휴대용 요역동학 검사 시 근전도 신호를 이용한 복압측정 방법 (A Non-invasive Measurement of Abdominal Pressure on Ambulatory Urodynamics Study Using Surface Electromyography)

  • 복압 측정을 위한 기존의 카데터 방법은 배뇨압에 영향을 줄 수 있는 많은 오차를 유발할 수 있으며 환자에게 불편함을 야기한다. 이러한 문제점을 해결하기 위해, 본 연구에서는 표면 근전도를 이용하여 비침습적으로 복압을 측정할 수 있는 새로운 방법을 개발하였다. 연구 결과, 기존 직장 카테터를 이용하여 측정된 복압과 복압측정 알고리즘을 이용하여 추정된 복압 간의 상관계수는 $0.79{\pm}0.06$이었으며, 두 신호간의 오차는 $0.10{\pm}0.07$이었다. 본 연구에서 개발된 복압추정 알고리즘을 이용하면 휴대용 요역동학 검사 시 편리하게 간접적으로 복압을 측정할 수 있을 것으로 생각된다.

근피로에 의한 하지 대퇴직근의 경직도 분석을 위한 근전도 변화에 관한 연구 (A Study on the Electromyography Change for Analysis of rectus femoris muscle stiffness with muscle fatigues)

  • 근경직으로 인한 질병을 판단하기 위한 새로운 비침습적 초음파 근경직도 측정 시스템을 검증하기 위해 근경직 발생시 근전도의 변화를 분석하였다. 근경직 유발을 위해 하지 대퇴직근에서 등척성 운동을 실시하였다. 실험 프로토콜에 따라 실험을 반복 실시하였다. 이때 바이오시스템3를 이용하여 무릎관절토크의 크기 및 토크를 실시간으로 측정하였다. 또한 대퇴직근에서 근전도 측정 시스템으로 근전도를 측정하였다. 피검자는 실험 프로토콜에 따라 최초 무릎관절 토크의 최대수의적 등척성수축(MVC : maximal voluntary contraction)을 측정하고,. MVC의 50%에서 -5%로 떨어질때까지 등척성 운동을 지속하였다. 근경직 유발시 근전도 신호를 신호처리 과정을 거쳐 중앙주파수변동과 실효값 변화로 나타내었으며, 이러한 결과가 근경직도 측정 시스템의 결과와 유의함을 보였다.

커프 신경전극을 위한 저잡음 증폭기 시스템 개발 (Development of a Low-Noise Amplifier System for Nerve Cuff Electrodes)

  • Cuff electrodes have a benefit for chronic electroneurogram(ENG) recording while minimizing nerve damage. However, the ENG signals are usually contaminated by electromyogram(EMG) activity from the surrounding muscle, the thermal noise generated within the source resistance, and the electric noise generated primarily at the first stage of the amplifier. This paper proposes a new cuff electrode to reduce the interference of EMG signals. An additional middle electrode was placed at the center of cuff electrode. As a result, the proposed cuff electrode achieved a higher signal-to-interference ratio compared to the conventional tripolar cuff. The cuff electrode was then assembled together with closure, headstage, and hermetic case including electronic circuits. This paper also presents a lownoise amplifier system to improve signal-to-noise ratio. The circuit was designed based on the noise analysis to minimize the electronic noise. The result shows that the total noise of the amplifier was below $1{\mu}V_{rms}$ for a cuff impedance of $1\;k{\Omega}$ and the common-mode rejection ratio was 115 dB at 1 kHz. In the current study, the performance of nerve cuff electrode system was evaluated by monitoring afferent nerve signals under mechanical stimuli in a rat animal model.

전침자극이 말의 위와 맹장의 운동성에 미치는 영향 (The effects of electroacupuncture on stomach and cecum motility in horses)

  • The effects of electroacupuncture(EA) on gastrointestinal motility were investigated in 6 horses. Three acupuncture points ; Guan Yuan Shu(BL-26), Wei Shu(BL-21) and Da Chang Shu(BL-25) were stimulated for 20 minutes by EA at separate occasions under varying condition ; 2V-1Hz, 2V-5Hz, 2V-30Hz, 4V-1Hz, 4V-5Hz and 4V-30Hz. Myoelectric activity of stomach and cecum was monitored to investigate the gastrointestinal motility. Electromyogram(EMG) recordings were carried out before, 0, 20 minutes after and 40 minutes after the EA stimulation. EMG bipolar electrode was surgically implanted in seromuscular layer of greater curvature in the stomach and between medial band and ventral band in the cecum. The EA stimulation and monitoring were not commenced until 15 days after electrode implantation. The EA stimulation of Wei Shu influenced on stomach motility and that of Da Chang Shu on, cecum motility. However, the EA stimulation of Guan Yuan Shu influenced on both the stomach and the cecum motility. The myoelectrical spike burst amplitude of the stomach and the cecum was significantly(p<0.05) increased by 2V-1Hz stimulation, but the myoelectrical spike burst frequence of the stomach and the cecum was significantly decreased by 2V-30Hz or 4V-30Hz stimulation. The myoelectrical spike burst duration of the stomach and the cecum was significantly lengthened by 4V-30Hz and 2V or 4V-30Hz stimulation, respectively.

