Exploring the Feasibility of Differentiating IEEE 802.15.4 Networks to Support Health-Care Systems

  • IEEE 802.15.4 networks are a feasible platform candidate for connecting all health-care-related equipment dispersed across a hospital room to collect critical time-sensitive data about patient health state, such as the heart rate and blood pressure. To meet the quality of service requirements of health-care systems, this paper proposes a multi-priority queue system that differentiates between various types of frames. The effect of the proposed system on the average delay and throughput is explored herein. By employing different contention window parameters, as in IEEE 802.11e, this multi-queue system prioritizes frames on the basis of priority classes. Performance under both saturated and unsaturated traffic conditions was evaluated using a novel analytical model that comprehensively integrates two legacy models for 802.15.4 and 802.11e. To improve the accuracy, our model also accommodates the transmission retries and deferment algorithms that significantly affect the performance of IEEE 802.15.4. The multi-queue scheme is predicted to separate the average delay and throughput of two different classes by up to 48.4% and 46%, respectively, without wasting bandwidth. These outcomes imply that the multi-queue system should be employed in health-care systems for prompt allocation of synchronous channels and faster delivery of urgent information. The simulation results validate these model's predictions with a maximum deviation of 7.6%.

  • To develop new health management system for the school children in korea, we have made a descriptive study in two phases. The first phase was to find out the situation and the problems of current school health management system. The second phase was to propose a new system based on the findings through literature review. We found that the health screening examination was carried out inefficiently in many aspects, i.e. procedure, items and methods of screening. We proposed a new health management system which was more efficient, less expensive, and more reliable.

  • Objectives : The Levator Ani Muscles Training(LAMT) is one of the well known very effective prevention and treatment method of some specific diseases in the anal organs, digestion system, urinary organs and genital organs, which means LAMT could be considered as an independent exercise. Methods : Chinese databases were searched and found one good source named CNKI(China National Knowledge Infrastructure) where there were downloaded 154 articles, out of which were finally selected 82 used for the introduction of the historical background, health-effectiveness, working mechanism, training methods, etc. Results : LAMT as an independent exercise, is considered very effective for the prevention of diseases in the anal organs, e.g. hemorrhoids, anal fissure, rectal prolapse; urinary organs, e.g. urinary incontinence, frequent micturition; digestion system, e.g. visceroptosis, diarrhea, chronic enteritis; genital organs, e.g. prostatitis, impotence, premature ejaculation, feminine vaginal relaxation, sexual indifference, etc. It is also told LAMT is effective for coronary heart disease, hyperyension and varicose veins of lower extremity to a certain degree. LAMT is also very similar to the Kegel exercise in couple of aspects. They are equally effective for the urinary incontinence and impotence. Conclusions : LAMT as an independent exercise has developed and become more and more health-effective, especially through the combination with the Qigong exercise, and thus many training methods have been introduced. There are not many scientific research and development with very limited accomplishments even in China. It is strongly required to strengthen the research and development activities so that LAMT will become one of the very effective natural healing soonest possible.

  • 응급상황은 일상생활의 어디에서나 잃어날 수 있다 응급상황의 발생을 감지하고 이에 대처하기 위한 e-health 시스템을 개발한다 생체신호를 측정하기 위하여 입는 형태의 Bio-Shirt를 제작한다. 측정된 신호는 블루투스 무선통신으로 PDA로 전송된다. PDA에서는 전송 받은 데이터를 분석하여 간단한 상황 판단 알고리즘을 실행한다. 또한 CDMA 모뎀을 통하여 웹 서버 형태로 구축된 ECMR로 사용자의 생체데이터를 전송한다. ECMR에서는 관리자가 사용자의 건강상태 및 응급상황 여부를 감시하고 위급상황으로 판단한 경우에는 구조센터나 담당의사 등과의 연락을 취한다. 프로토타입으로 개발한 e-health 시스템을 보인다.

Effect of Continuous Exposure to Reactive Oxygen Species on ${\gamma}$-Glutamyltranspeptidase Expression and Activity in HepG2 Cells (HepG2 세포에서 지속적인 활성 산소 노출이 ${\gamma}$-Glutamyltranspeptidase 발현과 활성에 미치는 영향)

  • The adverse health effects of a number of environment pollutions are related to the formation of free radicals. Induction of antioxidant defensive system in the response to an oxidative attack is an essential element of the cell to survive. CYP2E1 is easily induced by organic solvents and induces continuous formation of reactive oxygen species (ROS). ${\gamma}$-Glutamyltranspeptidase (${\gamma}$GT) plays an important role in glutathione metabolism and xenobiotic detoxification. To evaluate the characteristic of oxidative stress which induces GGT expression and to understand human antioxidant defensive response against oxidative stress induced by CYP2E1, we studied regulation of ${\gamma}$GT enzyme expression in response to various oxidative stresses in human HepG2 cells. The ${\gamma}$GT activity was not modified after exposure of acute oxidative stress inducing agents (ferric nitrilotriacetate, cumene hydroperoxide, ADP-Fe, O-tetradecanoylphorbol-13-acetate, tumor necrosis factor-alpha). To induce continuous exposure of cells to ROS, HepG2 cells were transfected by human CYP2E1 gene transiently. The CYP2E1 activity was verified with chlorzoxazone hydroxylation. Transfection of CYP2E1 showed continuous 60% increase in intracellular ROS and 240 % increase in microsomal ROS. CYP2E1 overexpressing cells showed increased ${\gamma}$GT activity (2.5-fold). The observed enhancement of ${\gamma}$GT activity correlated with a significant increase of ${\gamma}$GT mRNA (2.1-fold). Treatment with antioxidant strongly prevented the increase in ${\gamma}$GT activity. The CYP2E1 overexpression did not modify toxicity index and increased glutathione levels. These results show that continuous exposure of cells to ROS produced by CYP2E1 up-regulates ${\gamma}$GT; This may be one of the adaptive antioxidant responses of cells to oxidative insult. Present study also suggests that the induction of ${\gamma}$GT could be used as a marker of oxidative stress induced by exposure to organic solvents.

