A Rapid Assessing Method of Drug Susceptibility Using Flow Cytometry for Mycobacterium tuberculosis Isolates Resistant to Isoniazid, Rifampin, and Ethambutol

  • Background: The current conventional drug susceptibility test (DST) for Mycobacterium tuberculosis (Mtb) takes several weeks of incubation to obtain results. As a rapid method, molecular DST requires only a few days to get the results but does not fully cover the phenotypic resistance. A new rapid method based on the ability of viable Mtb bacilli to hydrolyze fluorescein diacetate to free fluorescein with detection of fluorescent mycobacteria by flow cytometric analysis, was recently developed. Methods: To evaluate this cytometric method, we tested 39 clinical isolates which were susceptible or resistant to isoniazid (INH) or rifampin (RIF), or ethambutol (EMB) by phenotypic or molecular DST methods and compared the results. Results: The susceptibility was determined by measuring the viability rate of Mtb and all the isolates which were tested with INH, RIF, and EMB showed susceptibility results concordant with those by the phenotypic solid and liquid media methods. The isolates having no mutations in the molecular DST but resistance in the conventional phenotypic DST were also resistant in this cytometric method. These results suggest that the flow cytometric DST method is faster than conventional agar phenotypic DST and may complement the results of molecular DST. Conclusion: In conclusion, the cytometric method could provide quick and more accurate information that would help clinicians to choose more effective drugs.

  • Sixty-three Salmonella Cultures were isolated from 600 msenteric lymphnodes and 600 rectal contents of apprently healthy pigs slaughtered at abattoris in Hapchun, changnyong, Keochang. The result observed were summaried as follows ; 1. Fifty-nine out of sixty-three cultures were serotyped and Salmonella enteritis(20 cultures) 2. Was found to be the most predominant serotpye. The remainder comprised of 15 serotypes were ; S. typhimuriu, S. derby, S. wagenia, S. infantis, S. thompson, S. tennessee, S. mission, S. anatum, S. give, S. nuwlands, S. newingtion, S. newhaw, S. neumuenstes, S. senftenberg, S. gbana 3. The antibiotic susceptibility of 9 were ; Baytril, Streptomycin, Cephalothin, Ampicillin, Tetracycline, Sulfadimethoxine, Rifampicin, Gentamicin, Kanamycln.

돈 폐염 유래 Pasteurella Multocida 혈청형 및 약제 감수성 (Serogroup and Drug Susceptibility of Pasteurella Mutocida Pneumonia in Pig)

  • The present study was conducted to investigate the incidence of pasteurella multocida infection in kyungbuk swine herds during the period from July 1989 to November 1989 and some properties of the isolated organisms. P. multocida was isolated from lungs of 155 slaughtered pigs, 43(27.7%) pigs were culture positive. The majority of biochemical and cultural properties of the P. multocida isolates were identical to those of the standard strains. The capsular serogroups and drug susceptibility of 43 isolates of P. multocida from pigs with pneumonic lesions were investigated. P. multocida isolates were typed for capsular serogroupes A by hyaluronidase inhibition of capsule and D by acriflavin auto agglutination. Most isolates(60.4%) were type A, 18.6% were type D, and the remaining 21.0% were untypable. In antimicrobial susceptibility test these isolates of P. multocida were susceptible in order of ampicillin (86.0%), trimethoprim sulfamethoxazole(83.7%), colistin(81.4%), chloramphenicol(79.1%), but the majority of them were resistant in order of streptomycin(30.2%), triple sulfa (4.6%).

  • The present study was conducted to investigate isolation and antimicrobial drug susceptibility of Pasteurella spp from pneumonic calves and cows (pneumonic lungs) in Gyeongnam south area from January to November 2000. Pasteurella multocida and P haemolytica were isolated 136 (53.8%) and 22(8.7%), respectively, from 158 of 253 pneumonic calves of 1 to 12 months of age and from pneumonic lungs and simultaneous isolation rate of P mutocida and P haemolytica was 4 (1.6%). Seasonal isolation rate of Pasteurella spp in calves and pneumonic lungs varied from 3 to 65.2%, and it was higher in Fall. The majority of biochemical and cultural properties of P multocida and P haemolytica isolated from calves and pneumonic lung were identical to those of the reference strains employed. Pneumonic lesions of the examined lungs were histologically identified as purulent bronchopneumonia and fibrinous pneumonia. All isolates were resistant to penicillin, amikacin, lincomicin, colistin, cloxacilling, sulfonamide, kanamycin, sulfamethoxasoletrimetoprim, and gentamicin(>80%), but some of them were susceptible to amoxicillin, norfloxacin, cephalexin, cefazolin, orbifloxacin, enrofloxacin, and ciprofloxacin(>60%).

