Distribution of medical status and medications in elderly patients treated with dental implant surgery covered by national healthcare insurance in Korea

  • Background: The purpose of this study was to investigate the distribution of systemic diseases and medications in patients older than 65 years of age who visited the hospital for implant treatment, as well as to investigate basic information about surgical complications that may occur after insured implant treatment. Method: A total of 126 patients over 65 years of age were treated for implant surgery from October 1, 2013 to October 30, 2016. Electronic chart review was conducted to obtain medical records, which included sex, age, systemic diseases, medication(s) being taken, and control of the medications. Five patients were excluded due to lack of medical records giving information about systemic disease. Results: Of the 126 patients, 112 (88.9%) were taking medication due to systemic disease and 9 patients (7.1%) were not. The sex distribution was 71 women and 55 men and the highest proportion of patients were between 65 and 69 years old. The most common diseases were, from most to least frequent, hypertension, cardiovascular disease, diabetes, and osteoporosis. The drug groups that can cause major complications after dental treatment were classified as hemorrhagic, osteoporotic, and immunosuppressive agents, and were taken by 45 (35.7%), 22 (17.5%) and 4 (3.2%) patients, respectively. Conclusion: Given that 88.9% of the elderly patients who were eligible for insurance implant treatment had systemic disease, it is necessary to carefully evaluate patients' medical histories and their general conditions in order to prevent emergencies during implant surgery.

푸드뱅크 기탁 조리식품의 미생물학적 위해분석 (II) (Microbiological Hazard Analysis of Cooked Foods Donated to Foodbank (II))

  • This study was conducted to estimate the safety level of non-cooking and cooking processed foods to propose the sanitary management of foods donated to foodbanks. The time and temperature were measured and the microbial levels of aerobic plate counts (APC), coliforms, E. coli, Salmonella spp., S. aureus, B. cereus, and E. coli O157:H7 were analyzed on ten food items donated to seven foodbanks. The amount of cooked foods donated to each foodbank was about 10 to 40 servings. All foodbanks hired a supervisor and had at least one refrigerator/freezer and one temperature-controlled vehicle, but only four foodbanks had the separate offices to manage the foodbank operation. The flow of donated foods was gone through the steps; production, meal service and holding at donator, collection by foodbank, transport (or holding after transport) and distribution to recipients. After production, the levels of APC of both non-cooking and cooking processed foods were complied with the standards by Ministry of Education & Human Resources Development, and were not increased till distribution. Only the level of coliforms in dried squid & cucumber salad (1.5×$10^3$ CFU/g) was not met the standards. E. coli and other pathogens were not detected in all tested samples. The microbial levels of delivery vessels and work tables were satisfactory, but the APC levels of two of four tested serving tables (6.9×$10^3$ and 5.3×$10^3$ CFU/100$cm^2$) and the coliforms level of one (1.1×$10^3$ CFU/100$cm^2$) were over the standards. The air-borne microflora level in serving room was estimated as satisfactory. It took about 3.0 to 6.5 hours from after-production to distribution and the temperatures of donated foods were exposed mostly to temperature danger zone, which had a high potential of microbial growth. These results imply that a checklist to monitor time and temperature in each step should be provided and the employees involving foodbank operation should be properly educated to ensure the safety of donated foods.

  • The clinical study was carried out the 33 patients with insomnia who were treated in Daejeon University Oriental Hospital from 17 March 1997 to 12 May 2001. The results were summarized as follows. 1. The ratio of male and female was 10:23 and sleep initiation insomnia and sleep maintenance insomnia were the highest frequence(97%), the beginning of insomnia was frequent at 30s(27.3%). 2. Living accident by mental attack was the most inducing factor and many patients came to our hospital by way of west-neuropsychiatry(55.6%). 3. In admission period most of the patients were within 15 days(78.8%) and Liver-Qi- depression was the main cause and in classification of Four Human coporeal constitution the number of patients Sho-Eum-In(少陰人) was remarked mostly and in distribution of the prescription drugs of growing heart and warming gall bladder such as GUIBIONDAMTANG(歸脾溫膽湯) and ONDAMTANGGAMI (溫膽湯加味) were many, 4. Relatively most patients were well treated(69.7%) and insomnia was mainly caused by Anxiety Disorder and Depression and Hwabyoung and in the age distribution the highest frequence was 30s but we know insomnia appeared at all ages. 5. In distribution of the period of the clinical history was various and within 15 days were all improved and the treatments group of drug and acupuncture and aid treatments etc. and hypnotics was more improved than not used hypnotics treatments group but using hypnotics was temporal. Sympatric Distribution of Three Human Taenia Tapeworms Collected between 1935 and 2005 in Korea

  • Taeniasis has been known as one of the prevalent parasitic infections in Korea. Until recently, Taenia saginata had long been considered a dominant, and widely distributed species but epidemiological profiles of human Taenia species in Korea still remain unclear. In order to better understand distribution patterns of human Taenia tapeworms in Korea, partial nucleotide sequences of mitochondrial cox1 and ITS2 (internal transcribed spacer 2) were determined, along with morphological examinations, on 68 Taenia specimens obtained from university museum collections deposited since 1935. Genomic DNA was extracted from formalin-preserved specimens. Phylogenetic relationships among the genotypes (cox1 haplotype) detected in this study were inferred using the neighbor-joining method as a tree building method. Morphological and genetic analyses identified 3 specimens as T. solium, 51 specimens as T. asiatica, and 14 specimens as T. saginata. Our results indicate that all 3 Taenia tapeworms are sympatrically distributed in Korea with T. asiatica dominating over T. saginata and T. solium.

