Synthesis, Characterization and In Vitro Evaluation of Triptolide-lysozyme Conjugate for Renal Targeting Delivery of Triptolide

  • A triptolide-lysozyme (TP-LZM) conjugate was synthesized to achieve renal specific delivery and to reduce the side effects of triptolide. Triptolide was coupled to lysozyme through succinic via an ester bond with an average coupling degree of 1 mol triptolide per 1 mol lysozyme. The lysozyme can specifically accumulate in the proximal tubular cells of the kidney, making it a potential carrier for targeting drugs to the kidney. The structure of triptolide succinate (TPS) was confirmed by IR, $^{1}H-NMR$, MS and UV. The concentrations of triptolide in various samples were determined by reversed-phase high-performance liquid chromatography (HPLC). In this study, the physicochemical and stability profiles of TP-LZM under various conditions were investgated the stability and releasing profiles of triptolide-lysozyme (TP-LZM) under various conditions. In vitro release trails showed triptolide-lysozyme was relatively stable in plasma (less than 30% of free triptolide released) and could release triptolide quickly in lysosome (more than 80% of free triptolide released) at $37^{\circ}C$ for 24 h. In addition, the biological activities of the conjugate on normal rat kidney proximal tubular cells (NRK52E) were also tested. The conjugate can effectively reduce NO production in the medium of NRK52E induced by lipopolysaccharide (LPS) but with much lower toxicity. These studies suggest the possibility to promote curative effect and reduce its extra-renal toxicity of triptolide by TP-LZM conjugate.

A Study on the Actual condition of the Middle School Students on Drug use (중학교 학생들의 약물사용실태에 관한 연구)

  • The purpose of this study was to survey the actual condition of the Middle School Students on a Drug. In this study, 500 middle school students in Seoul response to a questionnaire which is composed to analyse two categories, i.e. general facts, the actual condition of drug use (the frequency of drug use, the degree of perception on drugs, the people using a drug surrounding the degree of purchase on drugs). And the students are classified by satisfactory degree on their home backgrounds, satisfactory degree on their school life, and school record. The data is analysed by means of frequency, percentage, chi-square test and Pearson's correlation using the $SPSS-PC^+$ package program. The results of this study were as follow : The frequency of drug use except a alcohol and the degree of perception on a drug among female students are higher than among male. The female students are more permissive on the drug use and they are also tend to think more easily the purchase of a drug. The frequency of drug use and the degree of perception on drugs are higher in the group of the lower satisfactory degree on their home background. Especially, there are many people using drugs surrounding the students in the lower group. The frequency of drug use and the degree of perception on drugs are higher in the group that has the lower satisfactory degree on school life. The students in the group having low school record have more dangerous thought about the use of drugs than the students of high record. The degree of perception on a drug is higher in a higher group, and the students in a higher group also think more easily the purchase of drugs. The percentage of students using drugs are not high, but the high percentage of students are interested in the drugs and they have permissive attitudes on drugs. Thus, the preventive education for drug abuse must be done rapidly.

  • The elderly are more likely to have chronic medical conditions that require multiple drug therapies. Purposes of this study are to reveal necessity of elderly patient education by pharmacists, and to induce appropriate policy. We carried out literature research. Taking several drugs together increases risk of drug interactions and adverse reactions. We suggest that pharmacists have the legal authority to monitor prescription for efficient drug management, pharmacovigilance system be efficiently operated, and medication education fee be provided to allow pharmacists give more time to the elderly.

  • Background : The purpose of this study was to investigate the current status of medication compliance of outpatients and to analyze the factors contributing to medication non-compliance Methods : Telephone survey was conducted to the 1,000 outpatients who visited medical institutions during the period from January 2002 to April 2002. Subjects were randomly selected from the telephone directories of the nation, and the socio-demographic characteristics of the respondents such as age, gender and region were matched based on those of outpatients in 2001. Results : The results of survey revealed that those who complied with doctors' regimen in the right way accounted for 82.4%. The compliance increased with the strong belief in the medication, less unwanted side effects and inconvenience, more severity of disease, and lower perceived health status. Compliance rate was also higher in the patients group who experienced the drug education by the pharmacists than those who did not. Conclusion : In order to improve drug compliance, drug information on efficacy, adverse reaction, drug interactions, and basic disease information are to be provided to the patients. Drug education needs to be focused not only on providing knowledge of drugs and diseases but also changing attitude on drug use of the patients.

  • This study was done to examine consciousness and preventive strategies of adolescent drug abuse. This study design was descriptive survey. The data were collected from 1,830 students from 32 middle and high schools from November 11 to December 20, 1997, using qestionnaires. The data were analyzed using SAS computer programs. The study results are as follows ; Most of adolescent get the information on narcotics and drug abuse from TV, newspaper, magazine and advertisement. Some of them(48.8%) have received education on drug prevention. However, respondants (49.3%) answered unsatisfactory on preventive education. 69.2% of respondants do not know the legal regulation on alcohol and cigarette. The survey on the degree of cognition on drug abuse shows the serious results : cigarette(79.1%), alcohol(74.1%), bonds(55.1%), gas(49.8%), excitants(48.5%), narcotics(29%), marihwana(27.8%) and, diuretics (21.9%). The situation of adolescent drug abuse is also serious in the order of alcohol 38. 2%, cigarette 14.3%, marihwana 9.4%.

