Purpose: This study aimed to derive detailed insights into emergency medical technicians' (EMTs) perceptions regarding continuing education. Methods: We conducted a descriptive survey. The interview participants were 20 EMTs, divided into four groups: six Fire Department EMTs, five Hospital EMTs, five Clinical EMTs (excluding the ER and Fire Department), and four Non-Clinical EMTs. Data were collected through focus group interviews (FGIs) and semistructured questionnaires. The interview periods were March 25, 2022, and March 27, 2022. Results: Continuing education was primarily field-oriented, leading to lower satisfaction among non-field EMTs. To enhance satisfaction, training should be tailored to the specific roles and experiences of the EMTs. Additionally, quality management of lectures, instructor selection, and video quality require reconsideration. Key factors that enhance satisfaction include a wider variety of courses, up-to-date information, and relevant course content, regardless of job role or experience. Conclusion: Incorporating feedback into EMTs' perceptions of continuing education can lead to the development of more effective curricula and improved quality management. This approach is expected to enhance both educational satisfaction and job performance.

Emerging evidence has shown associations of microRNA-205 (miR-205) with crucial cell processes such as the epithelial-mesenchymal transition (EMT) and aberrant expression with tumorigenesis in many types of human malignancy. This prospective study characterized the contribution of miR-205 to the colorectal cancer (CRC) tumorigenesis. The real-time reverse transcription-polymerase chain reaction was used to examine miR-205 levels prospectively in 36 pairs of samples of CRC tissue and adjacent noncancerous tissue (>2 cm from cancer tissue). In addition, the relationship between miR-205 levels and clinicopathological features was explored. The capability of miR-205 to function as a tumor marker was also examined. miR-205 expression levels did not show significant changes overall. However, miR-205 was significantly downregulated in a group of CRC samples compared with matched noncancerous tissue samples. Moreover, decreased miR-205 correlated significantly with lymphatic metastasis. A receiver operating characteristic (ROC) curve also showed an optimum cut off point of $1.4{\times}10^{-3}$ to distinguish lymphatic metastatic CRCs from non-metastatic CRCs. Interestingly we found lymphatic metastasis in almost 80% of the depressed samples. This study suggested that miR-205 could be reduced in the majority of metastatic CRCs and the risk of CRC metastasis may be predicted by monitoring miR-205 in patient samples collected at the time of the initial diagnosis. Therefore, targeting miR-205 and its potential environmental activators might be a promising therapeutic option to prevent malignant progression toward metastasis.
Purpose : The purpose of this study was to verify the effects of academic self-efficacy on self-directed learning attitude of emergency medical students. Methods : A total of 209 emergency medical technology students filled out the questionnaire after receiving the informed consents. The questionnaire consisted of demographical characteristics, academic self-efficacy and self-directed learning attitude. Results : Academic self-efficacy and self-directed learning attitude of the subjects were above mean average. Higher correlations existed between academic self-efficacy and self-directed learning attitude of the subjects, and the significant positive correlations were found between sub-factors of the academic self-efficacy and self-directed learning attitude. The academic self-efficacy of the subjects had the significant influence on the self-directed learning, and self-efficacy. The self-confidence among sub-factors in the academic self-efficacy had a significant influence on self-directed learning attitude and sub-factors. Conclusion : The results of the study subjects of academic self-efficacy had a significant influence on the attitude of self-directed learning, In particular, self-efficacy and confidence of the academic self-efficacy sub-factors showed a significant impact on the sub-factors, attitudes and self-directed learning. This study will provide the basic data for the implementation of the educational programs. This will be effective in managing the knowledge, information, self-directed learning attitude and academic self-efficacy.
