U-Reuk, a laureate musician in the days of King Jin-Heung, Silla Dynasty, built the Eui Rim Reservoir about 1,400 years ago. This was one of the oldest man-made reservoir. The embankment of the reservoir collapsed by the clumsy artificial drainage on August 19, 1972 when the heavy rainfall of 462 mm/day caused a dangerous overflow of the reservoir. The result of the study on the mystic ancient embankment techniques are as follows: 1. Sandy loam derived from the weathering of granite which is the most widely distributed rock type in the area, was used in the embankment. Large size logs (embankment core) of 30-50 cm in diameter were buried lengthwise along the embankment. 2. The six stocks of Pinus densiflora, 3 stocks of Quercus acutissima, 1 stock of Quercus variabilis and 1 stock of Popolus maximowiczii, altogether 11 stocks are identified. Forest types in the nearby area during the days of the reservoir construction seem to be includde a considerable number of Pinus densiflora, Quercus and Populus species. 3. The angle of repose of the earth materials is taken into account during the embankment. On top of the embankment double layers of clay (20-30 cm indepth) were spread and consolidated. Layer of litter of 20-40 cm in deep covered on the clay layer of the embankment completely, and another layer of clay was consolidated over the litter. Finally, a layer of stones of 10-30 cm in diameter and clay (yellow soil layer) toped the embankment. 4. At the lower part of the embankment clay layer was thicker and became thinner as it goes upwards. At every layer, soil was consolidated and burned. When embankment was completed, it was covered evenly with heavy clay, and finally it was topped with general soil. 5. The heavy clay layer on the inner slope of the embankment showed gray phenomena and litter remained raw humus layer. The clay layer functioned as rubber in the water, and raw humus layer prevented water from seepages. Thus, the embankment was solidly built in this way. 6. The ancient embankment techniques used soils conveyed from nearby area taking the angle of repose into account. Once embankment was completed, clay and litter layers were added to have a plasticity and to withstand the water pressure. 7. It is an excellent technique that the reservoir was constructed with least labor for maximum effects while the recent embankment techniques requires a large amount of labor.

Purpose: Sentinel lymph node (SLNB) is the first confronted lymph node from primary lesion of tumor through lymphatic drainage, which is important for determining early metastasis and setting guidelines for treatment. We reported significant of sentinel lymph node biopsy in malignant melanoma (MM) and squamous cell carcinoma (SCC) of lower extremities. Materials and Methods: Twenty-five cases of surgically treatment and being possible for follow up more than 1 year among the patients who were diagnosed as MM and SCC in this institution from Sep. 2005 to Jan. 2009, and 10 cases of them were performed SLNB. Average age was 64 years old, and 15 cases of male and 10 cases of female were in this group. Results: 3 years overall survival rate was 100% and 3 years disease-free survival rate was 76%. Metastasis occurred in total 6 patients, 4 cases of inguinal lymph nodes, 1 case of soft tissue around knee, 1 case of left achilles tendon. In 15 cases of not performing SLNB, overall survival rate was 93.3% and disease-free survival rate was 73.3%. In 10 cases of performing SLNB, overall survival rate was 100% and disease-free survival rate was 90%. And only 1 case showed positive finding in the biopsy, and none of the 10 cases showed metastasis in follow-up. Conclusion: SLNB leads simpler and less complications compared to prior elective lymph node dissection, and shows high degree of accuracy. Throughout the SLNB, setting guidelines for treatment by accurate staging is thought to be helpful for increasing the survival rate in the patient with MM and SCC.