Developing A Medical Intelligence System in Medical Data Warehouse (의료 데이터 웨어하우스에서의 Medical Intelligence 시스템 개발)

  • This research discusses knowledge contents needed to build an OLAP system for medical sector, OLAP functionalities from past studies, and a medical intelligence system which is a kind of OLAP. The knowledge requirements which consist of nine contents and OLAP fundamental functionalities are applied to the system. Most past studies have focused on developing a medical data warehouse rather than OLAP. The medical intelligence system supplies health care providers (i.e., doctors, clinicians, researchers and nurses) and non-providers (i.e., managers and business analysts) with multidimensional OLAP functionalities. The system can be used to gain a deeper understanding of specific medical issues. In this research, we focus not on medical data warehouse, but on the technical challenges of designing and implementing an effective medical intelligence system for health care information. An architecture is applied to developing the medical intelligence system for a medical center in order to illustrate its practical usage. Six packages in the developed system are discussed in this research: Explorer, Analyzer, Reporter, Statistician, Visualizer, and Meta Administrator packages. Evaluation of the system and ongoing research directions conclude the research.

  • An individual's health data is very sensitive and private. Such data are usually stored on a private or community owned cloud, where access is not restricted to the owners of that cloud. Anyone within the cloud can access this data. This data may not be read only and multiple parties can make to it. Thus, any unauthorized modification of health-related data will lead to incorrect diagnosis and mistreatment. However, we cannot restrict semipublic access to this data. Existing security mechanisms in e-health systems are competent in dealing with the issues associated with these systems but only up to a certain extent. The indigenous technologies need to be complemented with current and future technologies. We have put forward a method to complement such technologies by incorporating the concept of blockchain to ensure the integrity of data as well as its provenance.

Blockchain-based Data Storage Security Architecture for e-Health Care Systems: A Case of Government of Tanzania Hospital Management Information System

  • Health information systems (HIS) are facing security challenges on data privacy and confidentiality. These challenges are based on centralized system architecture creating a target for malicious attacks. Blockchain technology has emerged as a trending technology with the potential to improve data security. Despite the effectiveness of this technology, still HIS are suffering from a lack of data privacy and confidentiality. This paper presents a blockchain-based data storage security architecture integrated with an e-Health care system to improve its security. The study employed a qualitative research method where data were collected using interviews and document analysis. Execute-order-validate Fabric's storage security architecture was implemented through private data collection, which is the combination of the actual private data stored in a private state, and a hash of that private data to guarantee data privacy. The key findings of this research show that data privacy and confidentiality are attained through a private data policy. Network peers are decentralized with blockchain only for hash storage to avoid storage challenges. Cost-effectiveness is achieved through data storage within a database of a Hyperledger Fabric. The overall performance of Fabric is higher than Ethereum. Ethereum's low performance is due to its execute-validate architecture which has high computation power with transaction inconsistencies. E-Health care system administrators should be trained and engaged with blockchain architectural designs for health data storage security. Health policymakers should be aware of blockchain technology and make use of the findings. The scientific contribution of this study is based on; cost-effectiveness of secured data storage, the use of hashes of network data stored in each node, and low energy consumption of Fabric leading to high performance.

Construction of e-Emergency Service System Infrastructure in Busan Metropolitan City (부산광역시 e-응급서비스시스템 인프라 구축)

  • The time taken for an ischemic heart disease patient to have a percutaneous coronary intervention because of acute myocardial infarction after arriving at the hospitals (door-to-balloon time) affects the patient's mortality significantly. To improve the emergency service system that has been previously used in the hospitals, this study focused on reducing door-data time and data-to-decision time among three time stages. The newly established e-emergency service system has set up the database of patients that had an emergency operation for acute myocardial infarction in the emergency service system of the hospital and has issued health cards for the patients that regularly visit the Busan National University Hospital. In addition, it has stored prior operation permits in the form of a certified electronic document. The new electronic system will reduce the complex treatment and operation procedures innovatively. Therefore, it is expected that this will make the life save (or the emergency patients easier and reduce the mortality. Moreover, it will also settle down the hospital staff's and patients' predicaments caused by the complex procedure of the legacy system.

Feasibility Study of a Custom-made Film for End-to-End Quality Assurance Test of Robotic Intensity Modulated Radiation Therapy System

  • This paper aims to verify the clinical feasibility of a custom-made film created by a laser cutting tool for End-to-End (E2E) quality assurance in robotic intensity modulated radiation therapy system. The custom-made film was fabricated from the Gafchromic EBT3 film with the size of $8^{{\prime}{\prime}}{\times}10^{{\prime}{\prime}}$ using a drawing that is identical to the shape and scale of the original E2E film. The drawing was created by using a computer aided design program with the image file, which is obtained by scanning original E2E film. Beam delivery and evaluations were respectively performed with the original film and the custom-made film using fixed-cone collimator on three tracking modes: 6D skull (6DS), Xsight spine (XS), and Xsight lung (XL). The differences between total targeting errors of the original and custom-made films were recorded as 0.17 mm, 0.3 mm, and 0.17 mm at 6DS, XS, and XL tracking modes, respectively. This indicates that the custom-made film could yield nearly equivalent results to those of the original E2E film, given the uncertainties caused by distortions during film scanning and vibrations associated with film cutting. By confirming the clinical feasibility of a custom-made film for E2E testing, it can be expected that economic efficiency of the testing will increase accordingly.
