An Fingerprint Authentication Model of ERM System using Private Key Escrow Management Server (개인키 위탁관리 서버를 이용한 전자의무기록 지문인증 모델)

  • Medical information is an important personal information for patients, and it must be protected. In particular, when medical personnel approach electronic medical records, authentication for enhanced security is essential. However, the existing public certificate-based certification model did not reflect the security characteristics of the electronic medical record(EMR) due to problems such as personal key management and authority delegation. In this study, we propose a fingerprint recognition-based authentication model with enhanced security to solve problems in the approach of the existing electronic medical record system. The proposed authentication model is an EMR system based on fingerprint recognition using PEMS (Private-key Escrow Management Server), which is applied with the private key commission protocol and the private key withdrawal protocol, enabling the problem of personal key management and authority delegation to be resolved at source. The performance experiment of the proposed certification model confirmed that the performance time was improved compared to the existing public certificate-based authentication, and the user's convenience was increased by recognizing fingerprints by replacing the electronic signature password.

  • This is a convergence study to provide an effective way of managing nurses' job stress by improving nurse satisfaction through an integrated identification of the effect satisfaction level on job stress for nurses using an electronic medical record(EMR) system. The data were collected from August 1 to September 1, 2014. The participants were 377 nurses from C university hospital of G metropolitan city were analyzed using SPSS 21.0. The result shows that job stress decreases as nurses' satisfaction level of using the EMR system increases. Thus, a practical job stress mediation program should be developed to continuously enhance nurses' satisfaction with the EMR system, which is expected to shorten the nursing record writing time and improve the quality of nursing service for patients. Furthermore, to improve the satisfaction with the EMR, appropriate management, such as continuous nursing education on the EMR system, will be needed.

  • Adverse drug reactions (ADRs) constitute key factors in determining successful medication therapy in clinical situations. Integrative analysis of electronic medical record (EMR) data and use of proper analytical tools are requisite to conduct retrospective surveillance of clinical decisions on medications. Thus, we suggest that electronic medical recording and human genetic databases are considered together in future directions of pharmacovigilance. We analyzed EMR-based ADR studies indexed on PubMed during the period from 2005 to 2017 and retrospectively acquired 1161 (29.6%) articles describing drug-induced adverse reactions (e.g., liver, kidney, nervous system, immune system, and inflammatory responses). Of them, only 102 (8.79%) articles contained useful information to detect or predict ADRs in the context of clinical medication alerts. Since insufficiency of EMR datasets and their improper analyses may provide false warnings on clinical decision, efforts should be made to overcome possible problems on data-mining, analysis, statistics, and standardization. Thus, we address the characteristics and limitations on retrospective EMR database studies in hospital settings. Since gene expression and genetic variations among individuals impact ADRs, pharmacokinetics, and pharmacodynamics, appropriate paths for pharmacovigilance may be optimized using suitable databases available in public domain (e.g., genome-wide association studies (GWAS), non-coding RNAs, microRNAs, proteomics, and genetic variations), novel targets, and biomarkers. These efforts with new validated biomarker analyses would be of help to repurpose clinical and translational research infrastructure and ultimately future personalized therapy considering ADRs.

  • 전자의무기록(Electronic Medical Record, EMR) 시스템은 기존에 수기로 작성하던 의무기록을 디지털화 한 것이다. 이는 다양한 장점이 있지만 의료인이 관리 도메인을 통해 환자의 정보를 세밀하게 수집할 수 있는 환자 개인의 프라이버시 침해 문제가 발생하게 된다. 즉, 관계자에 의해 의도적인 유출이 발생하거나 의료정보의 거래, 복제 등 위험성이 존재한다. 특히 일부 의료정보는 고용 차별이나 사회적 차별 등 환자에게 정신적 고통을 안겨줄 수 있다. 이러한 프라이버시 침해는 유전성 질환 유전자를 가진 사람에게 유전적 요인에 근거하는 고용 차별이 발생할 수 있다. 관련연구에서는 환자의 임상적(Clinical) 또는 유전적(Genomic) 정보가 자신의 신원과 연계되어 있다면 프라이버시 침해가 발생할 수 있음을 나타낸다. 이러한 프라이버시 문제로 인해 EMR 시스템에 기반을 둔 전자건강기록(Electronic Health Record, EHR) 시스템 또한 개인 프라이버시 침해의 위험이 존재하게 된다. 따라서 의료정보의 프라이버시 보호를 위해 부당한 고용 차별 보험 차별 사회적 차별로 연결될 수 있는 개인 의료정보의 유출방지, 타인에게 알려지고 싶지 않은 개인 의료정보가 무단으로 거래되지 않는 것을 보장해야 한다. 이를 위해 본 논문에서는 의무기록의 익명화를 통해 환자와 의무기록 간의 관계를 제거하는 여러 방법들을 소개한다.

  • In recent years, two major streams in medical information systems are:1) system integration among OCS(Order Communication System), EMR(Electronic Medical Record), PACS(Picture Archiving and Communication System), and ERP(Enterprise Resource Planning) and 2) system integration through medical collaboration between East and West medical service providers. One of the characteristics which differentiate the Korean medical industry from the western medical industry is the East-West medical collaboration. In many respects there are many differences between East and West medical treatment. Although East and West medical treatment have developed from different medical philosophies and standards, we assume that the better medical care can be provided by integrating their medical procedures effectively. The two possible approaches to the integration of East and West medical information systems are suggested in this paper:One is loosely coupled model and the other is tightly coupled model. EMR improves the quality of medical record which reflects the quality of clinical practice. It provides more efficient and convenient way of input, retrieval, storage, communication and management of medical data. We abstracted the standard medical procedures from the two medical procedures performed in Daejeon Oriental Hospital and Hehwa Clinic at Daejeon University and also abstracted database schema by analyzing the characteristics of information needed in East-West medical collaboration. Our EMR is composed of two types of data:one is structured data and the other is unstructured data, which are formalized by SOAP(Subjective, Objective, Assessment, Plan) format. Currently the integrated system is implemented and operated successfully for six months.

