EQ-5D를 이용한 강박장애 환자의 삶의 질과 증상 중증도의 연관성 연구 (Association between Quality of Life and Symptom Severity in Obsessive-Compulsive Disorder Patients Using EQ-5D)

  • Objectives Obsessive-compulsive disorder (OCD) is a disabling psychiatric disorder, and more attention is recently paid on the quality of life (QoL) in OCD patients. The Euro-QoL-5D (EQ-5D) is a widely used self-report to calculate a single score which represents 'health utilities'. The aim of this study was to assess the health-related QoL for patients with OCD using the EQ-5D and to examine the relationship between health-related QoL and symptom severity. Methods Seventy-three patients with a Diagnostic and Statistical Manual of Mental Disorders (DSM-5) diagnosis of OCD were recruited from the outpatient clinic in Seoul National University Hospital. Symptom severity was assessed using the Yale-Brown Obsessive-Compulsive Scale (YBOCS), and QoL was assessed with the EQ-5D-5L questionnaire. Using Korean valuation study, a single score of QoL was calculated. Results Most of the OCD patients were relatively young (< 45 years) with the mean YBOCS total score of 19.33. The mean EQ-5D score was 0.71 and significantly correlated with symptom severity (r = -0.483, p < 0.001). 25% of the EQ-5D score was predicted by the YBOCS total score (b = -0.011, p < 0.001) by regression analysis. Conclusions OCD patients suffer from lower health-related QoL and QoL significantly decreased as symptom severity increased. The results of the EQ-5D would enable further studies on QoL comparison across medical disease and mental disorders.

Evaluation of Health-Related Quality of Life among Patients with Cervical Cancer in Indonesia

  • Background: Evaluation of health-related quality of life (HRQOL) in cervical cancer patients is important in order to design the interventions for improving patient outcomes. Reports of HRQOL among cervical cancer patients in Indonesia are limited. Moreover, measurement using EQ-5D-3L is to our best knowledge has hitherto not been performed. This study aimed to examine the HRQOL of cervical cancer patients in Indonesia using EQ-5D-3L. Materials and Methods: A cross-sectional study was conducted by interviewing cervical cancer patients using the EQ-5D-3L questionnaire. Percentages of patients who reported having problems in each dimension of EQ-5D as well as EQ-5D index score (utility) were calculated. Results: Our findings indicated that the most frequent reported problems were pain/discomfort (67.8%) followed by anxiety/depression (57.5%). The mean of EQ-5D VAS was 75.8 (SD=17.0). The mean (SD) utility scores were 0.85 (0.19), 0.76 (0.20), 0.71 (0.21), and 0.77 (0.13) for cervical cancer patients in stage I, II, III, and IV, respectively. Conclusions: Cervical cancer significantly affects patient HRQOL. Efforts should be made to improve the quality of life of cervical cancer patients especially in terms of pain /discomfort and anxiety/depression reduction.

  • Objectives: This study intended to determine significant factors that influence the health-related quality of life ("HRQoL"; EuroQol 5 Dimension health-related quality of life (EQ_5D) & EuroQol visual analogue scale (EQ_VAS)) of the elderly in Korea. Methods: This study was based on 3,903 subjects aged 65 years or more who participated in the Fifth Korea National Health and Nutrition Examination Survey (KNHANES V), 2010~2012. The HRQoL was analyzed by various factors (general characteristics, health habits, mental health, chronic diseases, nutrient intakes). SPSS statistics for complex samples (Windows ver. 21.0) was used. Results: The HRQoL was higher in the males, those with higher educational level or higher income level while it was lower in those belong to single households. In particular, the EQ_5D was significantly higher in the group who reported walking practice, moderate physical activity (male), and the group who reported no vigorous physical activity (female). The EQ_VAS was significantly higher in the group who reported walking practice. Both EQ_5D and EQ_VAS were significantly lower in the group with stress, melancholy, suicidal thinking, and osteoarthritis. EQ_5D was significantly lower in the group with < 75% Estimated Energy Requirements (EER) in energy intake, and with < Estimated Average Requirements (EAR) in iron or niacin intake. A stepwise regression analysis revealed that i) higher educational level (male), and good self-rated health status significantly increased the EQ_5D, ii) age, alcohol intake (male), melancholy (female), suicidal thinking, osteoarthritis, and niacin intake deficiency (male) significantly decreased the EQ_5D, iii) higher income level (male) and good self-rated health status significantly increased the EQ_VAS, and iv) age (male), stress, suicidal thinking (female) and osteoarthritis significantly decreased the EQ_VAS. Conclusions: This study suggested that general characteristics, mental health, osteoarthritis, and niacin intake were associated with the HRQoL. Prospective research of long-term control is needed to establish the causal relationship between factors and the HRQoL.

