Immediate effects of single-leg stance exercise on dynamic balance, weight bearing and gait cycle in stroke patients

  • Objective: This study aimed to identify how various applications of weight bearing on the affected side of hemiplegia patients affect the ability of balance keeping of the affected leg and the gait parameters. Design: Cross-sectional study. Methods: Eighteen patients with hemiplegia participated in this study. There were twelve males and six females. This study investigated the effects of the single-leg stance exercise on dynamic balance, weight bearing, and gait ability compared with four conditions. Dynamic balance and weight bearing were measured using the step test (ST) of the affected side in stroke patients. In addition, gait parameters were measured using the optogait system for analysis of the spatial and temporal parameters of walking in stroke patients. Results: This study investigated the effect of the single leg stance exercise on the paralysis side. The ST showed significant findings for all conditions (p<0.05). Therefore, knee extension and flexion exercise on the affected side single-leg stance (condition 4) significantly improved dynamic balance and weight bearing on the affected side (p<0.05). In the condition of moving the knee joint in a single-leg stance was discovered that the stance phase time significantly increased more than in the condition of supporting the maximal voluntary weight on the affected side (p<0.05). Conclusions: Single-leg stance on the paralysis side with knee flexion and extension increased symmetry in weight bearing during stance phase time. This study suggests that single-leg stance exercises augments improved gait function through sufficient weight bearing in the stance phase of the affected side.

  • Purpose: This study aims to investigate the immediate effects of electromyography (EMG) biofeedback training of the gluteus medius on dynamic balance during single leg squats in healthy individuals. Methods: The sample size in this study was estimated using the G-power program at an effect size of 0.4, a significance level (α) of 0.05, and a testing power of 0.90. In addition, as a result of considering the 10% dropout rate, this study recruited 21 healthy individuals (8 males and 13 females). All subjects measured the Y-balance test-lower quarter (YBT-LQ) and limits of stability (LOS) before and after a single leg squat (SLS) and SLS with EMG biofeedback training of the gluteus medius (SLSEB). They were trained for 10 minutes for each exercise, and two dynamic balance tests were performed three times. Results: There was a significant difference in the YBT-LQ score between the two exercises (p < 0.05). In the YBT-LQ score, there was a significant difference before and after SLS and SLSEB (p < 0.05). SLSEB showed a significantly higher YBT-LQ score than SLS (p < 0.05). There was a significant difference in LOM between the two exercises (p < 0.05). However, there was no significant difference between the two exercises. Conclusion: A single-leg squat with EMG biofeedback exercises is an effective method to improve dynamic balance, such as the YBT-LQ.

뇌졸중 환자의 낙상 예측을 위한 Four Square Step Test의 활용에 관한 연구 (A Study of the Clinical Utility of the Four Square Step Test for Predicting Falls in Stroke Patients)

  • Purpose: This study was designed to evaluate the clinical utility of the Four square step test (FSST) for predicting falls in stroke patients, to compare the ability of the FSST test to discriminate between subgroups of fallers, and to determine if the test has any predictive value in identifying stroke patients who will fall. Methods: Stroke patients (N=37) who could walk at least 50 m with minimal assistance were recruited consecutively when attending a physical therapy session during their rehabilitation. Dynamic standing balance was measured using the FSST. The main outcome measures were FSST time and fall number. Numbers of falls were compared with FSST scores. Differences between the groups in FSST scores were examined using a t-test and 1-way analysis of variance. Post hoc analysis using the Tukey B procedure was used to identify specific group differences. Alpha was set at 0.05. Results: A total of 15 participants (40.5%) reported falls 6 had recurrent falls (2 falls) and 9 fell once. The mean FSST time differed significantly between groups with zero and multiple falls. A cutoff score of greater than 17 seconds on the FSST was associated with a sensitivity of 83% and a specificity of 84% for the identification of subjects with multiple risk factors for falls. Conclusion: The FSST is a feasible and valid clinical test of dynamic standing balance and can predict falls in post-stroke patients.

근전도-생체되먹임 훈련이 무릎관절 전치환술 환자의 하지 근활성도와 균형에 미치는 영향 (Effects of EMG-biofeedback Training on Total Knee Replacement Patients' Lower Extremity Muscle Activity and Balance)

  • Purpose: The purpose of the current study was to examine the effects of electromyography (EMG)-biofeedback training on lower extremity muscle activity and balance of patients with total knee replacement (TKR). Methods: Subjects were randomly allocated to two groups: experimental and control group. Subjects in the experimental group (n=10) were provided with quadriceps setting exercise by EMG-biofeedback (QSE+BF) and those in the control group were provided with QSE. Subjects in both groups were provided with the respective training programs for 20 minutes per session, five times per week, for a period of six weeks. To test significance, data analysis was performed using repeated-ANOVAs. Results: Statistically significant differences in muscle activity of the rectus femoris muscle and the vastus lateralis, and dynamic balance ability were observed in the experimental group, compared with the control group. In comparison of the muscle activity of the rectus femoris muscle and the vastus lateralis, and dynamic balance ability between different training periods within the groups, both groups showed statistically significant differences. Conclusion: EMG-biofeedback training is effective in improving lower extremity muscle activity and balance ability of patients with TKR, and should be effective in patients with other diseases.