  • The pre-existing medical treatment was done in person between doctors and patients. EMR (Electronic Medical Record) System computerizing medical history of patients has been proceed and has raised concerns in terms of violation of human right for private information. Which integrates "Identification information" containing patients' personal details as well as "Medical records" such as the medical history of patients and computerizes all the records processed in hospital. Therefore, all medical information should be protected from misuse and abuse since it is very important for every patient. Particularly the right to privacy of medical record for each patient should be surely secured. Medical record means what doctors put down during the medical examination of patients. In this paper, we applies fingerprint identification to EMR system login to raise the quality of personal identification when user access to EMR System. The system implemented in this paper consists of embedded module to carry out fingerprint identification, web server and web site. Existing carries out it in client. And the confidence of hospital service is improved because login is forbidden without fingerprint identification success.

Status of Herbal-drug-associated Adverse Drug Reactions Voluntarily Reported by EMR (1개 대학 한방병원에서 EMR을 통해 보고된 한약에 의한 약물유해반응의 현황)

  • Objectives : The aim of this study was to systematically investigate herbal-drug-associated adverse drug reactions (herbal ADRs) reports submitted by a single oriental hospital and to analyze the general characteristics, causative agents, clinical manifestations, severity and types of herbal medicines which caused herbal ADRs. Methods : This study proceeded with IRB approval. The data on herbal ADR were collected prospectively from January 2008 to February 2012 by EMR of Dongguk University Ilsan Oriental Hospital. The World Health Organization (WHO)-Uppsala Monitoring Center (UMC) criteria was used to determinate causality for each herbal ADR. WHO-Adverse Reaction Terminology (WHO-ART) System Organ Class (SOC) code and WHO severity category were also used in this study. Results : A total of twenty eight cases were reported. Twenty two cases were assessed to have over possible relations with herbal medication. The gender ratio of these cases were 64.6 percent female and 36.4 percent male, demonstrating no statistical significance. Patients aged over 60 were 59.1%. Gastro-intestinal system was reported to be the most frequently affected organ (38.8%), and followed by psychiatric system (22.4%), and integumentary system (22.4%). The most common clinical symptom was headache (12.2%), followed by diarrhea (10.2%), and pruritus (10.2%). The severity of most cases was assessed to be mild (89.8%). The percentage of moderate ones was 10.2%, and there were no severe cases. Conclusions : Progressive study and further analysis on herbal ADRs are warranted for safety in the clinical use of herbal medicines.

An Automatic LOINC Mapping Framework for Standardization of Laboratory Codes in Medical Informatics (의료 정보 검사코드 표준화를 위한 LOINC 자동 매핑 프레임웍)

  • An electronic medical record (EMR) is the medical system that all the test are recorded as text data. However, domestic EMR systems have various forms of medical records. There are a lot of related works to standardize the laboratory codes as a LOINC (Logical Observation Identifiers Names and Code). However the existing researches resolve the problem manually. The manual process does not work when the size of data is enormous. The paper proposes a novel automatic LOINC mapping algorithm which uses indexing techniques and semantic similarity analysis of medical information. They use file system which is not proper to enormous medical data. We designed and implemented mapping algorithm for standardization laboratory codes in medical informatics compared with the existing researches that are only proposed algorithms. The automatic creation of searching words is being possible. Moreover, the paper implemented medical searching framework based on database system that is considered large size of medical data.

  • The aim of the research is that disease database of the glycosuria and heart's blood is designed to manage the condition of the glycosuria and heart's blood patients periodically and continuously. Also we integrate patient database of existing OCS, PACS, EMR, ERP etc. and support optimal service timely that the patients want through intelligent integrated interface environment. For this, we will develop customized medical information convergence system. We construct intelligent database for disease of the glycosuria and heart's blood. And we support data integration environment for connection with existing systems - OCS, EMR, PACS etc. Also, in consideration of QoS, reliability, and expandability of customized medical information convergence system, we will design H/W, S/W, and data compatibility method.

A Study on the Practice Model for Practical Education for Health and Medical Information Management (보건의료정보관리 실습교육을 위한 실습모델 연구)

  • In this study, a practical model for health information management education using the EMR education system at universities for nurturing health care information managers was studied. Currently, there is no practical training course for health care information management in the standards for evaluation and certification of health care information management education introduced to strengthen the job competency of health care information managers. Accordingly, the program was constructed so that the practice program suggested as an educational environment in the Health and Medical Information Management Education Evaluation and Certification Manual can be practiced in the EMR education system. In addition, a practical model that can be performed according to the on-site practice guidelines for health and medical information management for each program was studied. Using the health care information management education EMR system, master data management, patient registration, doctor prescription, medical cost calculation, health insurance claim management, form management, discharge registration, cancer registration, unrecorded management, health care data management, health care statistics, A practice model was studied so that practice on information protection/security management can be performed. It will be possible to play a role as a health care information management expert by raising the quality level of health care information management education through systematic and standardized health care information management practice courses at universities. Accordingly, it is necessary to cultivate health care information management experts who develop and manage medical services based on medical data analysis through practical training of health care information managers.