한국 노인의 보철상태에 따른 건강관련 삶의 질 (The Effect of Korean Elderly's Prosthetic Status on Health Related Quality by Using EuroQol-5 Dimension)

  • 이 연구는 국민건강영양조사를 이용하여 우리나라 만 65세 이상 노인을 대상으로 보철 상태에 따른 건강관련 삶의 질을 조사하여, 노인의 구강건강증진의 기초자료를 확보하고자 하였으며, 다음과 같은 결론을 얻었다. 전체 연구대상자의 HRQoL은 0.86이었으며, 성별에 따른 HRQoL은 남자에서 0.90에 비해 여성에서 0.82로 남자가 유의하게 높았으며, 월 평균 소득이 증가할수록, 학력수준이 높을수록, 상실치아 개수가 낮을수록 HRQoL이 유의하게 높았다. 연구대상자 중 고정성 가공의치와 총의치 필요한 대상자는 HRQoL이 유의하게 낮았으며, 부분의치와 총의치를 장착하지 않은 대상자에서 HRQoL이 유의하게 높았다. EQ-5D의 현재의 건강 상태 5개의 항목에서 운동능력 항목은 부분의치 장착군에서 비장착군에 비해 어려움에 대한 응답한 비율이 높았으며, 자기관리 항목에서 총의치장착군에서 어려움이 있다고 응답한 비율이 높았다. 불안/우울 항목에서 총의치 장착군이 비장착군에 비해 어려움이 있다고 응답한 비율이 높았다.

  • 사육수의 담수화시 수용밀도에 대한 생리학적 반응을 비교하고자, 30마리와 60마리씩 실험수조에 수용하여 30분이내에 사육수를 담수로 교환하였다. 혈장의 Cortisol 농도는 실험개시시 30SW에서 $2.8\;ng\;mL^{-1}$,60SW는 $2.7\;ng\;mL^{-1}$로부터 30FW와 60FW는 24시간째 각각 $66.9\;ng\;mL^{-1}$, $314.1\;ng\;mL^{-1}$로 유의하게 높아졌다. 혈장의 Glucose농도는 60SW는 개시시 $15.8\;mg\;dL^{-1}$로부터 60FW는 3시간째 $257.7\;mg\;dL^{-1}$, 24시간째 $164.0\;mg\;dL^{-1}$로 개시시보다 높은 값으로 남아있었다. $Na^+$ 농도는 30SW와 60SW는 시간경과에 따라 차이를 보이지 않았으나, 30FW와 60FW는 시간경과에 따라 낮아지는 경향을 보였다. $Cl^-$농도는 60FW에서 개시시 $59.0\;mEq\;L^{-1}$로부터 3시간과 24시간째에 각각 $43.5\;mEq\;L^{-1}$, $30.0\;mEq\;L^{-1}$로 유의하게 낮아진 값을 보였다. 모든 실험구에서 생존율은 100%였다. 사육수를 24시간동안 담수로 교환시 사육밀도의 차이에 따른 혈장과 혈액학적 지표에서 다소 차이를 나타냈으며, 삼투압 조절 능력 및 생존율에서는 해수에서와 같은 경향을 보였다.

