  • Hemodialysis is essential for patients with end stage renal failure. It is important to improve the patency rate and to minimize occurrence of the stenosis. Also, the blood flow to the artificial kidney can affect the blood flow characteristics though arteriovenous graft. Thus, the delivered dose are important factors for analyzing hemodynamic characteristics during hemodialysis access. In this study, the numerical analysis was performed for the effect of the delivered dose during hemodialysis access on the blood flow through the graft. As a result, The adverse pressure gradient occurred in case of a larger delivered dose through a catheter than standard dose and the flow instability increased. Also the circulation flow appeared largely at anastomotic site of the vein when the delivered dose was exceeded about half blood flow of inlet blood flow.

고 선량율 감마선 조사에 따른 렌즈의 열화 (A CCD Camera Lens Degradation Caused by High Dose-Rate Gamma Irradiation)

  • Assumed that an IPTV camera system is to be used as an ad-hoc sensor for the surveillance and diagnostics of safety-critical equipments installed in the in-containment building of the nuclear power plant, an major problem is the presence of high dose-rate gamma irradiation fields inside the one. In order to uses an IPTV camera in such intense gamma radiation environment of the in-containment building, the radiation-weakened devices including a CCD imaging sensor, FPGA, ASIC and microprocessors are to be properly shielded from high dose-rate gamma radiation using the high-density material, lead or tungsten. But the passive elements such as mirror, lens and window, which are placed in the optical path of the CCD imaging sensor, are exposed to a high dose-rate gamma ray source directly. So, the gamma-ray irradiation characteristics of the passive elements, is needed to test. A CCD camera lens, made of glass material, have been gamma irradiated at the dose rate of 4.2 kGy/h during an hour up to a total dose of 4 kGy. The radiation induced color-center in the glass lens is observed. The degradation performance of the gamma irradiated lens is explained using an color component analysis.

Impact of radiation dose on concurrent chemoradiotherapy for limited-stage small-cell lung cancer

  • Purpose: To evaluate clinical outcomes according to radiation dose in patients with limited-stage small-cell lung cancer (LS-SCLC) treated with concurrent chemoradiotherapy (CCRT). Materials and Methods: From January 2006 to December 2015, 38 patients with LS-SCLC were treated with CCRT with etoposide and cisplatin. Total radiation doses ranged from 45 Gy to 66 Gy (1.8-2 Gy/fraction) and were classified into three groups: 45-54 Gy, 60-63 Gy, and 66 Gy. The impact of radiation dose on survival outcomes were evaluated. Toxicities were evaluated according to the Common Terminology Criteria for Adverse Events version 4.03. Results: The median follow-up period was 21 months. The 2-year overall survival (OS) and local failure-free survival (LFFS) rates were 45.8% and 67.5%, respectively. The 2-year LFFS rates were 33.3% for 45-54 Gy group, 68.6% for 60-63 Gy group, and 87.1% for 66 Gy group (p = 0.014). In multivariate analysis, radiation dose was a significant factor for LFFS (p = 0.015). Although radiation dose was not a significant factor for OS and disease-free survival (DFS) in multivariate analysis, both OS and DFS of 66 Gy group tended to be better than that of 45-63 Gy group in univariate analysis. However, there were no differences in severe toxicities among three groups. Conclusion: Higher radiation dose achieved better local control in patients with LS-SCLC treated with CCRT. In addition, a total dose of 66 Gy tended to improve OS and DFS.

Preoperative chemoradiation for locally advanced rectal cancer: comparison of three radiation dose and fractionation schedules