Purpose : This study examines satisfaction of students at the department of Emergency Medical Technology with clinical practice and relationship between motivation of achievement and satisfaction with clinical practice and aims to provide basic materials contributing to the improvement of clinical practice education. Method : It selected 237 students in the second and third grade at the dept. of Emergency Medical and having experience in clinical practice as the subjects, collected data from Sept. 1 to 30, 2007 and analyzed 237 questionnaires collected with SPSS 12.0 program. Result : 1. Degree of motivation of achievement scored 3.46 of full marks of five points and they showed motivation of achievement over average score. Students who entered the dept. of Emergency medical after beginning social career(3.78) showed higher motivation of achievement in the period of entrance and group having will to be volunteer(3.66) showed the highest motivation of achievement in selecting this major. The better personal relationship they have, the greater hospital grade and the higher their transcript, the higher motivation of achievement they have. In the desired area of employment after graduation, first-aid staffs at the field of industry group(3.85) showed the highest motivation of achievement. 2. Degree of their satisfaction with clinical practice scored 3.53 of full marks of five and they showed satisfaction with it over average score. They showed the highest satisfaction in the contents of practice(3.64). 3. There was statistically significant positive correlation between motivation of achievement and satisfaction with clinical practice(r = .165, p < .05). Conclusion : There was statistically significant positive correlation between motivation of achievement and satisfaction with clinical practice. Repeated research through sampling based on probability to generalize relevance of motivation of achievement and satisfaction with clinical practice is needed and further researches on developing program to inspire motivation of achievement in theoretical learning as well as in clinical practice are required.
Ptdlns(4,5)$P_2$ is a key cellular phosphoinositide that localizes in separate and distinctive pools in subcellular membrane and vesicular compartments. In membranes, Ptdlns(4,5)$P_2$ acts as a precursor to second messengers and is itself a main signaling and targeting molecule. Specific subcellular localization of type I PIP kinases directed by interacting with specific targeting module differentiates Ptdlns(4,5)$P_2$ production in a spatial and temporal manner. Several lines of evidences support the idea that Ptdlns(4,5)$P_2$ is generated in very specific pools in a spatial and temporal manner or by feeding Ptdlns(4,5)$P_2$ directly to effectors. In this concept, the interaction of PIPKI isoforms with a specific targeting module to allow precise subcellular targeting modulates highly specific Ptdlns(4,5)$P_2$ synthesis and channeling overall effectors. For instance, localization of PIPKI${\gamma}$661 to focal adhesions by an interaction with talin results in spatial and temporal production of Ptdlns(4,5)$P_2$, which regulates EGF-stimulated directional cell migration. In addition, Type $I{\gamma}$ PIPK is targeted to E-cadherin in cell adherence junction and plays a role in controlling dynamics of cell adherence junction and endocytosis of E-cadherin. Characterizing how PIP kinase isoforms are regulated by interactions with their targeting modules, as well as the mechanisms by which their product, Ptdlns(4,5)$P_2$, exerts its effects on cellular signaling processes, is crucial to understand the harmonized control of numerous cellular signaling pathways. Thus, in this review the roles of the Ptdlns(4)P(5) kinases and Ptdlns(4,5)$P_2$ were described and critically reviewed in terms of regulation of the E-cadherin trafficking, cell migration, and formation of cell adherence junction which is indispensable and is tightly controlled in epithelial-to-mesenchymal transition process.
With the rapid development of information and communication technology, a lot of multi-media learning programs are being developed and reported in the field of Emergency medicine both home and abroad. In this connection, this study was aimed at developing a foreign-body airway obstruction courseware in adults for EMT. The development period of CAI courseware lasted from May 2003 through November 2003. Among CAI courseware patterns, private instruction and repeat practice and simulation patterns were used as an instruction-learning strategy. The learning contents of the CAI courseware consisted of five chapters concerning (1) A relief of partial FBAO in the responsible victim, (2) A relief of complete FBAO in the responsible victim, (3) In case of unconsciousness in the responsible victim without removing all foreign body, (4) In case of consciousness in all victims after getting removed all foreign body and (5) A complete airway obstruction in victims without consciousness on the basis of assess responsiveness and the degree of airway obstruction. The way to use this courseware, with just a click on one specific chapter, was developed to proceed a course with progressive algorithm, a method of solving problems by choosing one between two situations. A characteristic of this CAI courseware is the enhanced efficiency of an instruction-learning method by providing an opportunity of choice based on situations in its effort to encourage learners to use a self-initiated learning method, not one-way method and to enhance problem solving skills among situations. Moreover, this courseware went through the diverse phases such as development, application, feedback in connection with learning process by practicing teachers, so that the courseware could be used frequently in the future. The contents of this courseware were written with the web, so that, if necessary, the contents could be continuously modified and complemented and handed out in the form of CD-ROM. This study indicates that the development of a variety of CAI courseware requires institutional and financial assistance and initiatives reflecting a reality in terms of learning process, technical assistance and resources.
