한방병원에 내원한 월경통 환자의 한의진료 실태 조사 (An Analysis of the Current Status of dysmenorrhea patients visiting Korean Medicine Hospital)

  • Objectives: This review aimed to present the characteristics of dysmenorrhea patients visiting a Korean Medicine hospital. Methods: One hundred thirteen dysmenorrhea patients visiting a Korean Medicine hospital from 2017 to 2021 were studied. We evaluated the general characteristics, type of dysmenorrhea, menstrual characteristics and overall therapies. Results: The average age of the 113 subjects was 29.4 years old, with 38.1% in their 20s and 25.7% in their 30s. The average pain intensity was NRS 7.5, and the menstrual pain severity of 79.6% of the subjects was grade 2 or 3. The types of dysmenorrhea were classified as primary dysmenorrhea in 62.0% and secondary dysmenorrhea in 38.1%. The average treatment period of the patients was 1-2 months in most cases (44.25%). The most common treatment method was herbal medicine. Conclusions: We figured out the status of dysmenorrhea patients and treatment in a Korean medicine hospital.

도시지역 일부 여자고등학생들의 월경통과 관련요인 (Dysmenorrhea and Its Related Factors among Women's High School Srudents in an Urban Area)

  • Purpose: The present study intended to reveal the association between the physique of adolescent women and dysmenorrhea, and the factors related to the frequency of dysmenorrhea. Methods: The study subjects included 511 girls of a women's high school in Daejeon city and they were, during the period of March 1st through April 30th, 2005, given self-administered questionnaires about frequency of menstruation, age, physique, exercise, and such menstruation-related items as age of menarche, menstrual period and days, and amount. Results: As with the frequency of dysmenorrhea, 38.4% experienced it 「always」, 44.6% 「occasionally」, and 17.0% 「almost scarcely」, and those with dysmenorrhea accounted for 83.0% of the subjects. The frequency of dysmenorrhea was not significant difference according to the BMI and HPI. However, the lower the age of menarche and the greater the amount of menstrual flow, the more frequent dysmenorrhea. The symptoms of dysmenorrhea included the highest frequency of low back pain(68.5%) and it was followed by abdominal pain(65.9%), sensibility(54.0%), fatigue(51.7%), and nervousness(49.5%). These accompanied symptoms tended to be worse as the frequency of dysmenorrhea increased. The multiple logistic regression analysis revealed that the relative risk of frequency of dysmenorrhea is 2.2 times in age of 14, by a standard age of 15 as menarchial age, 2.2 in 13, and 3.1 in below 12. The menstrual days was 3.0 times in both below 2 days and over 8 days with the standard days of 3-7 days. The menstrual amount was 1.9 times in the group with a greater amount of menstrual bleeding and 1.5 times in the group with a less amount than the normal group, respectively. Conclusion: The above results suggest that the rate of experiencing dysmenorrhea was up to 83.0% and 38.4% from these had dysmenorrhea upon every menstruation. It is thought that dysmenorrhea could be a great disadvantage sufficient to impair optimal health to a larger body of school girls. Further, for quality of life, it is required that more fundamental strategies instead of pain killers or others for palliation of dysmenorrhea would be established among adolescents.

STAXI-K를 이용한 원발성 월경통 양상과 분노 감정 표현 정도와의 상관성 고찰 (Comparative Study of Primary Dysmenorrhea and Anger Expression by STAXI-K)

  • Purpose: This study was conducted to investigate the relationship between primary dysmenorrhea. Methods: Intensity of dysmenorrhea was evaluated by scoring system of Andersch and Milsom. Anger state, trait, expression of the patients suffering from dysmenorrhea was measured by STAXI-K. And each value was analyzed by ANOVA testm as to intensity of dysmenorrhea, student t test as to existence of dysmenorrhea. Results: The numerical value of anger expression in the primary dysmenorrhea existent group was significantly higher than that in the primary dysmenorrhea non-existent group. There were significant differences among the numerical value of anger expression in the groups classified as intensity of primary dysmenorrhea, the value of three positive group() was significantly higher than that of negative group(-). Conclusion: This study showed the relationship between anger expression and primary dysmenorrhea. But there was no clue to find out their causal relationship, because this study was cross-sectional. So further study will be needed to prove the causality between them. And more precise scale has to been developed to evaluate various type of anger accurately.

