Purpose : Bony strength is dependent on bone mass and bony structure. So this study was designed to investigate the relationship between the bone mass and bony trabecular characteristics. Subjects and Methods : Study subjects were 51 females (average age 68.6 years) and 20 males (average age 66.4 years). Bone mineral density (BMD, $grams/cm^2$) of proximal femur was measured by a dual energy X-ray absorptiometry (DEXA). Regions of interest (ROIs) were selected from the digitized radiographs of proximal femur. A customized computer program processed morphologic operations (MO) of ROIs. 44 skeletal variables of MO were calculated from ROIs on the Ward's triangle and greater trochanter of femur. WHO BMD classes were predicted by MO variables of the same ROI. Classification and Regression Tree analysis was used for calculating weighted kappa values, sensitivity and specificity of MO. Results : The discriminating factors of morphologic operation were branch point, branch point [per cm sq]. Age also played important role in distinguishing osteoporotic classes. The sensitivity of MO at Ward's triangle and Greater Trochanter was 91.8%, 65.6%, respectively. The specificity of MO was 100% at Ward's triangle and Greater Trochanter. Conclusion : Bony trabecular characteristics obtained using radiological bone morphometric analysis seem to be related to bone mass.

Objectives Excess weight and obesity are a crucial public health problem worldwide and are considered as the main cause of many chronic diseases. The present study evaluated the effects of Aronia melanocarpa extract (AMEX) supplementation on body compositions in overweight or obese people. Methods This randomized, double-blind, placebo-controlled clinical trial was carried out on 66 healthy overweight or obese peoples. The eligible subjects were divided into AMEX and placebo supplement treatment for 12 weeks. Anthropometrics, body composition (dual-energy X-ray absorptiometry), and blood analysis were performed preand post intervention. Results We observed significant reductions in the body weight and body mass index in both groups; however, the decrease was higher in the AMEX group. Body fat mass and percent body fat showed a tendency to decreases after AMEX supplementation. No clinically significant changes were observed for any safety parameter. Conclusions In conclusion, the data of this trial indicate that AMEX were not effective in reducing body compositions, but as a safe supplement, it may help weight loss in overweight or obese people.
Objectives The purpose of this study was to examine the relationship between cold hypersensitivity and bone mineral density. Methods 28 outpatients who visited OO University Oriental Hospital from July 11th, 2013 to August 28th, 2013 were analyzed. Patients were subjected to thermometer, and those with thermal difference greater than 0.3 ℃ between upper arm and palm and also with more than VAS 4 of cold hypersensitivity were diagnosed with cold sensitivity. Lumba spine (L2-4) and heel bone measurement were obtained on dual energy X-ray absorptiometry (DEXA) and Quantitative Computed Tomography (QCT). Results 1. There was statistically significant correlation between T-score of DEXA and body weight (0.522), BMI (0.643), OSTA (Osteoporosis elf-assesment Tool for Asians, 0.579), BMI, T-score and Z-score of QCT. OSTA and T-score of QCT (0.521) also had statistically significance. 2. There was no statistically significant correlation between body fat, abdominal obesity rate, degree of cold hypersensitivity, upper arm and palm's temperature difference, palm temperature, height and Z-score, T-score of QCT or T-sore of DEXA. Conclusions Bone mineral density was lower in women who had cold hypersensitivity of hand and feet. But there was no statistically significant correlation between bone mineral density and degree of cold hypersensitivity, upper arm and palm’s temperature difference.
Background: Obesity is associated with menstrual disorder. Especially, upper-body obesity affects on female reproductive function. Objectives: The goals were to investigate relation between fat distribution and menstrual disorder in obese pre-menopausal Korean females. The hypothesis were tested that there is a relation between upper body obesity and menstrual disorder. Design: A cross-sectional evaluation of 66 Females (baseline age $32.15{\pm}7,32)$ with body mass index $31.22{\pm}4.05\;kg/m^2$. Body composition was measured using bioimpedence analysis (BIA) and anthropometry was done by same observer. VAS and Multidimensional verval rating scale(MVRS) were checked for menstrual pain. Dual-Energy X-ray Absorptiometry (DXA) was measured for evaluating body fat distribution. Menstrual disorder and body fat distribution were compared using statistical methods. Results: TLFR and WH ratio was higher in menstrual cyclic disorder group than controls. There was negative relationship between VAS and TLFR. Conclusions: The data shows that disorder of menstrual cycle was associated with upper body obesity. WH ratio could be one of the factor of menstrual disorder. VAS was correlated with lower body obesity. Further studies for role of upper body fat distribution on female reproductive function and relationship between menstrual pain and fat distribution thought to be needed.