영남지방 돼지에서 분리한 Bordetella bronchiseptica의 약제 감수성 (Antimicrobial Drug Susceptibility of Bordetella bronchiseptica Isolated from Youngnam Swine Herds)

  • The in vitro drug susceptibility of 83 strains of Bordetella bronchiseptica recovered from Korean pigs with atrophic rhinitis was investigated by the use of disk diffusion method. The majority of the organisms were highly resistant in order of prevalence to penicillin(98.7%), ampicillin(91.5%), streptomycin(90.3%), triple sulfa(83.1%), and trimethoprim/sulfamethoxazole(70.7%) while none of them were resistant to gentamicin, only 3.6% to colistin, chloramphenicol and kanamycin and 6.0% to tetracycline. The percentage of the organism resistant to bicozamycin, cephalothin and neomycin were 34.9%, 34.1% and 18.4%, respectively. A high prevalence of multiple drug resistance was observed and the 3 most common resistant patterns among 35 patterns noted were Am Pc Sm Sss Sxt(26.5%), Am Cf Pc Sm Sss Sxt(12.%) and Am Bm Pc Sm Sss Sxt(9.6%) patterns.

  • The present study was conducted to investigate biochemical properties and antimicrobial drug susceptibilities of 36 strains of Pasteurella haemolytica(P haemolytica) isolated from pneumonic calves in Kyongbuk province during the period from January 1990 to December 1990. P haemolytica was isolated from 36 of 111(32.4%) pneumonic calves of 1 to 6 months of age. The majority of biochemical and cultural properties of P haemolytica isolated from calves were identical to those of the reference strains employed. All isolated were susceptible to baytril, gentamicin, and kanamycin, some of them were resistant to tetracycline, amikacin, streptomycin, and ampicillin.

  • In recent years, in spite of medical advancement, tuberculosis (TB) remains a worldwide health problem. Although many laboratory methods have been developed to expedite the diagnosis of TB, delays in diagnosis remain a major problem in the clinical practice. Because of the slow growth rate of the causative agent Mycobacterium tuberculosis, isolation, identification, and drug susceptibility testing of this organism and other clinically important mycobacteria can take several weeks or longer. During the past several years, many methods have been developed for direct detection, species identification, and drug susceptibility testing of TB. A good understanding of the effectiveness and practical limitations of these methods is important to improve diagnosis. This review summarizes the currently-used advances in non-molecular and molecular diagnostics.

고위험병원체 결핵균의 신속진단을 위한 액체배양시스템 평가 (Evaluation of Liquid Culture System in Sputum Culture and Drug Susceptibility of Mycobacterium tuberculosis)

  • 본 연구에서는 객담 내 결핵균의 배양시험과 약제 감수성 시험 방법으로서 BacT/Alert 액체 배지, Ogawa 배지 및 $L\ddot{o}wenstein$-Jensen 배지 배양법을 비교하였다. 신속 액체배양시스템은 기존의 Egg-based media (Ogawa, $L\ddot{o}wenstein$-Jensen 배지)에 비해 높은 민감도와 빠른 배양으로 결핵환자의 초기 객담 검사와 약제 감수성 시험에 널리 사용되어 왔다. 객담은 N-Acetyl-L-cystine과 4% NaOH로 처리한 후 BacT/Alert 액체 배지 시스템과 Ogawa 배지에 접종하고 배양하여 비교하였다. 결핵환자 객담 검체 135개 중 도말검사결과 양성은 95개였고, Ogawa 배지 배양결과 양성으로 확인된 수는 89개(65.9%)였으며, BacT/Alert 액체 배지에서는 97개(71.9%)가 양성으로 확인되었다. 결핵균 배양 양성 검체의 평균배양일은 Ogawa 배지에서 22.4일, BacT/Alert 액체 배지는 11.3일로 단축되는 것을 확인하였다. 약제 감수성 시험에는 32개 결핵균주를 이용하였으며, isoniazid를 첨가한 BacT/Alert 액체 배지와 $L\ddot{o}wenstein$-Jensen 배지에서의 약제 감수성 시험 검사결과 일치률은 87.5%를 보였고, rifampicin에서의 일치률은 90.6%로 나타났다.

