To evaluate the effect of dose and image quality for Chest Digital Tomosynthesis(CDT) using sensitivity and tube voltage(kV). CDT images of the phantom were acquired varying sensitivity 200, 320, 400 according to set tube voltage of 125 kV and 135 kV. The dose and Dose Area Product(DAP) according to change of sensitivity and kV were evaluated and Image quality was evaluated by PSNR, CNR, SNR using Image J. Dose were lowered 14~23% less than sensitivity 200, 125 kV and DAP were lowered 13~26% less than sensitivity 200, 125 kV. PSNR were over 27 dB, which were significant value and CNR, SNR were better as sensitivity value was lower. But there were different statistical significant to each item. CNR and SNR were not statistically significant at sensitivity 320, 135 kV(P>0.05). CDT can improve image quality with lower radiation dose using better than quality and correction power at digital radiography system.

Diagnostic reference level (DRL) is employed to optimize the radiation doses of patients. The objective of this study is to review the DRLs for interventional procedures in Korea and abroad. Literature review was performed to investigate radiation dose index and measurement methodology commonly used in DRL determination. Dose area product (DAP) and fluoroscopy time within each major procedure category were systematically abstracted and analyzed. A wide variation was found in the radiation dose. The DAP values and fluoroscopy times ranged 0.01-3,081 Gy·㎠ and 2-16,878 seconds for all the interventional procedures, 8.5-1,679 Gy·㎠ and 32-5,775 seconds for the transcatheter arterial chemoembolization (TACE), and 0.1-686 Gy·㎠ and 16-6,636 seconds for the transfemoral cerebral angiography (TFCA), respectively. The DRL values of the DAP and fluoroscopy time were 238 Gy·㎠ and 1,224 seconds for the TACE and 189 Gy·㎠ and 686 seconds for the TFCA, respectively. Generally, the DRLs of Korea were lower than those of other developed countries, except for the percutaneous transluminal angioplasty with stent in arteries of the lower extremity (LE PTA and stent), aneurysm coil embolization, and Hickman insertion procedures. The wide variation in the radiation doses of the different procedures suggests that more attention must be paid to reduce unnecessary radiation exposure from medical imaging. Furthermore, periodic nationwide survey of medical radiation exposures is necessary to optimize the patient dose for radiation protection, which will ultimately contribute to patient dose reduction and radiological safety.
The author investigated interventional radiology patient doses in several other countries, assessed accuracy of DAP meters embedded in intervention equipments in domestic country, conducted measurement of patient doses for 13 major interventional procedures with use of Dose Area Product(DAP) meters from 23 hospitals in Korea, and referred to 8,415 cases of domestic data related to interventional procedures by radiation exposure after evaluation the actual effectives of dose reduction variables through phantom test. Finally, dose reference level for major interventional procedures was suggested. In this study, guidelines for patient doses were $237.7Gy{\cdot}cm^2$ in TACE, $17.3Gy{\cdot}cm^2$ in AVF, $114.1Gy{\cdot}cm^2$ in LE PTA & STENT, $188.5Gy{\cdot}cm^2$ in TFCA, $383.5Gy{\cdot}cm^2$ in Aneurysm Coil, $64.6Gy{\cdot}cm^2$ in PTBD, $64.6Gy{\cdot}cm^2$ in Biliary Stent, $22.4Gy{\cdot}cm^2$ in PCN, $4.3Gy{\cdot}cm^2$ in Hickman, $2.8Gy{\cdot}cm^2$ in Chemo-port, $4.4Gy{\cdot}cm^2$ in Perm-Cather, $17.1Gy{\cdot}cm^2$ in PCD, and $357.9Gy{\cdot}cm^2$ in Vis, EMB. Dose referenece level acquired in this study is considered to be able to use as minimal guidelines for reducing patient dose in the interventional radiology procedures. For the changes and advances of materials and development of equipments and procedures in the interventional radiology procedures, further studies and monitorings are needed on dose reference level Korean DAP dose conversion factor for the domestic procedures.
This study aims to find out geometric parameters which practitioner adjustable to reduce dose in coronary angiography. We take fluoroscopy and cine exposure by use of phantom, and got dose use the dose-area product(DAP) meter of angiography device, than convert DAP to effective dose. As results, Cine exposure shows higher dose measurement about 6-7 times than fluoroscopy. Dose in frame per second(FPS) mode could be decrease down to 70%, as lower FPS. In view of X-ray tube angle, LAO $45^{\circ}$+Caudal $30^{\circ}$ shows highest dose measurement. More use of Collimator, lower dose measurement. Source-image intensifier distance(SID) get longer to 10cm, dose of each fluoroscopy and cine exposure increase up to 25-30%. Image magnification of field of view(FOV) could increase dose up to 1.21-2 times. Also table-image intensifier distance get longer to 10cm, dose increased 1.11-1.25 times. Practitioner can adjust several geometric parameters, as FPS mode, tube angle, Collimation, SID, table-image intensifier distance, FOV. And each factors can reduce radiation dose in coronary angiography.
This research aims at measuring images density of according to DAP(dose area product), and suggesting the need to quality control of exposure dose. When tube voltage was fixed as 80 kVp and tube current was set as 1, 25, 50, 80, and 100 mAs, with the increase of DAP from 25 mAs to 50 mAs, the dose also rose 1.88 times as much as before, and with the increase from 50 mAs to 100 mAs, it got 2.05 time higher than before. However, the images density obtained as film grew as much as 48% with the increase from 25 mAs to 50 mAs, and 29% with the increase from 50 mAs to 100 mAs. In addition, it has been found out that the higher the DR images density got from 25 mAs to 50 mAs, the bigger it became by 12%, and that it got bigger by 30% with the increase from 50 mAs to 100 mAs. In other words, the differences in the image density by the increase of the dose with the digital imaging equipment in a proper condition was proved to be less than in the film images. Based on the results of this research, medical institutions using a digital imaging equipment are expected to be able to reduce exposure dose of each region of interest than now through the quality control of radiation dose.
