Increased expression of interleukin-1β in triglyceride-induced macrophage cell death is mediated by p38 MAP kinase

  • Triglycerides (TG) are implicated in the development of atherosclerosis through formation of foam cells and induction of macrophage cell death. In this study, we report that addition of exogenous TG induced cell death in phorbol 12-myristate 13-acetate-differentiated THP-1 human macrophages. TG treatment induced a dramatic decrease in interleukin-$1{\beta}$ (IL-$1{\beta}$) mRNA expression in a dose- and time-dependent manner. The expression of granulocyte macrophage colony-stimulating factor and platelet endothelial cell adhesion molecule remained unchanged. To identify signaling pathways involved in TG-induced downregulation of IL-$1{\beta}$, we added p38 MAPK, protein kinase C (PKC) or c-Raf1 specific inhibitors. We found that inhibition of p38 MAPK alleviated the TG-induced downregulation of IL-$1{\beta}$, whereas inhibition of PKC and c-Raf1 had no effect. This is the first report showing decreased IL-$1{\beta}$ expression during TG-induced cell death in a human macrophage line. Our results suggest that downregulation of IL-$1{\beta}$ expression by TG-treated macrophages may play a role during atherogenesis.

Total Spinal Anesthesia following Epidural Block for Correction with IIizarov Apparatus in an Achondroplasia Patient -A case report- (연골무형성증(Achondroplasia) 환자에서 술 후 교정시술을 위한 경막외차단 중 발생한 전척추 마취 -증례보고-)

  • Epidural analgesia using an epidural catheter is an effective method to relieve the pain during the rehabilitating procedure for postoperative orthopedic patients. Total spinal anesthesia is one of the possible complications of epidural catheterization which can lead to a life-threatening condition. Achondroplasia is the most common form of short-limbed dwarfism resulting from a failure of endochondral bone formation. In patients suffering with short stature syndrome like achondroplasia, the incidence and risk of total spinal anesthesia during epidural anesthesia may increase because of the technical difficulty and structural anomaly of the spine. We report here on a 35-year old female patient with a height of a 115 cm. She was diagnosed as achondroplasia and she had a previous Ilizarov operation; both tibial lengthening and correction of valgus were done. No specific event occurred during epidural catheterization. Immediately after the injection of a test dose via epidural catheter, the patient became hypotensive, drowsy and showed weakness of both her upper and lower extremities. The symptoms were disappeared after 40 minutes. The catheter was removed on the next day. We concluded that the total spinal anesthesia was caused by intrathecal injection of local anesthetics through the epidural catheter, and the anesthesia then migrated into the subarachonoid space.

  • Background: Airborne radioactive particulate in many important nuclear facilities (particularly nuclear power plants) will have a strong impact on the relative public dose if they are released into the corresponding environment traversing the stack or vents. The radiation protection researchers have regarded the relative environment assessing and estimation of public doses. And the model of assessing impact of discharges radioactive substance to the environment have been recommended by many international organizations (e.g. IAEA) with the nuclear energy safety and radiation protection. Materials and Methods: This paper introduced the generic models that were suggested by International Atomic Energy Agency (IAEA), for use in assessing the impact of discharges of radioactive substances to the environment (e.g. IAEA Safety Report Series No.19). Results and Discussion: The writers of this paper, based on the recommend methods, assessed the discharge limits in some airborne radioactive substances discharging standards. The reasons that IAEA method are introduced are mainly the following considerations: IAEA is one of international organizations with some authorities in the nuclear energy safety and radiation protection; and, more important, the recommend modes are operational methods rather than the methods having little operations such as that have used by some researchers. Conclusion: It is wish that the introduced methods in this paper can be referenced in draft or revise of the standards related to discharges of radioactive substances to the environment.

  • Bisphosphonate are a class of drugs with a chemical structure which inhibit bone resorption, actually used for metastatic bone disease, osteoporosis, Paget's disease and multiple myeloma. Significant complication associated with their use is reported recently : mandibular and maxillary osteomyelitis or osteonecrosis. So we here report our case about the patient who was diagnosed of prostate cancer in 2004 April and treated with bisphosphonate(Zoledronic acid-$Zometa^{(R)}$, Novartis Co.) intravenously every 3 to 4weeks at a dose of 4mg to prevent bone metastasis, and also, the patient who came to the hospital due to the bony exposure of mandible and pain in 2006 November and was diagnosed osteomyelitis of mandible as a result of biopsy, bone scan, PET CT examination.