건강 기능성 게임의 확산을 위한 유통 전략 연구: 유효성과 안전성에 대한 사용자 인식을 중심으로 (Which is the More Important Factor for Users' Adopting the Serious Games for Health? Effectiveness or Safety)

  • 게임을 통해 건강을 증진할 수 있는 건강 기능성 게임(SGHs)에 대한 관심이 고조되고 있다. 디지털 치료제(DTx)는 유효성과 안전성을 확보한 치료제이기 때문에 전통적인 의약품 유통방식을 따라야 하지만, SGHs는 웰니스 제품으로 DTx에 비해 보급과 확산이 유연하다. 또한 SGHs는 일시적인 치료 효과가 아닌 지속적인 모니터링과 피드백을 통해 맞춤형 서비스를 제공할 수 있어 효과적이며, SGHs를 인지장애치료나 행동교정 등에 적용할 경우 오작동과 부작용이 경미하다. 본 연구는 SGHs의 지각된 혜택과 위험을 동시에 고려하는 밸런스 프레임워크(Valence Framework)를 기반으로 142명의 학부생을 대상으로 설문을 수집하여 지각된 혜택만이 SGHs 사용 의도에 통계적으로 유의한 정(+)의 영향을 준다는 점을 실증하였다. 이러한 결과를 바탕으로 일반 사용자를 대상으로 한 SGHs 수용에 있어 혜택(유효성)을 중심으로 홍보하고, 데이터 주도형 (data-driven) 접근방법의 중요성과 데이터 주도형 유통 및 분석 플랫폼 전략의 필요하다는 시사점을 제시하였다.

방제드론 전용노즐의 유효살포폭 내 액적분포 및 수치해석 시뮬레이션 (Simulation of The Effective Distribution of Droplets and Numerical Analysis of The Control Drone-Only Nozzle)

  • 최근 농업분야의 스마트 농기계로 분류되고 있는 방제드론은 농촌지역의 고령화 시대를 맞이하여 작업 시간 단축과 방제효과를 높이기 위해 하드웨어 및 소프트웨어를 결합하여 스마트 방제 및 자동방제 시스템 구축을 위해 노력하고 있다. 본 논문에서는 관리관제 및 자동방제 시스템 구축을 위한 기초연구로 방제드론 전용노즐의 특성을 분석하였다. 다양한 드론 모델의 종류, 방제사, 바람, 비행속도, 비행고도, 날씨 조건, UAV 농약 종류 등 다양한 변수를 고려하기 위해서는 노즐의 특성파악과 범용성을 고려하여 약제살포 기준 제시가 가능한 관련 연구가 필요하다. 따라서 다양한 변수 고려가 가능하도록 자체 설계노즐을 기반으로 유동해석(CFD) 시뮬레이션을 실시하고 감수지 실험을 통하여 액적분포의 이론값과 실험값을 비교 분석하였다. 추후 드론운용에 따른 다양한 변수를 고려하여 정확한 비산을 산출하고 관리관제 및 자동방제 시스템에 활용하고자 한다.

Pharmacokinetic and Pharmacodynamic Characterization of Gliclazide in Healthy Volunteers

  • Pharmacokinetic and pharmacodynamic properties of gliclazide were studied after an oral administration of gliclazide tablets in healthy volunteers. After an overnight fasting, gliclazide tablet was orally administered to 11 volunteers; Additional 10 volunteers were used as a control group (i.e., no gliclazide administration). Blood samples were collected, and the concentration determined for gliclazide and glucose up to 24 after the administration. Standard pharmacokinetic analysis was carried out for gliclazide. Pharmacodynamic activity of the drug was expressed by increase of glucose concentration ($\Delta$PG), by area under the increase of glucose concentration-time curve ($AUC_{$\Delta$PG}$) or by the difference in increase of glucose concentration ($D_{$\Delta$PG}$) at each time between groups with and without gliclazide administration. Pharmacokinetic analysis revealed that $C_{max}, T_{max}$, CL/F (apparent clearance), V/F (apparent volume of distribution) and half-life of gliclazide were $4.69\pm1.38 mg/L, 3.45\pm1.11 h, 1.26\pm0.35 L/h, 17.78\pm5.27 L, and 9.99\pm2.15 h$, respectively. When compared with the no drug administration group, gliclazide decreased significantly the $AUC_{$\Delta$PG}$ s at 1, 1.5, 2, 2.5, 3 and 4 h (p<0.05). The $\Delta$PGs were positively correlated with $AUC_{gliclazide}$ at 1 and 1.5 h (p<0.05), and the correlation coefficient was maximum at 1 h (r = 0.642) and gradually decreased at 4 h after the administration. The $AUC_{$\Delta$PG}$s were positively correlated with $AUC_{gliclazide}$ at 1, 2, 3 and 4 h (p<0.05), and the maximum correlation coefficient was obtained at 2 h (r=0.642) after the administration. The $D_{$\Delta$PG}$ reached the maximum at 1 h, remained constant from 1 h to 3 h, and decreased afterwards. Therefore, these observations indicated that maximum hypoglycemic effect of gliclazide was reached at approximately at 1.5 h after the administration and the effect decreased, probably because of the homeostasis mechanism, in health volunteers.

