운동침대 틸트 각도 변화에 따른 노인의 승모근, 다열근 및 슬괵근 근육의 생리학적 특성 변화 연속 증례 연구 (Changes in Physiological Characteristics of Trapezius, Multifidus and Hamstring Muscles According to Tilt Angle of Exercise Bed in Elderly)

  • Purpose: This study investigated the effects of changing the tilt angle of the exercise bed on physiological characteristics of trapezius, multifidus, and hamstring muscle function. Methods: Twenty elderly who complains of back and lumbar pain or discomfort participated in this study (average age: $64.70{\pm}4.55$ years). The physiological characteristics, including muscle tone (F), dynamic stiffness (S), and elasticity (D) of trapezius, multifidus, and hamstring muscles were measured by a Myoton device at three tilt angles (0, 15, and 30 degrees) during exercise. Results: The muscle tone, dynamic stiffness, and elasticity of the right trapezius and left hamstring muscle showed significant differences (p<0.05) according to exercise bed tilt angle, while the other muscles showed no significant differences. Conclusions: Our results demonstrate that physiological characteristics of the upper and lower extremity muscles are affected ipsilaterally, according to the tilt angles of the exercise bed.

The Effects of Core Stability Exercise on the Ability of Postural Control in Patients With Hemiplegia

  • Core stability exercises for patients with hemiplegia have become increasingly important and a variety of exercises have been developed over the years to give the hemiplegic patients more stable postural control. This study examined the therapeutic effects of the core stability exercises on the ability of static and dynamic postural control. Fifteen hemiplegic patients (7 males, 8 females, age ranging from 46 to 76 years) hospitalized in a Daejoen rehabilitation hospital were enrolled in this study. Nine and 6 patients had a cerebral infarction and cerebral hemorrhage, respectively. The subjects participated in a core stability exercise program consisting of a total of 12 sessions 3 times each week over a 4-week period with each exercise lasting approximately 15 minutes. The ability of static and dynamic postural control by Berg Balance Scale (BBS) and Timed Up and Go (TUG), respectively, were measured before and after the core stability exercise. A Wilcoxon signed ranks test was used to compare the effects of the ability of static and dynamic postural control before and after core stability exercise in patients with hemiplegia. The ${\alpha}$=.05 level of significance was used for the statistical tests. Core stability exercises were effective in improving the ability of static postural control; BBS (p<.05). Core stability exercises were also effective in improving the ability of dynamic postural control; TUG (p<.05). Overall, core stability exercise is believed to be an important therapeutic method in rehabilitation programs for hemiplegic patients.

현미덤벨 운동시 가압처치가 노인여성의 신체구성, 체력과 동적 및 정적평형성에 미치는 융합적 효과 (The Converge Effects of Kaatsu Treatment on Body Compositions, Physical Fitness and Dynamic and Static Balance during Unmilled Rice Dumbbell Exercise in Older Women)

  • 본 연구는 노인여성들을 대상으로 현미덤벨 운동시 가압처치와 비가압처치를 하였을 때 신체구성, 체력과 동적 및 정적 평형성에 어떠한 융합적인 영향이 있는가를 규명하는데 목적이 있다. 이 연구결과, 덤벨운동트레이닝 후 체중에서는 집단간에 차이가 없었고, 체지방율은 집단간에 차이가 없었으나 시기간에는 유의한 차이가 있었다(P<.001). 제지방 체중과 BMI에서는 집단간에 유의한 차이가 없었다. 체력요인에서는 좌측 악력이 집단간에 유의한 차이를 보였다(P<.05). 근지구력에서는 집단간에 유의한 차이가 있었고(P<.05), 유연성과 민첩성에서는 시기간에 통계적으로 유의한 차이가 나타났다(P<.001). 그리고 현미덤벨 운동트레이닝 후 동 정적평형성에서는 10m 구간걷기와 2.44m 왕복걷기시 집단간에 통계적으로 유의한 차이가 나타났다(각각 P<.001, P<.05).

음악줄넘기 운동이 지적장애 남성의 동·정적 평형성 및 신체구성에 미치는 영향 (The effects of music rope skipping exercise on dynamic and static balance and body compositions in intellectual disabilities men)

  • 본 연구의 목적은 지적장애를 가진 학생을 대상으로 장기간의 음악줄넘기 운동프로그램을 적용시킨 후에 신체균형능력과 신체구성성분에 어떠한 차이가 있는가를 알아보고, 지적장애인들을 위한 운동프로그램 계획에 대한 정보를 제공하는데 있다. 참가한 피험자들은 M특수학교에 재학중인 학생을 두 집단으로 구분(운동집단 10명, 비운동집단 10명)하였다. 이들의 나이는 17~19세 이다. 운동집단의 학생들은 음악줄넘기 운동을 12주 동안 일주일에 3일, 하루 60분씩 실시하였다. 이 연구에서 측정된 항목은 동 정적 평형성 및 신체구성성분이었다. 이 연구결과, 음악줄넘기 운동프로그램 후에 동적 및 정적 평형성이 향상되었다. 또한 체중과 체지방율이 유의하게 차이가 있었다. 따라서 지적장애인들에게 12주 동안의 음악줄넘기 운동은 신체의 균형감각 향상과 신체구성성분의 개선에 효과적이었다.