  • Purpose: The standard radiation dose for patients with locally rectal cancer treated with preoperative chemoradiotherapy is 45-50 Gy in 25-28 fractions. We aimed to assess whether a difference exists within this dose fractionation range. Materials and Methods: A retrospective analysis was performed to compare three dose fractionation schedules. Patients received 50 Gy in 25 fractions (group A), 50.4 Gy in 28 fractions (group B), or 45 Gy in 25 fractions (group C) to the whole pelvis, as well as concurrent 5-fluorouracil. Radical resection was scheduled for 8 weeks after concurrent chemoradiotherapy. Results: Between September 2010 and August 2013, 175 patients were treated with preoperative chemoradiotherapy at our institution. Among those patients, 154 were eligible for analysis (55, 50, and 49 patients in groups A, B, and C, respectively). After the median follow-up period of 29 months (range, 5 to 48 months), no differences were found between the 3 groups regarding pathologic complete remission rate, tumor regression grade, treatment-related toxicity, 2-year locoregional recurrence-free survival, distant metastasis-free survival, disease-free survival, or overall survival. The circumferential resection margin width was a prognostic factor for 2-year locoregional recurrence-free survival, whereas ypN category was associated with distant metastasis-free survival, disease-free survival, and overall survival. High tumor regression grading score was correlated with 2-year distant metastasis-free survival and disease-free survival in univariate analysis. Conclusion: Three different radiation dose fractionation schedules, within the dose range recommended by the National Comprehensive Cancer Network, had no impact on pathologic tumor regression and early clinical outcome for locally advanced rectal cancer.

자궁경부암의 고선량율 강내치료 선량계획 분석 (Analysis of High Dose Rate Intracavitary Radiotherapy(HDR-ICR) Treatment Planning for Uterine Cervical Cancer)

  • Purpose : This study was done to confirm the reference point variation according to variation in applicator configuration in each fractioation of HDR ICR. Materials and Methods : We analyzed the treatment planning of HDRICR for 33 uterine cervical cancer patients treated in department of therapeutic radiology from January 1992 to February 1992. Analysis was done with respect to three view points-Interfractionation A point variation, interfractionation bladder and rectum dose ratio variation, interfractionation treatment volume variation. Interfractionation A point variation was defined as difference between maximum and minimum distance from fixed rectal point to A point in each patient. Interfractionation bladder and rectum dose ratio variation was defined as difference between maximum and minimum dose ratio of bladder or rectum to A point dose in each patient, Interfractionation treatment volume variation was defined as difference between miximum and minimum treatment volume which absorbed over the described dose-that is, 350 cGy or 400 cGy-in each patient. Results The mean of distance from rectum to A point was 4.44cm, and the mean of interfractionation distance variation was 1.14 cm in right side,1.09 cm in left side. The mean of bladder and rectum dose ratio was $63.8\%$ and $63.1\%$ and the mean of interfractionation variation was $14.9\%$ and $15.8\%$ respectively. With fixed planning administration of same planning to all fractionations as in first fractionation planning-mean of bladder and rectum dose ratio was $64.9\%$ and $72.3\%$.and the mean of interfraction variation was $28.1\%$ and $48.1\%$ reapectively. The mean of treatment volume was $84.15cm^3$ and the interfractionation variation was $21.47cm^2$. Conclusion : From these data, it was confirmed that there should be adapted planning for every fractionation ,and that confirmation device installed in ICR room would reduce the interfractionation variation due to more stable applicator configuration.

원자력발전소 계획예방정비 기간중 피폭최적화 경험 (The Optimization Experience of Occupational Exposure during Unclear Power Plant Outage)

  • By optimizing the radiation protection the collective dose and individual dose could be reduced during YGN #4 $5^{th}$ outage in 2001. The collective doses for the two high radiation jobs decreased to 85% and 65% of expected doses. The proportion of workers with low dose (below 1mSv) exposure increased 4% while the proportion of workers with over 3mSv and 5mSv exposure are decreased to 2%, 1% respectively. But none is exposed over 8mSv for the annual dose. To aid decision of utilizing the robot, cost- benefit analysis was performed and reasonable point was proposed to use the robot. For the first time job, repeated ALARA meeting and mock up training were implemented to set up working procedure by identifying the trouble. To easily set up standard procedure, mockup process was videotaped and reviewed during ALARA meeting. Monitoring is a good approach to chase radiological working condition such as working time, dose rate. behavior of workers, especially for high radiation work. Those data were estimated and adjusted from the stage of work planning to mock up. At the stage of actual work the monitoring data were compared to the estimation and recorded to database. This database will not only be used as a powerful tool for dose optimization at the following outage but also as a guideline to dose constraint set up for optimization for each specific situation.