A Case Report of Giant Cell Arteritis Combined with Oculomotor Nerve Palsy (동안신경마비가 동반된 거대세포 동맥염 환자의 치료 경험 - 증례보고 -)

  • Giant cell arteritis, which is also referred to as temporal arteritis, is defined as a systemic vasculitis in individuals over 50 years of age. Here, we report a case of giant cell arteritis combined with oculomotor nerve palsy. An 81-year old female patient experienced a headache for 10 days in her left temporoparietal area, that was characterized by a continuous dull ache and heaviness with intermittent shooting and lancinating pain. Her symptoms persisted in spite of receiving strong analgesics in another hospital. Upon physical examination, she was found to have marked tenderness over the left temporal area, especially along the path of the temporal artery as well as limitation of adduction, supraduction and infraduction of the left eyeball. At the time of admission, her erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP) were 52 mm/hr and 3.94 mg/dl. In addition her brain MRI revealed no specific findings. Giant cell arteritis was suspected based on the clinical symptoms and signs as well as the elevated ESR and CRP. Oral steroid therapy started was started with an initial dose of 40 mg of prednisolone per day that was gradually tapered to 5 mg a day for 2 weeks. Her headache subsided one day after the steroid therapy and oculomotor nerve palsy was markedly improved after 2 weeks of the therapy. After 2 months she had recovered completely from her symptoms.

  • Primary malignant melanoma of the vagina is an extremely rare genital neoplasm occurring mainly on postmenopausal women. It has a worse prognosis than cutaneous melanomas, because of the high rate of loco-regional recurrences and rapid systemic dissemination. In the past, radical surgical extirpation as the primary management had been recommended to improve loco-regional control, and possibly overall survival. However, the prognosis was poor In spite of such a radical approache. Recently, more conservative treatment such as wide local excision combined with adjuvant high-dose fraction radiotherapy seems to have premising results. Primary radiation therapy could be served as an alternative to surgery for patients with lesion less than 3 cm in diameter. We report 2 cases of primary vaginal malignant melanoma treated with radiotherapy.

A Case Report of Recurrent L858R Mutation Non-Small-Cell Lung Cancer with Brain Metastases Treated with Erlotinib and Traditional Korean Medicine After Failure with Gefitinib (Gefitinib 내성 후 Erlotinb과 한방 치료를 병행하여 호전된 뇌전이를 동반한 재발성 L858R 변이 비소세포폐암 환자 1례)

  • Objectives: The purpose of this study is to report the case of a patient with recurrent L858R mutation non-small-cell lung cancer with brain metastases treated with erlotinib and traditional Korean medicine after gefitinib failure. Methods: The patient was treated with erlotinib beginning in November 2021, and gamma knife surgery was performed on November 8, 2021. The dose of erlotinib was 150 mg/day every four weeks. At the same time, the patient was treated with traditional Korean medicine. Tumor size and cerebral edema were measured using computed tomography and magnetic resonance imaging, respectively. Adverse events were evaluated using the National Cancer Institute Common Terminology Criteria for Adverse Events, version 5.0. Results: After treatment with erlotinib and traditional Korean medicine for six months, the extent of the growing nodule in the right upper lobe decreased during the first three months and remained stable for the following three months. Peritumoral edema showed an increase three months after gamma knife surgery, but partial improvement of cerebral edema was confirmed with additional traditional Korean medicine six months after gamma knife surgery. The symptoms of discomfort and physical activity gradually improved. Conclusions: This case study suggests that the combination of EGFR-TKI and traditional Korean medicine may contribute to a reduction in tumor size and cerebral edema while improving quality of life.