Objective: A number of candidate genes have been in implicated in the pathogenesis of obesity in humans. Tumor necrosis factor-alpha $(TNF-{\alpha})$ is expressed primarily in adipocytes, and elevated levels of this cytokine have been linked to obesity and insulin resistance. Recently, the A allele of a polymorphism at position 308 in the promoter region of $TNF-{\alpha}$ (G-308A) has been shown to increase transcription of the gene in adipocytes. Therefore, we designed this study to test whether obese and non-obese subjects differ in $TNF-{\alpha}$ genotype distribution, and how the genotypes affect anthropometric parameters, including degrees of body mass index (BMI). Methods : The study included 153 obese but otherwise healthy women ($BMI{\geq}kg/m^2$, range 25-54.7, age range 15-40 years) and 82 non-obese healthy women ($BMI, age range 15-40 years). Total fat mass and percent body fat were determined by dual-energy X-ray absorptiometry. Genomic DNA was extracted and used for Ncol restriction fragment length polymorphism (RFLP) based genotyping of $TNF-{\alpha}$. Results: No differences were observed for allelic and genotype frequencies between the obese ($BMI{\geq}25$) and non-obese women. Also, no association of TNF-(l polymorphism was observed with body mass index (BMI) for genotype in obese women. In addition, age, pertent body fat, BMI, and cholesterol levels did not differ by $TNF-{\alpha}$ genotype. However, waist-tohip ratio (WHR) was significantly lower in subjects with $TNF-{\alpha}$ GA or AA genotype (0.94 0.07 vs. 0.920.03, P<0.005). Conclusion: These results suggest that $TNF-{\alpha}$ promoter polymorphism at position-308 is not a significant factor for BMI, but affects the WHR in obese healthy women from Koreans.
Purpose: This study was performed to evaluate the trabecular pattern on panoramic radiographs to predict age-related osteoporosis in postmenopausal women. Materials and Methods: Thirty-one postmenopausal osteoporotic women and 25 postmenopausal healthy women between the ages of 50 and 88 were enrolled in this study. The bone mineral density (BMD) of the lumbar vertebrae and femur were calculated using dual-energy X-ray absorptiometry (DXA), and panoramic radiographs were obtained. Fractal dimension (FD) was measured using the box counting method from 560 regions of interest ($51{\times}51$ pixels) in 6 sites on the panoramic radiographs. The relationships between age and BMD and between FD and BMD were assessed, and the intraobserver agreement was determined. Results: There was a significant difference in the FD values between the osteoporotic and normal groups (p<0.05). There was a significant difference in the FD values at three sites in the jaws (p<0.05). Age was significantly correlated with the BMD measurements, with an odds ratio of 1.25. However, the FD values were not significantly correlated with the BMD measurements, with an odds ratio of 0.000. The intraobserver agreement showed relatively higher correlation coefficients at the upper premolar, lower premolar, and lower anterior regions than the other sites. Conclusion: Age was an important risk factor for predicting the presence of osteoporosis in postmenopausal women. The lower premolar region was the most appropriate site for evaluating the FD value on panoramic radiographs. However, further investigation might be needed to predict osteoporosis using an FD value on panoramic radiographs.
Objectives : Osteoporosis is the most common metabolic disease of the bone, and one of the most important public health problems around the world. Obesity rate gradually increases as women grow older. The purpose of the study was to investigate the changes between bond mineral density(BMD) and body mass index(BMI) according to sex and age. Methods : We measured the bone mineral density(BMD) of lumber spine(L2-L4) and femoral neck in 10000 normal adult, using dual energy X-ray absorptiometry (DEXA; DPX-alpha). and also measured the body mass index(BMI) in 9810 normal adult. then we analysed the changes of BMD and BMI according to sex and age. Results : 1. As getting older, male's T-score of lumbar spine(L2-L4) was slowly decreased. On the other hand, female's T-score of lumbar spine was sharply dropped after 46 years old. 2. As getting older, male's T-score of femoral was slowly decreased. On the other hand, female's T-score of femoral was sharply dropped after 49 years old. 3. Irrespective of age, male's BMI keep up the extent within a definite period of numerical value. As getting older, female's BMI continued its upward trend. Conclusions : Much more attention should be needed for female patient suffering from osteoporosis and obesity than male.