CNN-LSTM 기반의 상지 재활운동 실시간 모니터링 시스템 (CNN-LSTM-based Upper Extremity Rehabilitation Exercise Real-time Monitoring System)

  • 재활환자는 수술 치료 후 신속한 사회복귀를 목적으로 신체적 기능 회복을 위하여 통원치료 및 일상에서 재활운동을 수행한다. 병원에서 전문 치료사의 도움으로 운동을 수행하는 것과 달리 일상에서 환자 스스로 재활운동을 수행하는 것은 많은 어려움이 있다. 본 논문에서는 일상에서 환자 스스로 효율적이고 올바른 자세로 재활운동을 수행할 수 있도록 CNN-LSTM 기반의 상지 재활운동 실시간 모니터링 시스템을 제안한다. 제안한 시스템은 EMG, IMU가 탑재된 어깨 착용형 하드웨어를 통해 생체신호를 계측하고 학습을 위한 전처리 과정과 정규화를 진행하여 학습 데이터세트로 사용하였다. 구현된 모델은 특징 검출을 위한 3개 합성곱 레이어 3개의 폴링 레이어, 분류를 위한 2개의 LSTM 레이어로 구성되어 있으며 검증 데이터에 대한 학습 결과 97.44%를 확인할 수 있었다. 이후 Teachable machine과의 비교평가를 진행하였으며 비교평가 결과 구현된 모델은 93.6%, Teachable machine은 94.4%로 두 모델이 유사한 분류 성능을 나타내는 것을 확인하였다.

수면 분석을 위한 다중 모달 생체신호 측정 시스템 (Multimodal Bio-signal Measurement System for Sleep Analysis)

  • In this paper, we designed a multimodal bio-signal measurement system to observe changes in the brain nervous system and vascular system during sleep. Changes in the nervous system and the cerebral blood flow system in the brain during sleep induce a unique correlation between the changes in the nervous system and the blood flow system. Therefore, it is necessary to simultaneously observe changes in the brain nervous system and changes in the blood flow system to observe the sleep state. To measure the change of the nervous system, EEG, EOG and EMG signal used for the sleep stage analysis were designed. We designed a system for measuring cerebral blood flow changes using functional near-infrared spectroscopy. Among the various imaging methods to measure blood flow and metabolism, it is easy to measure simultaneously with EEG signal and it can be easily designed for miniaturization of equipment. The sleep stage was analyzed by the measured data, and the change of the cerebral blood flow was confirmed by the change of the sleep stage.

물리치료 분야에서 인공지능 및 바이오센싱 기술의 현장적용 및 전망에 관한 연구: 맞춤형 재활치료를 중심으로 (A Study on the Field Application and Prospect of Artificial Intelligence and Bio-Sensing Technology in Physical Therapy: Focusing on Customized Rehabilitation Treatment)

  • PURPOSE: This study analyzed the impact of AI and biosensors on physical therapy, identifying the stage of customized technology development and future prospects. AI and biosensors improve the efficiency, establish customized treatment plans, and expand patient treatment opportunities. The study employed a literature review by searching databases and collecting research. METHODS: This study searched various databases related to the topic, collected existing research, papers, and reports, evaluated the literature, and summarize the results. RESULTS: Exercise therapy utilizing artificial intelligence can provide personalized and optimal exercise plans while monitoring rehabilitation progress. In addition, biosensors such as EMG sensors and accelerometers can monitor the individual progress in physical therapy, particularly in stroke patients, which can help improve physical therapy strategy and promote patient recovery. CONCLUSION: This study suggested that artificial intelligence can be applied in many areas of physical therapy, such as exercise therapy, customized treatment plans, rehabilitation and management, pain management, neuro rehabilitation, and auxiliary devices. Using AI technology, it is possible to analyze and improve exercise and posture, retrain the central nervous system, establish customized treatment plans for individual patients, predict and compare patient progress before and after treatment, and provide customized pain analysis and treatment methods. In addition, AI can provide neuro rehabilitation programs and customized auxiliary devices.