Purpose: Intra-abdominal fluid collection is one of the risk factors associated with postoperative morbidity or mortality for patients who undergo gastric cancer surgery. The aim of this study was to analyze the clinicopathological characteristics of the patients with postoperative intra-abdominal fluid collection and to identify the indications for inserting a percutaneous drain (PCD) in patients with intra-abdominal fluid collection. Materials and Methods: Among the 1,277 patients who underwent operations for gastric cancer at Seoul National University Hospital between April 2005 and July 2006, the data of 117 patients with an intra-abdominal fluid collection were reviewed. Results: The number of patients' with pathologic stage I, II, III and IV disease was 42 (36.8%), 23 (20.2%), 16 (14%) and 33 (28.9%), respectively. Forty-three patients (36.3%) underwent PCD insertion and the other 43 patients received conservative management. A univariate analysis of multiple clinical variables revealed that age, gender, diabetes, liver disease, lymph node dissection, the pathologic stage and the body mass index (BMI, $kg/m^2$) were not significantly associated with PCD insertion (P>0.05). However, the univariate analysis showed that two characteristics were associated with a significantly high incidence of PCD insertion: a diameter of an intra-abdominal fluid collection greater than 4 cm and infectious signs such as leukocytosis, fever and bacteremia. Conclusion: About two thirds of the intra-abdominal fluid collections after surgery for gastric cancer were managed with only conservative method without other morbidities of mortality. Surgeons should consider performing PCD insertion if the largest diameter of an intra-abdominal fluid collection is over 4 cm or if infectious signs are seen.

BACKGROUND: Most of the tailings have been left without any management in abandoned metalliferous mines and have become the main source of heavy metal contamination for agricultural soils and crops in the these areas. METHODS AND RESULTS: This experiment was carried out to investigate the assessment of the heavy metal contamination in paddy soils located on downstream of the closed metalliferous mine. The average total concentrations of cadmium (Cd), copper (Cu), lead (Pb), zinc (Zn), and arsenic (As) in paddy soils were 8.88, 56.7, 809, 754, and 37.9 mg/kg, respectively. Specially, the average concentrations of Cd, Pb and Zn were higher than those of warning criteria for soil contamination(4 mg/kg for Cd, 200 mg/kg for Pb, and 300 mg/kg for Zn) in agricultural soil established by Soil Environmental Conservation Act in Korea. The proportions of 0.1 M HCl extractable Cd, Cu, Pb, Zn, and As concentration to total concentration of these heavy metals in paddy soils were 27.7, 21.3, 35.1, 13.8 and 10.5%, respectively. The pollution index of these five metals in paddy soils ranged from 0.42 to 11.92. Also, the enrichment factor (EFc) of heavy metals in paddy soils were in the order as Cd>Pb>Zn>Cu>As, and the enrichment factor in paddy soil varied considerably among the sampling sites. The geoaccumulation index (Igeo) of heavy metals in soils were in the order as Cd>Pb>Zn>Cu>As, specially, the average geoaccumulation index of Cd, Pb, and Zn (Igeo 2.49~3.10) were relatively higher than that of other metals in paddy soils. CONCLUSION: Based on the pollution index, enrichment factor, and geoaccumulation index for heavy metal in paddy soils located on downstream of closed metalliferous mine, the main contaminants are mine waste materials and mine drainage including mine activity.
Background/Aim : Common bile duct (CBD) stones may cause jaundice, cholangitis, or pancreatitis. Extracorporeal shock wave lithotripsy (ESWL) may be needed whenever endoscopic procedure are failed to extract common bile duct stones. The aim of this study is to provide the standard for patient's best choice on ESWL for treatment of CBD stones resistant to endoscopic extraction. Materials and Methods : Fourty-six patients failed in endoscopic stone extraction including mechanical lithotripsy were treated by ESWL. In all patients, endoscopic sphincterotomy and nasobiliary drainage tube was done before ESWL using the ultrasonography for stone localization with a spark-gap type lithotriptor. Patients were sedated with an intravenous injection of 50 mg of Demerol. None were treated under general anesthesia. Results : Overall complete clearance rate of CBD stone was 89.1% (41/46). In 82.6% of the patients, the stones were extracted endoscopically after ESWL, and spontaneous passage was observed in 6.5%. In the clearance rate after ESWL, there were no noticeable differences with regard to number (single: 82.8%, two or three: 100%, more than three: 100%) and size of the stone (less than 33mm: 92.9%, 33 mm or larger: 83.3%), whereas there were significant differences with regard to the ratio of sum of long-axis length of the all stones to sum of long-axis length of the CBD excluding stone (1:2.4, 1:2.1) and diameter of the largest stone to diameter of CBD excluding stone (1:0.9, 1:0.4) for patients with complete clearance compared with those without. Conclusion : We propose that stones without the fragments are travelable sufficient space in CBD or extractable sufficient diameter of CBD regardless of stone size and number should be treated by other technique to prevent time and cost consuming, such as percutaneous transhepatic cholangioscopylithotomy.