8방향 경사와 회전 운동이 만성요통환자의 통증과 동적균형에 미치는 영향 (Effect of 8 Direction Incline and Rotation Exercise on Pain and Dynamic Balance in the Patients with Chronic Low Back Pain)

  • 본 연구는 8방향 경사와 회전운동이 만성요통환자의 통증과 동적균형에 미치는 영향을 알아보고자 시행되었다. 20명의 만성요통환자가 실험에 참여하였다. 10명(실험군)은 물리치료기구[온열팩{Hot Pack(H/P), 경피신경자극{Transcutaneous Electric Nerve Stimulation(TENS)}, 초음파{Ultra Sound(U/S)}]를 이용한 치료를 받은 후에 8방향 경사와 회전운동을 실시하였고 나머지 10명(대조군)은 물리치료기구를 이용한 치료만 실시되었다. 치료 중재는 주 3회, 6주 동안 적용되었다. 동적균형과 통증은 Star Excursion Balance Test(SEBT)과 Visual Analog Scale(VAS)를 이용하여 측정하였다. VAS 점수는 두 그룹 모두 감소하였으나 실험군에서 보다 유의하게 감소하였다. 동적균형 능력 또한 실험군에서 보다 유의한 증가를 나타내었다. 따라서 8방향 경사와 회전운동은 만성요통환자의 통증과 동적균형에 효과가 있었다고 사료된다.

안뜰감각 자극운동이 성인여성의 안정성한계, 동적체중이동 및 팔다리 반응시간에 미치는 영향 (Effect of Vestibular Sensory Stimulation Exercise on Limit of Stability, Dynamic Weight Shift, and Upper and Lower Extremities Reaction Time in Adult Women)

  • Purpose : The purpose of this study was to investigate the effect of vestibular sensory stimulation exercise on the limit of stability, dynamic weight shift, and upper and lower extremities reaction time in adult women. Methods : This study was conducted with 30 female. All subjects were randomly and equally assigned to an experimental group and a control group of 15 each. Subjects assigned to the experimental group received vestibular sensory stimulation training for 6 weeks. For the intervention, vestibular sensory stimulation exercises were conducted by referring to the Hamid exercise method and the Cawthorne-Cooksey exercise method, and the control group did not receive any intervention. All subjects were tested for limit of stability, dynamic weight shift, and upper and lower extremities reaction time before and after the intervention. Results : The results of this study showed significant differences between groups in reaction time, moving velocity, and directional control in the limit of stability test after intervention. In the dynamic weight shift test after intervention, there was a significant difference between the groups in the slow directional control of left and right. And in the upper and lower extremities reaction time test after intervention, both scores and reaction time showed significant differences between groups. Conclusion : As a result, the three vestibular sensory stimulation exercises applied in this study improved the limit of stability, dynamic weight shift, and upper and lower extremities reaction time.

슬링을 이용한 중간볼기근 강화훈련이 만성 발목 불안정성을 가진 20대 성인의 발목 불안정성, 균형, 근력에 미치는 영향 (Effects of Gluteus Medius Strengthening Training Using Sling on Ankle Instability, Muscle Strength, Balance in Adults in Their 20s with Chronic Ankle Instability)

  • Purpose : The purpose of this study was to investigate the effect of ankle strengthening exercise combined with sling-assisted gluteus medius strengthening on ankle instability score, and static and dynamic balance ability, muscle strength in adults in their 20s with chronic ankle instability. Methods : Twenty-eight adults in their 20s with chronic ankle instability were recruited. After screening test, they were randomized through R studio program as an experimental group (n=14) to apply an ankle strengthening exercise combined with sling-assisted gluteus medius strengthening and a control group (n=14) to apply ankle strengthening exercise. The intervention lasted two times a week for 6 weeks. To compare the intervention effects, the Cumberland ankle instability tool (CAIT) score, static and dynamic balance ability, and muscle strength of lower extremities were measured. Results : The experimental group showed a significant increase in pre and post-intervention Cumberland ankle instability tool (CAIT) score, static and dynamic balance ability, and muscle strengt (p<.05). The control group showed a significant increase in pre and post-intervention CAIT score, dynamic balance ability, and muscle strength (p<.05). The experimental group showed a significant increase in CAIT score, dynamic balance ability, and muscle strength compared to the control group (p<.05), and showed a high effect size. Conclusions : The results of this study confirmed that ankle strengthening exercise combined with sling-assisted gluteus medius strengthening on people with chronic ankle instability the possibility that it could be effective in improving ankle instability and improving dynamic balance ability, and strength by movement. Although additional research is needed to increase the number of participants due to the small sample size, it is hoped that this study will be an optimistic clinical protocol for people with chronic ankle instability.