액체금속을 활용한 신축성 전자소재 개발 동향 (Study for the Liquid Metals Enabled Stretchable Electronics)

  • Stretchable and flexible electronics that comply with dynamic movements and micromotion of the human tissues can enable real-time monitoring of physiologic signals onto the human skin and in the brain, respectively. Especially, gallium based liquid metal stretchable electronics can offer human-interactive biosensors to monitor various physiologic parameters. However, the liquid-like nature, surface oxidation and contamination by organic materials, and low biostability of the liquid metals have still limited the long-term use as bioelectronics. Here we introduced electrochemical deposition without oxidation pathways to overcome these practical challenges in liquid metal bioelectronics. CNT/PDDA composite with reduction way and PEDOT:BF4 with oxidation way under organic solvent are suggested as rationally designed material engineering approaches. We confirmed that the structures with the soft, flexible, and stretchable liquid metal platform can successfully detect dopamine with a high sensitivity and selectivity, record neural signals including action potentials without scar formation, and monitor physiologic signals such as EMG and ECG.

소뇌교각 수술 중에 안면운동유발전위의 검사방법과 기능적 예측인자 (Facial Motor Evoked Potential Techniques and Functional Prediction during Cerebello-pontine Angle Surgery)

  • 다중펄스경두개전기자극(mpTES)을 이용한 안면운동유발전위(FMEP)는 자유 진행 근전도와 직접적인 안면 신경 자극법의 한계점을 보완하고 소뇌교각 종양 수술 중에 안면 신경의 기능적인 완전성을 예측할 수 있다. 본 논문의 목적은 이 검사의 표준화된 검사방법과 안면 신경의 기능예측인자로서의 유용성을 알아보고 수술 후 중대한 후유증인 안면마비 발생률을 최소화하는 것이다. Mz (음극)-M3, M4 (양극) 전극으로 경두개전기자극을 주고 안면운동유발전위의 단일펄스반응(SPR)의 부재와 10 ms이상의 잠복기를 확인해서 직접적인 두개 외 말초 안면 근육 자극을 배제하고 구륜근(orbicularis oris)과 턱근(mentalis)에서 동시에 측정하면 구륜근에서만 측정했을 때보다 안면운동유발전위의 정확도와 성공률을 높일 수 있다. 본 논문에서는 안면운동유발전위의 50% 진폭감소를 경고기준으로 해서 수술 직후 안면 신경의 결과를 효과적으로 예측할 수 있었다. 결론적으로, 소뇌교각 종양 수술 중에 FMEP는 자유 진행 근전도와 직접적인 안면 신경 자극법과 더불어서 수술 후 중대한 후유증인 안면 마비 발생률을 최소화 할 수 있는 유용한 검사방법이다.

척수 종양 제거 술 중 운동유발전위의 호전과 근력 호전의 관계 (Increased Intraoperative Motor Evoked Potentials and Motor Recovery after Spinal Cord Tumor Removal)

  • Objective: To evaluate whether the increase of the amplitude of motor evoked potentials (MEPs) during surgery can imply favorable prognosis postoperatively in spinal cord tumor surgery. Method: MEPs were monitored in patients who underwent spinal cord tumor surgery between March 2016 and March 2018. Amplitude changes at the end of monitoring compared to the baselines in limb muscle were analyzed. Minimum and maximum changes were set to $MEP_{min}$ (%) and $MEP_{max}$ (%). Strengths of bilateral 10 key muscles which were documented a day before ($Motor_{pre}$), 48 h ($Motor_{48h}$) and 4 weeks ($Motor_{4wk}$) after the surgery were reviewed. Results: Difference of $Motor_{48h}$ from $Motor_{pre}$ ($Motor_{48h-pre}$) and $Motor_{4wk}$ from $Motor_{pre}$ ($Motor_{4wk-pre}$) positively correlated with $MEP_{min}$, suggesting that smaller the difference of MEPs amplitude, less recovery of muscle strength. There was a negative correlation between the amount of bleeding and $MEP_{min}$, indicating that the greater the amount of bleeding, the smaller the $MEP_{min}$, implying that MEPs amplitude is less likely to improve when the amount of bleeding is large. It also showed significant difference between patients with improved or no change of motor status and patients with motor deterioration after surgery according to anatomical tumor types. Conclusion: Improve of muscle strength was less when the increase of MEPs amplitude was small, and improvement of MEPs amplitude was less when the amount of bleeding was large. Correlation between changes of status of muscle strength after surgery and tumor types was observed. With amplitude increase in MEPs monitoring, restoration of muscle strength can be expected.