Purpose: The purpose of this study was to investigate the extended job of paramedic after the 1st job description in 2000 by the National Health Personnel Licensing Examination Board. Methods: This study was carried out from May 2012 to July 2013 on the Developing A Curriculum (DACUM) method to analysts 330 responses among 400 questionnaires (response rate 82.5%) after analysing 30 pilot questionnaires. In order to enhance validity and reliability of this job description, 2 job analysts in universities and 10 EMT-Paramedics in hospitals and fire stations were recruited. Results: There were differences between 166 (year 2000) and 240 (2012) in task element. There were higher answering at triaging patient(2.64 frequency, 4.15 importance), calling for help(2.68 frequency, 4.07 importance) in disaster emergency care. There were higher answering at measuring vital sign (3.95 frequency, 4.22 importance), measuring ECG(3.84 frequency, 4.14 importance), wound care(3.48 frequency, 3.86 importance), bleeding control & wound care(3.46 frequency, 3.86 importance), applying $O_2$(3.40 frequency, 3.94 importance) in physician assistant. Conclusion: Scope of extended practice as a physician assistant in hospital setting recommended us to replace the present curriculum and national exam with new curriculum and national written & skill test.

Purpose: This descriptive study investigated the relationship between death awareness and life ethics awareness, according to life stress, among students in the department of paramedics. Methods: General characteristics were identified using means and frequency, and the differences between the two military models were analyzed using the χ2-test and t-test by dividing them into lower and higher groups based on the mean life stress score (99.76 points). Results: Those with high life stress had higher death awareness than those with low life stress at 114.11 points. In contrast, those with low life ethics experienced more severe life stress with a score of 145.61 points (t=-2.609, p=.010)(t=-2.953, p=.003). The death recognition attitude and bioethics according to the degree of living stress-showed a significant correlation between the low and high groups (r=.188, p=.043) (r=.201, p=.042). Conclusion: Paramedic students require education on how to cope with life stress. However, access to education is limited to people living in modern times. As a potential solution to this problem, observing videos on the Internet is recommended. Moreover, we suggest accessing Internet and smart phone applications for advertising/educational purposes.

Tight junction protein 1 (TJP1), a component of tight junction, has been reported to play a role in protein networks as an adaptor protein, and TJP1 expression is altered during tumor development. Here, we found that TJP1 expression was increased at the RNA and protein levels in TGF-${\beta}$-stimulated lung cancer cells, A549. SB431542, a type-I TGF-${\beta}$ receptor inhibitor, as well as SB203580, a p38 kinase inhibitor, significantly abrogated the effect of TGF-${\beta}$ on TJP1 expression. Diphenyleneiodonium, an NADPH oxidase inhibitor, also attenuated TJP1 expression in response to TGF-${\beta}$ in lung cancer cells. When TJP1 expression was reduced by shRNA lentiviral particles in A549 cells (A549-sh TJP1), wound healing was much lower than in cells infected with control viral particles. Taken together, these data suggest that TGF-${\beta}$ enhances TJP1 expression, which may play a role beyond structural support in tight junctions during cancer development.

Recently, electric power systems have been operating with tight margins and have reached their operational limits. Many researchers consider a microgrid as one of the best solutions to relieve that problem. The microgrid is generally powered by renewable energies that are connected through power converters. In contrast to the rotational machines in the conventional power plants, the converters do not have physical rotors, and therefore they do not have rotational inertia. Consequently, a stand-alone microgrid has no inertia when it is powered by the only converter-based-generators (CBGs). As a result, the relationship between power and frequency is not valid, and the grid frequency cannot represent the power balance between the generator and load. In this paper, a superconducting flywheel energy storage system (SFESS) is applied to an inertia-free stand-alone (IFSA) microgrid. The SFESS accelerates or decelerates its rotational speed by storing or releasing power, respectively, based on its rotational inertia. Then, power in the IFSA microgrid can be balanced by measuring the rotor speed in the SFESS. This method does not have an error accumulation problem, which must be considered for the state of charge (SOC) estimation in the battery energy storage system (BESS). The performance of the proposed method is verified by an electromagnetic transient (EMT) simulation.
