The growth of the older population is expected to further increase social problems associated with population aging, such as isolation, poverty, and depression. The emerging issues associated with the older population are also expected to provide further momentum on studies about the dwelling environment as factors that ensure the health of older people as well as improve their quality of life. Therefore, approaches for explaining the issues of the older age group should be diversified using a variety of factors and appropriate analytic tools. Studies on measuring depression have principally focused on assessing an objective self-report questionnaire, usually in a highly structured, textual form which may not reflect the cognitive impairment of older adults. The aim of this study was to define and measure dwelling depression among older adults in Korea. There are two specific hypotheses in this study as follows: (a) there will be statistically significant relationships with dwelling dissatisfaction and depression, and (b) dwelling depression tools containing text and images will be, respectively, assessment tools that have a good construct with content validity and reliability. In the first experiment, to define and measure dwelling depression, 301 people over 65 years old living in single and two-person households were surveyed using a text-based dwelling depression questionnaires from September 1-30, 2017. In the second experiment, to examine whether the projective image questionnaire could serve as a suitable replacement for the text-based questionnaires, the same participants were surveyed from January 22 to February 2, 2018. The results show that depression has a close correlation with dwelling dissatisfaction. In addition, the geriatric dwelling depression index (GDDI) based on the projective image was refined. Additionally, the projective image questionnaire has a close correlation with the text-based questionnaire. Finally, through ROC curve analysis, it was found that the projective image questionnaire can accurately predict a depression group. To this end, this preliminary study examined the validity of the projective image questionnaire in older adults to make this instrument feasible for older populations and to contribute to a profound understanding of geriatric depression due to the living environment. We hope they will provide a basis for further research on psychological diagnoses using projective images.
우리나라는 급격히 증가하고 있는 고령 인구 증가에 따라 고령자의 사회적 고립 및 우울 등의 문제가 심각하게 제기되고 있다. 또한, 고령자의 건강과 삶의 질의 향상 요인으로 주거 환경에 대한 중요성이 부각되면서 고령자 주거에 대한 논의는 더욱 강조되고 있다. 본 연구에서는, 이와 같은 현실을 고려하며, 2017년도 9월 1일부터 9월 30일까지 7개 시 구에 거주하는 65세 이상 남 여 단독가구 고령자 350명을 대상으로 단독가구 고령자의 주거우울 개념 및 주거우울 유형을 실증적으로 정립 분석하였다. 측정도구의 신뢰도 및 타당성 검토와 우울과 주거만족의 상관분석을 실시하였고, 주거우울 유형 분류를 위해 회귀분석을 시행하였다. 그 결과, 고령자 우울과 주거만족의 상관을 확인하였고, 주거우울 지수를 산출하여 단독가구 고령자의 우울 상태를 판정할 수 있도록 하였다. 본 연구를 통해 새롭게 개발된 주거우울 척도와 유형이 단독가구 고령자의 주거우울정도에 대한 정확한 진단을 가능하게 할 것으로 기대된다.
Purpose: The purpose of this study was to investigate the factors that influence health-related quality of life (HRQoL) in community-dwelling older adults. Methods: The participants were 133 elderly people dwelling in a community. Data were analyzed using descriptive statistics, Pearson's correlation coefficients, and Stepwise multiple regression with SPSS. Results: The predictors of quality of life in the elderly were depression, physical symptoms, and social support. Depression and physical symptoms had a significant negative correlation with HRQoL and social support had a significant positive correlation with HRQoL. Among them, depression was shown as the most significant predictor of HRQoL in the elderly ($R^2$=.573, p<.001). Conclusion: These results indicate that strategies to relieve depression should be integrated in interventions to improve the QoL of older adults dwelling in a community residence.
