Preimplantation genetic diagnosis (PGD) has become an assisted reproductive technique for couples who are at risk that enables them to have unaffected baby without facing the risk of pregnancy termination after invasive prenatal diagnosis. The molecular biology and technology for single-cell genetics has reached an extremely high level of accuracy, and has enabled the possibility of performing multiple diagnoses on one cell using whole genome amplification. These technological advances have contributed to the avoidance of misdiagnosis in PGD for single gene disorders. Polymerase chain reaction (PCR)-based PGD will lead to a significant increase in the number of disorders diagnosed and will find more widespread use, benefiting many more couples who are at risk of transmitting an inherited disease to their baby. In this review, we will focus on the molecular biological techniques that are currently in use in the most advanced centers for PGD for single gene disorders, including biopsy procedure, multiplex PCR and post-PCR diagnostic methods, and multiple displacement amplification (MDA) and the problems in the single cell genetic analysis.
Purpose : To evaluate the treatment outcome for patients with locally advanced, unresectable esophageal cancer treated with relatively high dose radiation therapy(RT). Materials and Methods : From January 2000 to December 2008, 32 patients with locally advanced unresectable or medically inoperable esophageal cancer were treated with radiation therapy(RT) with or without concurrent chemotherapy. Ten patients were excluded from analysis because of distant metastasis and drop off. Patient distributions according to AJCC stages II, III IVa were 7(31.8%), 12(54.6%), 3(13.6%) respectively. The locations of tumor were cervical/upper thorax 3 (13.6%), mid thorax 13(59.1%), and lower thorax/abdominal 6(27.3%), respectively. Eleven patients received RT only, and 11 patients received cisplatin based concurrent chemoradiotherapy(CCRT). Median radiation dose was 65 Gy(range 57.6~72 Gy). Results : The median follow-up was 9.1 months(range 1.9~43.8 months). The response rates for complete response, Partial response, stable disease and Persistent disease were 6(27.3%), 11(50.0%), 4(18.2%) and 1(4.5%), respectively. Two patients(9.1%) suffered from esophageal stenosis and stents were inserted. Two patients(9.1%) had Grade 3 radiation pneumonitis and one of them expired due to acute respiratory distress syndrome(ARDS) at 36 days after completion of radiation therapy. The recurrence rate was 11(50.0%). The patterns of recurrence were persistent disease and local progression in 5(22.7%), local recurrence 3(13.7%) and concomitant local and distant recurrence in 3(13.7%). The overall survival(OS) rate was 32.1% at 2 years and 21.4% at 3 years(median 12.0 months). Disease free survival(DFS) rate was 17.3% at 2 and 3 years. All patients who had no dysphagia at diagnosis showed complete response after treatment and 100% OS at 3 years(p=0.0041). The OS for above 64.8 Gy group and 64.8 Gy or below group at 3 years were 60.6% and 9.1%(p=0.1341). The response to treatment was the only significant factor affecting OS(p=0.004). Conclusion : Relatively high dose radiation therapy in unresectable esophageal cancer tended to have a better outcome without increased complication rate. Further study with more patients is warranted to justify improved result.

Background: In July 2016, the Infectious diseases society of america and the american thoracic society (IDSA & ATS) published a guideline recommending piperacillin/tazobactam (Pip/Tazo) 18 g/day as the anti-pseudomonal dose for the treatment of pathogenic pneumonia. After the guideline was published, the Pip/Tazo dose used for the treatment of pathogenic pneumonia was changed from 13.5 g/day to 18 g/day in a superior general hospital intensive care unit (ICU). In this study, we analyzed the effectiveness and safety of the new dose. Methods: Adult patients aged ${\geq}19years$ who were diagnosed with pneumonia in ICU and who received Pip/Tazo for 7 days or more from September 1, 2015 to May 31, 2017 were included in the study. The electronic medical record (EMR) was retrospectively analyzed. Results: At baseline, there was a significant difference between 44 patients treated with 13.5 g/day and 31 patients treated with 18 g/day of Pip/Tazo. The 18 g/day-treatment group comprised more elderly patients than the 13.5 g/day-treatment group (p=0.028). The results of the treatment-effects analysis showed no significant difference between the two groups. In case of safety data, there were significant differences in two parameters related to blood count, namely hemoglobin (p=0.016) and platelet count (p=0.011). Conclusion: Based on the significant difference in baseline age, there is a possibility that high-dose Pip/Tazo showed improved therapeutic effect. However, when high-dose Pip/Tazo was used, the blood cell count was found to drop from the reference value more frequently. Therefore, blood cell count should be monitored carefully when high-dose Pip/Tazo is administered.

