Ulcerative colitis (UC) is a chronic idiopathic inflammatory bowel disease. Mesalizine for the first-line therapy of UC has adverse effects include pancreatitis, pneumonia and pericarditis. UC complicated by two coexisting conditions, however, is very rare. Moreover, drug-related pulmonary toxicity is particularly rare. An 11-year-old male patient was hospitalized for recurring upper abdominal pain after meals with vomiting, hematochezia and exertional dyspnea developing at 2 weeks of mesalizine therapy for UC. The serum level of lipase was elevated. Chest X-ray and thorax computed tomography showed interstitial pneumonitis. Mesalizine was discontinued and steroid therapy was initiated. Five days after admission, symptoms were resolved and mesalizine was resumed after a drop in amylase and lipase level. Symptoms returned the following day, however, accompanied by increased the serum levels of amylase and lipase. Mesalizine was discontinued again and recurring symptoms rapidly improved.
A female pug was presented with severe anemia, dyspnea and hemoglobinuria. By B- and M-mode echocardiography, the large number of heartworm were founded in the right ventricle, right atrium and pulmonary artery. The heartworm mass was moved from the right ventricle to the right atrium with the motion of the tricuspid value. Paradoxic septum motion were visible in M-mode of left ventrcular outflow tract and chordae tendineae level short-axis view. Flattened interventricular septum was find in B-mode of papillary muscle level short-axis view. In autopsy, 81 adult heartworms and 7 filarial debris were found. Most of adult heartworm were found in the right ventricle and pulmonary artery. 3 of them were found in the venaca and right atrium, respectively.
Methemoglobinemia is rare. It is classified into two types: congenital methemoglobinemia and acquired methemoglobinemia. Methemoglobin is incapable of binding oxygen, leading to complications such as cyanosis, dyspnea, headache, and heart failure. In the present case, a 35-year-old man with congenital methemoglobinemia underwent general anesthesia for thyroidectomy. The patient was diagnosed with hemoglobin M at 7 years of age. Ventilation was performed with FiO2 1.0. Arterial blood gas analysis showed that the pH was 7.4, PaO2 439 mmHg, PaCO2 40.5 mmHg, oxyhemoglobin level of 83.2%, and methemoglobin level of 15.5%. The patient had a stable course, although cyanosis was observed during surgery.

Background : The B-type natriuretic peptide (BNP) is a cardiac neurohormone that is specifically secreted from the ventricles in response to volume expansion and a pressure overload. Differentiating congestive heart failure from the pulmonary causes of dyspnea is very important for patients presenting with acute dyspnea. Methods : A retrospective study was carried out on 261 patients who were admitted to the emergency department of Hanyang University Hospital due to acute dyspnea from March to July 2004. The serum BNP levels of the patients were measured using the ELISA method. Results : The BNP levels were 382, 111-1140 pg/ml (median, interquartile range) in the heart failure group (n=119) and 29, 7-81 pg/ml in the non-heart failure group (n=142). The BNP levels according to the subgroups of heart failure were 820, 354-1620 pg/ml, 1650, 239-1990 pg/ml, and 378, 106-1120 pg/ml for the chronic obstructive pulmonary disease (COPD) with combined left heart failure (n=5), cor pulmonale (n=3), and left heart failure groups (n=111), respectively. The BNP levels according to the subgroups of non-heart failure were 39, 21-101 pg/ml, 59, 10-129 pg/ml, and 15, 6-47 pg/ml for the COPD (n=20), other pulmonary diseases (n=56), and other causes groups (n=66), respectively. The BNP levels were significantly different according to the underlying etiology (p<0.001), and were significantly higher in the COPD patients with left heart failure than in those without (p=0.002). When the patients with no cardiovascular risk factor such as diabetes mellitus, hypertension, smoking or renal failure were analyzed, the BNP levels were also significantly higher in the patients with heart failure than in those without(p<0.001). When 133 pg/ml was designated as the BNP cut-off level the sensitivity for predicting heart failure was 73 percent and the specificity was 87 percent. Conclusion : BNP measurements are useful for determining the presence of heart failure in the patients presenting with acute dyspnea. The possibility of heart failure must be seriously considered in patients with high BNP levels even when they have a pre-existing pulmonary disease such as COPD or no risk factors for heart failure.