Purpose: The purpose of this study was to investigate the effects of health behavior-related characteristics, self-esteem, activities of daily living, and family support on depression in the community-dwelling elderly. Methods: The participants were 229 elders sampled from Seoul and other five provinces. A structured questionnaire was used to collect data, and data were analyzed by applying t-test, ANOVA, Pearson correlation coefficient, and stepwise multiple regression with SAS 8.12. Results: There was a negative correlation between depression, MMSE-K, self-esteem, ADL, IADL, and family support. Depression and health behavior-related characteristics showed a significant difference according to stress level and the reason of stress. Major factors that affected the elders depression were self-esteem, the amount of alcohol drinking, sleeping hours, ADL and the cognition of health status, which explained 59.4%. Conclusion: Based on the findings of this study, self-esteem and health behavior-related characteristics including alcohol drinking, sleeping hours and ADL were the influencing factors of depression in the community-dwelling elderly. Therefore, effective psychological and physical health promotion methods need to be developed and applied in nursing interventions to prevent depression in the community-dwelling elderly.
Purpose: The purpose of this study was to investigate the relationships among quality of sleep, depression, late-life function and disability in community-dwelling older women with urinary incontinence. Methods: A stratified random sampling method was conducted to recruit participants from May 10 to August 17, 2007. Data were collected by questionnaires, which were constructed to include lower urinary tract symptoms, quality of sleep, depression, and late-life function and disability in 128 community-dwelling older women. Results: The major findings of this study were as follow: 1) 56.3% of participants belonged to urinary incontinence group. 2) There were significant relationships between depression and sleep latency, sleep duration, daytime dysfunction, quality of sleep, function component, frequency dimension, and limitation dimension. 3) Depression was significantly associated with frequency dimension, limitation dimension in capability which explained 44% of variance in depression. Conclusion: These results may contribute to a better understanding of sleep quality, depression, late-life function and disability in the community-dwelling older women with urinary incontinence. Therefore, health programs for prompting older women's health should be planned based on results of the study.
고령자 인구의 급격한 증가로 고령자 삶의 질을 개선하고, 고령자건강 등 심리상태를 측정하고자 하는 사회적요구가 증가하고 있지만, 현재 주로 적용되는 텍스트 기반의 척도는 시력 저하 등 고령자의 인지적 문제를 반영하지 못하고 있다. 따라서 본 연구는 K-HTP를 활용하여 주거환경에 따른 고령자우울을 측정하고, 개발한 척도의 타당성을 검증하는 것을 목표로 한다. 이를 위해, 2018년도 1월 22일부터 2월 2일까지 대전과 주변지역 7개 시 구에 거주하는 65세 이상 남 여 단독가구 고령자 301명을 대상으로 조사를 진행하여, 주거우울척도와 K-HTP 기반 주거우울척도의 상관관계를 규명하고, 요인 분석을 통해 척도의 신뢰도를 규명하였다. 고령자주거우울지수(GDDI)와 K-HTP 기반 고령자주거우울지수(GDDI-K)의 상관관계를 규명하고, GDDI-K의 정확도를 분석하였다. 연구 결과, K-HTP 척도가 주거우울을 진단할 수 있음을 확인하였다. 본 연구는 고령자의 인지적 상황을 고려하여 주거우울을 진단할 수 있는 K-HTP를 활용한 척도를 제안함으로써 투사이미지를 활용한 고령자 우울진단 연구의 발판을 제공한 데 의의가 있다.
주거환경이 고령자 삶의 질을 결정하는 중요한 요인으로 인식되면서 고령자우울 측정의 필요성이 부각된다. 특히, 고령자 인지특성, 생활환경 등 다양한 변수를 고려하여 측정 도구를 개발하는 것은 정확한 심리 진단에 매우 중요한 일이다. 본 연구에서는, 2018년도 1월 22일부터 2월 2일까지 7개 시 구에 거주하는 65세 이상 남 여 단독가구 고령자 301명을 대상으로 주거환경 속에서 나타나는 우울을 투사적 이미지를 적용하여 측정하였다. 이를 위해 주거우울과 투사이미지의 상관관계를 규명하였고, 요인분석을 통해 투사이미지 기반 측정도구를 보완하였다. 고령자주거우울지수(GDDI)와 '투사이미지 기반 고령자주거우울지수'의 상관관계를 규명하였으며, 척도의 정확도를 분석하였다. 연구 결과, 투사이미지를 활용한 설문은 서술형 설문과 높은 상관관계를 갖고 있음을 확인하였다. 본 연구는 주거우울을 측정할 수 있는 새로운 도구를 제안하여 투사기법을 이용한 심리 진단 연구의 발판을 제공한데 그 의의를 찾을 수 있다.