개심술시 체외순환에 의한 혈청 POTASSIUM 변동에 관한 연구 (A Clinical Study of Changes in Serum Potassium Ion Concentration Before and After Extracorporeal Circulation with Heart-Lung Machine)

  • Since the open heart surgery was performed, various kinds of problem concerning the extracorporeal circulation[EGG] have been known. The author investigated the changes of serum potassium ion before and after ECC among the 102 patients including 63 adults and 39 children who underwent open heart surgery from April 19S6 to February 1990 in Chung-Ang University Hospital. The mean values of potassium ion before and after ECC were analyzed according to the influencing factors such as priming solution, aortic cross clamping time, the underlying disease, the type of oxygenator and the amount of cardioplegic solution. The results were as follows: l. In the aspect of congenital and acquired heart disease groups, the mean values of serum potassium ion[Mean\ulcornerS.D.] before and after ECC revealed a significant change only in the acquired heart disease group[congenital; 3.87$\pm$0.48mEq /L vs. 4.05$\pm$0.73mEq /L, P>0.05, acquired: 4.40 $\pm$0.98mEq /L vs. 4.11$\pm$0.52mEq /L, P<0.05]. Between the two groups, the changes of the mean values of serum potassium ion before and after ECC were significant[P<0.05]. But all values were within normal limits. 2. In the aspect of the aortic cross clamping time[ACCT], in the groups of less or more than 120 minutes, the mean values of serum potassium ion before and after ECC revealed no significant change[less than 120 min; 3.97+-0.64mEq /L vs. 3.99+0.67mEq /L, P>0.05, more than 120 min; 4.34+0.82mEq /L vs. 4.27+0.62mEq /L, P>0.05], and The changes of mean values of serum potassium ion between the two groups were not significant[P>0.05]. 3. In both membrane and bubble oxygenator groups, the mean values of serum potassium ion before and after ECC did not reveal a significant difference respectively [membrane; 4.74 +1.40mEq /L vs. 4.28+0.3lmEq /L, P>0.05, bubble; 4.02 +0.60mEq /L vs. 4.05 L0.68mEq/L, P>0.05], and no differences between the membrane and bubble oxygenator groups[P >0.05]. 4. In the groups of membrane and bubble oxygenator in the cases of ACCT more than 120 minutes, the mean values of serum potassium ion before and after ECC did not reveal a significant difference respectively[membrane; 4.36$\pm$0.85mEq /L vs. 4.37$\pm$0.26mEq /L, P>0.05, bubble; 4.30 $\pm$0.80mEq/L vs. 4.23$\pm$0.67mEq/L, P>0.05], and no differences between the two groups[P>0.05]. 5. In spite of increased amount of cold potassium cardioplegic solution, the mean values of serum potassium ion before ECC were similar to those of serum potassium ion after ECC[less than 20ml /kg

성인의 구강건강 상태와 건강관련 삶의 질 관련성: EuroQoL-5 Dimension 구성요소를 중심으로 (Association between Oral Health Status and Health Related Quality of Life (EuroQoL-5 Dimension))

  • 본 연구는 2013년 KNHANE 자료를 이용하여 우리나라 성인의 구강건강 상태와 HRQoL 관련성을 EQ5D 구성요소를 중심으로 평가하고자 하였다. 16~64세 성인 3,252명을 최종 분석대상자로 하여 다음과 같은 결론을 얻을 수 있었다. 치아통증과 운동능력, 통증 및 불편은 통계적으로 유의한 관련성을 나타냈고, 저작 불편과 말하기 불편은 EQ5D 구성요소 5가지 모두 통계적으로 유의한 관련성을 나타냈다. 주관적 구강건강 인식은 자기관리, 일상활동, 통증 및 불편, 불안 및 우울과 통계적으로 유의한 관련성을 나타내어 구강건강 상태와 EQ5D 구성요소의 관련성을 확인하였다. 또한 EQ5D 구성요소에 미치는 영향을 통제변수를 보정 후 로지스틱 회귀분석으로 살펴본 결과 치아통증, 저작 불편, 말하기 불편, 주관적 구강건강 인식이 불안 및 우울에 영향을 미치는 것으로 나타났다. 구강건강 증진은 HRQoL을 향상시킬 수 있는 방안이 되므로, 구강건강의 중요성을 인식하고 예방차원의 교육 및 프로그램이 필요하다고 생각된다.