세포 염색 방법을 이용한 결핵균 감수성 검사법 (Trial for Drug Susceptibility Testing of Mycobacterium tuberculosis with Live and Dead Cell Differentiation)

  • 연구배경 : 결핵치료에서 어려움을 주는 가장 중요한 요인의 하나가 약제 내성균에 감염된 경우이다. 근래 다제내성 결핵균의 증가는 신속한 결핵균 감수성검사방법 개발에 대한 필요성을 더욱 증가시키고 있다. 활발하게 개발이 진행되고 있는 분자생물학적 기법들도 신속한 내성여부의 구분에 많은 도움을 주고 있지만, 아직까지는 검사약제가 제한되어 있고 보완할 점들이 많이 남아있다. 따라서 저자들은 결핵균 세포 염색 방법에 의해 생균과 사균을 구분할 수 있는 신속하고도 정확한 결핵균 약제 감수성 검사 방법을 검토하여 보았다. 방 법 : 본 연구의 대상으로 대표적인 4가지 항결핵 약제(Isoniazid, Rifampicin, Streptomycin, Ethambutol)에 모두 내성인 임상분리 결핵균 20 균주와 모든 약제에 감수성인 임상분리 결핵균 20 균주를 사용하였다. 약제감수성검사시의 최소 희석배수였던 MacFarland #1 탁도로부터 10배 희석한 결핵균액을 7H9 배양액 $30m{\ell}$에 접종한 후 $37^{\circ}C$에서 24시간 배양한 다음, 핵산 염색액인 Syto 9 (MolecularProbes, USA)과 세포질 염색액인 프로피디움(propidium iodide, $C_{27}H_{34}I_2N_4$)을 결핵균과 잘 섞은 후 실온의 암소에서 15 분간 방치한 후 슬라이드에 $5{\mu}{\ell}$씩 점적하여 형광 현미경으로 관찰하였다. 결 과 : 실험에 사용한 약제내성 결핵균은 4가지의 항결핵약제의 각 농도가 함유된 7H9 배양액에서 사멸하지 않고 생존하고 있음을 형광 현미경상에서 확인 할 수 있었다. 프로피디움(propidium iodide)은 살아있는 세균의 경우 세포핵은 염색시키지 못하고 세포질만 염색함으로써 살아있는 결핵균은 형광현미경 시야에서 녹색을 띄게 되고, 세포의 핵산을 염색시키는 Syto 9 은 사멸한 세포의 세포질을 통과하여 결핵 약제에 감수성인 결핵균의 세포핵을 염색시켜 형광 현미경 시야에서 붉은 색으로 관찰 되었다. 결 론 : Acridin (Syto9)과 propidium 성분을 이용하여 세포를 형광 염색시켜 세포의 사멸 및 생육을 판단하는 방법을 결핵균 약제감수성검사에 적용한 결과, 간편하고도 신속하게 24시간 이내에 내성균과 감수성균을 구분할 수 있었다. 생균과 사균 세포의 판별법으로 기존의 결핵균 감수성 방법을 대체하기 위해서는 형광현미경을 비롯한 실험실 장비와 숙련된 검사자가 필요하지만, 배양된 균으로 검사하는 데만 4주 이상 소요되는 기존의 결핵균 감수성 검사 방법을 대체할 수 있는 매우 저렴하고도 간편한 검사방법으로 판단되었다.

급성 폐혈증형 돼지단독예에서 분리한 Erysipelothrix rhusiopathiae의 생화학적 특성 및 약제감수성 (Biochemical properties and antimicrobial drug susceptibility of Erysipelothrix rhusiopathiae isolated from the cases of acute septicemic swine erysipelas)

  • The present study was conducted to investigate biochemical properties and antimicrobial drug susceptibilities of 47 strains of E rhusiopathiae isolated from the cases of acute septicemic swine erysipelas in Youngnam and Kyunggi provinces during the period from June 1988 to December 1989. The isolants were identified as E rhusiopathiae on the basis of cellular and colonial morphology, and characteristic reactions in some biochemical tests. All the organisms produced hydrogen sulfide in triple sugar iron agar and showed the characteristic "pipe cleaner" type of growth in gelatin stab cultures. The majority of biochemical and cultural properties of E rhusiopathiae isolated from pigs affected with acute erysipelas were identical to those of the reference strains employed. All the isolates were highly susceptible to penicillin G, ampicillin, erythromycin (MIC:0.025~0.39IU or ${\mu}g/ml$), and moderately susceptible to oleandomycin, oxytetracycline, chloramphenicol(MIC:$0.78{\sim}25{\mu}g/ml$). Kanamycin and sulfadimethoxine showed no activity against the isolates(MIC:>$400{\mu}g/ml$). The MICs of dihydrostreptomycin presented two distribution peaks; of 47 strains, 5(10.6%) were resistant to dihydrostreptomycin (MIC:$400{\mu}g/ml$), while the majority of them were susceptible to the drug.