Purpose: This study examined the effectiveness of a forest-experience-integration intervention in community dwelling cancer patients. Methods: The study was done with a nonequivalent control group pretest-posttest design. The subjects of the present study were 53 community dwelling cancer patients who were registered in a community health center in Gyeongsangbuk-do. The subjects were divided into an experimental group (n=26) who participated in the forest-experience-integration intervention and a control group (n=27) who did not participate. Data were collected from May to June in 2011. Collected data were analyzed using SPSS PC+ 19.0 through $x^2$ test, and independent t-test. Results: There were significant differences between the groups in depression (t=-4.51, p<.001), self-regulation resilience (t=6.95, p<.001), interpersonal resilience (t=10.10, p<.001), positivity resilience (t=9.67, p<.001), and total resilience (t=13.93, p<.001) measurements. Conclusion: The forest- experience-integration intervention delivered to community dwelling cancer patients was an effective method for relieving depression and enhancing self-regulation resilience, interpersonal resilience, positivity resilience, and total resilience, and can be utilized as an effective nursing intervention for community dwelling cancer patients.
Purpose: The purpose of this study was to identify the mediating effect of depression in the relationship between muscle strength of extremities and falls among community-dwelling elderly. Methods: Two hundred forty-seven participants were recruited from a public health center, a hall for the aged and a school for the aged in B city. Face-to-face interviews were conducted using questionnaires from May to September of 2007. Data was analyzed with descriptive statistics, Pearson correlation, and multiple regression using the SPSS WIN 14.0 program. Results: There was a significantly negative relationship between muscle strength of lower extremities and falls, muscle strength of left upper extremity and falls, and muscle strength of right upper extremity and falls. Depression positively correlated with falls. Depression showed mediating effects between muscle strength of extremities and falls. Weakness of muscle strength of extremities increased depression and the increased depression increased the frequencies of falls. Conclusion: For the effective management and prevention of community-dwelling older adults' falls, exercise programs including depression-decreasing strategies should be established. These exercise programs can decrease depression which is the mediator role between the degrees of muscle strength of extremities and falls.
Purpose: The purpose of this study was to identify the influence of physical activity and depression on sleep quality among the young-old and old-old community-dwelling elderly. Methods: Participants were 216 community-dwelling older adults in Korea aged 65 or above. Data were collected using structured questionnaires with face-to-face interviews that included demographic and health-related characteristics, International Physical Activity Questionnaires (IPAQ), the Short Form Geriatric Depression Scale (SGDS) and the Pittsburgh Sleep Quality Index (PSQI). A hierarchical multiple regression was conducted to examine whether physical activity and depression would predict sleep quality under other controlled factors. Results: There were differences in demographic and health-related characteristics, physical activity, and depression by age groups, but not in sleep quality. In the young-old elderly, physical activity (${\beta}=-0.22$, p=.043) and depression (${\beta}=0.31$, p=.002) were significantly associated with sleep quality (F=4.46, p=.001, Adjusted $R^2=.16$). In the old-old elderly, physical activity (${\beta}=-0.29$, p=.001) and depression (${\beta}=0.41$, p<.001) were significantly associated with sleep quality (F=10.79, p<.001, Adjusted $R^2=.29$). Conclusion: These finding highlight physical activity and depression as important contributors to sleep quality in both young-old and old-old elderly.