The Effect of a Kettlebell Complex Program according to the Support Surface on Lower Extremity Muscle Activity and Balance in Baseball Players

  • Purpose: The purpose of this study was to investigate the effect of a kettlebell complex program according to the support surface on the lower extremity muscle activity and balance of baseball players. Methods: The participants were divided into two groups; unstable support surface group 1 (11 people) performed the kettlebell complex program on an unstable support surface, and stable support surface group 2 (10 people) performed the kettlebell complex program on a stable support surface. Muscle activity was measured by surface electromyography. Dynamic balance was measured with a balance-measuring equipment. A paired t-test was used to compare groups before and after the experiment. An independent t-test was performed to determine the difference in the degree of change between the two groups before and after the experiment. Results: The intragroup comparison between stable support surface group 1 and 2 showed significant differences in muscle activity and sense of balance. In the comparison between the groups, the difference in muscle activity in unstable support surface group1 was significant in the biceps femoris and rectus femoris muscles, and significant differences were also found in the sense of balance. Conclusion: These results suggest that a kettlebell exercise on an unstable support surface is more effective in improving muscle activity and sense of balance than a kettlebell exercise on a stable support surface.

The association between pain, balance, fall, and disability in patients with lumbar spinal stenosis with vascular claudication

  • Background: The effect of lumbar spinal stenosis (LSS) and peripheral vascular disease (PVD), which occurs with similar degenerative conditions, when seen together, has not been studied. The aim of this study is to examine and compare the relationship between pain, balance, disability, fear of falling, and kinesiophobia in LSS patients with intermittent vascular claudication (IVC). Methods: Seventy-two patients diagnosed with LSS using magnetic resonance imaging participated in this study. Thirty-five patients with IVC symptoms and showing vascular lesions by lower extremity venous and arterial Doppler ultrasonography imaging were included in the IVC-LSS group. The pain, static balance, dynamic balance, disability, fear of falling, and kinesiophobia were evaluated using the numeric rating scale, single leg stance test, Time Up and Go (TUG), the Oswestry Disability Index (ODI), Fall Efficacy Scale-International (FES-I), and Tampa Scale for Kinesiophobia (TSK), respectively. Results: Age and female sex were found to be higher in the IVC-LSS group (P = 0.024; P = 0.012). The IVC-LSS group had a shorter single leg stance time and TUG test duration, pain intensity, ODI, FES-I, and TSK scores were higher than patients with LSS (P = 0.001). Pain, fear of falling, and kinesiophobia were moderately correlated with disability in the IVC-LSS group. No relationship was found between pain and dynamic balance. Also, the pain was not related to kinesiophobia. Conclusions: The findings indicated that IVC causes loss of balance and an increase in pain, disability, fear of falling, and kinesophobia in patients with LSS.

미사일 모델의 피치 동안정미계수 측정을 위한 강제진동기법의 적용 (Application of Forced Oscillation Technique for Pitch Dynamic Stability Derivatives of a Missile Model)

  • This paper presents an application of forced oscillation technique to measure pitch dynamic stability derivatives of a missile model in the low speed wind tunnel. The missile model is oscillated by D.C. electric servomotor with constant amplitudes and frequencies. Phase shift is determined as the difference of peak values between input and output signals from the dynamic stability balance installed at the center of gravity of the model. Stability derivatives were calculated by using phase shifts, amplitudes, forcing moments and input frequencies. Test results show the proper usage of the force oscillation technique with good damping effects.