The present study makes an investigation into expression of genes related to cardiac development in chicken, quail and chicken-quail hybrids during the early stage of embryogenesis. Real-time PCR was used to detect mRNA expressions of Nkx2-5, GATA4 and TBX5 in the heart of chicken, quail and chicken-quail hybrids embryos during the 3rd to 7th days of incubation. Results showed that NKX2-5 mRNA displayed a similar expression trend in chicken, quail and chicken-quail hybrids. The initial and highest expression of Nkx2-5 was focused on the 3rd day of incubation, then it declined till 5th day of incubation, thereafter, it fluctuated. Expression of Nkx2-5 gene in quail was significantly higher than in chicken and chicken-quail hybrids, and no significant difference was observed between the two latter species. GATA4 mRNA showed a similar expression trend between chicken and quail, which displayed a steady increase from 3rd to 6th d, then, the expression level decreased. However, GATA4 mRNA expression in chicken-quail hybrids was significantly higher than that in chicken and quail from 3rd to 5th d (p<0.01), but significantly lower than that in chicken and quail during the later stage of the experiment (p<0.05), due to the dramatic drop from 5th d onwards (p<0.01). TBX5 mRNA expression in chicken and quail showed the same trend as GATA4 expressed in the two species. Furthermore, TBX5 expression in chicken-quail hybrids was significantly higher than that in chicken and quail during the whole course of experiment, although relatively lower TBX5 expression was detected in the early stage. In conclusion, Nkx2-5, GATA4 and TBX5 genes showed dynamic changes during the process of cardiac development in chicken, quail and their hybrids embryos. In addition, the expression trend in chicken was similar to that in quail, and there was no significant difference for gene expression level, except NKX2-5. However, expression of these genes in chicken-quail hybrids was significantly different from their parents, the difference mechanism needs to be further explored.

The purpose of this study is to investigate the types of domestic safety accidents involving children and to verify the difference of safety knowledge according to variables of parents. Subjects include 226 parents who have 3-5 year old infants enrolled in kindergartens and day care centers in I-city, Jeollabuk-do. The study tools are modified and complemented questionnaire for the study purpose referred to the preceding research data and the literature. For the processing of collected data, frequency analysis, descriptive statistics, one-way ANOVA, and the Duncan post-test were utilized using the SPSS program. The results of this study are as follows: First, safety accidents occurred at the majority of homes which have infant children. On average, the occurrence frequency was 1.78 times per six months. The accidents involving infants occurred in the order of drop or slip, crash or impact, stricture or jamming, fall, stab or cut, burn, swallow or inhalation. Second, both parents had an average level of safety knowledge. There was a distinction in the areas of 'fall' and 'first aid' according the mother's age. In addition, stay-at-home mothers had the higher level of safety knowledge compared with working mothers in the areas of 'general safety knowledge' and 'fire'. There was a distinction in the area of 'fire' according to the father's age. Given these findings, safety training programs for parents to improve safety knowledge needs to be developed.
Polyetherimide (PEI) membranes for separators in Zn air batteries were prepared via phase inversion process from casting solution composed of PEI, n-methylpyrolidone (NMP), and polyvinylpurrolidone (PVP). Furthermore, Zn air batteries were fabricated with the separators. The effects of PEI content and PVP addition in the casting solution on the morphology, mechanical strength, ionic conductivity were investigated through SEM, stress-strain test and ac impedance test. The elelctrochemical performances of the batteries were evaluated through galvanostatic discharge analysis. The mechanical strength of the membrane increased with increasing PEI composition in the casting solution. Little effect of PVP addition into the solution on the mechanical strength of the membrane was investigated. The ionic conductivity value decreased with increasing PEI composition in the solution. With addition of PVP, ionic conductivity of membrane increased until 10 wt% to show the maximum value of 0.1 S/cm. In the higher range of PVP addition over 10%, the ionic conductivity decreased with increasing PVP addition. Ionic conductivity of separator strongly affected the capacity of Zn air battery, and the battery assembled with the separator which showed high ionic conductivity showed high capacity.