  • Dermatofibrosarcoma protuberans (DFSP) is a rare soft tissue sarcoma, with an incidence of about 0.8% to 5% per million people per year, accounting for 1% of soft tissue sarcomas. In its early stage, DFSP is typically found as a violet or pinkish macule or patch, and it can develop into a palpable mass with ulceration or bleeding. The standard treatment for DFSP is wide local excision of the tumor with a 2- to 3-cm negative margin, and radiation therapy or chemotherapy can be conducted with surgical treatment. A 35-year-old man had a palpable mass on the left side of his occipital scalp without color change, ulceration, or bleeding, which typically are present in malignancy. A magnetic resonance imaging (MRI) scan showed a 3-cm homogenous enhanced mass without adhesion between the scalp and the mass. Unexpectedly, a biopsy revealed the round mass to be DFSP. A wide excision and rotation of the scalp flap were performed. The patient recovered without any complications and received adjuvant radiotherapy at a dose of 60 Gray (Gy) for six weeks. There was no recurrence through six months of follow-up. Here we report this unique case of DFSP with atypical presentation.

  • There have been several case reports of neurological manifestations in pediatrics as severe acute respiratory syndrome coronavirus-2 infection in children is being increased. We report a case of a 4-year-old boy who presented febrile seizure during follow-up in a negative pressure isolation room after confirmed coronavirus disease 2019, which has not yet been reported in Korea. He has no symptoms other than fever. The seizure was controlled after one dose of intravenous lorazepam, and there was no respiratory support during the hospitalization. He was discharged 12 days later without neurological sequelae.

Effects of Ondamtang, Samulansintang and Shihosogansan Water Extract on the Obesity and Stress (온담탕(溫膽湯)과 사물안신탕(四物安神湯) 및 시호소간산(柴胡疏肝散)이 비만(肥滿)과 스트레스에 미치는 영향(影響))

  • Many phychiatrists have reported that the change of serotonin concentration would cause mental disorder and affect the pathological conditions such as schizophrenia, depression and eating behabior. The end product of serotonin metabolism was excreted as 5-HIAA in urine. Serum lipids, according to the report, were concerned with obesity, said it was. This study aims to observe the changes of plasma serotonin, urinary 5-HIAA and serum lipids, making use of Fat Cell Mass rate of 27 normal persons and 42 psychosomatic patients. For this, I also observed the change of serotonin, 5-HIAA and lipids of the psychosomatic patients by the use of 3 kinds of herbs as treatment medication on the basis of physical symptoms and the results were obtained as follows; 1. Urinary 5-HIAA is correlated with the body water rate(r=0.381), while reversely correlated with the Fat Cell Mass rate(r=-0.330). 2. Compared the control group with the patients group for the serum lipids value, they showed the significant results ; $146.4{\pm}5.71$ mg/dl and $166.9{\pm}6.56$ mg/dl in the total cholestrol value over- weights, $471.2{\pm}42.99$ mg/dl in the total lipid value, and $158.1{\pm}6.50$ mg/dl and $181.1{\pm}6.30$ mg/dl in the phospholipid of the obesity, respectively. 3. With comparison of each group to other group to the others for Fat Cell Mass rate, the total cholesterol showed the significant differences when the Fat Cell Mass rate was 20% or more, HDL-cholesterol value when 30% or more, and triglyceride when 30% or more, respectively. 4. there was significant variations in the relations between Fat Cell Mass rate and body water, which body mass index was increased as the body water was decreased. 5. Fat Cell Mass rate was correlated with Cholesterol(r = 0.420), triglyceride (r = 0.443), and $\beta$-lipoprotein(r = 0.450) of serum lipids, while reversely correlated with HDL -Cholesterol(r = -0.354) and urinary 5- HIAA had the correlation coefficient of -0.330. 6. What related with body water rate urinary 5-HIAA (r = 0.381) and $\beta$-lipoprotein(r = -0.405). 7. there were significant changes in the total cholesterol value and HDL-Cholesterol Value of serum lipids after dose of Ondamtang, significant increase in the 5-HIAA value after dose of Samulansintang, and significant devrease in the Trigylceride value of serum lipids after dose of Shihosogansan. As a result, it was seen that there was direct correlations among the Fat Cell Mass, urinary 5-HIAA, and serum lipids and stress from the mental conditions was not correlated directly to Serotonin, 5-HIAA, and serum lipids. I would like to conclude, therefore, that the detailed study should be performed on the function of serotonin of hypothalamus.