  • Purpose - The aim of this study is to investigate the benefit-sharing activities. They affect contract renewal intention between franchisers and franchisees that are linked through a contract with the intention of building their relationship from the level based on dynamic trust. This study will show a new approach on qualitative growth and consolidation of competitive power and may well be able to suggest practical ways to strengthen directions in the field of franchise industries. Research design, data, and methodology - In this study, based on literature review, we separated logistical-supporting activities and non-logistical-supporting activities from benefit-sharing activities. We modeled how benefit-sharing activities influence contract renewal intention of franchisees by dynamic trust. In particular, considering the possibility of trading concepts and characteristics of trust between providers opportunistic behaviors was defined as variables, depending on the situation and dynamic characters reveal their relationship trust concept. That is, dynamic trust having the possibility of changing depending on opportunistic risks and interdependencies by Lewis & Weigert(1985, 2012) was classified into transitional-based trust, calculated based trust, relational-based trust and balance-based on trust. The total of 104 samples from domestic franchisees being with franchisees were collected by using a structural composed questionnaire. Results - The results are as following. Benefit-sharing activities in logistical supporting activities appeared to affect the multidimensional concept of transitional trust, calculated based on trust, relationship-based trust and balance based on trust dynamic trust with all-defined impact. Non-logistical supporting activities appeared to affect significantly the positive (+) to the dynamic trust concept except transitional trust. Lewis & Weigert(1985; 2012) have proposed a dynamic confidence calculated from relations within-based trust relationship based on trust appears to affect the significant positive (+) to the balance-based confidence transactions confidence mechanism of the system side in the verified relationship. Finally, a relationship based on trust and confidence in the balance based on the Influence of dynamic renewed confidence showed significantly affect the positive(+). In particular, the balance based trust showed the acts as a mediating factors between relational trust and confidence and renew calculated. Conclusion - From the above these results, the significance from this study is as following. First, the proposed activity is to share and validate the approach by identifying the potential in the relationship from the contract with building dynamic trust, and one of the ways from the franchise headquarters and affiliated merchants mutual growth through win-win cooperation in sharing activities. Next, the dynamic segmentation confidence to suggest the possibility can be broken down over the validity and reliability test for each concept. In addition, by identifying the relationship between the dynamic level of confidence granular multi-dimensional concept, to determine the level of trust with a dynamic relationship between risk and interdependencies can be possible in franchise industries. In order to maintain and develop their mutual relationship, franchise industries should propose confidence-building steps to promote their quality improvement.

편마비 환자의 트레드밀과 체중지지의 트레드밀 훈련이 균형능력 및 보행능력에 미치는 영향 (The Effect of Treadmill and Body Weight Support Treadmill Training on Balance and Gait Ability in Hemiplegia Patients)

  • Purpose: The purpose of this study is to investigate and to verification of changes that the effect of treadmill and body weight support treadmill training on balance and gait ability for sub-acute stroke patients during 4 week. Method: 16 subjects who was diagnosed stroke were divided into 2 groups(8-treadmill training group, 8-body weight support treadmill group) by randomized control trial. Both training programs were consisted with 40 minuted, 5 times a week for 4 weeks and after training programs. Analysis: We analysed effects and changes on balance and gait ability. Analyses were performed using PASW ver. 18.0 and results were reported as mean ${\pm}$ standard deviation (S.D.). To investigate within group comparisons and to verification on effects of exercise, we did paired t test and repeated measured ANOVA test. Significance was set at p<.05. Result: Both training programs showed positive changes in Limit of Stability but significant results which is area of hemiside, area of intact side, area of posterior, total area were reported in body weight support treadmill training group. Changes of Berg Balance Scale was significantly increase and it had significant correlation between groups. Changes of 6 Minutes Walking Test was significant increase in both groups but there was no significant changes on Foot print and Romberg test. Conclusion: After considering all factors, both training programs showed effect on improvement of balance and gait ability in sub-acute stroke patients, but body weight support treadmill training group had better improvement in dynamic balance than treadmill training group. For the increase of balance and gait ability in sub-acute stroke patients, we need to continues study on difference of treadmill and body weight support treadmill and then we will give stroke patients a better satisfaction if we develop and provide a rehabilitation program for improvement of balance and gait ability.

보수볼과 밸런스패드 위 런지운동 시 균형 능력 비교 (Comparison of Balance Ability on Lunge Exercise on BOSU Ball and Balance Pad)

  • 본 연구는 불안정한 지지면에서 런지운동과 지면에서 런지운동 후 동적균형운동의 변화를 확인하고자 하였다. 보수훈련군, 밸런스패드훈련군 ,지면훈련군으로 나눠 런지운동을 시행하였다. 그 결과 기능적 팔뻗기검사는 보수훈련군과 지면훈련군에서 유의하게 증가하였으며, Y 균형검사에서는 밸런스패드훈련군에서만 유의하게 증가하였다. 각 군간의 유의한 차이는 확인하지 못하였다. 본 연구를 통해 지면훈련과 불안정한 지지면에서의 런지운동의 차이는 확인할 수 없었으나 런지운동이 균형능력에 영향을 주는 것을 확인할 수 있었다.