Protein S deficiency is one of the several risk factors for thrombophilia and can cause blood clotting disorders such as deep vein thrombosis and pulmonary embolism. A 54-year-old man was admitted with the complaint of dyspnea and was diagnosed with pulmonary embolism. The patient had very low level of free protein S, total protein S antigen, and protein S activity (type I protein S deficiency). In history taking, we found that his mother, 78 year old, had a history of same disease 10 years ago, and confirmed the pronounced low level of protein S. The patient's son also had very low level of protein S, however there had not been any history of pulmonary embolism yet. This case study suggests that asymptomatic persons with a family history of protein S deficiency and pulmonary embolism should be checked regularly for early detection of the disease, as protein S deficiency can be suspected.
Rupture of a bronchus is an unusual result of nonpenetrating trauma to the chest. This case was a 19 year old male who was a worker in the mine. The patient had sustained a compression chest injury without external wound or rib fracture. At five days after trauma, he was suffered from dyspnea and the symptom was progressively increased. Two weeks after the chest trauma, a diagnosis of left main bronchial obstruction due to traumatic bronchial rupture was made by means of bronchoscopy and bronchography. He was operated upon. The left lung was completely atelectatic and the left main bronchus was found to be transected at the level of 3cm from carina. End-to-end anastomosis of the bronchus was performed and the left lung was aerated very well. postoperative course was uneventful.

In order to evaluate a health effect of air pollution, we designed exposure group (taxi driver, street sweeper, street trader,) and non -exposure group (office clerk). We analysed exposure and biologic marker by using personal sampler. Mean NO$_2$ and benzene level in each group were statistically significant. Also, respiratory symptom, chronic cough, sputum, and dyspnea on exertion were statistically significant in each group.
This 51 years old male patient was admitted to the department of thoracic and cardiovascular surgery via OPD because of anterior chest pain. 7days before admission, He got the chest trauma after traffic accidents,the lateral chest roentgenogram showed complete transverse sternal fracture. He also complained of mild dyspnea. We also noticed that he had depressed anterior chest wall. It looks like funnel chest. The operative findings revealed dislocated & callus formations at the both 4th and 5th costochondral junction and transverse fracture of sternal body between 4th and 5th costochondral junction, the upper end of sternal fracture was situated below the lower end of sternal fracture. The two ends of sternal fracture were situated at the same level and reapproximated the two ends by two-interrupted wire sutures. The patient is well on the road to recovery after the operation.
A 73-year-old non-small cell lung cancer (NSCLC) patient admitted due to cough, sputum, and dyspnea, aggravated a week ago. She was diagnosed as pneumonia based on the assessment of inflammation markers, chest X-ray and sputum culture. Computed tomography (CT) scan was conducted to exclude malignant tumor metastasis. At the initiation of treatment, considering underlying disease and inflammation marker level, herbal medicine and antibiotics were concurrently used and antibiotics had been discontinued after 10days. Using the monotherapy of herbal medicine in the next 6 days, chest X-ray showed remarkably decreased infiltration in right middle lung and right lower lung. This case represented additional improvement of chest X-ray when treated only with herb medicine after termination of antibiotic therapy and demonstrated the possibility of applying herbal medicine in patients with limited use of antibiotics.
Purpose: This study was conducted to determine the level of medical care required for mass-gatherings and to describe the types of illness and injury that may occur during demonstrations. Methods: We conducted a retrospective review of the medical records for patients injured during demonstrations. Results: From May to August at 2008, a total of 932,000 participants attended demonstrations. Most patients were occurred from June to July, a total of 126 patients were evaluated and treated at the emergency center of our hospital. The mean patient age was $31.8{\pm}9.8$ years, and men predominated over women. The vast majority of patients were experienced trauma (88.9%). The diagnostic categories were contusion (49.2%), laceration (20.6%), fracture (6.3%), syncope/dizziness (5.5%), ocular injuries (3.9%), dyspnea (3.9%), other trauma (3.1%), and abdominal complaints (1.5%). Two patients were admitted. Conclusion: The rate and the acuity of patients seen at these demonstrations was low. Nevertheless, a full on-site physician and transportation system is recommended during similar incidents.