Soil-steel structures have been used for the underpass, or drainage systems in the road embankment. This type of structures sustain external load using the correlations with the steel wall and engineered backfill materials. Buried flexible conduits made of corrugated steel plates for the coastal road was tested under vehicle loading to investigate the effects of live load. Testing conduits was a circular structure with a diameter of 6.25m. Live-load tests were conducted on two sections, one of which an attempt was made to reinforce the soil cover with the two layers of geo-gird. Hoop fiber strains of corrugated plate, normal earth pressures exerted outside the structure, and deformations of structure were instrumented during the tests. This paper describes the measured static and dynamic load responses of structure. Wall thrust by vehicle loads increased mainly at the crown and shoulder part of the conduit. However additional bending moment by vehicle loads was neglectable. The effectiveness of geogrid-reinforced soil cover on reducing hoop thrust is also discussed based on the measurements in two sections of the structure. The maximum thrusts at the section with geogrid-reinforced soil cover was 85-92% of those with un-reinforced soil cover in the static load tests of the circular structure; this confirms the beneficial effect of soil cover reinforcement on reducing the hoop thrust. However, it was revealed that the two layers of geogrid had no effect on reducing the overburden pressure at the crown level of structure. The obtained values of DLA decrease approximately in proportion to the increase in soil cover from 0.9m to 1.5m. These values are about 1.2-1.4 times higher than those specified in CHBDC.

This study was carried out to investigate the construction characteristics and the landscaping design skill for Wiyiam at Ongnyucheon(玉流川) in the rear garden of Changdeok Palace focusing on constructing technique, space composition and transformational process of the site. The results of this study were summarized as follows; First, Wiyiam of Ongnyucheon was constructed as garden ornament which was modified the huge bedrock into the shape of the mountain. The waterfall of Wiyiam was originally made of torrent which water flowed into the bottom, but it was reconstructed to take the effect of Inak(離落: the method to drop water from high place) by carving rock into square shape at the era of King Gojong(高宗). Second, main characteristics of Wiyiam appeared in Donggwoldo was the hill built up with a square pond and ground at the back of Wiyiam and the profound scenery of mountains and stream from the view of Wiyiam. Also, pavilions such as Soyojeong(逍遙亭), Cheongujeong and Taegeukjeong(太極亭) built in Ongnyucheon seemed to be constructed as the spots to appreciate the view and waterfall sound of Wiyiam. Also the spots were not bounded by the function of creating special water systems such as Cheongujeong pond and Taegeukjeong pond and showed the outstanding landscape design skill to make people feel unusual interest from each spot. On the other hand, this study considered that the evening scenery of Wiyiam, beautiful sound of falling water, unusual water system with beautiful plant materials were landscaped for the function of the psychological peace and stability to the appreciators. Lastly, the extreme change of space composition in Ongnyucheon was assumed as a strategy to improve the existing poor drain environment by confirming that the mountain stream and wall of Soyojeong were removed and drainage in both side of Soyojeong was installed in the 21th year of King Gojong's reign(1884).