여대생의 월경특성, 스트레스, 불안 및 생활습관자세가 월경통에 미치는 영향 (Factors affecting Dysmenorrhea in Undergraduate Students)

  • Purpose: The purpose of this study was to investigate dysmenorrhea in undergraduate students and its affecting factors. Method: The survey was conducted with 453 students at 4 nursing school in Seoul, Incheon, Chungcheongbuk and Jeollabuk Province. Data were collected from September to December 2013, and analyzed with SPSS Windows 21.0. Results: In a final model of hierarchial multiple regression, menarcheal age (${\beta}$=-0.13, p=.003), menstrual amount (${\beta}$=0.17, p=.001), stress (${\beta}$=0.18, p=.010) habit of sitting by crossing one's legs (${\beta}$=0.14, p=.003) were associated with dysmenorrhea in undergraduate students. These factors attributed to 12.0% of variance in the dysmenorrhea of undergraduate students. Conclusion: These results suggest that dysmenorrhea in undergraduate student could be influenced by habit of sitting by crossing one's legs as well as menstrual history and stress. Ultimately, intervention program including correct of cross-legged sitting postures can be effective for undergraduate students to alleviate dysmenorrhea.

원발성 생리통에 대한 계지복령환(桂枝茯笭丸) 단독치료와 침(鍼) 치료 병행 차이 연구 (A Pilot Study of the Difference between Gyejibongnyeong-hwan and Gyejibongnyeong-hwan combined Acupuncture Therapy on the Primary Dysmenorrhea)

  • Purpose : The purpose of this study is to identify the difference of clinical effects of gyejibongnyeong-hwan and acupuncture therapy on primary dysmenorrhea. Methods : We studied with ten patients recruited from April, 2003 to Feburary, 2004. Women with organic disease as uterine myoma, ovarian cysts and pelvic inflammatory disease were excluded from this study. We treated them with gyejibongnyeong-hwan or gyejibongnyeong-hwan combined acupuncture therapy for 8weeks. The severity of dysmenorrhea was measured by VAS(Visual Analog Scale). Results : gyejibongnyeong-hwan significantly decreased the severity of dysmenorrhea. And Gyejibongnyeong-hwan combined Acupuncture therapy significantly decreased the severity of dysmenorrhea. The change of VAS of gyejibongnyeong-hwan group is significantly higher than gyejibongnyeong-hwan combined acupuncture. Both of Gyejibongnyeong-hwan and acupuncture did not show hepatic and renal virulence. Conclusion : This study shows that gyejibongnyeong-hwan has remarkable effects on dysmenorrhea patients. Obviously further researches concerning all these area still necessary.

중.고등학교 여학생의 월경 특성과 월경곤란증에 관한 연구 (A Study on Middle and High School Girl Students' Menstruation Characteristics and their Menstruation Dysmenorrhea)

  • Middle and high school girl students' menstruation characteristics, appearance of menstruation dysmenorrhea, and coping with the menstruation dysmenorrhea were examined in order to give information in instructing girl students about menstruation dysmenorrhea, in delivering health education in the school, and establishing health policy. 970 girl students in middle and high schools in Cheju-do were selected and surveyed using the questionnaire the author developed based on the previous studies. The findings are as follows. First, the age of first menstruation was average $12.9{\pm}1.22$ years. The younger they were, the earlier their first menstruation came. The most common menstrual cycle was such one that continuously irregular from the beginning or that regular and irregular cycle run together. The first irregular menstrual cycle changed regular one within 1 or 2 years. Most of the respondents had normal menstrual cycle of $21{\sim}35$ days and menstruation period of $3{\sim}8$ days. Second, symptom of menstruation dysmenorrhea was orderly; hurt in the underbelly, tired, hurt in the waist, and headache. It was higher with high school students than middle school students. For the most part, time of heavy menstruation dysmenorrhea was the 2nd day after menstruation starts and period of menstruation dysmenorrhea was 2 days. More than the half of the respondents felt that the present menstruation dysmenorrhea was similar with the first one and they felt not comport in daily life due to menstruation dysmenorrhea, and 'hard to live a normal life' was higher with the high school girl students than with the middle school students. Third, how to cope with menstruation dysmenorrhea was enough sleep, relax and rest, taking a hot water shower, listening to music or watching a movie, taking medicine, and doing excercise. Few of the respondents have ever visited a doctor due to menstruation dysmenorrhea, and the reason was mostly irregular menstruation and heavy dysmenorrhea. Forth, when they took medicine due to menstruation dysmenorrhea, they gathered information on the medicine from family members and friends, while extremely low from a pharmacist or a doctor. The most of them have ever took medicine during menstruation once and took standard dose. While excess dosage was higher with high school students than with middle school students. From these findings, a good many girls are experiencing menstruation dysmenorrhea since their first menstruation. To discover early the severely abnormal findings and to treat, more interest from home and school, counseling and sexual education on time, and expert's diagnosis are necessary. Rather than unconditionally taking pain-relievers which can plainly solve the pain, continuous health education at home and in the school should be carried on for the girls to practice proper measures one think effective or an expert recommends.