  • Objectives: The aim of this study was to estimate the single oral toxicity of Peucedani Radix (PR) ethanol extracts. PR is one of the important herbs for removal of phlegm, the viscous turbid pathological product that can accumulate in the body and cause a variety of diseases. However, research on the pharmacologic toxicity of PR is lacking. Methods: In this study, PR was orally administered to 5-week-old male ICR mice at an oral dose of 2,000, 3,000, or 5,000 mg/kg. After a single-dose administration, the mortality and behavioral changes were observed daily and body weights were measured every two days. After 14 days, the organ weight, organ index, macroscopy, hematological analysis, and serum biochemistry analysis were determined. Results: No mortality, body weight changes, abnormal behavioral changes, or anatomical signs of toxicity were found. The organ weight, organ index, hematological analysis, and serum biochemistry analysis were also within the normal ranges. Conclusions: These results suggest that the 50% lethal dose of PR is more than 5,000 mg/kg. This could indicate that PR is a safe drug without acute toxicity and side effects.

  • This study was performed to evaluate single and repeated-dose toxicities of Bee Venom Extracts (F1, F3) in Spraque-Dawley. F1 was injected subcutaneously to rat at dose levels of 0, 0.0002, 0.002, 0.02 mg/kg/day for single-dose toxicity study and repeated-dose toxicity study. F3 was injected subcutaneously to rat at dose level of 0, 0.003, 0.03, 0.3 mg/kg/day for single-dose toxicity study and repeated-dose toxicity study. In both studies, there were no dose related changes in mortality, clinical sign, body weight change, food and water consumption, opthalmoscopy, organ weights, urine analysis, biochemical examination, and hematological findings of all animals treated with Bee Venom (F1, F3). Gross and histopathological findings revealed no evidence of specific toxicity related to Bee Venom (F1, F3). These results suggest that the subcutaneous NOEL (No Observed Effect Level) of Bee Venom (F1, F3) may be over F1 -0.02 mg/kg, F3-0.3 mg/kg.

Radiation Exposure from Nuclear Power Plants in Korea: 2011-2015

  • Background: On June 18, 2017, Korea's first commercial nuclear reactor, the Kori Nuclear Power Plant No. 1, was permanently suspended, and the capacity of nuclear power generation facilities will be adjusted according to the governments denuclearization policy. In these circumstances, it is necessary to assess the quality of radiation safety management in nuclear power plants in Korea by evaluating the radiation dose associated with them. Materials and Methods: The average annual radiation dose per unit, the annual radiation dose per person, and the annual dose distribution were analyzed using the radiation dose database of nuclear reactors for the last 5 years. The results of our analysis were compared to the specifications of the Nuclear Safety Act and Medical Law in Korea. Results and Discussion: The annual average per unit radiation dose of global major nuclear power generation was 720 man-mSv, while that of Korea's nuclear power plants was 374 manmSv. No workers exceeded 50 mSv per year or 100 mSv in 5 years. The individual radiation dose according to occupational exposure was 0.59 mSv for nuclear workers, 1.77 mSv for non-destructive workers, and 0.8 mSv for diagnostic radiologists. Conclusion: The radiation safety management of nuclear power plants in Korea has achieved the best outcomes worldwide, which is considered to be the result of the as-low-as-reasonably-achievable (ALARA) approach and strict radiation safety management. Moreover, the occupational exposures were also very low.

Fluoride removal using Alum & PACl in batch & continuous mode with subsequent microfiltration

  • In this study, defluoridation efficiency by aluminium sulphate (alum) and polyaluminium chloride (PACl) were compared for recommended Nalgonda dose (100%) and 80% of this dose in both batch and continuous modes. The residual turbidity was found to be higher in case of alum as compared to PACl with 80% dose representing lesser efficient settling of suspensions, which primarily comprise alumino-fluoro complexes that result in high residual aluminium in the treated water and this was confirmed by TEM and Zeta analysis. Moreover, the application of PACl also resulted in much lesser addition to the TDS and also required lesser lime for pH compensation due to its lower acidity. Hence this reduced dose was recommended for defluoridation. It was also observed that in case of alum, residual aluminium in treated water was 0.88 mg/L (100% dose) & 0.72 mg/L (80% dose) and in case of PACl, it was 0.52 mg/L(100% dose) & 0.41 mg/L(80% dose). After subsequent microfiltration, residual aluminium was 0.28 & 0.21 mg/L for 100% & 80% dose respectively and in case of alum and in case of PACl, it was 0.16 & 0.11 for 100% & 80% dose respectively, which conform to the Al standards(<0.2 mg/L).
