 수 있다. 가족간병인의 간병 부담과 삶의 질을 개선하기 위해서 지역에서 실질적으로 필요한 사회경제적 지원 및 환자와 가족들의 요구 사항을 확인하고 추가적인 지원을 위한 노력이 필요하다.

The Characteristics related to Pulmonary Rehabilitation in Patients with Chronic Obstructive Pulmonary Disease: A Cross-sectional Study, Data from the Korea National Health and Nutrition Examination Survey 2015-2019.

  • Objective: Patients with chronic obstructive pulmonary disease (COPD) may experience reduced physical activity and quality of life (QoL) due to decreased pulmonary function. The purpose of this study was to investigate the level of pulmonary function, physical activity, and QoL of COPD patients. Design: Cross-sectional observational study. Methods: This study examined the published data of the Korea National Health and Nutrition Examination Survey in 2015-2019. Among 39,759 subjects who participated for 5 years, data from 151 patients diagnosed with COPD were analyzed separately. For the pulmonary function, the results of forced vital capacity (FVC), forced expiratory volume in 1 second (FEV1), FEV6, forced expiratory flow 25-75%, and peak expiratory flow were observed. Physical activity was identified as frequency and duration. For the QoL, EQ-5D-3L evaluation results were examined, and the frequency and index of the Korean version were investigated. Results: In pulmonary function, all variables were found to be lower than age and weighted matched normal values. COPD patients showed to perform very low levels of high/medium physical activity and sitting time was confirmed to be more than 8 hours a day. In QoL, it was found that the highest reporting rate of some problems was the "pain and discomfort" and "mobility". Conclusions: It was found that COPD patients showed that the prevalence of circulatory disease was relatively high, lowered pulmonary function, and QoL. These can be improved through regular physical activity, and it is thought that this can be achieved through optimization of pulmonary rehabilitation.