Darangnon in Gachon Village, Namhae-gun, Gyeongsangnam-do, is the first designated environment among Korea's industrial-based historical and cultural sites and is still the only one that involves agricultural activities. Although the stonework, which is the infrastructure environment here, has limitations that inevitably lead to natural and artificial deformation due to its nature, there has been no research to consider the situation so far. As of the end of May 2020, this study investigated and analyzed the damage in the sub-area of the survey, which is approximately 30% of the scope of the designation of the scenic spot, from a quantitative and qualitative perspective. As a result of the study, the state of loss, which reveals the physical damage of the arctic rice paddy stone retaining wall in the environment under investigation, was particularly serious around the coast, in the northern area with high slopes, and near tourist information centers and parking lots. On the other hand, the qualitative aspect of the damage to the stonework was noticeable in the repair of heterogeneous materials on the stone retaining wall adjacent to the village and parking lot, and the landscape damage caused by the cladding of plants was found in an environment far away from the residence. In addition, natural environmental factors such as slopes, elevations, and soil showed a close relationship with the degree of physical damage of the stone retaining wall, the higher the slope, the higher the elevation, and the better the soil drainage, the greater the impact. These results suggest that humanities environmental factors such as cultivation activities and management entities have important factors in the physical damage and management of stone retaining wall. Therefore, it is deemed essential to find management measures with local residents along with improving the agricultural environment, such as securing agricultural water and soil improvement, for the preservation of tuna paddies and stone retaining wall in the future.

Purpose: Septic arthritis of the knee is an orthopedic emergency that requires early diagnosis and surgical treatment. This study examined the effectiveness of drain insertion and irrigation in the treatment of septic arthritis of the knee under local anesthesia. Materials and Methods: A retrospective study was conducted on nine cases (eight patients) diagnosed with septic arthritis of the knee from September 2017 to February 2020 and treated with drain insertion and irrigation under local anesthesia. After penetrating through the superolateral portal to the superomedial portal and inserting the drain, daily irrigation of approximately 3 L of normal saline was done. The following were investigated: age, sex, underlying disease, cause, degree of osteoarthritis, time from diagnosis to surgery, duration of hospitalization, duration of normalization of C-reactive protein, and smear and culture. Results: The initial white blood cell count of joint fluid was 71,472±51,667/mm3 (32,400-203,904/mm3), and polymorphic leukocytes were 91.1%±2.6% (86%-95%). The average time from diagnosis to surgery was 8.3±1.3 hours (6-10 hours), and the irrigation period was 8.2±3.2 days (4-15 days). The average length of hospitalization was 20.8±8.7 days (9-37 days). There was no reoperation or recurrence. Smear and culture tests were not identified. Conclusion: In the treatment of septic arthritis of the knee, the insertion of a drain tube and irrigation under local anesthesia is a relatively fast and simple method to reduce pain by repetitive draining of purulent joint fluid and can be used as an alternative treatment for patients with a risk of general or spinal anesthesia.

Purpose To evaluate the usefulness and effectiveness of bronchial occluders in the treatment of postoperative bronchopleural fistula (BPF). Materials and Methods The subjects of the study were six out of seven postoperative BPF patients who underwent surgery due to tuberculosis or lung cancer between 2009 and 2019. Each patient had a bronchial occluder inserted to treat BPF that occurred after surgery. Of the six patients, five had lung cancers and one had tuberculosis. Five were male and one was female; their ages ranged from 59 to 74 years, with an average of 69 years. The diagnosis of BPF was based on findings from bronchoscopy and CT, and treatment was initiated approximately 1 to 2 weeks after diagnosis. The technical and clinical success of the bronchial occluders in the treatment of BPF was evaluated. The study assessed the postoperative clinical effects of the occluders, survival duration, and additional treatments. Results All six patients were successfully treated. Clinical success was achieved in five patients, while partial clinical success was achieved in one; there was no clinical failure. No complications during the migration of the device or device perforations were observed. Two patients were diagnosed with BPF by CT, while four were diagnosed by bronchoscopy. Lobectomy, bilobectomy, and pneumonectomy were performed on two patients each. The periods between surgery and diagnosis ranged from 1 to 34 months; the average was 10 months. Four patients (59-103 days; an average of 80.5 days) died and two (313 days, 3331 days) survived. The causes of death were aggravation of the underlying disease (n = 2), pulmonary edema and pleural effusion (n = 1), and pneumonia (n = 1). Additional catheter drainage was performed in one patient, and a chest tube was maintained in two patients. Conclusion Bronchial occluders are useful and effective in the treatment of BPF after pulmonary resection.