The purpose of this study was to evaluate the relationship between the osteoporotic condition and periodontal condition in postmenopausal women with periodontitis. Forty three female postmenopausal patients with no systemic disease were grouped into 3 groups by their periodontal conditions; 12 mild periodontitis, 11 moderate periodontitis and 20 advanced periodontitis. From each patient, age of menopause was taken, alkaline phosphatase(ALP) and osteocalcin (OC) in blood and deoxypyridinoline (DPD) in urine were measured. Bone mineral density (BMD) of lumbar spine (L2-L4) was measured by dual energy X-ray absorptiometry. Periodontal and osteoporotic parameters were compared among the groups and correlation coefficient between them was evaluated. The blood ALP and OC levels were similar among the groups with different periodontal condition, whereas the urine DPD level and BMD were significantly lower in advanced periodontitis group than the other groups(p<0.01). Probing depth was negatively related with BMD (r=-0.5, p<0.01) and positively related with patient age and the duration of menopause (r= 0.32 and 0.35 respectively, p<0.05). Clinical attachment loss was negatively related with BMD (r=-0.66, p<0.01), and positively related with urine DPD (r= 0.37, p<0.05). These results showed that postmenopausal women with advanced periodontitis had significantly decreased bone mineral density and suggests that decreased bone mineral density in postmenopausal women could be associated with periodontal tissue breakdown.
In bone density examinations, a change in the measured BMD occurs owing to the differences between the measured areas. To address this problem, we aimed to develop a new auxiliary device that could be rotated by $15^{\circ}$ by fixing the ankle to the distal femur neck. Dual energy X-ray absorptiometry (DXA) of BMD examinations were performed once a year, but 10 patients were examined over three sessions to analyze the area for measuring the femur neck BMD. The goal of this test was to determine the device's reliability, and the results were expressed in terms of the standard deviation of measurements. After performing bone density measurements using the new auxiliary device on 10 normal patients, with three measurements for each patient, the obtained standard deviation was 0.03. The standard deviation of the measured BMD was 0.19 when using the currently existing auxiliary device, while the standard deviation of the measured BMD was 0.03 when using the new auxiliary device. By using the new auxiliary device, the standard deviation could be reduced by ~80%. Accurate rotation of the femur neck was possible in all examinations, and the standard deviation of BMD measurements could be reduced by up to 80% compared with the measurements performed using the currently existing auxiliary device. We hope that this advantageous new design can be used as a standard auxiliary device for measuring the femur neck BMD.
Objectives: The purpose of the study is to investigate the relationship between the number of remaining teeth and bone health status among adults over 65 years old. Methods: The study subjects were 1,843 adults over 65 years old drawn from the Korea National Health and Nutrition Examination Survey (KNHANES) in 2008-2010. Oral health status included the number of remaining teeth, and prosthetic appliance status. Bone health statuses were assessed using dual-energy X-ray absorptiometry (DEXA). Bone health statuses were classified into normal (T-score ${\geq}-1.0$), osteopenia (-2.5 < T-score < -1.0), and osteoporosis (T-score ${\leq}-2.5$). Complex samples chi-square test was used to estimate the relationship between the number of remaining teeth and bone health status, and related factors included in the model were analyzed with the complex samples logistic regression analysis. Results: Using the multinomial logistic regression analytic method, the elderly having 20 or more remaining teeth and those having less than 20 remaining teeth with prosthesis were compared. The latter group had 1.89 times higher rate of being diagnosed as having osteoporosis. Conclusions: This study suggests that oral health is an important factor for geriatric osteoporosis. Therefore, in order to prevent fractures due to osteoporosis and osteoporosis in old age, it is necessary to be aware of the relationship between oral health and osteoporosis, and oral health should be considered when preparing preventive management strategies.