The Study on the Attitude about Drugs, the Actual Conditions on Drug Abuse and Drug Education of Primary School Students (국민학생의 약물에 대한 태도와 남용경험 및 약물교육실태)

  • The purposes of this study were to suggest the need for drug prevention program in primary school and to provide basic data for preventing the youth from their drug abuse. A total of 680 students of primary school in Pusan were selected to answer this questionnair. The data was collected from July 1 to July 15, 1994. The major findings of this study were as follows : 1) The attitude about drugs of primary school students was desirable. By groups, girl students showed more desirable attitude than boy students. 2) 5.9% of subjects had smoking experience, and for the motives of smoking 77.5% of them, the largest numbers, was with curiosity. 3) 39.8% of subjects had experienced alcohol use, and for the motives alcohol use 50.5% of them, the largest numbers, was with curiosity. 4) 1.8% of subjects had experienced inhalants (gas, butane gas), and for the motives of inhalants use 58.4% of them, the largest numbers, was also with curiosity. 5) The rates of smoking, alcohol use and inhalants use were higher in boy students than girl students. 6) The attitude points about drugs in case of smoking, alcohol use and inhalants use were lower than the points in case of not using them. 7) Knowledge level about .drugs of subjects was appeared poor. 8) For the need of drug education 65.9% of subjects responded 'necessary', and they responded most suitable educator as school nurse. 9) The students had received drug education from school in 38.2%, from their parents in 8% and from mass media in 63.5%.

Effects of a Structured Drug Education Program on Knowledge and Medication Compliance for Hemodialysis Patients (혈액투석환자를 위한 약물교육프로그램이 약물에 대한 지식과 약물복용 이행도에 미치는 효과)

  • Purpose: The purpose of this study was to analysis the effects of a structured drug education program on knowledge and medication compliance for hemodialysis patients. Method: Hemodialysis patients were recruited from a kidney center, at a university hospital located in G city, Korea. Thirty subjects in the control group received no intervention and 30 subjects in the experimental group received a structured drug education program. Result: There were significant increases in medication knowledge and medication compliance in the experimental group compared to the control group. Conclusion: According to the above results, this education program for hemodialysis patients increased knowledge and medication compliance. These findings suggest that a structured drug education program can be used as an efficient nursing intervention for hemodialysis patients.

Effects of Image-Use Medication Education on Knowledge, Self-Efficacy, and Misuse of Medication in Elderly Inpatients (이미지 활용 약물교육이 노인환자의 약물지식, 복약자기효능감 및 복약오류에 미치는 효과)

  • Purpose: This quasi-experimental study was done to develop image-use medication education for older inpatients and to evaluate the effects on their knowledge, self-efficacy, and misuse of medication. Methods: Fifty nine elders (experimental group - 30, and control group - 29) received medication education once a week for 3 weeks. Data were collected before (pretest), right after (post 1), and 4 weeks after the program finished (post 2). Drug Use Knowledge Scale, Self-efficacy for Appropriate Medication Use Scale, and Drug Misuse Scale were used. Analysis included descriptive statistics, $x^2$ test, repeated measured ANOVA. Results: Findings showed significant differences in knowledge of medication and drug misuse between groups according to time(pretest, post 1, and post 2). At post 1 and post 2, knowledge and self-efficacy levels were significantly higher and misuse scores were lower in the experimental group compared to the control group. Conclusion: Findings indicate that elder-tailored medication education consisting of group education and individual guidance with images is effective and practical for medicine safety in elderly inpatients. Moreover, it could lead to a healthier life for elders, even elders with multiple chronic diseases and taking several medications.

A Study on the Health Management of Polypharmacy Use in the Elderly

  • The purpose of this study is to identify the level of polypharmacy use, drug knowledge, and drug misuse behavior in the elderly, and to understand the correlation between them and their effect on drug misuse behavior. The study design was a descriptive survey study, and the participants of the study were 215 elderly people from the local community center. The research tool used drug knowledge, drug misuse behavior, and the data collection period was from February 8 to 19, 2021. The data analysis were descriptive statistics, t-test, one-way ANOVA, Pearson's correlation coefficient, and regression analysis. As a result of the study, a significant correlation variable for the drug knowledge of the elderly showed a significant correlation with prescription and non-prescription, r=.145 (p<0.05), and r=.-. 136, which showed a negative significant correlation (p<0.05). As for the significant correlation variable in the drug misuse behavior of the elderly, when prescription and non-prescription were combined, there was a significant correlation with r=.256 (p<0.01), and when not using drugs, r=.-.225 was negative. showed a significant correlation (p<0.01). In terms of the effect on drug misuse behavior, chronic disease =.145, prescription and non-prescription use = .233, which had a positive effect, and non-prescription = -.328, indicating a negative and significant effect. The provision of education on the safe use of drugs by the elderly should first be provided in the community. In addition, we need systematic education and social support for the transmission of correct knowledge on multi-drug use by the elderly and for health management.

Inhibitory Potential of Bilobetin Against CYP2J2 Activities in Human Liver Microsomes

  • Cytochrome P450 2J2 (CYP2J2) is a member of the cytochrome P450 superfamily, and is known to be arachidonic acid epoxygenase that mediates the formation of four bioactive regioisomers of epoxyeicosatrienoic acids (EETs). CYP2J2 is also involved in the metabolism of drugs such as albendazole, astemizole, danazol, ebastine, and terfenadine. CYP2J2 is highly expressed in the heart and cancer tissues. In this study, the inhibitory potential of ten natural products against CYP2J2 activity was evaluated using human liver microsomes and tandem mass spectrometry. Among them, bilobetin, which is a kind of biflavonoid, exhibits a strong inhibitory effect against the CYP2J2-mediated astemizole O-demethylation (IC50 = 0.73 μM) and terfenadine hydroxylation (IC50 = 0.89 μM). This result suggests that bilobetin can be used as strong CYP2J2 inhibitor in drug metabolism study.