뇌졸중 환자에 대한 요추부 안정화 운동이 정적, 동적균형 및 보행에 미치는 영향 (Effects of Lumbar Stabilization Exercise on Static and Dynamic Balancing and Gait of Stroke Patients)

  • 본 연구는 뇌졸중 환자에게 요추부 안정화 운동프로그램을 적용하여 정적, 동적운동의 균형 및 보행에 어떠한 영향을 미치는지 보고자 하였다. 27명의 뇌졸중 환자 중 실험군 14명과 대조군 13명으로 구분하고 하루 30분 주 4회로 6주간 실시하였다. 변인은 요추부 안정화 운동 전-후로 정적, 동적기립균형 능력과 보행 능력 확인 하였다. 결과는 IBM SPSS(19.0 version)을 이용하여 전-후 차이검증과 집단간 차이 검증을 위해 t검증과 이원량분석을 실시하였다. 유의수준은 α=.05로 설정하고 다음과 같은 결과를 얻었다. 첫째, 요추부 강화 운동 후 정적기립 균형에서 통계적으로 유의한 차이를 보였다(P<.05). 둘째, 요추부 강화 운동 후 동적 기립균형에서 통계적으로 유의한 차이를 보였다(P<.05). 셋째, 요추부 강화 운동 후 보행동작에서 통계적으로 유의한 차이를 보였다(P<.05). 집단 간 상호 작용에서도 유의한 차이를 보였다. 이러한 결과는 뇌졸중 환자에게 다양한 요추부 운동의 필요성이 제기 되고 요추부 안정화 운동을 통해 신체치료 및 건강증진에 도움을 줄 수 있을 것으로 판단된다. 뇌졸중 환자의 삶의 질에서도 긍정적 영향을 미칠 것으로 사료 되며 다양한 요추부 운동 적용 연구가 이루어져야겠다.

가상현실 운동프로그램이 여성노인의 근활성도와 균형능력에 미치는 영향 (Effects of Virtual Reality Exercise Program on Muscle Activity and Balance Abilities in Elderly Women)

  • Purpose: The aim of this study was to investigate the effects of an exercise program using a virtual reality game and a gait exercise program using a treadmill on % maximum voluntary isometric contraction (%MVIC) and static and dynamic balancing capabilities. Methods: A total of 26 elderly women were included in the study. 13 women were assigned to an exercise program using a virtual reality game, and 13 to a gait exercise program using a treadmill. The subjects performed the exercise for 40 min per session, three sessions per week, for eight weeks. Results: The %MVIC of the vastus medialis was significantly increased from $28.91{\pm}2.03%$ to $32.98{\pm}2.6%$ in the virtual reality game exercise group (p<0.00). The %MVIC of the vastus lateralis was significantly increased from $27.17{\pm}1.93%$ to $31.50{\pm}2.18%$ (p<0.00) in the gait exercise program group. The whole path length with both feet on the floor and eyes open was significantly decreased from $1570.92{\pm}820.6mm$ to $1343.62{\pm}242.41mm$ (p<0.00). The whole path length with both feet on the floor and eyes closed was significantly decreased from $1819.85{\pm}361.14mm$ to $1581.05{\pm}285.11mm$ (p<0.00). The length of a functional reach was significantly increased from $25.2{\pm}4.23cm$ to $27.68{\pm}4.04cm$ (p<0.00). Conclusion: The exercise program using a virtual reality game is effective for improving the %MVIC and static and dynamic balancing capabilities in elderly women aged 65 years and more.

Comparative Immediate Effects of Isometric Chin-tuck and Dynamic Neuromuscular Stabilization on Neck Flexor Muscle Thickness and Upright Sitting Height Posture

  • Background: Cervical dysfunction is a common pathomechanical marker in individuals with forward head posture (FHP). To overcome the limitations of the isometric chin-tuck (ICT) exercise, dynamic neuromuscular stabilization (DNS), which emphasizes an entire spinal chain exercise, has recently shown promising clinical results. Objects: Purpose of this study was to compare the immediate effects between ICT and DNS techniques. Methods: 43 young subjects (mean age, $24.0{\pm}5.0$ years) were recruited. Group of subjects with FHP were measured under baseline, ICT, and DNS conditions. Outcome measures included sitting height, longus colli (LC) and sternocleidomastoid (SCM) muscle thickness and LC/SCM thickness ratio. One-way repeated measures ANOVA was used to compare the continuous dependent variables among FHP, ICT, and DNS conditions at p<.016. Results: Both ICT and DNS exercise conditions yielded significantly increased LC muscle thickness, LC/SCM thickness ratio and sitting height than did FHP condition (p<.0001, respectively). Sitting height was significantly greater in DNS exercise than in the ICT exercise (p<.0001). Conclusion: The present results demonstrated that sitting height was greater in the DNS exercise than in the ICT exercise, as well as both corrective postural training exercises were effective on LC/SCM muscle balance ratio when compared with the baseline FHP condition. Therefore, it is considered that DNS exercise can be the recommended exercise for people with FHP.