Synergistic Effects of Exemestane and Aspirin on MCF-7 Human Breast Cancer Cells

  • Objective: The purpose of this study is to investigate the combined effects of exemestane and aspirin on MCF-7 human breast cancer cells. Methods: Antiproliferative effects of exemestane and aspirin, alone and in combination, on growth of MCF-7 human breast cancer cells were assessed using the MTT assay. Synergistic interaction between the two drugs was evaluated in vitro using the combination index (CI) method. The cell cycle distribution was analyzed by flow cytometry and Western blotting was used to investigate the expression of cyclooxygenase-1, cyclooxygenase-2 and Bcl-2. Results: MTT assays indicated that combination treatment obviously decreased the viability of MCF-7 human breast cancer cells compared to individual drug treatment (CI<1). In addition, the combination of exemestane and aspirin exhibited a synergistic inhibition of cell proliferation, significantly arrested the cell cycle in the $G_0/G_1$ phase and produced a stronger inhibitory effect on COX-1 and Bcl-2 expression than control or individual drug treatment. Conclusion: These results indicate that the combination of exemestane and aspirin might become a useful method to the treatment of hormone-dependent breast cancer. The combination of the two inhibitors significantly increased the response as compared to single agent treatment, suggesting that combination treatment could become a highly effective approach for breast cancer.

의약품 해외 수출을 위한 한국 제약산업의 해결과제 (Barriers Impeding Domestic Pharmaceutical Industry's Exportation to Developed Countries)

  • Background: For Korean pharmaceutical industry to continue to grow, it is requisite to enter the global markets of developed countries. However, the export volume has fallen short of 10% of the gross sales and the industry has only recently warming up to prepare the globalization along with suchlike the Columbus Project. Purpose: This research was conducted to identify the difficulties Korean pharmaceutical companies perceived and to discover the gap in the needs for the government aid the companies have been seeking in the purpose of entering the developed pharmaceutical markets. Method: A survey method was used for this research. Six experts were surveyed and provided comments for the pre-questionnaire. Then, a final questionnaire was developed consisting of 10 items on regulatory-related and another 10 items on non-regulatory-related factors in drug exportation using the Likert scale (1 to 5). The survey sample was 30 Korean companies which have participated in the Columbus Project since 2010. Results: Nineteen (63%) companies responded to the survey. Most companies perceived difficulty (mean = 4.19) over the entire pathway of the regulatory process of global markets. Clinical trials and post-marketing surveillance were remarked as the most difficult barrier to follow the regulatory globalization. Among non-regulatory related factors, marketing, arranging a distribution network, obtaining experts, and projecting a timeline in exportation were brought up as the most difficulty. Conclusion: Especially, cost and language barrier were considered as the main cause producing these difficulties across regulatory and non-regulatory processes and accordingly, securing both long term budget and experts at governmental level was suggested by the domestic pharmaceutical companies.

Prediction of Pharmacokinetics and Penetration of Moxifloxacin in Human with Intra-Abdominal Infection Based on Extrapolated PBPK Model

  • The aim of this study is to develop a physiologically based pharmacokinetic (PBPK) model in intra-abdominal infected rats, and extrapolate it to human to predict moxifloxacin pharmacokinetics profiles in various tissues in intra-abdominal infected human. 12 male rats with intra- abdominal infections, induced by Escherichia coli, received a single dose of 40 mg/kg body weight of moxifloxacin. Blood plasma was collected at 5, 10, 20, 30, 60, 120, 240, 480, 1440 min after drug injection. A PBPK model was developed in rats and extrapolated to human using GastroPlus software. The predictions were assessed by comparing predictions and observations. In the plasma concentration versus time profile of moxifloxcinin rats, $C_{max}$ was $11.151{\mu}g/mL$ at 5 min after the intravenous injection and $t_{1/2}$ was 2.936 h. Plasma concentration and kinetics in human were predicted and compared with observed datas. Moxifloxacin penetrated and accumulated with high concentrations in redmarrow, lung, skin, heart, liver, kidney, spleen, muscle tissues in human with intra-abdominal infection. The predicted tissue to plasma concentration ratios in abdominal viscera were between 1.1 and 2.2. When rat plasma concentrations were known, extrapolation of a PBPK model was a method to predict drug pharmacokinetics and penetration in human. Moxifloxacin has a good penetration into liver, kidney, spleen, as well as other tissues in intra-abdominal infected human. Close monitoring are necessary when using moxifloxacin due to its high concentration distribution. This pathological model extrapolation may provide reference to the PK/PD study of antibacterial agents.