This study examined the moral judgments and ethical decision-making and analyzed ethical dilemmas experienced in practice by nursing students. The data were collected using self-reported questionnaires and reports from 189 college students in their 2nd ~ 4th grades. Data collection period was from Nov 6th - 20th in 2012. Each grade's P(%) scores were 47.92, 43.74, and 43.75 respectively.. For stage 4 score, each grade's scores were 22.37, 22.98, and 19.74. This result shows that, compared to juniors, senior student's P(%) scores did not drop and stage 4 scores rather decreased, which is in line with the results of previous studies about the effects of ethics education. This finding could be attributed to the regular and voluntary case presentations and discussions required for the senior participants. Regarding ethical decision-making type, the most favored was type 3(35.45%). Among seven categories of the ethical dilemmas facing the students, the most commonly reported was the patient's rights and dignity, followed by practices based on nursing standards, arranging for dying patients. This study raises the need that the curriculum is revised to encourage student's participation in the analysis of ethical issues they confront in the field.

DELAY M9 is an ammunition used in combination with the fuse M904 and M905 of aviation drop-off ammunitions. Due to the properties of the ammunition, the performance or safety of the ammunition cannot be verified until it is used, because of its one-time function, no reproduction and is finally destroyed at the same time. Long-term storage of ammunition in use by the army is inevitable due to its operational characteristics, and a test evaluation is required to maintain and verify the performance of stored ammunition for immediate use in case of emergency. This paper estimates the reliability and shelf life of the DELAY M9 ammunition by using the test data obtained through ASRP performed from the past to the present. The result of the performance evaluation test for the currently stored DELAY M9 ammunition is analyzed, and the reliability and shelf-life of the stored ammunition are assessed by examining the number of defects according to the storage time and production year. This research result can be used to manage the stored ammunition, such as ASRP planning and evaluation test, result analysis and munition state decision.
This study was carried out to(1) visualize the installation effect of an anti-wind net; (2) evaluate structural stability of typical anti-wind nets in Jeju; and (3) present the optimal specification of pipes in an anti-wind net for maximum instant wind velocities of 40 m/s and 45 m/s. The analyses were done for anti-wind nets with a mesh of 4 mm and a height of 3 m by using CFX and ANSYS. The results showed that the wind went down due to flow resistance when passing through an. anti-wind net. The anti-wind net with the supporting pipe being installed every two main columns was certainly unstable because the main column not sustained by the supporting pipe became cantilever. With regard to the position of a fixing point of the supporting pipe, von Mises stress on pipes was certainly increased as vertical positions of the supporting pipe were changed to be too lower or higher than an adequate position but there was little difference according to horizontal positions. The adequate vertical position was $2{\sim}2.5\;m$ high from the ground. For a maximum instant wind velocity of 40 m/s, the optimal specification of pipes was a main column of ${\varphi}48.1{\times}2.1$ t@2,000, cross beams(bottom and top) of ${\varphi}26.7{\times}1.9\;t$, cross beams(center) of ${\varphi}33.5{\times}2.1$ t/2ea and a supporting pipe of ${\varphi}31.8{\times}1.5$ t@2,000. In case of a maximum instant wind velocity of 45 m/s, the optimal specification of pipes with structural stability was a main column of ${\varphi}48.6{\times}3.25$ t@2,000, cross beams(bottom and top) of ${\varphi}26.7{\times}1.9\;t$, cross beams(center) of ${\varphi}48.1{\times}2.1$ t/2ea and a supporting pipe of ${\varphi}31.8{\times}1.5$ t@2,000.
Purpose: The purpose of this study is to identify the changes to nursing activities of nurses on patients with DNR (Do-Not-Resuscitate) order and factors associated to the changes. Methods: Data were collected using a structured questionnaire for 173 nurses at general hospitals. Logistic regression analysis was performed on the data using SAS 9.4. Results: With 39 nursing activities, an average of 60.4 (34.9%) nurses reported an increase in the activities, 102.4 (59.2%) no change and 10.1 (5.9%) a drop. The activity increase was the greatest in the social area, and the physical area was where the activities decreased the most. The activity increase was associated knowledge competency (9 items), attitudes (2 items), practical competency (4 items) and work load (14 items were). Conclusion: To offer systematical care for DNR patients, it is necessary to expand nurses' knowledge through end-of-life education and adjust their workload and provide a support system at the department level.