In this study, we evaluated the DAP(Dose Area Product) reduction effect of the newly developed IRIS collimator by measuring the DAP of the Rectangle collimator and the IRIS collimator depending on the field, SID(Source to Image recpetor Distance) change, and AEC mode use. The results were as follows. The IRIS collimator decreased DAP by 34.91, 29.33, and 29.04%, respectively, compared to the Rectangle collimator when the field was increased to $8{\times}8$, $12{\times}12$, $16{\times}16inch$. And also, when the SID was increased to 100, 120 and 140 cm, the IRIS collimator decreased DAP by 10.73, 33.68 and 46.22%, respectively, compared to the Rectangle collimator. In AEC mode and none-AEC mode, DAP in IRIS collimator was reduced by 32.71 and 21.69%, respectively, compared with the Rectangle type. The IRIS collimator can reduce DAP by 29.62% on average compared to Rectangle type, which is statistically significant. These results suggest that the newly developed IRIS collimator can be used in medical field to alleviate radiation exposure.
Interventional cardiology procedures can involve relatively high radiation doses compared to conventional radiography. During CAG, CAG + PCI and PCI the same area is exposed to radiation for a long period. In this study, radiation exposure data of 421 examinations in Gyeongsang area were collected, and the DRLs and ADs in actual medical practice for three types of interventional cardiology procedures in Korea were established. In CAG 286 case, 75th percentile DRLs and ADs of the total DAP were 55.89 Gy·cm2 and 37.47 Gy·cm2 , respectively. In CAG + PCI 92 case, those values were 222.84 Gy·cm2 and 117.51 Gy·cm2 respectively. In PCI 43 case, those values were 198.73 Gy·cm2 and 120.13 Gy·cm2 respectively. In this study, for the first time, the diagnostic reference level of interventional cardiology procedures in Gyeongsang area were established. Using the diagnostic reference level of interventional cardiology procedures derived from this study, it will help to identify and improve the level of exposure dose in the region and country.
Objective: The purpose of the study was to calculate the effective and absorbed organ doses of cone-beam computed tomography (CBCT) in pediatric patient using personal computer-based Monte Carlo (PCXMC) software and to compare them with those measured using thermoluminescent dosimeters (TLDs) and anthropomorphic phantom. Materials and Methods: Alphard VEGA CBCT scanner was used for this study. A large field of view (FOV) (20.0 cm × 17.9 cm) was selected because it is a commonly used FOV for orthodontic analyses in pediatric patients. Ionization chamber of dose-area product (DAP) meter was located at the tube side of CBCT scanner. With the clinical exposure settings for a 10-year-old patient, DAP value was measured at the scout and main projection of CBCT. Effective and absorbed organ doses of CBCT at scout and main projection were calculated using PCXMC and PCXMCRotation software respectively. Effective dose and absorbed organ doses were compared with those obtained by TLDs and a 10-year-old child anthropomorphic phantom at the same exposure settings. Results: The effective dose of CBCT calculated by PCXMC software was 292.6 μSv, and that measured using TLD and anthropomorphic phantom was 292.5 μSv. The absorbed doses at the organs largely contributing to effective dose showed the small differences between two methods within the range from -18% to 20%. Conclusion: PCXMC software might be used as an alternative to the TLD measurement method for the effective and absorbed organ dose estimation in CBCT of large FOV in pediatric patients.
The purpose of this study is to suggest a method to reduce the dose by Analyzing the dose area product (DAP) and image quality according to the change of tube current using NEMA Phantom. The spatial resolution and low contrast resolution were used as evaluation criteria in addition to signal to noise ratio (SNR) and contrast to noise ratio (CNR), which are important image quality parameters of intervention. Tube voltage was fixed at 80 kVp and the amount of tube current was changed to 20, 30, 40, and 50 mAs, and the dose area product and image quality were compared and analyzed. As a result, the dose area product increased from $1066mGycm^2$ to $6160mGycm^2$ to 6 times as the condition increased, while the spatial resolution and low contrast resolution were higher than 20 mAs and 30 mAs, Spatial resolution and low contrast resolution were observed below the evaluation criteria. In addition, the SNR and CNR increased up to 30 mAs, slightly increased at 40 mAs, but not significantly different from the previous one, and decreased at 50 mAs. As a result, the exposure dose significantly increased due to overexposure of the test conditions and the image quality deteriorated in all areas of spatial resolution, low contrast resolution, SNR and CNR.
This study used a adult absorption dose phantom (CIRS model 701-G, USA) made of human equivalent material and the vascular imaging equipment Allura Xper FD 20 (Philips, Netherlands). Optically stimulated luminescent dosimeters (OSLD) were inserted into the anatomical positions corresponding to each organ, and the exposure dose was measured. Dose area product (DAP) and air kerma (AK) measured by the dose meter in the equipment were compared. Continuous imaging was performed at two angles for a total of 20 minutes, with a frame per seconds of 3.75 and 7.5 fps and an FOV of 42 cm, 37 cm, and 31 cm, respectively, under the conditions of fluoflavor I, II, and III, each selected for 5 repetitions. This study was found that selecting a lower fps was the most effective way to reduce patient exposure dose, and adjusting the fluoflavor was a good alternative method for reducing patient exposure dose at high fps. Therefore the method of condition change with the greatest dose reduction effect is to set the minimum FPS and can reduce patient exposure dose according to geometric conditions and fluoflavor characteristics.