A Study on Abdominal Temperature of Dysmenorrhea Patients

  • Dysmenorrhea is one of the common gynecologic disorders of menstruation women. Primary dysmenorrhea is menstrual pain without pelvic pathology. whereas secondary one is painful mestruation with underlying pathology. The cause of primary dysmenorrhea is increased endometrial prostaglandin. The mechanisims underlying secondary dysmenorrhea are not eluciated. There are many blood vessels under the skin and they play a very important role in the thermal control of peripheral part. The control of blood circulation is mainly controlled by autonomic nervous system and it is known that D.I.T.I. (digital infra-red thermographic imaging) is an objective method showing the body temperature. I observed the abdominal themerature of 49 patients complaning of dysmenorrhea who visited the gynecologic department of Pundang Oriental Medical Hospital during 1997. In order to rule out thermal abnormality due to obesity. the causes whose obesity index were above I. were excluded in this study. The mean temperature of lower abdomen of the dysmenorrhea group was $35.22{\pm}1.33^{\circ}C$ and control group was $36.01{\pm}0.74^{\circ}C$.

청소년의 월경곤란증과 영향 요인 (Factors Affecting Dysmenorrhea among Adolescents)

  • Purpose: This research was to determine the characteristics of dysmenorrhea among girls, identify the factors affecting dysmenorrhea. Method: The study subjects were 1653 high school student in Busan Metropolitan city. The data were collected by a self-administered questionnaire between November 1 and November 30, 2002. Descriptive statistics, t-test, one-way ANOVA with Scheffe' test, and stepwise multiple regression analysis in SPSS program were used for data analysis. Result: Students suffering from slight dysmenorrhea made up the majority 52.9%, whereas those with serious cases represented 29.8%. Regarding the measures taken to cope with dysmenorrhea, most of the respondents took analgesics 52.8%. A high points of dysmenorrhea was associated with irregular meal habits(p<0.01), unbalanced diet(p<0.05), stress(p<0.01), and health awareness(p<0.01), first menstruation before they reached 13 (p<0.01), heavy bleeding(p<0.01), family history(p<0.01). Factors affecting the dysmennorrhea of the respondents included the extent of bleeding, family history, health awareness, age during the first menstruation, and degree of stress, accounting for 13.1% of the total variance. Conclusion: To manage dysmenorrhea among girls, the results of the research indicate that students need to have proper awareness of health and manage stress through health education. Likewise, they need to be given positive follow-up management through a holistic nursing approach.

소양인, 소음인 월경통 환자의 체표온도 비교 (A study on the thermography of dysmenorrhea patients(Soyangin, Soeumin))

  • Dysmenorrhea is one of the most common gynecologic disorders in menstruating women. Dysmenorrhea refers to the pain appearing in the lower abdomen and lower back before. after or during menstruation. In oriental medicine. the causes of dysmenorrhea have been recognized as something penetrated such as qi. blood. and pathologic factors. The purpose of this study was to observe the difference in the temperature between the Soyangin dysmenorrhea patients group and the Soeumin dysmenorrhea patients group by thermography. We observed the 55 dysmenorrhea patients that visited ** Oriental Medicine center from July 19 to August 27. 2004. We diagnosed the constitution by QSCCII and the body temperature was assessed by DITI. Thermographic measurements were performed on 3 areas(Chon-jung(CV17), Chung-wan (CV12), Kwan-won(CV4)). In this study we observed that the temperature of Chon-jung(CV17) is the highest and Chung-wan (CV12) is higher than Kwan-won(CV4). The temperature of Soeumin group is higher than that of Soyangin group in all areas and there is a significant difference in 3 area temperature between Soeumin group and Soyangin group.

현부리경탕가감(玄附理經湯加減)의 기체혈어형(氣滯血瘀型) 월경통(月經痛)에 미치는 효과에 대한 임상적 고찰 (Clinical efficacy of Hyunburikyungtang-gagam for dysmenorrhea caused qi-stagnation and blood clots)

  • Purpose This study was designed to know how Hyunburikyungtang-gagam effects on dysmenorrhea Method 47 patients visited Oriental Gynecology in Conmaul Oriental Hospital from Jan, 2001 to Jan. 2002 because of their dysmenorrhea were randomly assigned. Hyunbruikyungtang-gagam were prescribed to the patients. Primary and secondary dysmenorrhea change were studied by VASS, MVRS. Written down by the patients before and after the treatment. Results Their ages were $28.62{\pm}4.9$, 18 patients were married and the others were not married. prescription period were $20.59{\pm}0.97$ days, After the treatment, numerical index for the pain was obviously reduced in both primary and secondary dysmenorrhea. Numerical index reduced $5.43{\pm}1.63$(VASS), $3.04{\pm}1.58$(MVRS) in primary dysmenorrhea, and $5.43{\pm}1.63$(VASS), $3.04{\pm}1.58$(MVRS) in secondary dysmenorrhea and there is no difference in effect between primary and secondary dysmenorrhea. But prescribed period was not correration with treatment effects and Pain-reduced period. Conclusion Hyunburikyungtang-gagam has good efficacy on primary and secondary dysmenorrhea, but some more studies are needed on the relationship between treatment effects and Pain-reduced period.