50세 이상 한국인의 식물성 단백질 섭취와 건강 관련 삶의 질과의 연관성: 국민건강영양조사 2016-2018 (Plant Proteins in Relation to Health-related Quality of Life in South Korean Individuals Aged 50 Years or Older: Korea National Health and Nutrition Examination Survey 2016-2018)

  • 본 연구에서는 50세 이상 한국인을 대상으로 식물성 단백질 섭취와 건강관련 삶의 질과의 연관성을 분석하였다. 2016~2018년 국민건강영양조사 자료를 이용하여 7,956명(남성 3,434명, 여성 4,522명)이 연구에 포함하였다. 건강관련 삷의 질은 신체 기능, 자기 관리, 일상 활동, 통증/불쾌감, 불안/우울 문제로 구성된 삶의 질 5차원 지표(EQ5D)를 사용하여 측정하였다. 식물성 단백질 일일 섭취량과 건강관련 삶의 질 사이의 연관성은 회귀 분석을 사용하여 평가하였다. 총 식물성 단백질, 콩과 견과류 종자(LNS) 및 발효 콩(FERMB) 단백질의 섭취는 남성과 여성 모두에서 건강관련 삷의 질과 건강관련 삷의 질과 유의미한 연관이 있었다(p < 0.01). EQ5D의 지표 중 신체 기능과 일상 활동이 식물성 단백질 섭취와 가장 밀접한 관련이 있었다. LNS 단백질이나 FERMB 단백질 섭취에 비해 총 식물성 단백질 섭취가 건강관련 삷의 질과 더 큰 연관성을 보였다. 결론적으로, 식물성 단백질 섭취와 건강관련 삷의 질 사이에는 유의미한 연관성이 있었다. 이는 식물성 단백질의 적절한 섭취가 한국 성인의 건강관련 삷의 질 유지 또는 개선에 도움이 될 수 있음을 시사한다.

누크관 수종 제거 이후 후유증에 대한 한방치료 1례 : 증례보고 (Improvement of Symptoms after Excision of Hydrocele of Canal of Nuck with Korean Medicine: A Case Report)

  • Objective: The purpose of this case study is to report the hydrocele of the canal of Nuck and the effectiveness of Korean medicine in post-surgery improvement of symptoms. Method: A patient who was diagnosed with hydrocele of the Nuck canal and had symptoms after Lichtenstein's operation was treated using Korean medicine, including acupuncture, herbal medicine, and moxibustion, every day. The evaluation was performed using the Numeral Rating Scale (NRS), the 5-level EQ-5D version (EQ-5D-5L), the EQ visual analogue scale (EQ-VAS), and the World Health Organization Quality of Life-Brief (WHOQOL-BREF). Results: After treatment, changes were observed in NRS (from 8 to 2-3) and EQ-VAS (from 60 to 85), whereas there was no difference in EQ-5D-5L (both 6). There was an overall improvement in the domains of WHOQOL-BREF and subjective symptoms. Conclusion: This study suggests that Korean medicine may improve symptoms after surgery of hydrocele of the canal of Nuck and can be evaluated using an overall QoL questionnaire.

Perceptions of the General Public About Health-related Quality of Life and the EQ-5D Questionnaire: A Qualitative Study in Korea

  • Objectives: The aim of this qualitative study was to investigate how members of the general public in Korea interpret the concept of health, and which dimensions of health are most important to them. We also explored their perceptions of the EuroQoL 5-Dimension (EQ-5D), including the EuroQoL visual analogue scale (EQ-VAS). Methods: We conducted face-to-face, in-depth interviews with 20 individuals from the general population, using a semi-structured interview guide. Content analysis was performed with verbatim transcripts and field notes to identify codes and categorize them according to their similarities and associations. Results: In total, 734 different codes were derived and classified into 4 categories. Participants cited the importance of both the mental and physical aspects of health, although they emphasized that the physical aspects appeared to play a larger role in their conceptualization of health. Participants noted that the EQ-5D has the advantage of being composed of 5 dimensions that are simple and contain both physical and mental areas necessary to describe health. However, some of them mentioned the need to add more dimensions of mental health and social health. Participants showed great satisfaction with the visually well-presented EQ-VAS. However, participants opined that the EQ-VAS scores might not be comparable across respondents because of different ways of responding to the scale. Conclusions: While physical health is a fundamental aspect of health, mental and social aspects are also important to Koreans. The content of the EQ-5D broadly matched the attributes of health considered important by Koreans.
